Evaluation of quantitative contrast harmonic imaging to assess malignancy of liver tumors:A prospective controlled two-center study

AIM: To establish the extent to which contrast enhancement with SonoVue in combination with quantitative evaluation of contrast-medium dynamics facilitates the detection of hepatic tumors. METHODS: One hundred patients with histologically confirmed malignant or benign hepatic tumor (maximum size 5 cm) were analyzed. Contrast-enhanced ultrasound (bolus injection 2.5 mL SonoVue) was carried out with intermittent breath-holding technique using a multifrequency transducer (2.5-4 MHz). Native vascularization was analyzed with power Doppler. The contrast-enhanced dynamic ultrasound investigation was carried out with contrast harmonic imaging in true detection mode during the arterial,portal venous and late phases. Mechanical index was set at 0.15. Perfusion analysis was performed by post-processing of the raw data time intensity curve (TIC) analysis. The cut-off of the gray value differences between tumor and normal liver tissue was established using Receiver Operating Characteristic (ROC) analysis 64-line multi-slice computed tomography served as reference method in all cases. Magnetic resonance tomography was used additionally in 19 cases. RESULTS: One hundred patients with 59 malignant (43 colon,5 breast,2 endocrine metastases,7 hepatocellular carcinomas and 2 kidney cancers) and 41 benign (15 hemangiomas,7 focal nodular hyperplasias,5 complicated cysts,2 abscesses and 12 circumscribed fatty changes) tumors were included. The late venous phase proved to be the most sensitive for classification of the tumor type. Fifty-eight of the 59 malignant tumors were classified as true positive,and one as false negative. This resulted in a sensitivity of 98.3%. Of the 41 benign tumors,37 were classified as true negative and 4 as false negative,which corresponds to a specificity of 90.2%. Neonatal erythema multiforme associated with a rotavirus infection:A case report

CYP2C19 polymorphism influences Helicobacter pylori eradication

The known factors that have contributed to the decline of Helicobacter pylori (H. pylori) eradication rate include antibiotic resistance, poor compliance, high gastric acidity, high bacterial load, and cytochrome P450 2C19 (CYP2C19) polymorphism. Proton pump inhibitor (PPI) is important in the eradication regimen. The principal enzyme implicated in the metabolism of PPIs is CYP2C19. The effects of PPI depend on metabolic enzyme, cytochrome P450 enzymes, and CYP2C19 with genetic differences in the activity of this enzyme (the homozygous em, heterozygous em (Hetem), and poor metabolizer). The frequency of the CYP2C19 polymorphism is highly varied among different ethnic populations. The CYP2C19 genotype is a cardinal factor of H. pylori eradication in patients taking omeprazole- based or lansoprazole-based triple therapies. In contrast, the CYP2C19 polymorphism has no significant effect on the rabeprazole-based or esomeprazole-based triple therapies. The efficacy of levofloxacin-based rescue triple therapy might be also affected by the CYP2C19 polymorphism, but CYP2C19 genotypes did not show obvious impact on other levofloxacin-based rescue therapies. Choice of different PPIs and/or increasing doses of PPIs should be individualized based on the pharmacogenetics background of each patient and pharmacological profile of each drug. Other possible factors influencing gastric acid secretion (e.g., IL-1β- 511 polymorphism) would be also under consideration.

Development of electrodes with high electrocatalytic activity and stability is essential for solving problems that still restrict the extensive application of vanadium redox flow batteries(VRFBs).Here,we designed a novel negative electrode with superior electrocatalytic activity by tailoring nitrogen functional groups,such as newly formed nitro and pyridinic-N transformed to pyridonic-N,from the prenitrogen-doped *** was experimentally confirmed that an electrode with pyridonic-N and nitro fuctional groups(tailored nitrogen-doped graphite felt,TNGF) has superior electrocatalytic acivity with enhanced electron and mass *** functional theory calulations demonstrated the pyridonic-N and nitro functional groups promoted the adsorption,charge transfer,and bond formation with the vanadium species,which is consistent with expermental *** addition,the V2+/V3+redox reaction mechanism on pyridonic-N and nitro functional groups was estabilised based on density functional theory(DFT) *** TNGF was applied to a VRFB,it enabled enhanced-electrolyte utilization and energy efficiencies(EE) of 57.9% and 64.6%,respectively,at a current density of 250 mA cm^(-2).These results are 18.6% and 8.9% higher than those of VRFB with electrode containing graphitic-N and pyridinicN ***,TNGF-based VRFB still operated with an EE of 59% at a high current density of300 mA cm^(-2).The TNGF-based VRFB exhibited stable cycling performance without noticeable decay of EE over 450 charge-discharge cycles at a current density of 250 mA cm^(-2).The results of this study suggest that introducing pyridonic-N and nitro groups on the electrode is effective for improving the electrochemical performance of VRFBs.

