Gastric splenosis mimicking a gastrointestinal stromal tumor:A case report

BACKGROUND Mass lesions located in the wall of the stomach(and also of the bowel)are referred to as“intramural.”The differential diagnosis of such lesions can be challenging in some *** such,it may occur that an inconclusive fine needle aspiration(FNA)result give way to an unexpected diagnosis upon final surgical ***,we present a case of an intramural gastric nodule mimicking a gastric gastrointestinal stromal tumor(GIST).CASE SUMMARY A 47-year-old Caucasian woman,who had undergone splenectomy for trauma at the age of 16,underwent gastroscopy for long-lasting epigastric pain and *** revealed a 15 mm submucosal nodule bulging into the gastric lumen with smooth margins and normal overlying mucosa.A thoraco-abdominal computed tomography scan showed in the gastric fundus a rounded mass(30 mm in diameter)with an exophytic growth and intense enhancement after administration of intravenous *** ultrasound scan showed a hypoechoic nodule,and fine needle FNA was *** GIST was considered the most probable diagnosis,and surgical resection was proposed due to symptoms.A laparoscopic gastric wedge resection was *** postoperative course was uneventful,and the patient was discharged on the seventh postoperative *** final pathology report described a rounded encapsulated accumulation of lymphoid tissue of about 4 cm in diameter consistent with spleen parenchyma implanted during the previous *** Splenosis is a rare condition that should always be considered as a possible diagnosis in splenectomized patients who present with an intramural gastric nodule.

Rectal-cancer radiotherapy damages the perineal muscle floor

The interesting contribution of Qin et al.[1]about radiation injury on the rectum deserves to be integrated with observations regarding the equivalent damage to the perineal striated *** approaching this topic,not yet extensively studied,we were interested at first in delineating its basic pathology by histological methods,as already proposed[2,3].For this,we selected from our anatomopathological archive 20 surgical specimens of abdominoperineal *** the first group,we enclosed 10 surgical specimens of lower rectal cancers operated on 5-6 weeks after a neoadjuvant radiochemotherapy,consisting of 50 Gy,fractioned in 5 weeks,and associated with a FOLFOX pharmacological *** the second group,we included 5 cases of the same pathology,at a lower stage,and submitted for surgery 10 days after a‘short-term’neoadjuvant radiotherapy of 25 Gy in 5 fractions over 1 week.A third group encompassed 5 surgical specimens of abdominoperineal resection,performed for recurrent anal cancer 1 year after the same radiation treatment of 50 Gy as applied in the first *** our study,we excluded cases with neoplastic infiltration of the perineal muscles,previous perineal surgery,radiation or trauma,systemic diseases such as diabetes,vasculitis,connective-tissue or musculoskeletal disorders,manifest atherosclerosis,morbid obesity,and poor ***,the second and third groups were implemented for dose-and timerelated controls,in comparison to the first *** perineal striated muscles found in the specimens were extensively studied with serial histological sections of 4mm and different staining techniques(Haematoxylin-Eosin,Trichrome Masson,and Mallory Azan).

Aim:We aimed to analyze temporal trends in mortality from intrahepatic(ICC)and extrahepatic(ECC)cholangiocarcinoma in selected countries ***:Official death certification data for ICC and ECC and populations estimates for 29 countries worldwide(17 from Europe,8 from the Americas,and 4 from Australasia)and for Hong Kong Special Administrative Region of the People’s Republic of China(SAR),from 1995 to 2018,were extracted from the World Health Organization and the Pan American Health Organization ***-standardized mortality rates were computed.A joinpoint regression analysis was ***:In both sexes,ICC mortality rates increased in most countries considered,including the USA,the UK,and Australia;in some countries,including Italy and France,the increasing trends leveled off over the most recent *** men,around 2016,the highest rates(1.7-2.3/100,000)were observed in Hong Kong SAR,Portugal,France,Spain,Australia,Austria,the UK,and Canada;Latin American countries and some eastern European countries had the lowest rates(0.2-0.8/100,000).A similar pattern was observed in women,but with lower rates(from 1.7/100,000 in Hong Kong SAR to 0.14/100,000 in Argentina).ECC mortality declined in most European and Australasian countries,but it tended to increase in *** both sexes,rates were below 1/100,000 around 2016,with the only exceptions being Japan(2.6/100,000 men and 1.2/100,000 women)and Hungary(1.5/100,000 men and 1.1/100,000 women).Conclusion:ICC mortality increased in most areas of the world,likely due to increased prevalence of risk factors and improved cancer recognition and *** mortality fell in most countries,largely due to the widespread use of cholecystectomy.

