Importin proteins were originally characterized for their central role in protein transport through the nuclear pores, the only intracellular entry to the nucleus. This vital function must be tightly regulated to control access by transcription factors and other nuclear proteins to genomic DNA, to achieve appropriate modulation of cellular behaviors affecting cell fate. Importin-mediated nucleocytoplasmic transport relies on their specific recognition of cargoes, with each importin binding to distinct and overlapping protein subsets. Knowledge of importin function has expanded substantially in regard to three key developmental systems： embryonic stem cells, muscle cells and the germ line. In the decade since the potential for regulated nucleocytoplasmic transport to contribute to spermatogenesis was proposed, we and others have shown that the importins that ferry transcription factors into the nucleus perform additional roles, which control cell fate. This review presents key findings from studies of mammalian spermatogenesis that reveal potential new pathways by which male fertility and infertility arise. These studies of germline genesis illuminate new ways in which importin proteins govern cellular differentiation, includ ng v a d rect ng proteins to d st nct ntrace ular compartments and by determining cellular stress responses.

Peripheral nerve injuries induce a severe motor and sensory deficit. Since the availability of autologous nerve transplants for nerve repair is very limited, alternative treatment strategies are sought, including the use of tubular nerve guidance conduits(tNGcs). However, the use of tNGcs results in poor functional recovery and central necrosis of the regenerating tissue, which limits their application to short nerve lesion defects(typically shorter than 3 cm). Given the importance of vascularization in nerve regeneration, we hypothesized that enabling the growth of blood vessels from the surrounding tissue into the regenerating nerve within the tNGc would help eliminate necrotic processes and lead to improved regeneration. In this study, we reported the application of macroscopic holes into the tubular walls of silk-based tNGcs and compared the various features of these improved silk^(+) tNGcs with the tubes without holes(silk^(–) tNGcs) and autologous nerve transplants in an 8-mm sciatic nerve defect in rats. Using a combination of micro-computed tomography and histological analyses, we were able to prove that the use of silk^(+) tNGcs induced the growth of blood vessels from the adjacent tissue to the intraluminal neovascular formation. A significantly higher number of blood vessels in the silk^(+) group was found compared with autologous nerve transplants and silk^(–), accompanied by improved axon regeneration at the distal coaptation point compared with the silk^(–) tNGcs at 7 weeks postoperatively. In the 15-mm(critical size) sciatic nerve defect model, we again observed a distinct ingrowth of blood vessels through the tubular walls of silk^(+) tNGcs, but without improved functional recovery at 12 weeks postoperatively. Our data proves that macroporous tNGcs increase the vascular supply of regenerating nerves and facilitate improved axonal regeneration in a short-defect model but not in a critical-size defect model. The ability to modulate the future liver remnant(FLR) is a key component of modern oncologic hepatobiliary surgery practice and has extended surgical candidacy for patients who may have been previously thought unable to survive liver resection. Multiple techniques have been developed to augment the FLR including portal vein embolization(PVE), associating liver partition and portal vein ligation(ALPPS), and the recently reported transhepatic liver venous deprivation(LVD). PVE is a well-established means to improve the safety of liver resection by redirecting blood flow to the FLR in an effort to selectively hypertrophy and ultimately improve functional reserve of the FLR. This article discusses the current practice of PVE with focus on summarizing the large number of published reports from which outcomes based practices have been developed. Both technical aspects of PVE including volumetry, approaches, and embolization agents; and clinical aspects of PVE including data supporting indications, and its role in conjunction with chemotherapy and transarterial embolization will be highlighted. PVE remains an important aspect of oncologic care; in large part due to the substantial foundation of information available demonstrating its clear clinical benefit for hepatic resection candidates with small anticipated FLRs.

Acute and chronic respiratory illnesses cause widespread morbidity and mortality,and this class of illness now includes the novel coronavirus severe acute respiratory syndrome that is causing coronavirus disease-2019(cOVID-19).The world is experiencing a major demographic shift toward an older,obese,and physically inactive *** factor assessments based on pandemic data indicate that those at higher risk for severe illness from cOVID-19 include older males,and people of all ages with obesity and related comorbidities such as hypertension and type 2 *** in and of itself leads to negative changes in innate and adaptive immunity,a process termed *** causes systemic inflammation and adversely impacts immune function and host defense in a way that patterns *** primary prevention strategies to reduce the risk for cOVID-19 at both the community and individual levels include mitigation activities and the adoption of lifestyle practices consistent with good immune *** and human studies support the idea that,in contrast to high exercise workloads,regular moderate-intensity physical activity improves immunosurveillance against pathogens and reduces morbidity and mortality from viral infection and respiratory illnesses including the common cold,pneumonia,and *** odds are high that infectious disease pandemics spawned by novel pathogens will continue to inflict morbidity and mortality as the world’s population becomes older and more ***-19 is indeed a wake-up call,a tocsin,to the world that primary prevention countermeasures focused on health behaviors and hygiene demand our full attention and support.

A quasi two-dimensional two-phase flow cylinder model of slug motion in a voided line developed in this paper can predict reasonably the change of flow pattern of the slug, air entrainment, holdup and the distribution of axial velocity. However, if using the theory of transfer momentum to estimate the pressure-time history of slug at the elbow, the result of calculation is not good in agreement with that of experiment. Analysis of the experiment results and the estimations of using the theory of transient two-phase flow indicate that an evident water hammer response occurs in the slug, when the high-speed slug impacts on the elbow, and the peaks and waveform of pressure-time history are dependent mainly on the propagation of transient two-phase flow in the slug.

Solid organ transplantation has rapidly developed into the therapy a choice for end-stage organ failure. The expansion of its use has resulted is a large deficiency in organ supply. To address this, the field of organ transplantation has attempted to develop new strategies that would increase the availability of organs for transplant. Some of these strategies include expansion of the donor pool by increasing the number of living donors or using deceased donor organs that may be marginal or ＂expanded＂ The intent is to bring life-saving therapy to individuals in need; however, much of this expansion has been brought forward without clear prospective guidelines. This article focuses on the current disparity between organ supply and demand, and how this has impacted the use of living donors and development of the ＂expanded donor＂ concept.
