Associating liver partition and portal vein ligation for staged hepatectomy(ALPPS)approach emerged as a promising surgical strategy for rapid and large hypertrophy of the future liver remnant(FLR)when a major liver resection is *** liver metastasis(CRLM)is their main ***,the promising results published so far,are very difficult to interpret since they usually focus on the technique and not on the underlying ***,they are usually made up of complex populations,which received different chemotherapy schemes,with the ALPPS technical variations implemented over time and without consistent long-term follow-up results as ***,its role in CRLM should be analyzed as carefully as possible to indicate and select the best candidates who will benefit the most from this *** conducted a computerized search using PubMed and Google Scholar for reports published so far,using mesh headings and keywords related to the ALPPS and CRLM.

BACKGROUND Hepatocellular adenoma(HCA) is a rare benign liver tumor usually affecting young women with a history of prolonged use of hormonal *** the majority is asymptomatic, a low proportion may have significant complications such as bleeding or malignancy. despite responding to the hormonal stimulus, the desire for pregnancy in patients with small HCA is not contraindicated. However, through this work we demonstrate that intensive hormonal therapies such as those used in the treatment of infertility can trigger serious complications CASE SUMMARY A 33-year-old female with a 10-year history of oral contraceptive use was diagnosed with a hepatic tumor as an incidental finding in an abdominal ultrasound. The patient showed no symptoms and physical examination was unremarkable. Laboratory functional tests were within normal limits and tests for serum tumor markers were negative. An abdominal magnetic resonance imaging(MRI) was performed, showing a 30 mm × 29 mm focal lesion in segment VI of the liver compatible with HCA or Focal Nodular Hyperplasia with atypical behavior. After a total of six years of follow-up, the patient underwent ovulation induction treatment for infertility. On a following MRI, a suspected malignancy was warned and hence, surgery was decided. The surgical specimen revealed malignant transformation of HCA towards trabecular hepatocarcinoma with dedifferentiated areas. There was non-evidence of tumor recurrence after three years of clinical and imaging follow-up.,CONCLUSION HCAs can be malignant regardless its size and low-risk appearance on MRI when an ovultation induction therapy is indicated.

The Antarctic Paleogene marine fossil record has been the key to reconstructing the evolution of the Weddellian Sea and final dismemberment of Southern *** this context,Eocene marine vertebrates from Seymour(Marambio)Island have provided valuable *** present the first Eocene record of marine reptiles from the southern Atlantic Coast of South *** corresponds to several postcranial turtle remains represented by a proximal end of the right humerus,three caudal and one thoracic vertebrae,a fragment of the left pubis,and ten ossicles of the dorsal carapace,coming from the Leticia Formation(late-mid Eocene)at Cabo Tiburones,Tierra del Fuego,*** materials show severalfeatures such as the size and general morphology of the humerus and vertebrae,and the presence of relatively small,irregular,smooth,and unkeeled ossicles,which allow us to assign them to dermochelyidae *** are a cosmopolitan group ofcryptodiran turtles,registered from the late Cretaceous up to the recent,with some physiological-biological peculiarities(e.g.,endothermy and an exclusive jellyfish-based diet)and characterized by the presence of an osseous carapace formed by *** new finding from the Leticia Formation is an addition to the scarce and extremely fragmentary record of Eocene dermochelyids from the southern seas like those from the La Meseta and Submeseta formations(Antarctica)and the Waihao and Burnside formations(New Zealand).This new information allows us to discuss the presence of these turtles in such high latitudes in the past and its implication in the evolution of the Weddellian fauna.

The Ria de Aveiro lagoon has been considered one of the most important Southern European long-term monitoring sites(LTER-site)concerning climate change and anthropogenic ***,studies concerning pesticide contamination herein are few and mostly ***,56 pesticides dissolved in the aqueous phase(DAP)and suspended particulate matter(SPM)were examined to fill the knowledge gap,considering the hydrophobic nature of most evaluated *** samples were taken seasonally from eight sites,and both DAP and SPM fractions were analysed by gas-chromatography-mass *** annual average concentrations of fungicides,herbicides and insecticides were≈57 ng/L,≈409 ng/L and≈1058 ng/L in DAP and≈7μg/g,≈28μg/g and≈67μg/g in *** pesticides and the excessive phosphate concentrations suggest that the lagoon's water quality may be hazardous to biota and *** were identified as the most threatened trophic level using deterministic risk assessment,with chlorpyriphos and chlorfenvinphos being the primary *** experiments with Artemia and Daphnia supported the suppositions.

