Combination of C-reactive protein and fibrinogen is useful for diagnosing periprosthetic joint infection in patients with inflammatory diseases

Background:The screening of periprosthetic joint infection(PJI)in patients with inflammatory diseases before revision arthroplasty remains uncertain.Serum C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),plasma fbrinogen(FIB),monocyte/lymphocyte ratio,and neutrophil/lymphocyte ratio(NLR)can help screening PJI,but their values in patients with infammatory diseases have not been determined.Methods:Patients with inflammatory diseases who underwent revision hip or knee arthroplasty at West China Hospital,Sichuan University,from January 2008 to September 2020 were divided into infected and non-infected groups based on the 2013 International Consensus Meeting criteria.Sensitivity and specificity of the tested biomarkers for diagnosing infection were determined based on receiver operating characteristic(ROC)curves,and optimal cutoffs were determined based on the Y ouden index.The diagnostic ability of these biomarkers was re-assessed after combining them with each other.Results:A total of 62 patients with inflammatory diseases were studied;of them 30 were infected.The area under the ROC curve was 0.813 for CRP,0.638 for ESR,0.795 for FIB,and 0.656 for NLR.The optimal predictive cutoff of CRP was 14.04 mg/L with a sensitivity of 86.2%and a specificity of 68.7%,while FIB had a sensitivity of 72.4%and a specificity of 81.2%with the optimal predictive cutoff of 4.04 g/L.The combinations of CRP with FIB produced a sensitivity of 86.2%and specificity of 78.1%.Conclusion:CRP with a slightly higher predictive cutoff and FIB are useful for screening PJI in patients with inflammatory diseases,and the combination of CRP and FIB may further improve the diagnostic values.

To the Editor:Malnutrition is associated with an increased risk of complications after total knee arthroplasty(TKA),including persistent wound drainage,delay of wound healing,superficial surgical site and even deep periprosthetic joint infections,and intensive care unit admission.[1]Albumin(ALB)is the most abundant protein in human plasma and is considered a reliable and sensitive biomarker of nutritional status and malnutrition.Like malnutrition,low serum ALB levels are not only associated with various post-operative complications following TKA including pulmonary infection,renal impairment,superficial and deep infections,unplanned intubation,and even mortality;but also associated with increased treatment costs,longer length of stay and higher risk of readmission.[2]

Tumor Microenvironment-Adaptive Nanoplatform Synergistically Enhances Cascaded Chemodynamic Therapy

Chemodynamic therapy(CDT),a noninvasive strategy,has emerged as a promising alternative to conventional chemotherapy for treating tumors.However,its therapeutic effect is limited by the amount of H_(2)O_(2),pH value,the hypoxic environment of tumors,and it has suboptimal tumor-targeting ability.In this study,tumor cell membrane-camouflaged mesoporous Fe_(3)O_(4) nanoparticles loaded with perfluoropentane(PFP)and glucose oxidase(GOx)are used as a tumor microenvironment-adaptive nanoplatform(M-mFeP@O_(2)-G),which synergistically enhances the antitumor effect of CDT.Mesoporous Fe_(3)O_(4) nanoparticles are selected as inducers for photothermal and Fenton reactions and as nanocarriers.GOx depletes glucose within tumor cells for starving the cells,while producing H2O2 for subsequent⋅OH generation.Moreover,PFP,which can carry O_(2),relieves hypoxia in tumor cells and provides O_(2) for the cascade reaction.Finally,the nanoparticles are camouflaged with osteosarcoma cell membranes,endowing the nanoparticles with homologous targeting and immune escape abilities.Both in vivo and in vitro evaluations reveal high synergistic therapeutic efficacy of M-mFeP@O_(2)-G,with a desirable tumor-inhibition rate(90.50%),which indicates the great potential of this platform for clinical treating cancer.

To the editor:A 26-month-old girl presented with atrial tachycardia as the main cardiac manifestation,which lasted for more than 1 year.The girl was born to healthy,non-consanguineous parents and had no family history of similar diseases.There was no asphyxia at birth and no abnormal feeding history.Her development was normal in the early stages.However,at the age of 17 months she did not respond to her name and hardly communicated with her parents.She was also unable to grasp things and repeatedly chewed on her hands.In the meantime,she frequently started grinding her teeth when awake.

