Quantification of the Erythropoietic Response to Anemia and Determination of the Hb ×log(Epo) Component as an Independent Parameter, Indicating the Response to Hypoxic Stress

Serum erythropoietin (Epo) levels are influenced by the integrity of renal function, the severity and etiology of anemia, the percentage of hemoglobin F and other factors. We aimed to create a parametric expression for the response to anemic/hypoxic stress and we evaluated serum Epo levels in 1096 subjects with normal renal function, of whom 837 were anemic (Hb 16.5 g/dl). Hb ranged between 3.6 and 23.8 g/dl and the corresponding Epo between <3.4 and 2670 mIU/ml. Between log-Epo and Hb the correlation co-efficient r was ?0.706. The function linking Hb and log-Epo was determined as log(Epo) = 3.05 - 0.131 × Hb. Investigating various mathematic models, which could parametrically express the “response to anemia/hypoxia”, we found that the Hb × log(Epo) product was a stable parameter and fitted a normal distribution. Arbitrarily defining as normal range about one SD between the mean (12 - 21) we found that only 224 patients (20.8%) exhibited a Response to Anemia Index (RAI) beyond these limits. Below the lower limit there were 106 patients, diagnosed with polycythemia vera (12/38, 31.6%) anemia of chronic disease (10/34, 29.4%), megaloblastic anemia (17/56, 30.4%) and β-thalassemia trait (11/41, 26.8%). Forty-eight patients (45.3%) were diabetic. Above the defined upper normal RAI limit there were 118 patients, mainly diagnosed with secondary erythrocytosis (24/34, 70.6%), aplastic anemia (8/20, 40%), hemolytic anemia (3/14, 21.4%) and myelodysplastic syndromes (46/326, 14.1%). RAI was a rather constant parameter for each individual, with minimal variation in different evaluations, when Epo was estimated in the absence of inflammatory conditions. We have validated the superiority of RAI for the prediction of response to Epo in a cohort of 669 patients, who received Epo treatment. It is concluded that RAI is a reliable parameter describing the response to hypoxic/anemic stress.

Resolution of non-alcoholic steatohepatitis by rosuvastatin monotherapy in patients with metabolic syndrome

AIM: To investigate the effect of rosuvastatin monotherapy on non-alcoholic steatohepatitis(NASH). At present there is no effective treatment for non-alcoholic fatty liver disease or its advanced form ***: This prospective study included 20 biopsy proven patients with NASH, metabolic syndrome(Met S) and dyslipidaemia. Biochemical parameters of the blood of the patients and an ultrasonography of the liver were performed at baseline. Then patients receivedlifestyle advice and were treated for a 12 mo period with rosuvastatin(10 mg/d) monotherapy. Patients were re-evaluated during the study at 3 mo intervals, during which biochemical parameters of the blood were measured including liver enzymes. A repeat biopsy and ultrasonography of the liver were performed at the end of the study in all 20 patients. Changes in liver enzymes, fasting plasma glucose, serum creatinine, serum uric acid(SUA), high sensitivity C reactive protein(hs CRP) and lipid profile were assessed every 3 mo. The primary endpoint was the resolution of NASH and the secondary endpoints were the changes in liver enzyme and lipid ***: The repeat liver biopsy and ultrasonography showed complete resolution of NASH in 19 patients, while the 20 th, which had no improvement but no deterioration either, developed arterial hypertension and substantial rise in triglyceride levels during the study, probably due to changes in lifestyle including alcohol abuse. Serum alanine transaminase, aspartate transaminase, and γ-glutamyl transpeptidase were normalised by the 3rd treatment month(ANOVA P < 0.001), while alkaline phosphatase activities by the 6th treatment month(ANOVA, P = 0.01). Fasting plasma glucose and glycated haemoglobin were significantly reduced(P < 0.001). Lipid values were normalised by the 3rd treatment month. No patient had Met S by the 9th treatment month. Accumulating evidence supports that psoriasis may be a potential multisystem inflammatory disease associated with a range of co-morbidities showing an overlapping pathology and an important health impact such as metabolic *** is associated with an increased risk of obesity,metabolic syndrome(Mets)and diabetes mellitus type 2,following a"dose-response"relationship from mild to severe ***,recent evidence from large prospective studies suggests that obesity constitutes a risk factor for psoriasis and psoriatic ***,a dyslipidemic profile may precede psoriasis *** obesity,Mets and psoriasis,characterized as chronic inflammatory states,stem from a shared underlying pathophysiology exhibiting common genetic predisposition and risk factors such as high caloric intake,physical inactivity and psychological *** weight may potentiate the inflammation of psoriasis through the deregulation of adipocytokines while,at the same time,it may help the development of ***,recent translational data has shown that psoriasis,through increased T-helper inflammatory cytokines in skin and sera,may exert a plethora of effects on insulin regulation and lipid ***-based prospective cohort and longitudinal studies are needed to unravel the association between psoriasis and metabolic *** recognition of the intricate complex interplay between psoriasis and metabolic co-morbidities may help dermatologists to be aware of associated metabolic co-morbidities in order to screen for metabolic diseases and manage holistically and effectively the psoriatic patient.