CYP2C19 polymorphism has no influence on rabeprazole-based hybrid therapy for Helicobacter pylori eradication

AIM To evaluate the impact of cytochrome P450 2C19(CYP2C19) and interleukin-1β(IL-1β) polymorphisms on the efficacy of Helicobacter pylori(H. pylori) eradication by using rabeprazole-based hybrid *** A total of 88 H. pylori-infected patients were recruited to receive 14-d of hybrid therapy from March 2013 to May 2014. Three patients were excluded from analysis because of incomplete compliance. Either a follow-up endoscopy or 13 C-urea test was performed to determine the results of H. pylori eradication therapy. The genotypes of CYP2C19 and IL-1β were analyzed to investigate the impact on treatment effect. RESULTS The total eradication rate of H. pylori was 92.94%(79/85). According to the CYP2C19 genotypes, the rates of H. pylori eradication were 89.19% in extensive metabolizers(em) and 95.83% in non-em. The *** eradication rates regarding the IL-1β genotypes were 92.59% in the normal acid secretion group and 93.10% in the low acid secretion group. After multivariable logistic regression analysis, both the genotypes of CYP2C19 and IL-1β had no significant influences on the eradication rates of H. *** The CYP2C19 and IL-1β polymorphisms are not significantly independent factors of *** eradication using rabeprazole-based hybrid therapy.

A flexible electromagnetic interference(emI)shielding film was innovatively fabricated using both low melting temperature solder arrays and amorphous soft magnet particulates consolidated on a polyethylene terephthalate(PET)***,the IneSneBi solder arrays presented a low melting point of 99.4C,which enabled attachment to the heat-sensitive plastic substrate without any thermal damage,and a low electrical resistivity of 14.1 mU cm,making them very effective at interrupting em ***,the solder arrays with a high thermal conductivity of 61.2 W/(m$K)at 298.15 K were also useful as a thermal conductor for a heat ***,the solder arrays provide efficient electrical and thermal channels for electron transport induced by abruptly or consistently created em waves due to suddenly turning on and the long term operation of electronics,***,the Fe-based magnetic particulates were added and resulted in effective saturation magnetization of 161.1 emu/g,remanence of 2.9 emu/g,and coercivity of 12.5 G,thus were able to disturb the em pathway because of their soft magnetic ***,the hybrid emI shielding film with exceptionally high electrical and thermal conductivity and superior soft magnetic properties provided remarkable shielding effectiveness of 102.5 dB at 10.2 GHz.

In this research,Chlorellasp and Chodatella *** cultivated in the incubator with constant temperature and light *** algae reaching stable stage were separated from algal water by the ***,adding the suitable concentration of sodium perchlorate into the isolated algae,called algal solution,is in order to keeping all experimental runs having constant ion *** prepared algal solutions were sonicated with the different powers and reaction times using *** filtrate after sonication was filtered by 0.45 μm membrane filter and analyzed by high performance liquid chromatography（HPLC） equipped with evaporized laser scattered detector（ELSD） for molecular weights and polysaccharide *** excitation-emission matrix（EEFM） was measured by fluorescent excitation-emission *** the sonication power of 100 W and reaction time of 7 min,released organic matter from Chlorella *** detected with three sugars including glucose,sucrose and rhamnose;however;both glucose and sucrose were found for the released organic matter from Chodatella *** molecular weight distribution of released organic from algal species was ranged from 1× 102 to 4× 106 *** the observation of the EEFM,aliphatic protein（Ex/em=230/310～230/340 nm） and tryptophan-protein could be found in the filtrate of Chlorella *** both fluorescent intensities were raised with increased power and reaction *** humic-like substance（Ex/em=280/320～280/330 nm） was appeared in 100 W of power and 15 min of reaction time on the Chlorella *** protein andtryptophan-protein were still found in the filtrate ofChodatella sp.,however,fulvic-like substance（Ex/em=220/380nm） was observed in 100 W of power and 7 min of reaction time.

This research focus on developing an approach to innovative design problems based on the TRIZ and extension method. The proposed innovative design procedure combines the Su-Field modeling procedure, contradiction matrix and extension method to improve the efficiency and extent of concept evolutions. The extensibility of matter-element can be used to exchange the descriptions of design problems and solutions into creative fields. In this research, we will introduce the TRIZ contradiction matrix and concept extension of matter-element into symbolic developments to derive out more creative solutions. This study proposes a flexible and extensible innovative design approach with the help of extension method. The bicycle design examples are adopted to explain and verify feasibility of the proposed approach. The case study successfully demonstrates that the proposed approach is feasible and efficient.
