Patient-reported outcomes measures form the backbone of outcomes evaluation in orthopaedics,with most of the literature now relying on these scoring tools to measure change in patient health *** patient-reported information is increasingly collected routinely by orthopaedic providers but use of the data is typically restricted to academic *** in electronic data capture and the outcome tools themselves now allow use of this data as part of the clinical *** review evaluates the role of patient reported outcomes data as a tool to enhance daily orthopaedic clinical practice,and documents how developments in electronic outcome measures,computer-adaptive questionnaire design and instant graphical display of questionnaire can facilitate enhanced patientclinician shared decision making.

AIM： To compare the outcomes of vision using two different intraocular lens（IOL） replacement techniques,iris-fixated foldable intraocular lens（IF-IOL） and scleralfixated foldable intraocular lens（SF-IOL） in patients with insufficient capsular ***： Total 63 eyes（62 patients） with insufficient posterior capsule support underwent replacement of IF-IOL or SF-IOL between January 2008 and August *** measures included changes in visual acuity, slit lamp examination, refractive indices and corneal curvatures. RESULTS： The mean improvement of uncorrected visual acuity（UCVA） was greater in IF-IOL group compared to the SF-IOL group（0.43 D±0.19 D vs 0.35 D±0.18 D, P 〈0.05）. Moreover, 12（38.71%） eyes in IF-IOL group and 4（12.50%） in SF-IOL group had a higher postoperative UCVA than preoperative best corrected visual acuity（BCVA） while 9（29.03%） eyes in IF-IOLgroup and 18（56.25%） in SF-IOL group had a lower postoperative UCVA than preoperative BCVA. The myopic mean manifest sphere and mean cylinder magnitude were lower in the IF-IOL group than that in the SF-IOL group（-0.47 D±0.58 D vs 0.50 D±0.43 D, P 〈0.01; 0.84 D ±0.53 D vs 1.23 D ±0.70 D, P 〈0.05）. No difference of corneal astigmatism and surgically induced astigmatism was found between the two groups. In addition, fewer complications were observed in IF-IOL eyes. CONCLUSION： IF-IOL implantation can give a significant improvement in vision with fewer complications than SF-IOL in patients with insufficient capsular support.

Comminuted olecranon fracture fixation with pre-contoured plate:Comparison of composite and cadaver bones

AIM: To determine whether use of a precontoured olecranon plate provides adequate fixation to withstand supraphysiologic force in a comminuted olecranon fracture ***: Five samples of fourth generation composite bones and five samples of fresh frozen human cadaveric left ulnae were utilized for this study. The cadaveric specimens underwent dual-energy X-ray absorptiometry(DEXA) scanning to quantify the bone quality. The composite and cadaveric bones were prepared by creating a comminuted olecranon fracture and fixed with a pre-contoured olecranon plate with locking screws. Construct stiffness and failure load were measured by subjecting specimens to cantilever bending moments until failure. Fracture site motion was measured with differential variable resistance transducer spanning the fracture. Statistical analysis was performed with two-tailed Mann-Whitney-U test with Monte Carlo Exact ***: There was a significant difference in fixation stiffness and strength between the composite bones and human cadaver bones. Failure modes differed in cadaveric and composite specimens. The load to failure for the composite bones(n = 5) and human cadaver bones(n = 5) specimens were 10.67 nm(range 9.40-11.91 nm) and 13.05 nm(range 12.59-15.38 nm) respectively. This difference was statistically significant(P ? 0.007, 97% power). Median stiffness for composite bones and human cadaver bones specimens were 5.69 nm/mm(range 4.69-6.80 nm/mm) and 7.55 nm/mm(range 6.31-7.72 nm/mm). There was a significant difference for stiffness(P ? 0.033, 79% power) between composite bones and cadaveric bones. No correlation was found between the DEXA results and stiffness. All cadaveric specimens withstood the physiologic load anticipated postoperatively. Catastrophic failure occurred in all composite specimens. Looking at the history of epididymal research provides a level of confidence in predictions of cause and effect that looking at the future cannot achieve. Thus, when one of us （hamilton, 2002） wrote a short essay on the history of the testicular excurrent ducts it became fairly obvious that advances in understanding the epididymis followed advances in other fields, particularly those that provided new instrumentation to probe at ever finer levels of detail. Can this same theme lead to predictions about what will happen in epididymis research over the next century？

Cysteine-rich secretory protein-1 （CRISP-1） is a glycoprotein secreted by the epididymal epithelium. It is a member of a large family of proteins characterized by two conserved domains and a set of 16 conserved cysteine residues. In mammals, CRISP-1 inhibits sperm-egg fusion and can suppress sperm capacitation. The molecular mechanism of action of the mammalian CRISP proteins remains unknown, but certain non-mammalian CRISP proteins can block ion channels. In the rat, CRISP-1 comprises two forms referred to as Proteins D and E. Recent work in our laboratory demonstrates that the D form of CRISP-1 associates transiently with the sperm surface, whereas the E form binds tightly. When the spermatozoa are washed, the E form of CRISP-1 persists on the sperm surface after all D form has dissociated. Cross-linking studies demonstrate different protein-protein interaction patterns for D and E, although no binding partners for either protein have yet been identified. Mass spectrometric analyses revealed a potential post-translational modification on the E form that is not present on the D form. This is the only discernable difference between Proteins D and E, and presumably is responsible for the difference in behavior of these two forms of rat CRISP- 1. These studies demonstrate that the more abundant D form interacts with spermatozoa transiently, possibly with a specific receptor on the sperm surface, consistent with a capacitation-suppressing function during sperm transit and storage in the epididymis, and also confirm a tightly bound population of the E form that could act in the female reproductive tract. （Asian J Androl 2007 July; 9： 508-514）
