BACKGROUND Primary thumb carpometacarpal(CMC)osteoarthritis is one of the most common conditions encountered by hand *** the vast number of operations that have been proposed,none have demonstrated results significantly superior to trapeziectomy *** The purpose of our study was to determine why surgeons opt for their technique in treating CMC *** A cross-sectional survey of active members of the American Society for Surgery of the Hand was conducted to evaluate the reasons behind their preferred technique in the treatment of isolated thumb CMC *** were contacted by email once and provided with a link to a de-identified survey consisting of 5 treatment questions and 5 demographic *** Of 950 responses were received.40.5%of surgeons preferred trapeziectomy+ligament reconstruction tendon interposition(LRTI),followed by trapeziectomy+suspensionplasty(28.2%),suture button suspension(5.9%),trapeziectomy alone(4.6%),prosthetic arthroplasty(3.2%),arthrodesis(1.1%),and other(6.6%).Proponents of trapeziectomy+LRTI cited familiarity(73.2%),exposure during fellowship(48.8%)and less proximal migration(60%)to be the main reasons affecting their *** who preferred trapeziectomy+suspensionplasty most reported simplicity(74.9%),fewer complications(45.3%),less proximal migration(43.8%),and avoidance of autogenous tissue harvest(42.7%).Advocates of suture button suspension cited avoidance of autogenous tissue harvest(80.4%),shorter immobilization(76.8%),and quicker recovery(73.2%)with their *** of trapeziectomy alone cited simplicity(97.7%),fewer complications(86.4%),and avoidance of autogenous tissue harvest(59.1%).In their comments,45%of surgeons choosing trapeziectomy alone cited evidence as an additional *** of prosthetic arthroplasty cited improved pinch strength(83.3%)and improved range of motion(63.3%),while those preferring arthrodesis cited better pinch strength(90%)and freque

Participants in mixed-species bird flocks(MSFs)have been shown to associate with species that are similar in body size,diet,and evolutionary history,suggesting that facilitation structures these *** addition,several studies have suggested that species in MsFs resemble each other in their plumage,but this question has not been systematically investigated for any MsF *** the nonbreeding season of 2020 and 2021,we sampled 585 MSFs on 14 transects in 2 habitats of Tongbiguang Nature Reserve in western Yunnan Province,*** performed social network analysis and the Multiple Regression Quadratic Assignment Procedure to evaluate the effect of 4 species traits(body size,overall plumage color,distinctive plumage patterns,and diet)and evolutionary history on species association strength at the whole-MSF and within-MSF *** 41 significant relationships showed that species with stronger associations were more similar in their various *** size had the strongest effect on association strength,followed by phylogeny,plumage patterns,and plumage color;diet had the weakest *** results are consistent with the hypotheses that the benefits of associating with phenotypically similar species outweigh the potential costs of interspecific competition,and that trait matching can occur in plumage characteristics,albeit more weakly than in other *** explanations exist as to why similarities in plumage may occur in MSFs,including that they could reduce predators'ability to target phenotypically"odd"*** trait matching in plumage occurs through assortative processes in ecological time or is influenced by co-evolution requires furtherstudy.

The Impacts of Flood and Local Communities’ Coping Strategies along the River Gambia

Flood disasters as Climate change hazards are common in developing countries, particularly in communities along the river Gambia. Local communities, for instance, had their local coping strategies that enabled them to stay in their communities even amid these ordeals, and climate change disaster threats. This work strives to understand flood impacts and the local peoples’ adaptation or coping strategies along the River Gambia basin. A community-based cross-sectional research study of 422 research participants of which 294 are males (69.7%) and females 128 (30.3%), and a focus group discussion of 10 groups which comprised 5 female groups and 5 male groups respectively found that 98.6% of the households experienced floods in their community, and 70.6% experienced flood in their houses, 2.1% have impending flood information and 88.4% do not know evacuation centres. The majority of the households had some local coping strategies, but they acknowledged their insufficient effectiveness. The result also shows that the impact of floods on farmlands, roads, buildings, and livestock was greatly felt. Coping strategies such as sandbags, raised elevations, contour bonds, dikes, and buildings on highlands were all found to be common mechanisms the local people used. The study opines that floods affect communities, but the effects vary depending on individual assets.

Stroke Patients’ Reintegration into Normal Living Post-Discharge from Inpatient Rehabilitation: An Integrative Review

Background: Stroke rehabilitation professionals have historically focused rehabilitation on physical functions and overlooked the concept of community reintegration after discharge from inpatient rehabilitation. The lack of focus on psychosocial functions post-stroke may lead to lower levels of satisfaction during community reintegration. Methods: This integrative review synthesized findings from research literature on stroke patients’ reintegration into the community after inpatient rehabilitation to address three research questions: a) What specific physical and psychosocial functions have been identified as predictors of successful reintegration into normal living after stroke?, b) How do physical and psychosocial functions promote successful reintegration into normal living after stroke?, and c) What factors have been identified that hinder stroke patients’ reintegration into normal living after stroke? Results: A systematic search of literature identified sixteen studies that provided significant context for the research questions. What physical and psychosocial functions of stroke patients included, for example, improved mobility, independence in daily activities, reduced disability, psychological well-being, self-efficacy, social support, and personal relationships. How physical and psychosocial functions promote reintegration included, for example, disability management, emotional well-being, self-care independence, sense of purpose, and employment influence. Factors that hinder stroke patients’ reintegration consisted of longer stride time, impaired balance/mobility, activities limitation, severe stroke, presence of comorbidity, depressive symptoms, speech and language challenges, inadequate self-efficacy, fear of falling, older age, low educational level, lack of social support, and social isolation. Brine shrimp(Artemia)has existed on Earth for 400 million years and has major ecological importance in hypersaline *** a crucial live food in aquaculture,brine shrimp cysts have become one of the most important aquatic products traded ***,our understanding of the biodiversity,prevalence and global connectedness of viruses in brine shrimp is still very limited.A total of 143 batches of brine shrimp(belonging to seven species)cysts were collected from six continents including 21 countries and more than 100 geographic locations worldwide during 1977–*** total,55 novel RNA viruses were identified,which could be assigned to 18 different viral families and related *** viruses were dsRNA viruses,16 were+ssRNA viruses,and 28 were−ssRNA *** analyses of the RNA-directed RNA polymerase(RdRp)showed that brine shrimp viruses were often grouped with viruses isolated from other invertebrates and ***,most brine shrimp viruses were related to those from different hosts that might feed on brine shrimp or share the same ecological niche.A notable case was the novel brine shrimp noda-like virus 3,which shared 79.25%(RdRp)and 63.88%(capsid proteins)amino acid identity with covert mortality nodavirus(CMNV)that may cause losses in *** addition,both virome composition and phylogenetic analyses revealed global connectedness in certain brine shrimp viruses,particularly among Asia and Northern *** highlights the incredible species diversity of viruses in these ancient species and provides essential data for the prevalence of RNA viruses in the global aquaculture *** broadly,these findings provide novel insights into the previously unrecognized RNA virosphere in hypersaline ecosystems worldwide and demonstrate that human activity might have driven the global connectedness of brine shrimp viruses.
