We report a case of acute chylous peritonitis mimicking acute appendicitis in a man with acute on chronic pancreatitis. Pancreatitis, both acute and chronic, causing the development of acute chylous ascites and peritonitis has rarely been reported in the English literature. This is the fourth published case of acute chylous ascites mimicking acute appendicitis in the literature.

Agricultural intensification and the subsequent decline of mixed farming systems has led to an increase in continuous cropping with only a few fallow or break years,undermining global soil ***-ley rotations incorporating temporary pastures(leys) lasting 1–4 years may alleviate soil degradation by building soil fertility and improving soil ***,the majority of previous research on arable-ley rotations has utilized either grass or grassclover leys within ungrazed *** leys,containing a mix of grasses,legumes,and herbs,are rapidly gaining popularity due to their promotion in agri-environment schemes and potential to deliver greater ecosystem services than conventional grass or grass-clover *** grazing in arable-ley rotations may increase the economic resilience of these systems,despite limited research of the effects of multispecies leys on ruminant health and greenhouse gas *** review aims to evaluate previous research on multispecies leys,highlighting areas for future research and the potential benefits and disbenefits on soil quality and livestock *** botanical composition of multispecies leys is crucial,as legumes,deep rooted perennial plants(e.g.,Onobrychis viciifolia and Cichorium intybus) and herbs(e.g.,Plantago lanceolata) can increase soil carbon,improve soil structure,reduce nitrogen fertilizer requirements,and promote the recovery of soil fauna(e.g.,earthworms) in degraded arable soils while delivering additional environmental benefits(e.g.,biological nitrification inhibition and enteric methane reduction).Multispecies leys have the potential to deliver biologically driven regenerative agriculture,but more long-term research is needed to underpin evidence-based policy and farmer guidance.

Background Sub-Saharan Africa experiences a disproportionate amount of pediatric surgical disease,with 80%of children lacking access to timely,affordable,and safe surgical *** study aims to characterize the burden of disease and outcomes of pediatric surgical conditions at Connaught Hospital,the main pediatric referral hospital in Sierra *** This retrospective and hospital-based study included children up to 15 years old who were operated on between 2015 and June 2016 at Connaught Hospital in Freetown,Sierra *** and inferential statistics were used to characterize the distribution of disease and compare all variables against age category and *** A total of 215 patients were included in this study of which 72.5%(n=132)were male and 27.5%(n=50)were *** of the patients were diagnosed with congenital anomalies(60.9%;n=131).However,infection was the leading diagnosis(60.5%;n=23)among patients aged 5–10 years(n=38).Inguinal hernia was the leading condition(65.0%;n=85)among patients presenting with a congenital *** condition with the highest mortality was infections(17.0%;n=8),followed by other conditions(9.1%;n=2)and congenital anomalies(3.1%;n=4).Based on the results of this study,over 7000 children with inguinal hernias remain untreated annually in Freetown,Sierra *** This study quantifies the burden of surgical disease among children,a foundational step toward the prioritization of pediatric surgical care in national health agendas,the development of evidence-based interventions,and the strategic allocation of resources in Sierra Leone.

BACKGrOUND The impact of age on outcomes in cardiogenic shock(CS)is poorly described in the pre-hospital *** assessed the impact of age on outcomes of patients treated by emergency medical services(EMS).METHODS This population-based cohort study included consecutive adult patients with CS transported to hospital by *** linked patients were stratified into tertiles by age(18-63,64-77,and>77 years).Predictors of 30-day mortality were assessed through regression *** primary outcome was 30-day all-cause *** A total of 3523 patients with CS were successfully linked to state health *** average age was 68±16 years and 1398(40%)were *** patients were more likely to have comorbidities including pre-existing coronary artery disease,hypertension,dyslipidemia,diabetes mellitus,and cerebrovascular *** incidence of CS was significantly greater with increasing age(incidence rate per 100,000 person years 6.47[95%CI:6.1-6.8]in age 18-63 years,34.34[32.4-36.4]in age 64-77 years,74.87[70.6-79.3]in age>77 years,Pre was a step-wise increase in the rate of 30-day mortality with increasing age *** adjustment,compared to the lowest age tertile,patients aged>77 years had increased risk of 30-day mortality(adjusted hazard ratio=2.26[95%CI:1.96-2.60]).Older patients were less likely to receive inpatient coronary *** Older patients with EMS-treated CS have significantly higher rates of short-term *** reduced rates of invasive interventions in older patients underscore the need for further development of systems of care to improve outcomes for this patient group.

This study examines the impact of spatial landscape configuration(e.g.,clustered,dispersed)on land-surface temperatures(LST)over Phoenix,Arizona,and Las Vegas,Nevada,*** classified detailed land-cover types via object-based image analysis(OBIA)using Geoeye-1 at 3-m resolution(Las Vegas)and QuickBird at 2.4-m resolution(Phoenix).Spatial autocorrelation(local Moran’s I)was then used to test for spatial dependence and to determine how clustered or dispersed points were ***,we used Advanced Spaceborne Thermal Emission and reflection radiometer(ASTEr)data acquired over Phoenix(daytime on 10 June and nighttime on 17 October 2011)and Las Vegas(daytime on 6 July and nighttime on 27 August 2005)to examine day-and nighttime LST with regard to the spatial arrangement of anthropogenic and vegetation *** Moran’s I values of each land-cover type were spatially correlated to surface *** spatial configuration of grass and trees shows strong negative correlations with LST,implying that clustered vegetation lowers surface temperatures more *** contrast,clustered spatial arrangements of anthropogenic land-cover types,especially impervious surfaces and open soil,elevate *** findings suggest that city planners and managers should,where possible,incorporate clustered grass and trees to disperse unmanaged soil and paved surfaces,and fill open unmanaged soil with *** findings are in line with national efforts to augment and strengthen green infrastructure,complete streets,parking management,and transit-oriented development practices,and reduce sprawling,unwalkable housing development.
