Hypof.actionated versus Conventionally f.actionated Radiotherapy in Post-m.stectom. Breast Cancer Patients

Background & Objective: Hypof.actionation has been used in curative setting in breast conservative surgery, but still no adequate inf.rm.tion about its application in the adjuvant setting af.er m.stectom. in breast cancer patients. The aim.of.this trial was to assess the ef.icacy and toxicity of.hypof.actionation radiotherapy (40 Gy in 15 f.actions) in post m.stectom. breast cancer patients and to com.are these results with those of.post m.stectom. patients treated retrospectively by conventional radiotherapy (50 Gy in 25 f.actions) as regard overall survival (OS), disease f.ee survival (Df.), locoregional disease f.ee survival (LDf.), and toxicities. Patients & m.thods: One hundred post m.stectom. breast cancer patients were included into this study, they were divided into 2 groups, the 1st included 50 patients treated prospectively with hypof.actionated radiotherapy regim.n (40 Gy in 15 f.actions), and the 2nd (control group) included 50 patients treated retrospectively with conventionally f.actionated radiotherapy regim.n (50 Gy in 25 f.actions). Results: The 2 year overall survival were 96% & 94% respectively (p = 0.7), while the disease f.ee survival were 91% & 89.8%, respectively (p = 0.9), and the LDf. were 95.8% & 93.3%, respectively (p = 0.9), G1 acute derm.titis was observed in 22 (44%) & 25 (50%) patients in group I & II respectively, G2 in 8 (16%) & 10 (20%) patients respectively, no G4 skin toxicity was detected. Radiation pneum.nitis was observed in 2 patients (4%) only in group II. Conclusion: post-m.stectom. hypof.actionated radiation therapy achieved com.arable survival and toxicity to the conventionally f.actionated radiotherapy with the advantage of.reducing overall treatm.nt tim., treatm.nt burden & cost.

Background:Thyroid carcinom.(TC)accounts f.r alm.st 0.5%–1%of.total *** incidence is increasing rapidly *** studies have drawn up the epidem.ological prof.le of.TC and its clinical and pathological ***,to date,no sim.lar studies have been conducted in ***:To establish an epidem.ological prof.le of.TC in a Tunisian health care institute and to analyze its clinical and histopathological characteristics in our *** and m.thods:We present a retrospective study reviewing the cases of.TC diagnosed in our institution in a 4-year ***:We collected a sam.le of.192 cases of.*** consisted of.31 m.les and 161 f.m.les(83.8%)with a sex-ratio m.f.of.*** m.an age was 46.4 *** thyroid carcinom. was the m.st f.equent histological *** m.ltif.cality rate was 33.8%.The m.an size of.TC was 2.2±1.9 cm.60.9%of.TC were staged pT1 and 20.3%had nodal *** thyroid m.crocarcinom.s were noted in 37.5%of.***:Our results were consistent with those of.the literature.A high proportion of.pT1 and pN0 tum.rs were noted in our series,suggesting that TC's diagnosis and m.nagem.nt was perf.rm.d at an early stage of.the disease in our *** addition,our study enabled us to notice the im.act of.the Coronavirus disease 19 crisis on the m.nagem.nt of.TC in our *** studies are needed to establish the epidem.ological prof.le of.TC in Tunisia and to assess its clinical and pathological f.atures.

Cyclophospham.de and Etoposide as a Salvage Treatm.nt in m.tastatic Osteosarcom. Patients

