Coronary Artery Anomalies Detected by Multi-Detector Computed Tomography: A Single Centre Experience

Background: Coronary artery anomalies (CAA) prevalence represents less than 1% of congenital heart diseases. it includes anomalies in origin, course, and termination. its detection has been easier with advances in imaging techniques using multi-detector computed tomography (MDCT). MDCT helps not only detection of the anomalous origin, but it allows delineation of the course and termination of the arteries, differentiation between benign and malignant courses, and guiding therapeutic interventions. Results: There were consecutive patients with a low-to-intermediate probability of coronary artery disease scanned with 128 MDCT. Each patient underwent a non-contrast prospective gating acquisition for coronary calcium scoring followed by contrast-enhanced helical retrospective gated scans for the detection of coronary artery origin, course, termination, and detection of concomitant atherosclerosis. We scanned 1000 patients with a mean age of 57.5 ± 8.3, and 68% were males. Thirty-two anomalies were noted (3.2%) including;nineteen (1.9%) anomalous origin from the opposite sinus, three (0.3%) anomalous left coronary arteries from the pulmonary artery (ALCAPA), one (0.1%) super dominant left anterior descending artery (LAD) giving origin to the posterior descending artery (PDA), three single coronary arteries (0.3%)in which the left main (LM) and right coronary arteries were originating with a common stem from the right coronary sinus (RCS)and the LM took a pre-pulmonic course. Along with six (0.6%) dual LAD including five (0.5%) patients with type i (short LAD and long diagonal), there was one (0.1%) type 4 with an extra LAD originating from the RCS with a pre-pulmonic course. Conclusions: MDCT allows easy detection of coronary anomalies with high spatial resolution and overcomes limitations in conventional invasive coronary angiography. introduction: Hepatitis C virus (HCV) genotype 4 is the most prevalent in Egypt. Vis-ceral adiposity index (VAi) and (TyG) index are newly developed indices for assess-ment of metabolic syndrome (MS) and insulin resistance (iR). We aimed at comparing their levels in HCV-patients with healthy controls and validate their use for prediction of hepatic histopathological changes. Patient and Methods: 78 chronic HCV-infected patients proven by PCR, viral genotyping and hepatic histopathology, and 67 healthy controls were enrolled. Presence of MS, Homeostasis Model Assessment for iR esti-mation (HOMA-iR), TyG index, and VAi were assessed. Results: HOMA-iR, TyG and frequency of MS were significantly higher in patients’ group (p p p = 0.002). Conclusion: HCV genotype 4 is significantly associated with MS and increased values of HOMA iR and TyG index. TyG index and VAi are valuable simple indices that could predict the histopathological changes in Egyptian CHC pa-tients.

influence of Abiotic Elicitors on Accumulation of Thymol in Callus Cultures of Origanum vulgare L.

Callus cultures of Origanum vulgare L. were established from leaf discus on Murashige and Skoog （MS） medium containing different levels of growth regulators, i.e., 2,4-Dichlorophenoxyacetic acid （2,4-D）, Naphthalene acetic acid （NAA）, Benzyl Adenine （BA） and Kinetin （Kn） and incubated under dark condition. Callus tissues were employed to study the influence of abiotic elicitors on the production of thymol. Constant weights of callus （300 mg） were cultured on accumulation medium treated separately with each one of elicitors used （50 g/L sucrose, 200 mg/L NaC1 and 50 or 100 mg/L proline）. The fresh and dry weights of callus were recorded after six weeks. The result indicated that maximum production of fresh and dry callus weight were 1,014 mg and 46.20 mg respectively achieved at 0.5 mg/L 2,4-D and 3 mg/L BA adding to the medium. Dry callus tissues were extracted with 70% methanol and analyzed by HPLC to determine the concentrations of thymol. The addition of abiotic elicitors to MS medium caused significant reduction in fresh weight of callus compared with control treatment. The concentration of thymol in the callus cultured on control treatment was 146.6 ppm. The data showed that 50 or 100 mg/L proline produced the highest yield of thymol 181.48 ppm and 174.58 ppm respectively, followed by sucrose 162.9 ppm, whereas the treatment with NaCi caused reduction in thymol concentration to percentage of 50.56% compared with the control.

