Lymphoepitelioma-like hepatocellular carcinoma: A case report and a review of the literature

Lymphoepitelioma is a particular form of undifferentiat-ed carcinoma, characterized by a prominent lymphoid stroma, originally described in the nasopharynx. Lym-phoid stroma-rich carcinomas arising in other organs have been termed lymphoepithelioma-like carcinoma (LELC). In the liver, primary LELCs are very rare, and the majority has been identified as cholangiocarcino-mas. Here a rare case of lymphoepithelioma-like hepa-tocellular carcinoma (HCC) is described. A 47-year old woman presented with abdominal pain. Ultrasonogra-phy revealed a liver nodule, 2.2 cm in diameter, local-ized in the right lobe, adjacent to the gallbladder. Viral markers for hepatic B virus (HBV), hepatic C virus (HCV) and Epstein-Barr virus (EBV) were negative. The nod-ule was hypoechogenic. The patient underwent sur-gery, with resection of the nodule. Histology showed hepatocellular carcinoma, characterized by a promi-nent lymphoid infiltrate. At immunocytochemistry, tumor cells were reactive for Hep Par1 and glypican 3. Immunophenotyping of tumor infiltrating lymphocytes evidenced the predominance of CD8+ cytotoxic sup.pressor T cells. The postoperative clinical outcome was favorable and the patient was recurrence-free 15 mo after resection. This case, to the best of our knowl-edge, is the first reported non EBV and non cirrhosis-associated lymphoepithelioma-like hepatocellular carci-noma. The association between the lack of EBV infec-tion, the absence of cirrhosis, a "cytotoxic profile" of the inflammatory infiltrate and a good prognosis could identify a variant of lymphoepithelioma-like HCC with a favorable clinical outcome.

This study has been conducted to analyze the chemical composition and antimicrobial, antioxidant and antiplasmodial properties of ethanol extracts of Pycnostachys erici-rosenii *** and Leucas martinicensis (Jack.) *** that are not yet well known. The HPLC-DAD-MS-ESIsup.+sup. method was used to investigate the chemical profile, the disc diffusion and microdilution method for the antimicrobial activities, the DPPH and FRAP assays for antioxidant property and SYBR Green I-based growth inhibition assay on Pf 3D7 (CQ-sensitive) strain for antiplasmodial properties. The results indicated significant amounts of hydroxybenzoic acid, flavonol and hydroxycinnamic acid in the two species. The total polyphenols analysis gave 209.0 ± 16.1 mg GAE/100g DW for P. erici-rosenii and 175.3 ± 1.6 mg GAE/100g DW for L. martinicensis while for total flavonoids analysis 33.4 ± 1.8 mg TE/100g DW was found for P. erici-rosenii and 39.6 ± 0.6 mg TE/100g DW for L. martinicensis. Moreover, antimicrobial tests revealed 0.125 mg/mL on E. faecalis ATCC25922 for ethanol extracts P. erici-rosenii and L. martinicensis. The antioxidant assays revealed IC50 56.17 μg/mL and 78.43 μg/mL for DPPH assay, and 112.03 μM TE/100mL and 90.46 μM TE/100mL) for FRAP assay for ethanol extracts of P. erici-rosenii and L. martinicensis respectively. Finally, for Antiplasmodial activity, IC50 was 4.07 μg/mL for P. erici-rosenii and 12.29 μg/mL for L. martinicensis. Therefore, the antimicrobial, the antioxidant, and the antiplasmodial results suggest that the ethanolic extracts of the aerial parts of P. erici-rosenii and L. martinicensis contain compounds with antimicrobial, antioxidant, and antiplasmodial properties, which can be searched in new drug discovery for treatment of infectious diseases.

Osteopathia striata with cranial sclerosis(OSCS, OMIM#300373) is an X-linked dominant sclerosing bone dysplasia that shows a distinct phenotype in females and males. In 2009, Zandra Jenkins et al found that germline mutations in the FAM123 B /WTX /AMER1 gene, mapped to chromosome Xq11.2, cause both the familial and sporadic forms of OSCS. Intriguingly, the WTX gene was already known as a putative tumor sup.ressor gene, since in 2007 Rivera et al had reported inactivating WTX mutations in Wilms' tumor(WT), the most frequent renal tumor of childhood. Here we review the heterogeneous clinical presentation of OSCS patients and the involvement of WTX anomalies in OSCS and in WT.