Background and Objective: Osteosarcom. is a rare bone cancer with approxim.tely 30% - 35% of.patients who will relapse either system.cally or locally, with the lung being the com.onest site of.relapse. The objective of.this trial was to evaluate the ef.icacy of.cyclophospham.de and etoposide, in treatm.nt of.m.tastatic osteosarcom. patients progressed af.er one or m.re chem.therapy lines, with the progression f.ee survival and treatm.nt response as the prim.ry endpoints, while the secondary endpoints were overall survival and treatm.nt toxicity. Patients and m.thods: Twenty seven m.tastatic osteosarcom. patients were enrolled into this trial and received cyclophospham.de and etoposide chem.therapy. Cyclophospham.de was given at a dose of.500 m./m. per day, I.V f.r 5 days and etoposide (100 m./m. per day I.V f.r 5 days). Response was assessed af.er 3 cycles according to Response Evaluation Criteria in Solid Tum.rs (RECIST) version 1.1. Chem.therapy Toxicity was graded according to the Com.on Term.nology Criteria f.r Adverse Events (CTCAE). Results: The m.dian overall survival tim. and progression-f.ee survival were 12 m.nths and 8 m.nths, respectively. f.ur patients (14.8%) achieved partial response;14 patients (51.9%) had stationary disease (SD);and 9 (33.3%) expressed tum.r progression. Hem.tologic toxicity was the m.in toxicity. None of.the patients had G4 or lif. threatening toxicities. Conclusion: The com.ination of.cyclophospham.de and etoposide represents an ef.icient and tolerable treatm.nt option f.r patients with m.tastatic osteosarcom..

Objective:To explore the ef.ect of.Persea am.ricana supplem.ntation on inf.am.ation,oxidative stress,and lipid prof.les in ovariectom.zed rats f.d with a high-f.t diet and exposed to ***:The control group was sham.operated,while groups 2-5 were ovariectom.zed and f.d a high-f.t *** 4 and 5 were exposed toγ-radiation(1 Gy/week f.r 5 weeks)af.er *** 3 and 5 were treated with 1 m./250 g/day of.Persea am.ricana f.r one *** levels of.estrogen,alanine am.notransf.rase,aspartate am.notransf.rase,cholesterol,triglycerides and lipoproteins were ***,hepatic oxidative stress,inf.am.atory and f.brogenic m.rkers were ***:Persea am.ricana treatm.nt reduced the oxidative stress m.rkers as well as the levels of.triglyceride,total cholesterol,and low-density lipoprotein cholesterol,which in turn lowered hepatic f.t ***,it suppressed hepatic inf.am.atory m.diators(interleukin-6,tum.r necrosis f.ctor-α,and C-reactive protein)and downregulated pro-f.brogenic m.rkers(transf.rm.ng growth f.ctor-βand tissue inhibitor of.m.talloproteinase-1).Conclusions:Persea am.ricana provides protection against ovariectom.,and gam.a radiation-m.diated hepatic inf.am.ation not only through its antioxidant,anti-inf.am.atory,lipid-lowering ef.ect but also by m.dulating the f.brogenic m.rkers.

Identif.cation and analysis of.a processed cytochrom. P450 pseudogene of.the disease vector Aedes aegypti

Objective: To clone cytochrom. P450 f.om.Aedes aegypti(Ae. aegypti) and determ.ne the characteristics using bioinf.rm.tics tools. m.thods: Cytochrom. P450 of.Ae. aegypti was am.lif.ed using polym.rase chain reaction, cloned and sequenced. Evolutionary relationship of.the sequence was inf.rred and bioinf.rm.tics tools were used to predict subcellular localisation, signal peptide, transm.m.rane helix, phosphorylation, O-glycosylation, secondary and tertiary structures of.the deduced protein. Results: Polym.rase chain reaction rather am.lif.ed a cytochrom. P450 pseudogene which was nam.d CYP4H44P(Gen Bank accession num.er Kf.79932). The pseudogene has 1537 nucleotides and an open reading f.am. of.335 am.no acids containing cytochrom. P450 m.tif. except the Wxxx R m.tif. It is highly hom.logous to CYP4H28 and CYP4H28v2. Phylogenetic analysis and evolutionary divergence showed strong clustering with CYP4H28 alleles and least divergence f.om.the alleles respectively. The deduced protein was predicted to be f.und in the cytoplasm.and likely to be phosphorylated but devoid of.signal peptide, transm.m.rane helix and O-glycosylated sites. The secondary and tertiary structures were also generated. Conclusions: A cytochrom. P450 pseudogene, CYP4H44 P was cloned f.om.Ae. aegypti. The pseudogene is hom.logous with CYP4H28 alleles and seem. to have recently diverged f.om.this group. Isolating this pseudogene is an im.ortant step f.r evaluating its biological role in the m.squito and f.r the evolutionary analysis of.Ae. aegypti CYPs.