目的:经提出并证实,采用Pro-Argin TM漱口水工艺制成的含有0.8%精氨酸、PVM/MA共聚物、焦磷酸盐和0.05%氟化钠的精氨酸漱口水的作用方式,可利用多种体外技术产生封闭作用。方法:采用定量和定性实验室技术研究这种新型精氨酸漱口水的作用方式。利用共焦激光扫描显微镜检查(CSLM)和原子力显微镜检查(AFM)研究牙本质表面的水合层。化学分析电子光谱学(ESCA)、二次离子质谱学(SIMS)和近红外光谱学(NIR)提供了关于这种漱口水的化学性质的信息。结果:使用精氨酸漱口水后,采用CLSM观察暴露牙本质小管上的水合层的形成。经荧光研究证实,水合层可渗透到牙本质小管内壁。经AFM研究证实,精氨酸漱口水对牙本质表面具有亲合力,这与其粘合性质相一致。NIR表明,使用漱口水数次后,精氨酸会沉积下来,而ESCA/SIMS检测到磷酸基团和有机酸基团的存在,表明PVM/MA共聚物和焦磷酸盐是连同精氨酸一起沉积下来的。结论:研究表明:富含精氨酸层连同来自于含有0.8%精氨酸、PVM/MA共聚物、焦磷酸盐和0.05%氟化钠的无酒精漱口水的共聚物和磷酸盐离子一起沉积下来后,可封闭牙本质表面。

Acute bacterial sternoclavicular osteomyelitis in a long-term renal transplant recipient

Kidney transplantation is the treatment of choice for a significant number of patients with end-stage renal disease. Although immunosuppression therapy improves graft and patient's survival, it is a major risk factor for infection following kidney transplantation altering clinical manifestations of the infectious diseases and complicating both the diagnosis and management of renal transplant recipients(RTRs). Existing literature is very limited regarding osteomyelitis in RTRs. Sternoclavicular osteomyelitis is rare and has been mainly reported after contiguous spread of infection or direct traumatic seeding of the bacteria. We present an interesting case of acute, bacterial sternoclavicular osteomyelitis in a long-term RTR. Blood cultures were positive for Streptococcus mitis, while the portal entry site was not identified. Magnetic resonance imaging of the sternoclavicluar region and a three-phase bone scan were positive for sternoclavicular osteomyelitis. Eventually, the patient was successfully treated with Daptomycin as monotherapy. In the presence of immunosuppression, the transplant physician should always remain alert for opportunistic pathogens or unusual location of osteomyelitis.

Factors affecting breastfeeding duration in Greece:What is important?