Patterns and Changes in the Treatment of Patients with Type ii Diabetes Mellitus in Egypt: The DiSCOVER Program

The DiSCOVER study is a three-year, non-interventional prospective study conducted in 37 countries, including Egypt, to assess the treatment patterns and outcomes in patients with type 2 diabetes mellitus initiating a second-line antidiabetic therapy (add-on or switch). in this report of the Egyptian cohort of DiSCOVER, baseline data were collected according to routine clinical practice at 38 study sites, using a standardized electronic case report form, in the period from December-2014 to November-2019. We enrolled 583 patients (mean age: 52.9 ± 9.8 years and median duration since diagnosis: median 36.5, iQR 18.1, 70.4 months). The mean HbA1c value at baseline was 8.6 ± 1.4%, indicating poor glycemic control. The most commonly prescribed first-line medications were metformin or sulfonylurea monotherapy. For second line-therapy, the majority of patients switched to dual therapy with metformin plus sulfonylureas or DPP-4 inhibitors. Fewer patients switched to triple therapy, treatment by four or more medications, or insulin treatment (15, 12, and 35 patients, respectively). The most commonly cited reasons for switching to second-line therapy were lack of efficacy, weight gain, hypoglycemic events, and side effects (549, 54, 25, and 21 patients, respectively). The set treatment target of enrolled patients at the initiation of second-line therapy was an HbA1c level of 6.9%. Follow-up data will assess the outcomes of such changes in the Egyptian population.

E-mail security becomes critical issue to research community in the field of information security. Several solutions and standards have been fashioned according to the recent security requirements in order to enhance the e-mail security. Some of the existing enhancements focus on keeping the exchange of data via e-mail in confident and integral way. While the others focus on authenticating the sender and prove that he will not repudiate from his message. This paper will survey various e-mail security solutions. We introduce different models and techniques used to solve and enhance the security of e-mail systems and evaluate each one from the view point of security.

Knowledge, Attitude and Practice (KAP) Assessment of intestinal Parasitic infection among School Children in Asmara, Eritrea

Purpose: Poor hygiene practice, inadequate sanitary conditions and lack of awareness of correct mode of transmission are some factors that attribute to high intestinal parasitic infection among children. Therefore, having proper knowledge, good attitude and practice (KAP) against the aforementioned factors can significantly mitigate the spreading of intestinal infection. Methods: The present study was a cross-sectional study which comprised of 127 students who were selected by stratified random sampling. A self-administered questionnaire was used to record KAP of students against intestinal parasitic infection. Total 126 students gave stool samples which were analysed by formol-ether concentration technique. in addition, sewage samples were also collected and analysed for any parasitic load. Statistical data were generated by Chi-square test. Results: A total of 126 stool samples were examined in the study of which 46 (36.50%) were found infected with one or more intestinal parasites. Among the parasites identified, the most common was Hymenolepsis nana with a prevalence rate of 35 (76.0%). Here, most of the students 96 (76.1%) were having poor knowledge about the correct mode of transmission of intestinal parasites. Majority of the students 91 (72.2%) practicing defecation in open air when they are at home, and all 126 (100%) are doing that at school, and significant number of students were not washing hands after defecation. Conclusion: Lack of knowledge regarding mode of transmission and practice of defecation in open air and not washing hands after defecation are issues of great concern, because many parasitic infection have faecal-oral mode of transmission. So, health authorities have to take these issues seriously and action would be vital to resolve it.

