A Unique Modelling Strategy to Dynamically Simulate the Performance of a Lobe Pump for Industrial Applications

The performance of a newly designed tri-lobe industrial lobe pump of high capacity is simulated by using commercial CFD solver Ansys Fluent. A combination of user-defined-functions and meshing strategies is employed to capture the rotation of the lobes. The numerical model is validated by comparing the simulated results with the literature values. The processes of suction, displacement, compression and exhaust are accurately captured in the transient simulation. The fluid pressure value remains in the range of inlet pressure value till the processes of suction and displacement are over. The instantaneous process of compression is accurately captured in the simulation. The movement of a particular working chamber is traced along the gradual degree of lobe’s rotation. At five different degrees of lobe’s rotation, pressure contour plots are reported which clearly shows the pressure values inside the working chamber. Each pressure value inside the working chamber conforms to the particular process in which the working chamber is operating. Finally, the power requirement at the shaft of rotation is estimated from the simulated values. The estimated value of power requirement is 3.61 BHP FHP whereas the same calculated theoretically is 3 BHP FHP. The discrepancy is attributed to the assumption of symmetry of blower along the thickness.

In this editorial,we comment on the article by Chen et *** specifically focus on the risk factors,prognostic factors,and management of brain metastasis(BM)in breast cancer(BC).BC is the second most common cancer to have BM after lung *** risk factors for BM in BC are:HER-2 positive BC,triplenegative BC,and germline BRCA *** factors associated with BM are lung metastasis,age less than 40 years,and African and American *** though risk factors associated with BM in BC are elucidated,there is a lack of data on predictive models for BM in *** studies have been made to formulate predictive models or nomograms to address this issue,where age,grade of tumor,HER-2 receptor status,and number of metastatic sites(1 vs>1)were predictive of BM in metastatic ***,none have been used in clinical *** Comprehensive Cancer Network recommends screening of BM in advanced BC only when the patient is symptomatic or suspicious of central nervous system symptoms;routine screening for BM in BC is not recommended in the *** decreases the quality of life and will have a significant psychological *** studies are required for designing validated nomograms or predictive models for BM in BC;these models can be used in the future to develop treatment approaches to prevent BM,which improves the quality of life and overall survival.

Readmission rate and early complications in patients undergoing total knee arthroplasty:A retrospective study

BACKgROUND Total knee arthroplasty(TKA)can improve pain,quality of life,and functional *** uncommon,postoperative complications are extremely consequential and thus must be carefully tracked and communicated to patients to assist their decision-making before *** of the risk factors for complications and readmissions after TKA,taking into account common causes,temporal trends,and risk variables that can be changed or left unmodified,will benefit this *** To assess readmission rates,early complications and their causes after TKA at 30 days and 90 days *** This was a prospective and retrospective study of 633 patients who underwent TKA at our hospital between January 1,2017,and February 28,*** the 633 patients,28 were not contactable,leaving 609 who met the inclusion *** inpatient and outpatient hospital records were retrieved,and observations were noted in the data collection *** Following TKA,the 30-day and 90-day readmission rates were determined to be 1.1%(n=7)and 1.8%(n=11),*** unplanned visit rate at 30 days following TKA was 2.6%(n=16)and at 90 days was 4.6%(n=28).At 90 days,the unplanned readmission rate was 1.4%(n=9).Reasons for readmissions included medical(27.2%,n=3)and surgical(72.7%,n=8).Unplanned readmissions and visits within 90 days of follow-up did not substantially differ by age group(P=0.922),body mass index(BMI)(P=0.633),unilateral vs bilateral TKA(P=0.696),or patient comorbidity status(30-day P=0.171 and 90-day P=0.813).Reoperation rates after TKA were 0.66%(n=4)at 30 days and 1.15%(n=8)at 90 *** average length of stay was 6.53 *** In this study,there was a low readmission rate following *** was no significant correlation between readmission rate and patient factors such as age,BMI,and co-morbidity status.