This study aim.d to evaluate the anti-hyperglycem.c and antioxidant role of.Actinidia deliciosa(kiwif.uits)aqueous extract in streptozotocin-treated *** were distributed into;control,*** aqueous extract(ADAE;1 g/kg orally),streptozotocin(STZ;50 m./kg,i.p,single dose),and STZ plus ADAE,*** showed that ADAE had high antioxidant and radical scavenging *** in blood sugar level,lipid peroxidation(LPO),kidney f.nction biom.rkers,and perturbations in hem.tological param.ters were observed in diabetic ***,enzym.tic and non-enzym.tic antioxidants,protein content,and alkaline phosphatase(ALP)activity ***,histological,im.unohistochem.cal alpha-sm.oth m.scle actin im.unoreactivity(α-Sm.-ir)and histochem.cal(collagen,total protein,DNA,and RNA)alterations were observed in rat ***,STZ produced upregulation of.inf.am.atory associated genes(tum.r necrosis f.ctor-alpha;TNf.αand transf.rm.ng growth f.ctorβ1;TGf.β1)and triggered apoptosis by upregulating apoptotic related gene[Bcl2-associated X protein(Bax)]and downregulating anti-apoptotic related gene B-cell lym.hom.-2(Bcl-2)based on real-tim. AIm.To study the relation between hepatitis C virus (HCV) genotype 4 and m.croalbum.nuria and renal im.airm.nt in relation to hepatic histology, and virem.a in the absence of.cryoglobulinem.a, and to exam.ne the ef.ect of.treatm.nt on ***: Three hundred subjects, including 233 HCV genotype-4 inf.cted patients, were tested f.r cryoglobulinem.a, m.croalbum.nuria, album.n creatinine ratio (ACR), urea, creatinine, and estim.ted glom.rular f.ltration rate (eGf.). The param.ters were m.asured again in the HCV patients af.er 48 wk of.treatm.nt with pegylated interf.ron and ***: Signif.cantly higher levels of.m.croalbum.n- uria were detected in HCV-positive patients com.ared to HCV-negative controls (m.dian 9.5 vs 5.9, respectively, Kruskal-Wallis P=0.017). Log m.croalbum.nuria was signif.cantly correlated with hepatic inf.am.ation (r=0.13, P=0.036) and f.ibrosis (r=0.12, P=0.061), but not with viral load (r=-0.03, P=0.610), or alanine transam.nase (r=-0.03, P=0.617). Diabetes m.llitus neither signif.cantly m.derated (χ2=0.13, P=0.720), nor m.diated (Sobel test P=0.49) the HCV ef.ect. HCV status was signif. cantly associated with log m.croalbu-m.nuria (χ2=4.97, P=0.026), adjusting f.r age, gender, diabetes, cryoglobulinem.a, urea and creatinine. A positive HCV status was not signif.cantly associated with low eGf. (m./m.n every 1.73 m.) [odds ratio (OR): 0.5, 95% conf.dence interval (CI):0.2-1.4], nor with high ACR (OR: 1.7, 95% CI:0.7-4.1). End-of.treatm.nt response (ETR) was achieved in 51.9% of.patients. Individuals with ETR had signif.cantly lower m.croalbum.nuria post-treatm.nt (χ2=8.19, P=0.004).CONCLUSION: HCV af.ected the developm.nt of.m.croalbum.nuria independent of.diabetes or cryoglobulinem.a. Com.ination therapy of.pegylated interf.ron-ribavirin had a positive ef.ect in reducing m.croalbum.nuria.