During the rainy season in Brazil,landslides can have catastrophic consequences,including loss of life,damage to urban infrastructure and significant economic ***,more than eight million people in Brazil live in disaster-prone *** objective of this article is to present a chronological review of the principal policies and programmes at the national,state,and municipal levels,with an emphasis on their key actions aimed at mitigating the impact of disasters related to natural hazards,particularly *** analysis was based on official records of public policies from the national,state,and municipal *** results show that several initiatives were initially developed at the local and regional ***,it was not until the late 1980s that a comprehensive organisation of civil protection at the national level emerged as a response *** is possible to highlight three phases(understanding,coexistence,and prevention)of institutional action focused on risk management in the face of *** the growing knowledge of risk scenarios,disasters continue to occur in Brazil on an annual basis,revealing several obstacles to reducing their impact,particularly given the social and economic disparities between Brazilian regions and the applicability of public policies that must overcome the limitations of each municipality.

Tropical regions have provided new insights into how ecological communities are *** dry coastal communities,water stress has been hypothesized to determine plant assembly structure by favoring preadapted lineages from neighboring ecosystems,consistent with functional ***,it is unclear whether this hypothesis is sufficient to explain how coastal communities in tropical ecosystems are ***,we test whether water stress or other factors drive community assembly in woody plant communities across the coastal zone of Brazil,a tropical *** characterized functional and phylogenetic structures of these communities and determined the underlying environmental factors(e.g.,water stress,historical climate stability,edaphic constraints,and habitat heterogeneity)that drive their community *** of coastal woody species show geographically varied patterns,including stochastic arrangements,clustering,and overdispersion of species relative to their traits and phylogenetic *** complexity,water vapor pressure,and soil nutrient availability best explained the gradient in the functional *** deficit,water vapor pressure,and soil organic carbon were the best predictors of variation in phylogenetic *** results support the water-stress conservatism hypothesis on functional and phylogenetic structure,as well as the effect of habitat heterogeneity on functional structure and edaphic constraints on functional and phylogenetic *** effects are associated with increased phenotypic and phylogenetic divergence of woody plant assemblages,which is likely mediated by abiotic filtering and niche opportunities,suggesting a complex pattern of ecological assembly.

In this paper we examine several definitions of vaccine efficacy(VE)that we found in the literature,for diseases that express themselves in outbreaks,that is,when the force of infection grows in time,reaches a maximum and then *** fact that the disease occurs in outbreaks results in several problems that we *** propose a mathematical model that allows the calculation of VE for several *** trials usually needs a large number of volunteers that must be ***,all volunteers should be enrolled in approximately the same time,but this is generally impossible for logistic reasons and they are enrolled in a fashion that can be replaced by a continuous density function(for example,a Gaussian function).The outbreak can also be replaced by a continuous density function,and the use of these density functions simplifies the ***,for example Gaussian functions,one of the problems one can immediately notice is that the peak of the two curves do not occur at the same *** model allows us to conclude:First,the calculated vaccine efficacy decreases when the force of infection increases;Second,the calculated vaccine efficacy decreases when the gap between the peak in the force of infection and the peak in the enrollment rate increases;Third,different trial protocols can be simulated with this model;different vaccine efficacy definitions can be calculated and in our simulations,all result are approximately the *** final,and perhaps most important conclusion of our model,is that vaccine efficacy calculated during outbreaks must be carefully examined and the best way we can suggest to overcome this problem is to stratify the enrolled volunteer's in a cohort-by-cohort basis and do the survival analysis for each cohort,or apply the Cox proportional hazards model for each cohort.