Meigs syndrome(MS)is characterized by a benign ovarian tumor(fibroma),hydrothorax on the right side,and ascites;which can be resolved permanently after *** literature reveals that most MS surgeries were performed under general anesthesia(gA)[1].However,gA poses major risks to the *** the high risk of gastric content regurgitation,poor general condition or dyselectrolytemia may lead to delayed arousal,and associated multi-organ *** mechanical ventilation is also difficult due to ascites and hydrothorax(reduced cardiac output,impaired ventilation-perfusion in lungs causing hypoxia and hypercapnia)[1,2].In this letter,we report a case of MS tumor resection under the subarachnoid block(SAB)to mitigate these issues and also review the complications associated with both techniques.

One-Pot green Synthesis of 1, 4-Dihydropyridine Derivatives Using Polyindole TiO2 Nanocatalyst by Solvent Free Method

This study used a Polyindole in combination with TiO2 nanocatalyst as an efficient heterogeneous catalyst to carry out a multi-component Hantzsch reaction involving different aromatic aldehydes with methyl acetoacetate, and aqueous ammonium to create 1,4-dihydropyridine derivatives under solvent free condition at ambient temperature. A broad range of aldehydes and methyl acetoacetates, ranging from heteroaromatic to polyaromatic one, with high level of functional group tolerance can be used to provide the desired products possessing relevant medicinal moiety in high yields. This technology has prospective advantages over current protocols, including the utilization of a cheap, stable, recyclable, and safe catalyst, quicker reaction times with higher yields and simple product isolation.

Background Ni-kshay Poshan Yojana (NPY) is a direct benefit transfer scheme of the government of India introduced in 2018 to support the additional nutritional requirements of persons with TB (PwTB). Our recent nationwide evaluation of implementation and utilization of NPY using programmatic data of PwTB from nine randomly selected Indian states, reported a 70% coverage and high median delay in benefit credit. We undertook a qualitative study between January and July 2023, to understand the detailed implementation process of NPY and explore the enablers and barriers to effective implementation and utilization of the NPY *** We followed a grounded theory approach to inductively develop theoretical explanations for social phenomena through data generated from multiple sources. We conducted 36 in-depth interviews of national, district and field-level staff of the National Tuberculosis Elimination Programme (NTEP) and NPY beneficiaries from 30 districts across nine states of India, selected using theoretical sampling. An analytical framework developed through inductive coding of a set of six interviews, guided the coding of the subsequent interviews. Categories and themes emerged through constant comparison and the data collection continued until theoretical *** Stakeholders perceived NPY as a beneficial initiative. Strong political commitment from the state administration, mainstreaming of NTEP work with the district public healthcare delivery system, availability of good geographic and internet connectivity and state-specific grievance redressal mechanisms and innovations were identified as enablers of implementation. A Study to Observe Pulse Pressure Variation after Induction with Propofol for general Anesthesia

Background and Aims: Pulse pressure variation (PPV) is a reliable and predictive dynamic parameter presently being utilized for fluid responsiveness. In the operating room, fluid administration based on PPV monitoring helps the physician in deciding whether to volume resuscitate or use interventions in patients undergoing surgery. Propofol is an intravenous induction agent which lowers blood pressure. There are multiple causes such as depression in cardiac output, and peripheral vasodilatation for hypotension. We undertook this study to observe the utility of PPV as a guide to fluid therapy after propofol induction. Primary outcome of our study was to monitor PPV as a marker of fluid responsiveness for the hypotension caused by propofol induction. Secondary outcome included the correlation of PPV with other hemodynamic parameters like heart rate (HR), systolic blood pressure (SBP), and diastolic blood pressure (DBP);after induction with propofol at regular interval of time. Methods: A total number of 90 patients were recruited. Either of the radial artery was then cannulated under local anaesthesia with 20g VygonLeadercath arterial cannula and invasive monitoring transduced. A baseline recording of heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP) and PPV was then recorded. Patients were then induced with predetermined doses of propofol (2 mg/kg) and recordings of HR, SBP, DBP, and PPV were taken at 5, 10 and 15 minutes. Results: Intraoperatively, PPV was significantly higher at 5 minutes and significantly lower at 15 minutes after induction. It was observed that there were no statistically significant correlations between PPV and SBP or DBP. PPV was strongly and directly associated with HR. Conclusion: We were able to establish that PPV predicts fluid responsiveness in hypotension caused by propofol induction;and can be used to administer fluid therapy in managing such hypotension.