Oxazaphosphorine bioactivation and detoxification: the role of xenobiotic receptors

Oxazaphosphorines,with the most representative members including cyclophosphamide,ifosfamide,and trofosfamide,constitute a class of alkylating agents that have a broad spectrum of anticancer activity against many malignant ailments including both solid tumors such as breast cancer and hematological malignancies such as leukemia and lymphoma.Most oxazaphosphorines are prodrugs that require hepatic cytochrome P450 enzymes to generate active alkylating moieties before manifesting their chemotherapeutic effects.Meanwhile,oxazaphosphorines can also be transformed into non-therapeutic byproducts by various drug-metabolizing enzymes.Clinically,oxazaphosphorines are often administered in combination with other chemotherapeutics in adjuvant treatments.As such,the therapeutic efficacy,off-target toxicity,and unintentional drug–drug interactions of oxazaphosphorines have been long-lasting clinical concerns and heightened focuses of scientific literatures.Recent evidence suggests that xenobiotic receptors may play important roles in regulating the metabolism and clearance of oxazaphosphorines.Drugs as modulators of xenobiotic receptors can affect the therapeutic efficacy,cytotoxicity,and pharmacokinetics of coadministered oxazaphosphorines,providing a new molecular mechanism of drug–drug interactions.Here,we review current advances regarding the influence of xenobiotic receptors,particularly,the constitutive androstane receptor,the pregnane X receptor and the aryl hydrocarbon receptor,on the bioactivation and detoxification of oxazaphosphorines,with a focus on cyclophosphamide and ifosfamide.

Role of circulating long non-coding RNA for the improvement of the predictive ability of the CHA_(2)DS_(2)-VASc score in patients with atrial fibrillation

Background:The CHA_(2)DS_(2)-VASc score was initially applied to stratify stroke risk in patients with atrial fibrillation(AF)and was found to be effective in predicting all-cause mortality outcomes.To date,it is still unclear whether circulating long non-coding RNAs(lncRNAs)as emerging biomarkers,can improve the predictive power of the CHA_(2)DS_(2)-VASc score in stroke and all-cause mortality.Methods:Candidate lncRNAs were screened by searching the literature and analyzing previous RNA sequencing results.After preliminary verification in 29 patients with AF,the final selected lncRNAs were evaluated by Cox proportional hazards regression in 192 patients to determine whether their relative expression levels were associated with stroke and all-cause mortality.The c-statistic,net reclassification improvement(NRI),and integrated discrimination improvement of the patients were calculated to evaluate the discrimination and reclassification power for stroke and all-cause mortality when adding lncRNA expression levels to the CHA_(2)DS_(2)-VASc score model.Results:Five plasma lncRNAs associated with stroke and all-cause mortality in AF patients were selected in our screening process.Patients with elevated H19 levels were found to have a higher risk of stroke(hazard ratio[HR]3.264,95% confidence interval[CI]:1.364–7.813,P=0.008).Adding the H19 expression level to the CHA_(2)DS_(2)-VASc score significantly improved the discrimination and reclassification power of the CHA_(2)DS_(2)-VASc score for stroke in AF patients.In addition,the H19 level showed a marginally significant association with all-cause mortality(HR 2.263,95%CI:0.889–5.760,P=0.087),although it appeared to have no significant improvement for the CHA_(2)DS_(2)-VASc model for predicting all-cause mortality.Conclusions:Plasma expression of H19 was associated with stroke risk in AF patients and improved the discriminatory power of the CHA_(2)DS_(2)-VASc score.Therefore,lncRNA H19 served as an emerging non-invasive biomarker for stroke risk prediction in patients w

An Edge-Based Anisotropic Mesh Refinement Algorithm and its Application to Interface Problems

Based on an error estimate in terms of element edge vectors on arbitrary unstructured simplex meshes,we propose a new edge-based anisotropic mesh refinement algorithm.As the mesh adaptation indicator,the error estimate involves only the gradient of error rather than higher order derivatives.The preferred refinement edge is chosen to reduce the maximal term in the error estimate.The algorithm is implemented in both two-and three-dimensional cases,and applied to the singular function interpolation and the elliptic interface problem.The numerical results demonstrate that the convergence order obtained by using the proposed anisotropic mesh refinement algorithm can be higher than that given by the isotropic one.

Well-dispersed NiCoS_(2) nanoparticles/rGO composite with a large specific surface area as an oxygen evolution reaction electrocatalyst

Developing efficient oxygen evolution reaction(OER) electrocatalysts such as transition metal sulfides(TMSs) is of great importance to advance renewable hydrogen fuel toward further practical applications.Herein,NiCoS_(2) nanoparticles well decorated on double-sided N-doped reduced graphene oxide sheets(NiCoS_(2)/rGO) are prepared from an Al-containing ternary NiCoAl-layered double hydroxide precursor(NiCoAl-LDH) grown on GO support as an OER electrocatalyst.The Al-confinementassisted sulfurization,followed by selective acid treatment,endows the resulting NiCoS_(2)/rGO composite with the advantages:well-dispersed NiCoS_(2) nanoparticles,dualsided rGO support,as well as a large specific surface area of 119.4 m^(2)·g^(-1) and meso-/macroporous size distribution.The NiCoS_(2)/rGO electrocatalyst exhibits an overpotential of 273 mV at 10 mA·cm^(-2) and a good stability of 24 h,which outperform those of the counterparts of NiS_(2)/rGO and CoS_(2)/rGO.The results of electrochemical active surface area and electrochemical impedance spectra experimentally provide convincing rationales of the information of active sites and good conductivity,both underpin the enhanced electrocatalytic performances.
