A retrospective evaluation of the quality of malaria case management at twelve health facilities in four districts in Zambia

Impacts of Recent Infection Testing Integration into HIV Surveillance in Ekiti State, South West Nigeria: A Retrospective Cross Sectional Study

Background: Timely detection of recent HIV infections is critical in achieving the HIV epidemic. The aim of this study was to evaluate the effects of integrating the Recent Infection Testing Algorithm in HIV surveillance in Ekiti State, South West Nigeria. Method: Data from the Nigeria Medical Records System (NMRS)/National Data Repository (NDR) and Index Testing (IT) Services data capturing tools were abstracted for a retrospective cross-sectional study of 719 newly diagnosed HIV-positive cases who were screened for HIV incident infection using Asante rapid recency test kits between August 2020 and June 2022 at 5 selected recency testing health facilities. The venous blood (10 ml) of all the rapid Asante tested recent patients were collected into an EDTA tube and processed for viral load testing. The plasma level of HIV-1 RNA was quantified using the CAP/CTM and cobas 4800 platform at obafemi Awolowo University Teaching Hospital PCR Laboratory Ife. Chi-square was used to characterize the recency status of the patients. A measure of the association was done using Chi-square, while the p-value of 0.05 was considered statistically significant. Results: The median age of the participants was 39 years. The result from the study showed that of the total number of 719 new cases of diagnosed HIV-positive, 626 of them were screened for rapid Test for Recency infection (RTRI) (87%). overall, we found that 1.8% (11/626) of clients were classified as recent. The investigation also showed that from the index cases of the 11 confirmed recent infections, a total number of 125 additional positive cases were found. Conclusion: This result indicated a rise in the number of cases discovered in the State. Therefore, incorporating recency testing within regular HIV testing programs becomes essential in order to prioritize clients for index testing and provide useful information on transmission clusters.

The editor and coordinator of this new 284 pages study on the New Zealand Geological Timescale, Roger A. Cooper, and his team of 22 New Zealand and foreign experts are to be complimented for their achievement in creating this comprehensive regional time scale monograph. The previous, less ambitious update was in 1996. Behind the elegant cover design, depicting an hourglass emptying rock attributes of fossils and its strata in the New Zealand Cenozoic scale, hides a set of well-done and very readable chapters. These chapters are of interest also to non-New Zealanders, not only to follow the well laid-out stratigraphic philosophy and methodology, but also to learn how far New Zealand has come in regional standardization of stratigraphy. Stratigraphy has the same rules and regulations at different scales, independent of time interval or geography.

Impact of Age on Hormonal Profiles of Sub-Fertile Women Attending Fertility Clinic in Umuahia, Abia State, South Eastern Nigeria

Aim: This study evaluated the impact of age on the hormonal profiles of women diagnosed with infertility in a Fertility Clinic in Abia State, South-East, Nigeria. Methodology: Subjects comprised of 200 females: 150 subjects and 50 controls, aged Result: The means ± SEM of serum anti-Mullerian hormones were 1602.44 ± 54.42 pg/ml (control), 848.06 ± 23.04 pg/ml (group 1), 26.74 ± 1.28 pg/ml (group 2), 10.37 ± 1.26 pg/ml (group 3) values for follicle stimulating hormone in the control subjects was 4.90 ± 0.22 mIU/ml, 12.59 ± 0.79 mIU/ml (group 1), 30.59 ± 1.31 mIU/ml (group 2), and 41.59 ± 1.59 miU/ml (group 3). Similarly, the mean ± SEM of luteinizing hormone of control, group 1, group 2 and group 3 were 5.01 ± 0.22 mIU/ml, 15.02 ± 1.13 mIU/ml, 42.71 ± 1.82 mIU/ml and 58.22 ± 2.62 mIU/ml respectively while for estadiol the values were 63.16 ± 1.95 pg/ml, 94.10 ± 5.56 pg/ml, 58.84 ± 4.01 pg/ml and 36.7 ± 1.59 pg/ml for control, group 1, group 2 and group 3 respectively. The mean ± SEM of FSH/LH ratio for the control and experimental subjects were 0.98 ± 0.01, 0.89 ± 0.02, 0.74 ± 0.03 and 0.75 ± 0.02 for control, group 1, group 2 and group 3 respectively. The comparison of the means showed significant difference (p Conclusion: The evaluation of levels of the hormonal parameters across the age ranges of the population studied shows that women within the control and experimental group 1 (of achieving pregnancy naturally.

对纯镍及其表面离子注镧样品在900℃空气中的恒温氧化规律进行了研究。采用扫描电子显微镜(SEM)和透射电子显微镜(TEM)对Nio膜的微观形貌和结构进行测试。采用激光拉曼(Raman)谱和X射线衍射仪(XRD)对两种样品表面氧化膜的应力状态进行测量。采用二次离子质谱(SIMS)对氧化膜内元素Ni、o和La的深度分布情况进行了测量。结果表明,离子注镧显著降低了镍的恒温氧化速率,细化了表面Nio膜的晶粒尺寸,同时,将氧化膜生长的生长机制由未注镧前Ni2+阳离子向外扩散转变为注镧后o2-阴离子向内扩散为主。X射线衍射和激光拉曼测量均反映出注镧引起的膜内应力降低效应,并且,结合氧化膜内应力深度分布的不均匀性及膜生长过程中存在的稀土元素效应对两种应力测量结果之间存在的偏差进行了细致分析。

Ultrafine Structure of Calcific Deposits Developed in Calcific Tendinopathy

The ultrafine structure of tendons deposits formed in three patients, males aged 52 and 61 years and a female aged 71 years were evaluated by atomic force microscopy. Three distinctly different structures of deposit surface were identified: (i) compact, smooth and uneven surface composed of closely packed nanoparticles of diameter 30 nm;(ii) surfaces consisting of plate-like crystalline particles about 30 nm thick that formed larger entities divided by deep depressions;(iii) rough surface formed by individual or closely attached elongated needle-like particles with elliptical cross-section of diameter about 30 nm. These surface structures were developed by different formation mechanisms: (i) Aggregation of Posner’s clusters into nanoparticles formed on biological calcific able surfaces and in the bulk of body fluid surrounding the deposits that subsequently settled onto the deposit surface;(ii) Regular crystal growth on surface nuclei generated at low supersaturation of body fluid with respect to the phosphatic phase and/or in a narrow cavity containing a very limited volume of liquid;(iii) Solution mediated re-crystallization of the upper layers of a deposit or unstable crystalline growth governed by volume diffusion of building units to the particle tip. Small rods, 40 nm wide and from 100 to 300 nm long, with no apparent order were detected only on the surface of deposit formed in the female patient. These rods could be debris of collagen fibres that disintegrated into individual building units (macromolecules) with some showing breakdown into smaller fragments.