Hazard and population vulnerability analysis: a step towards landslide risk assessment

In this paper, an attempt to analyse landslide hazard and vulnerability in the municipality of Pahuatlfin, Puebla, Mexico, is presented. In order to estimate landslide hazard, the susceptibility, magnitude （area-velocity ratio） and landslide frequency of the area of interest were produced based on information derived from a geomorphological landslide inventory; the latter was generated by using very high resolution satellite stereo pairs along with information derived from other sources （Google Earth, aerial photographs and historical information）. Estimations of landslide susceptibility were determined by combining four statistical techniques：（i） logistic regression, （ii） quadratic discriminant analysis, （iii） linear discriminant analysis, and （iv） neuronal networks. A Digital Elevation Model （DEM） of lo m spatial resolution was used to extract the slope angle, aspect, curvature, elevation and relief. These factors, in addition to land cover, lithology anddistance to faults, were used as explanatory variables for the susceptibility models. Additionally, a Poisson model was used to estimate landslide temporal frequency, at the same time as landslide magnitude was obtained by using the relationship between landslide area and the velocity of movements. Then, due to the complexity of evaluating it, vulnerability of population was analysed by applying the Spatial Approach to Vulnerability Assessment （SAVE） model which considered levels of exposure, sensitivity and lack of resilience. Results were expressed on maps on which different spatial patterns of levels of landslide hazard and vulnerability were found for the inhabited areas. It is noteworthy that the lack of optimal methodologies to estimate and quantify vulnerability is more notorious than that of hazard assessments. Consequently, levels of uncertainty linked to landslide risk assessment remain a challenge to be addressed.

Gastrointestinal stromal tumour of the rectum: Report of a case and review of literature

Gastrointestinal stromal tumour （GIST） is a rare tumour of the gastrointestinal tract which does not generally originate in the rectum. The authors describe a case of a 70-year-old man who underwent an anterior resection of the rectum for a low-risk GIST. The patient was not given adjuvant chemotherapy with imatinib and is still disease-free 30 mo after surgery. The authors conclude that although rectal GIST is extremely uncommon, it should be included in differential diagnosis when a tumour in the rectum is detected. Biopsy of the tumour is essential, since this makes it possible to reach a sure preoperative diagnosis based on the immunohistological features of the CDl17 and CD34. Although complete surgical resection with negative tumour margins is the principal curative procedure for primary and nonmetastatic tumours, further studies are still needed for the determination of the most effective treatment strategy for patients with rectal GIST.

ESWL for difficult bile duct stones:A 15-year single centre experience

Non-invasive assessment of liver fibrosis in chronic hepatitis B

The goal of this review is to provide a comprehensive picture of the role,clinical applications and future perspectives of the most widely used non-invasive techniques for the evaluation of hepatitis B virus(HBV)*** the past decade many non-invasive methods have been developed to reduce the need for liver biopsy in staging fibrosis and to overcome whenever possible its limitations,mainly:invasiveness,costs,low reproducibility,poor acceptance by *** techniques conceived to assess liver stiffness,in particular transient elastography,and the most commonly used biological markers will be assessed against their respective role and limitations in staging hepatic *** evidence highlights that both liver stiffness and some bio-chemical markers correlatewith survival and major clinical end-points such as liver decompensation,development of hepatocellular carcinoma and portal *** the non-invasive techniques here discussed can play a major role in the management of patients with chronic HBV-related *** their prognostic value,transient elastography and some bio-chemical markers can be used to better categorize patients with advanced fibrosis and cirrhosis and assign them to different classes of risk for clinically relevant *** recent data indicates that the combined measurements of liver and spleen stiffness enable the reliable prediction of portal hypertension and esophageal varices development.