In this paper, a novel gesture spotting and recognition technique is proposed to handle hand gesture f.om.continuous hand m.tion based on Conditional Random.f.elds in conjunction with Support Vector m.chine. f.rstly, YCbCr color space and 3D depth m.p are used to detect and segm.nt the hand. The depth m.p is to neutralize com.lex background sense. Secondly, 3D spatio-tem.oral f.atures f.r hand volum. of.dynam.c af.ine-invariants like elliptic f.urier and Zernike m.m.nts are extracted, in addition to three orientations m.tion f.atures. f.nally, the hand gesture is spotted and recognized by using the discrim.native Conditional Random.f.elds m.del. Accordingly, a Support Vector m.chine verif.es the hand shape at the start and the end point of.m.aningf.l gesture, which enf.rces vigorous view invariant task. Experim.nts dem.nstrate that the proposed m.thod can successf.lly spot and recognize hand gesture f.om.continuous hand m.tion data with 92.50% recognition rate.

Can We Predict m.norrhagia with Intrauterine Contraceptive Device (IUCD) Insertion?

Objective: Studying sub endom.trial vascularity and blood f.ow in cases using intrauterine contraceptive devices f.r contraception with and without m.norrhagia com.ared to cases not using intrauterine contraceptive devices. m.thods: Three hundred and f.f.een wom.n attending gynecology and f.m.ly planning outpatient clinics in the m.ternity hospital, Ain Sham. University were included in the study. They were classif.ed into three groups, 105 wom.n using IUCD with m.norrhagia (group I), 105 wom.n using IUCD without m.norrhagia (group II), and 105 norm.l controls not using IUCD (group III). Af.er excluding local causes f.r bleeding, blood disease or any m.dical disorders, transvaginal ultrasound including three dim.nsional power Doppler (3DPD) ultrasound was done f.r all wom.n. Right and lef. uterine artery pulsatility index (PI) and resistance index (RI) were calculated, subednom.trial blood f.ow RI and PI were obtained then 3DPD Vascular indices (VI, f. and Vf.) of.subendom.trial blood f.ow were obtained f.r all cases. Statistical analysis was done to com.are between the three groups. Results: A signif.cant statistical dif.erence was f.und as regards subendom.trial vascularity indices, while there was no dif.erence as regards bilateral uterine arteries Doppler indices in the three groups. Conclusion: Subendom.trial vascularity in cases of.m.norrhagia with IUCD was m.rkedly higher than in cases without m.norrhagia and cases with no IUCD. 3DPD m.y be used f.r selection of.cases prior to insertion of.IUCD.

The separation of.rare earth elem.nts using diatom.te m.5 f.om.aqueous solutions was *** experim.ntal isotherm. f.r the adsorption of.trivalent lanthanum.cerium.and neodym.um.cations on this adsorbent were quantif.ed under strongly acidic conditions(pH 2)at 298-328 *** adsorption equilibria of.these earth elem.nts were analyzed using two statistical physics m.dels(hom.geneous and heterogeneous m.nolayer m.dels).The results show that the adsorption of.these ions im.lies a m.ltiionic m.chanism.which is ***-containing f.nctional groups are responsible f.r the adsorption of.these rare-earth elem.nts on the diatom.te surf.ce.A heterogeneous statistical physics m.del conf.rm. that two Si-based f.nctional groups participate in the separation of.these *** calculated adsorption capacities at saturation f.llow the order:neodym.um.cerium.*** interaction energies range f.om.28600 to 40100 J/m.l,indicating physical adsorption on diatom.te *** study dem.nstrates that diatom.te m.5 is a prom.sing separation m.dium.that can be used f.r the recovery of.REEs dissolved in aqueous solutions via adsorption.
