Surgeon preferences in the treatment of thumb carpometacarpal osteoarthritis

BACKGROUND Primary thumb carpometacarpal(CMC)osteoarthritis is one of the most common conditions encountered by hand *** the vast number of operations that have been proposed,none have demonstrated results significantly superior to trapeziectomy *** The purpose of our study was to determine why surgeons opt for their technique in treating CMC *** A cross-sectional survey of active members of the American Society for Surgery of the Hand was conducted to evaluate the reasons behind their preferred technique in the treatment of isolated thumb CMC *** were contacted by email once and provided with a link to a de-identified survey consisting of 5 treatment questions and 5 demographic *** Of 950 responses were received.40.5%of surgeons preferred trapeziectomy+ligament reconstruction tendon interposition(LRTI),followed by trapeziectomy+suspensionplasty(28.2%),suture button suspension(5.9%),trapeziectomy alone(4.6%),prosthetic arthroplasty(3.2%),arthrodesis(1.1%),and other(6.6%).Proponents of trapeziectomy+LRTI cited familiarity(73.2%),exposure during fellowship(48.8%)and less proximal migration(60%)to be the main reasons affecting their *** who preferred trapeziectomy+suspensionplasty most reported simplicity(74.9%),fewer complications(45.3%),less proximal migration(43.8%),and avoidance of autogenous tissue harvest(42.7%).Advocates of suture button suspension cited avoidance of autogenous tissue harvest(80.4%),shorter immobilization(76.8%),and quicker recovery(73.2%)with their *** of trapeziectomy alone cited simplicity(97.7%),fewer complications(86.4%),and avoidance of autogenous tissue harvest(59.1%).In their comments,45%of surgeons choosing trapeziectomy alone cited evidence as an additional *** of prosthetic arthroplasty cited improved pinch strength(83.3%)and improved range of motion(63.3%),while those preferring arthrodesis cited better pinch strength(90%)and freque

High efficiency synthesis of HKUST-1 under mild conditions with high BET surface area and CO2 uptake capacity

This study focuses on the development of a hydrothermal method for the rapid synthesis of good quality copper benzene-1,3,5-tricarboxylate（referred to as HKUST-1）with high yield under mild preparation conditions to address the issues associated with reported *** synthesis conditions and activation methods were studied to understand their influence on the properties of *** was found that mixing the precursors at50°C for 3 h followed by activation via methanol refluxing led to the formation of a product with the highest BET specific surface area of 1615 m2/g and a high yield of 84.1%.The XRD and SEM data illustrated that the product was highly *** sample was also tested on its capacity in *** results showed strong correlation between surface area of the sample and its CO2uptake at 1 bar and 27°*** HKUST-1 prepared in this study demonstrated a high CO2uptake capacity of 4.2 mmol/*** is therefore concluded that this novel and efficient method can be used in the rapid preparation of HKUST-1 with high surface area and CO2uptake capacity.

An advanced ash fusion study on the melting behaviour of coal,oil shale and blends under gasification conditions using picture analysis and graphing method

This study investigates the potential of solid fuel blending as an effective approach to manipulate ash melting behaviour to alleviate ashrelated problems during gasification,thus improving design,operability and *** ash fusion characteristics of Qinghai bituminous coal together with Fushun,Xinghua and Laoheishan oil shales(and their respective blends)were quantified using a novel picture analysis and graphing method,which incorporates conventional ash fusion study,dilatometry and sintering strength test,in a CO/CO_(2)*** imagebased characterisation method was used to monitor and quantify the complete melting behaviour of ash samples from room temperature to 1520℃.The impacts of blending on compositional changes during heating were determined experimentally via Xray diffraction and validated computationally using *** showed that the melting point of Qinghai coal ash to be the lowest at 1116℃,but would increase up to 1208℃,1161℃and 1160℃with the addition of 30%50%of Laoheishan,Fushun,and Xinghua oil shales,*** formation of highmelting anorthite and mullite structures inhibits the formation of lowmelting ***,the sintering point of Qinghai coal ash was seen to decrease from 1005℃to 855℃,834℃,and 819℃in the same blends due to the formation of lowmelting *** also showed that blending directly influences the sintering strength during the various stages of *** key finding from this study is that it is possible to mitigate against the severe ash slagging and fouling issue arising from high calcium and iron coals by cogasification with a high silicaalumina oil ***,blending coals with oil shales can also modify the ash melting behaviour of fuels to create the optimal ash chemistry that meets the design specification of the gasifier,without adversely affecting thermal performance.

DEM analysis of the influence of stirrer design on die filling with forced powder feeding

Die filling is a critical stage during powder compaction,which can significantly affect the product quality and *** this paper,a forced feeder is introduced attempting to improve the filling performance of a lab-scale die filling *** die filling process is analysed with a graphics processing units(GPU)enhanced discrete element method(DEM).Various stirrer designs are assessed for a wide range of process settings(i.e.,stirrer speed,filling speed)to explore their influence on the die filling performance of free-flowing *** results show that die filing with the novel helical-ribbon(i.e.,type D)stirrer design exhibits the highest filling ratio,implying that it is the most robust stirrer design for the feeder configuration ***,die filling performance with the type D stirrer design is a function of the stirrer speed and the filling speed.A positive variation of filling ratio(ηf>0%)can be ensured over the whole range of filling speed by adjusting the stirrer speed(i.e.,increasing the stirrer speed).The approach used in this study can not only help understand how the stirrer design affects the die filling performance but also guide the optimization of feeder system and process settings.

