Factors Associated with Splenomegaly amongst Patients with Sickle Cell Disease in Cameroon

Introduction: Sickle cell disease is the most common hemoglobinopathy in the world. Sickle cells are quickly fixed and destroyed by the elements of the reticuloendothelial system mainly in the spleen. It leads to a palpable increase in the volume of the spleen called splenomegaly. Splenomegaly is the cause of multiple complications that are relatively frequent and potentially serious in sickle cell disease, such as splenic sequestration seizures, spleen rupture, hypersplenism and splenic abscesses. We aimed at determining the prevalence of splenomegaly and to study the associated factors in patients with sickle cell disease. Materials and Methods: This was an analytical cross-sectional study, conducted from 1 January to 30 April 2019, at the Mother and Child Center of the Chantal BIYA Foundation. Patients were grouped into two groups: patients with splenomegaly (PS1) and patients without splenomegaly (PS0). As soon as we obtained the informed consent of the parents, we examined the children and recorded socio-demographic data, disease history and follow-up, documented complications, and clinical findings;then we performed the Rapid Malaria Diagnostic Test. The statistical analyzes were carried out using SPSS20 (Statistical Package for Social Sciences) and Microsoft Excel 2010 software. Results: We examined 403 children with sickle cell disease and 142 had splenomegaly (35%). Almost all of the study populations were homozygous SS. The Hackett 2 stage of splenomegaly was the most frequent (56.7%). The most common physical sign among PS1 was the presence of abdominal scarring (50.7% vs. 19.4%). PS1 had significantly lower levels of Hb (6.94 ± 1.67 vs 7.62 ± 1.43 p = 0.003) and platelets (297.45 ± 146.25 vs 398.70 ± 163.73 p lenomegaly (PS0). Communication Mediated through Natural language Generation in Big Data Environments: The Case of Nomao

Along with the development of big data, various Natural language Generation systems (NlGs) have recently been developed by different companies. The aim of this paper is to propose a better understanding of how these systems are designed and used. We propose to study in details one of them which is the NlGs developed by the company Nomao. First, we show the development of this NlGs underlies strong economic stakes since the business model of Nomao partly depends on it. Then, thanks to an eye movement analysis conducted with 28 participants, we show that the texts generated by Nomao’s NlGs contain syntactic and semantic structures that are easy to read but lack socio-semantic coherence which would improve their understanding. From a scientific perspective, our research results highlight the importance of socio-semantic coherence in text-based communication produced by NlGs.

Rapid HPlC Method for Determination of Parachloroaniline in Chlorhexidine Antiseptic Agent in Mouthrinses, Ophthalmic and Skin Solution

We described a simple and rapid method to quantify simultaneously chlorhexidine (CHD) and its major metabolite, para Chloroaniline (pCA) by HPlC with UV detection without the additional need of mobile-phase amine modifiers or ion-pairing reagents, with good resolution between pCA and CHD, symmetry peak of the compound and short run time. HPlC-UV analyses were performed using a Dionex? Summit liquid chromatograph (Dionex Corp, Sunnyvale, CA, USA). Chromatographic separations were carried out on a luna? 150 mm×3 mm i.d. column packed with 3 μm CN (cyano) particles (Phenomenex?), guarded by an on-line filter. Mobile phase consist of methanol:water with sodium chloride with 0.02% of formic acid (55:45). Wavelengths for pCA and for CHD are 238 and 255 nm respectively. Influence of methanol and of sodium chloride content in the eluant has been studied. linearity of CHD is very good, from 0.5 up to 21.2 μg/l while linearity of pCA is in the range of 0.05 to 10 μg/l with correlation coefficients above 0.999. Resolution between the components is above 4, asymmetry is about 1.3 and 1.7 for pCA and CHD respectively and the run time is less than 5 minutes. This method has been applied to CHD solution of different medical devices. No interference has been reported, and the analysis of direct injection of solution, without any treatment is achieved in less than five *** conclusion, we present a validated method for dosage of CHD and its major impurity pCA, known to be carcinogen, available into medical products or medicinal device for in-vitro diagnostic.

Influence of a liquid Nutritional Supplement on Water Intake in Experimental Beagle Dogs

The objectives were to evaluate the effects of a liquid nutritional supplement formulated for dogs on water intakes and urine output. A liquid nutritional supplement was tested by way of a cross-over design in 8 experimental healthy Beagle dogs (4 males and 4 females, aged 9.3 years). The supplement (87 percent water, 2.7 percent protein, 2.6 percent fat, 0.4 percent crude fiber) was added to water and tested at 2 incorporation rates (50 or 70 ml/day/dog—D50 or D70) versus the control placebo (CO-water only). The dogs were kept in a controlled environment;water intakes and urine output were measured. Individual water intakes were characterised by large variations. Mean water intake increased significantly by 28 percent in dogs receiving the liquid nutritional supplement, in both genders, irrespectively of the dosage. Urine output was also increased, by 55 percent. Faeces scores remained unchanged. It was concluded that the liquid supplement increased water intake and urine output in a safe way, without increasing dramatically the daily dietary sodium chloride intake. The recommended dosage of the manufacturer—50 ml/day for dogs weighing 10 - 20 kg BW is efficient. Increasing the dosage had no advantage, nor adverse effects. Increased water intake and urine output is of interest for dogs suffering from urolithiasis.

Malaria Characteristics in Children with Sickle Cell Disease

Background: The relationship between sickle cell disease and malaria is the subject of much controversy. However, there is a lack of data in our services. Our objective was to study the epidemiological, diagnostic and evolutionary characteristics of malaria in children with sickle cell disease followed in a specialized setting. Patients and Methods: We conducted a retrospective, descriptive, and analytical study of children with sickle cell disease (SCD) who presented with malaria and were followed at the Ambulatory Care Unit for Sickle Cell Children and Adolescents (USAD) at the Albert Royer National Children’s Hospital in Dakar, from January 1st, 2017, to December 31th, 2019. We included all the followed pediatric patients, less than 16 years, with sickle cell disease who presented at least one episode of malaria, confirmed by a positive thick drop, during this 3 years. We did not include patients with incomplete records or those older than 16 years. The clinical and biological signs, the follow up was collected and analyzed with Excel package 2019. Results: Of 3773 patients followed for sickle cell disease, 21 had presented malaria. The frequency was 0.5% or 7 cases/year. However, we exploited the data of 14 of them. The sex ratio was 6 boys for a girl and the mean age at admission was 7.3 years. The highest number of malaria cases was observed in 2018 and the peak frequency was observed in November with 8 cases (57.1%). Fever was the most frequent symptom, observed in 10 patients (71.4%). All patients were SS homozygous, with a mean baseline hemoglobin level of 7.5g/dl. All patients had a positive thick blood smear and Plasmodium falciparum was the only species found in the blood smear, with a mean parasite density of 1693 parasites/ml of blood. Place of Doppler Ultrasound in the Characterization of Ovarian Cysts: About 35 Cases

Pelvic ultrasound is the first-line examination for the diagnosis of ovarian cysts. When coupled with Doppler, it allows the diagnosis to be directed towards the benign or malignant nature of the cyst. These benign tumors are frequent in pubertal period. We report a descriptive study in 35 patients aged between 18 and 32 years of age who received a pelvic inflammatory etiology assessment. The purpose of this study was to show the place of pelvic ultrasound in the diagnosis of ovarian cysts.
