Robotic-assisted retroperitoneal lymph node dissection for stage II testicular cancer

Objective:To evaluate the perioperative as well as early oncological outcomes of patients undergoing robotic retroperitoneal lymph node dissection for treatment of testicular ***:We conducted a prospective consecutive case series of patients undergoing robotic assisted retroperitoneal lymph node dissection for metastatic testicular cancer between May 2018 and July 2021 at our *** were collected on patient and tumour characteristics,intraoperative and postoperative parameters,and functional and oncological *** statistics are ***:Nineteen patients were identified;18(94.7%)completed the procedure robotically and one was converted to open surgery;78.9%of patients had stage≥IIB and 12(63.2%)patients had undergone prior *** median operative time was 300(interquartile range[IQR]240-315)*** blood loss was 100(IQR 50-175)*** length of stay was 2(range 1-11)*** robotically completed patients commenced diet and passed flatus on Day 1 and were discharged by Day *** median lymph node yield was 40.5(IQR 38-51)*** patients undergoing nerve-sparing procedures recovered antegrade ejaculatory *** patient had a Clavien-Dindo III complication(chylous ascites requiring drainage).At a median follow-up of 22.3(IQR 16.3-24.9)months,one patient developed retroperitoneal recurrence,which was successfully treated with second-line chemotherapy;no other patients have had ***:Robotic retroperitoneal lymph node dissection is a safe and feasible alternative to open surgery in appropriately selected patients,offering low *** oncological outcomes are *** cohorts and longer follow-ups are required to validate our institution's findings.

Contributions of biofilm and suspended sludge to nitrogen transformation and nitrous oxide emission in hybrid sequencing batch system

Hybrid system combines the nature of suspended growth and attached growth has been widely applied to wastewater treatment. In this research, the contributions to N transformation and N2O emission by biofilm and suspended sludge in the hybrid sequencing- batch reactor for a simultaneous nitrification, denitrification and phosphorus removal process were investigated. For the hybrid system, nitrification occurred mostly in the suspended sludge, while the biofilm played the major role in denitrification. The interaction of the biofilm and the suspended sludge in the same reactor resulted in a better overall nitrogen removal performance with simultaneous nitrification and denitrification. However, N2O emission was the main end product of nitrogen removal for the hybrid system; while it was N2 for the biofilm. The relative low N2O emissions from the pure biofilm and the pure suspended sludge corresponded to the relatively high nitrate at the end of the aeration period compared with the hybrid system.

Quasi-static, Poiseuille Flow of Analgesics from an Elastomeric Pump: Theoretical Determination of Infusion Times and Toxicity Conditions

Background: Elastomeric pumps (elastic balls into which analgesics or antibiotics can be inserted) push medicines through a catheter to a nerve or blood vessel. since elastomeric pumps are small and need no power source, they fit easily into a pocket during infusion, allowing patient mobility. Elastomeric pumps are widely used and widely studied experimentally, but they have well-known problems, such as maintaining reliable flow rates and avoiding toxicity or other peak-and-trough effects. Objectives: Our research objective is to develop a realistic theoretical model of an elastomeric pump, analyze its flow rates, determine its toxicity conditions, and otherwise improve its operation. We believe this is the first such theoretical model of an elastomeric pump consisting of an elastic, medicine-filled ball attached to a horizontal catheter. Method: Our method is to model the system as a quasi-Poiseuille flow driven by the pressure drop generated by the elastic sphere. We construct an engineering model of the pressure exerted by an elastic sphere and match it to a solution of the one-dimensional radial Navier-stokes equation that describes flow through a horizontal, cylindrical tube. Results: Our results are that the model accurately reproduces flow rates obtained in clinical studies. We also discover that the flow rate has an unavoidable maximum, which we call the “toxicity bump”, when the radius of the sphere approaches its terminal, unstretched value—an effect that has been observed experimentally. Conclusions: We conclude that by choosing the properties of an elastomeric pump, the toxicity bump can be restricted to less than 10% of the earlier, relatively constant flow rate. Our model also produces a relation between the length of time that the analgesic fluid infuses and the physical properties of the fluid, of the elastomeric sphere and the tube, and of the blood vessel into which the analgesic infuses. We report on the novel heterometallic quaternary selenide EuCeCuse3,the fabrication of which has been a challenge until this *** structure of the reported selenide was elucidated from the powder X-ray diffraction data,which revealed the formation of EuCeCuse3with excellent yield(96.7%)accompanied with a minor fraction of Cese2(3.3%),and was best solved in orthorhombic space group Pnma with the BaLaCus3structural ***,the crystal structure of the title compound completes the row of the heterometallic quaternary selenides EuRECuse3(RE=La,Pr,Nd,sm,Eu,Gd,Tb,Dy,Ho,Er,Tm,Yb,Lu,Y),of which the cerium-based derivative exclusively belongs to the BaLaCus3structural *** distortion of the Cuse4polyhedron was compared for the whole series of EuRECuse3compounds using theτ4-descriptor for four coordinated ions,which revealed the highest degree of distortion for the Ce3+-containing selenide,followed by the La3+-based ***,the crystallographic and geometrical parameters of the reported selenide were discussed in comparison to the Ce3+-based sulfides srCeCus3and *** initio calculations of the crystal structure,a phonon spectrum and elastic constants for the crystal of EuCeCuse3were also *** types and wavenumbers of fundame ntal modes were determined and the involvement of ions participating in the phonon modes was *** experimental IR spectrum of the reported selenide was interpreted and found to be in agreement with the calculated *** experimental direct band gap of EuCeCuse3was measured to be 1.36 eV that is consistent with the concept of its origin due to interband transitions between orbitals emerging mainly from 4f(valence band)and 5d(conduction band)levels of the Eu2+*** dependence of the Young’s modulus on the direction demonstrates the anisotropy of the elastic properties,while the Vickers hardness for EuCeCuse3was calculated to be 5.2 ***,the title compound is paramagnetic above 4 K.

One formulation of D. Voiculescu's theorem on approximate unitary equivalence is that two unital representations π and ρ of a separable C*-algebra are approximately unitarily equivalent if and only if rank οπ = rank ορ. We study the analog when the ranges of π and ρ are contained in a von Neumann algebra R, the unitaries inducing the approximate equivalence must come from R, and 'rank' is replaced with 'R -rank' (defined as the Murray-von Neumann equivalence of the range projection).

CFD investigation of dense phase pneumatic conveying at a pipeline enlargement

The practice of stepping a conveying line is not new and there are many examples in industry; however, the details of the flow behaviour at the step have not been extensively studied. Three step geometries were modelled： an ＇abrupt＇ step and two ＇gradual＇ step-up geometries from a pipe bore of 75-100 ram. The flow behaviour of a single plug of material passing through these geometries was investigated using the ＇Eulerian＇ CFD model from Fluent. solids volume fractions were recorded at 0.01 s time steps at several point locations, and visualised throughout the pipe.