Soil respiration under three different land use types in a tropical mountain region of China

Soil respiration (SR) Wis one of the largest contributors of terrestrial CO_2 to the *** as well as physicochemical parameters influence SR and thus, different land use practices impact the emissions of soil CO_2. In this study, we measured SR, bi-monthly, over a one-year period in a terrace tea plantation, a forest tea plantation and a secondary forest, in a subtropical mountain area in Xishuangbanna, China. Along with the measurement of SR rates, soil characteristics for each of the land use systems were investigated. Soil respiration rates in the different land use systems did not differ significantly during the dry season, ranging from2.7±0.2 μmol m^(-2) s^(-1) to 2.8±0.2 μmol m^(-2) s^(-1). During the wet season, however, SR rates were significantly larger in the terrace tea plantation(5.4±0.5 μmol m^(-2)s^(-1)) and secondary forest(4.9±0.4 μmol m^(-2)s^(-1)) than in the forest tea plantation(3.7±0.2 μmol m^(-2) s^(-1)).This resulted in significantly larger annual soil CO_2 emissions from the terrace tea and secondary forest,than from the forest tea plantation. It is likely that these differences in the SR rates are due to the 0.5times lower soil organic carbon concentrations in thetop mineral soil in the forest tea plantation, compared to the terrace tea plantation and secondary ***, we suggest that the lower sensitivity to temperature variation in the forest tea soil is a result of the lower soil organic carbon concentrations. The higher SR rates in the terrace tea plantation were partly due to weeding events, which caused CO_2 emission peaks that contributed almost 10% to the annual CO_2 flux. Our findings suggest that moving away from heavily managed tea plantations towards low-input forest tea can reduce the soil CO_2 emissions from these systems. However, our study is a casestudy and further investigations and upscaling are necessary to show if these findings hold true at a landscape level.

Flight Characteristics Analysis Based on QAR Data of a jet Transport During Landing at a High-altitude Airport

Flight data of a twin-jet transport aircraft in revenue flight are analyzed for potential safety problems. Data from the quick access recorder （QAR） are first filtered through the kinematic compatibility analysis. The filtered data are then organized into longitudinal- and lateral-directional aerodynamic model data with dynamic ground effect. The dynamic ground effect requires the radio height and sink rate in the models. The model data are then refined into numerical models through a fuzzy logic algorithm without data smoothing in advance. These numerical models describe nonlinear and unsteady aerodynamics and are used in nonlinear flight dynamics simulation. For the jet transport under study, it is found that the effect of crosswind is significant enough to excite the Dutch roll motion. Through a linearized analysis in flight dynamics at every instant of time, the Dutch roll motion is found to be in nonlinear oscillation without clear damping of the amplitude. In the analysis, all stability derivatives vary with time and hence are nonlinear functions of state variables. Since the Dutch roll motion is not damped despite the fact that a full-time yaw damper is engaged, it is concluded that the design data for the yaw damper is not sufficiently realistic and the contribution of time derivative of sideslip angle to damping should be considered. As a result of nonlinear flight simulation, the vertical wind acting on the aircraft is estimated to be mostly updraft which varies along the flight path before touchdown. Varying updraft appears to make the descent rate more difficult to control to result in a higher g-load at touchdown.

Hybrid coronary revascularization vs. percutaneous coronary interventions for multivessel coronary artery disease

OBjECTIVE Hybrid coronary revascularization(HCR)combines a minimally invasive surgical approach to the left anterior descending(LAD)artery with percutaneous coronary intervention(PCI)for non-LAD diseased coronary *** is associated with shorter hospital lengths of stay and recovery times than conventional coronary artery bypass surgery,but there is little information comparing it to isolated PCI for multivessel *** objective is to compare long-term outcomes of HCR and PCI for patients with multivessel *** This cohort study used data from New York’s cardiac surgery and PCI registries in 2010−2016 to examine mortality and repeat revascularization rates for patients with multivessel coronary artery disease who underwent HCR and *** proportional hazards methods were used to reduce selection *** were followed for a median of four *** There was a total of 335 HCR patients(1.2%)and 25,557 PCI patients(98.8%)after *** was no difference in 6-year risk adjusted survival between HCR and PCI patients 83.17%vs.81.65%,adjusted hazard ratio(aHR)=0.90(95%CI:0.67−1.20).However,HCR patients were more likely to be free from repeat revascularization in the LAD artery(91.13%vs.83.59%,aHR=0.51(95%CI:0.34−0.77)).CONCLUSIONS For patients with multi-vessel coronary artery disease,HCR is rarely *** are no differences in mortality rates after four years,but HCR is associated with lower repeat revascularization rates in the LAD artery,presumably due to better longevity in left arterial mammary grafts.

The aging population is an important issue around the world especially in developed *** med-ical advances have substantially extended life span,the same cannot be said for the duration of health *** are seeing in-creasing numbers of elderly people who are frail and/or have multiple chronic conditions;all of these can affect the quality of life of the elderly population as well as increase the burden on the healthcare *** is mechanistically related to common medical conditions such as diabetes mellitus,ischemic heart disease,cognitive decline,and frailty.A recently accepted concept termed‘Accelerated Biological Aging’can be diagnosed when a person’s biological age—as measured by biomarkers of DNA methylation—is older than their corresponding chronological ***,a conditionally essential amino acid,has received much attention in the past few years.A substantial number of animal studies have provided a strong scientific foundation sug-gesting that this amino acid can improve cellular and metabolic health,including blood glucose control,so much that it has been labelled one of the‘longevity amino acids’.In this review article,we propose the rationale that an adequately powered random-ized-controlled-trial(RCT)is needed to confirm whether taurine can meaningfully improve metabolic and microbiome health,and biological *** trial should incorporate certain elements in order to provide the much-needed evidence to guide doctors,and also the community at large,to determine whether this promising and inexpensive amino acid is useful in improving human metabolic health.