A Validated High-Performance Thin-Layer Chromatography (Hptlc) Method for the Quantitative Determination of Tricin in Two <i>Spergularia</i>Species

A simple, sensitive, precise and robust high-performance thin-layer chromatographic (HPTLC) method for analysis of tricin has been developed and validated for the determination of tricin in genus Spergularia. Analysis of tricin was performed on pre-coated TLC aluminum plates with silica gel as the stationary phase. Linear ascending development was carried out in double twin trough glass chamber saturated with a mobile phase consisting of chloroform: methanol (37:3 v/v). Spectro-densitometric scanning was performed by TLC scanner iii (CAMAG) in absorbance mode at the wavelength of 270 nm. The system was found to give compact spots for tricin (Rf value of 0.34 ± 0.04). The linear regression analysis data for the calibration plots showed a good linear relationship with r2 = 0.9981 in the concentration range 2-15 μl with respect to peak area. According to the international Conference on Harmonization (iCH) guidelines, the method was validated for precision, recovery, and robustness. The tricin content quantified from Spergularia marina and S. daiandra was found to be 0.398 mg/100g and 1.642 mg/100g fresh weight, respectively. Statistical analysis of the data showed that the method is reproducible and selective for the estimation of tricin.

Estimates of the Prevalence of Tobacco Smoking in Egypt

The use of tobacco products constitutes the leading cause of preventable deaths in the world. in Egypt, data on the national prevalence of cigarette and waterpipe smoking are lacking, yet such data are vital to tobacco control efforts. We designed and implemented a sampling method to generate population-based prevalence estimates for tobacco smoking in Egypt. in 2005, a total of 3369 adults ages 18 and above, including 1867 females and 1502 males, participated in the survey. Among males, the national prevalence of former cigarette smoking was 18.1%, and 27.5% reported current smoking. Exclusive current waterpipe smoking was reported by 7.2% of adult males, and a further 3.4% reported smoking both waterpipes and cigarettes. Tobacco smoking was rarely reported by females;among them the national prevalence of current cigarette smoking was 0.3%. This study produced the first robust estimate of the national prevalence of cigarette and waterpipe smoking in Egypt based on a rigorous probability sample. Along with the more recent Global Adult Tobacco Survey, carried out by the World Health Organization in 2009, the results will be useful in assessing smoking trends in Egypt and in evaluating the impacts of recent tobacco control laws.

Relationship between Cerebral imaging and Executive Functions in Egyptian Elderly

This study aimed at assessing the relationship between executive functions and left sided hemiplegia due to non hemorrhagic supratentorial infarction in elderly Egyptians. it is considered a case control study which was conducted among 90 elderly participants, who were divided into two groups: a case group, 45 cases with cerebral infarctions of 6 months duration or more, and a control group who did not have previous cerebral infarctions. Both groups were selected from Ain shams University Hospital. Each participant was subjected to comprehensive geriatric assessment, executive functions assessment using, block design test, digit span, letter verbal fluency test, animal verbal fluency test and clock drawing test, and then a Computed Tomography (CT) brain was performed. it was found that cases were suffering from more functional impairment than controls, and have had significant lower scores in Mini Mental Status Examination (MMSE) (P ignificant difference was found between both groups as regards performance of executive function tests (P ignificant effect of Parietal lobe infarctions was found on Block Design Test, Digit Span Test, Animal Verbal Fluency and Clock Drawing Test (P value ion: There was a significant difference between cases and controls as regards their performance in cognitive and executive function tests.

Nuclear Factor-Kappa B and Other Oxidative Stress Biomarkers in Serum of Autistic Children

The aim of the present study was to investigate the status of oxidative stress in the serum of children affected with autism spectrum disorder. Twenty autistic children aged 3 to 12 years, were gender and age-matched with 20 typically developing children. Changes in the levels of the redox-sensing transcription factor nuclear factor-kappa B (NF-κB) was measured in serum of autistic children and controls. Other oxidative stress biomarkers such as malondialdehyde, reduced glutathione, total antioxidant capacity, catalase activity, and paraoxonase 1 activity were determined in serum as well. Significant increase was observed in serum NF-κB of autistic children compared to that in controls (by 138.6%). There was also marked increase in malondialdehyde level by 87.3% in autistic patients. Meanwhile, there were significant decreases in reduced glutathione (by 24%), catalase activity (by 40.8%), paraoxonase 1 activity (by 36.6%), and total antioxidant capacity (by 36.5%) compared to the control group. These data clearly demonstrate increased oxidative stress in serum of autistic children and suggest that the NF-κB signaling pathway is activated in autism, possibly due to increased oxidative burden.
