Neurodegenerative disorders such as Alzheimer's and Parkinson's diseases a re increasing in prevalence as world populations age.While tremendous progress has been made,our understanding of the mechanisms that underlie the development of these diseases remains far from com plete.More troubling,despite the growing emotional and financial toll being to ken by neurodegenerative disorders,existing treatment options are limited almost exclusively to those that help manage symptoms but that lack the ability to alter the progression of the disease(Liu et al.,2022).

Introduction: There are currently no national guidelines specifically for the management of NSCLC ALK-positive lung cancer in the UK. National guidelines exist for lung cancer in general and reference is made to ALK-positive lung cancer. However, the generic nature of these guidelines is not sufficient for the optimal management of this specific type of lung cancer, with its unique disease management requirements. Method: In 2021, members of the charity ALK POSITIVE LUNG CANCER (UK) completed anonymised surveys to gather real-world data about their treatment. Results: Data were gathered on the prescribing of blood thinners (n = 127);frequency of brain MRI scans (n = 123);prescribing bone strengthening medication (n = 127);frequency of CT scans (n = 123);mental health (n = 132). There was significant variation in each of these five aspects of treatment. Conclusions: The findings, from a representative population of ALK-positive patients, provide new information on the significant disparities in the treatment that patients receive. This health inequality is likely to have a detrimental effect on the survival outcomes for some of these patients. It would improve patient outcomes and overall survival if ALK-positive lung cancer disease management national guidelines were established and utilised across the NHS in the UK. Safe acupuncture and dry needling during pregnancy:New Zealand physiotherapists' opinion and practice

Objective: Acupuncture guidelines have advised caution when treating women during pregnancy,because historical ‘‘forbidden" acupuncture points are believed to stimulate miscarriage or early labor.Despite recent research demonstrating that acupuncture is a useful and safe treatment tool for pregnancy-related low-back pain(LBP) and pelvic girdle pain(PGP), it is postulated that fear of miscarriage and subsequent blame by association, restricts its provision. More recently, an increase in dry needling(DN) courses for physiotherapists has potentiated the rapid growth in DN practice in New Zealand(NZ). Many dry needlers do not consider DN to be a form of acupuncture; it is unknown if they have similar safety concerns.Methods: NZ registered physiotherapists practicing acupuncture and/or DN were invited to participate in an electronic survey to examine their practice and level of understanding in regard to safe needling during pregnancy.Results: Of 124 respondents, only 60(48%) would needle pregnant women, with a further 66% of those still expressing safety concerns. NZ physiotherapists practicing DN only, were more likely to needle areas related to ‘‘forbidden" points in all trimesters. However, overall, NZ physiotherapists were less likely to needle ‘‘forbidden" points than their UK peers.Conclusion: Conflicting literature and a ‘‘fear of blame" influences NZ physiotherapists' decisions to offer needling(both acupuncture and DN) during pregnancy. Further training in this field is recommended to ensure safe practice and adequate provision of acupuncture treatment options for pregnant women suffering musculoskeletal pain, such as LBP and PGP. Further research, particularly into DN, for women during pregnancy, is warranted.

Gene bank scheduling of seed regeneration: Interim report on a long term storage study

A major challenge for the management of gene banks is the maintenance of good seed health in the collections.Large germplasm collections >10 000 accessions often have been acquired from different sources over a range of dates,may differ in germination at time of deposition in the gene bank,and may have genetic differences in seed longevity.The major storage variables affecting seed longevity are temperature and seed moisture content.Two varieties of each pea(Pisum sativus L.),lentil(Lens culinaris Medikus subsp.culinaris),and chickpea(Cicer arietinum L.),were stored at three temperatures;40,20,and 2°C,each with three seed moisture levels of 10.9–13.8%(high),7.9–10.3%(medium),and 7–7.8%(low),in the Australian Temperate Field Crops Collection gene bank.Seed longevity at a given storage period was estimated by the corresponding germination percentage for each treatment.This paper is an interim report on seed viability decline in the first seven years of this seed longevity study,in which viability decline towards zero was almost completed in the three seed moisture treatments at 40°C and the 20°C high seed moisture treatment,but had not declined in the other treatments.Seed longevity positively responded to a reduction in temperature and then to a reduction in seed moisture.The number of days in storage for seed germination decline to 85%(p85),and to 50%(p50) for mean seed viability,are reported by storage/varietal treatment.Both p85 and p50 showed significant inverse linear responses with seed moisture at 40°C for pea and lentil varieties,with intra-specific variation for pea.This long term trial aims to provide informed timing of seed regeneration for accessions in a gene bank.

Pathologic hyperplasia of various pancreatic endocrine cells is rare but has been long known.β cell hyperplasia contributes to persistent hyperinsulinemic hypoglycemia of infancy,which is commonly caused by mutations in the islet ATP-sensitive potassium channel,and to noninsulinoma pancreatogenous hypoglycemia in adults,which may or may not be associated with bariatric surgery.α cell hyperplasia may cause glucagonoma syndrome or induce pancreatic neuroendocrine tumors.An inactivating mutation of the glucagon receptor causes α cell hyperplasia and asymptomatic hyperglucagonemia.Pancreatic polypeptide cell hyperplasia has been described without a clearly-characterized clinical syndrome and hyperplasia of other endocrine cells inside the pancreas has not been reported to our knowledge. Based on morphological evidence,the main pathogenetic mechanism for pancreatic endocrine cell hyperplasia is increased endocrine cell neogenesis from exocrine ductal epithelium.Pancreatic endocrine cell hyperplasia should be considered in the diagnosis and management of hypoglycemia,elevated islet hormone levels,and pancreatic neuroendocrine tumors.Further studies of pathologic pancreatic endocrine cell hyperplasia will likely yield insights into the pathogenesis and treatment of diabetes and pancreatic neuroendocrine tumors.
