Purpose： The clinical significance of isolated flee fluid （FF） without solid organ injury on computed to- mography （cT） continues to pose significant dilemma in the management of patients with blunt abdominal trauma （BAT）. Methods： We reviewed the incidence of FF and the clinical outcome amongst patients with blunt abdominal trauma in a metropolitan trauma service in South Africa. We performed a retrospective study of 121 consecutive cT scans over a period of 12 months to determine the incidence of isolated FF and the clinical outcome of patients managed in a large metropolitan trauma service, Results： Of the 121 cTs, FF was identified in 36 patients （30%）. Seven patients （6%） had isolated FF. Of the 29 patients who had free fluid and associated organ injuries, 33 organ injuries were identified. 86% （25/ 29） of all 29 patients had a single organ injury and 14% had multiple organ injuries. There were 26 solid organ injuries and 7 hollow organ injuries. The 33 organs injured were： spleen, 12; liver, 8; kidney, 5; pancreas, 2; small bowel, 4; duodenum, 1. Six （21%） patients required operative management for small bowel perforations in 4 cases and pancreatic tail injury in 2 cases. All 7 patients with isolated FF were initially observed, and 3 （43%） were eventually subjected to operative intervention. They were found to have an intra-peritoneal bladder rupture in 1 case, a non-expanding zone 3 haematoma in I case, and a negative laparotomy in 1 case. Four （5？%） patients were successfully managed without surgical interventions. conclusions： Isolated FF is uncommon and the clinical significance remains unclear. Provided that reli- able serial physical examination can be performed by experienced surgeons, an initial non-operative approach should be considered.

The Rheic Ocean was one of the most important oceans of the Paleozoic Era. It lay between Laurentia and Gondwana from the Early Ordovician and closed to produce the vast Ouachita- Alleghanian-Variscan orogen during the assembly of Pangea. Rifting began in the cambrian as a continua- tion of Neoproterozoic orogenic activity and the ocean opened in the Early Ordovician with the separation of several Neoproterozoic arc terranes from the continental margin of northern Gondwana along the line of a former suture. The rapid rate of ocean opening suggests it was driven by slab pull in the outboard Iapetus Ocean. The ocean reached its greatest width with the closure of Iapetus and the accretion of the peri- Gondwanan arc terranes to Laurentia in the Silurian. Ocean closure began in the Devonian and continued through the Mississippian as Gondwana sutured to Laurussia to form Pangea. The ocean consequently plays a dominant role in the Appalachian-Ouachita orogeny of North America, in the basement geology of southern Europe, and in the Paleozoic sedimentary, structural and tectonothermal record from Middle America to the Middle East. Its closure brought the Paleozoic Era to an end.

β-Glucans, mostly from oats, have been shown to reduce blood concentrations of total and LDL-cholesterol in animals and humans. After processing, spent brewer’s yeast, a by-product of the fermentation process, contains 85% - 90% β-glucans. The purpose of this study was to evaluate the effect of yeast-derived β-glucan fiber on plasma lipids and early atherosclerosis development in hamsters consuming a semi-purified hypercholesterolemic diet (HcD). Animals were fed either the HcD or the HcD containing psyllium or β-glucan fiber from yeast for 12 weeks. Both the psyllium and β-glucan fiber from yeast showed significant decreases in plasma total cholesterol, non-HDL-c, triacylglycerol, and aortic fatty streak area when compared to the HcD. Also, the β-glucan fiber from yeast had significantly less aortic fatty streak area compared to the psyllium diet. Findings from this study show that while both β-glucan fiber from yeast and psyllium produced similar reductions in plasma lipid and lipoprotein concentrations, the β-glucan fiber from yeast prevented the development early atherosclerosis better than psyllium in the hamsters.

Satellites have provided high-resolution(color(i.e.,remote sensing reflectance)and thermal emission imagery of aquatic environments since the early 1980s;however,global operational water quality products based on these data are not readily available(e.g.,temperature,chlorophyll-a,turbidity,and suspended particle matter).currently,because of the postprocessing required,only users with expressive experience can exploit these data,limiting their ***,we provide paths(recipes)for the nonspecialist to access and derive water quality products,along with examples of applications,from sensors on board Landsat-5,Landsat-7,Landsat-8,Landsat-9,Sentinel-2A,and *** emphasize that the only assured metric for success in product derivation and the assigning of uncertainties to them is via validation with in situ *** hope that this contribution will motivate nonspecialists to use publicly available high-resolution satellite data to study new processes and monitor a variety of novel environments that have received little attention to date.

Self-similar fractals are of importance in both science and engineering. Metal-organic Sierpin′ ski triangles are particularly attractive for applications in gas separation, catalysis and sensing. Such fractals are constructed in this study by using 1208 V-shaped 4,400-dicyano-1,10：30,100-terphenyl molecules and Fe atoms on Au（1 1 1）, and studied in detail by low-temperature scanning tunneling microscopy. Density functional theory calculations are employed to rationalize the invisible Fe atoms in STM images. Monte carlo simulations are performed to understand the formation mechanism of the surface-supported fractal crystals.

The treatment of peritoneal carcinomatosis (Pc) of colorectal origin with cytoreductive surgery(cRS) plus hyperthermic intraperitoneal chemotherapy (HIPEc) has a 5-year recurrence-free or cure rate of at least 16%, so it is no longer labeled as a fatal disease, and offers prolonged survival for patients with a low peritoneal carcinomatosis index. Metachronous Pc of colorectal origin is so predictable that there is a model which has been used to successfully determine the individual risk of each patient. Patients at risk are clearly identified; those with the highest risk have small peritoneal nodules present in the first surgery (70% probability of developing Pc), ovarian metastases(60%), perforated tumor onset or intraoperative tumor rupture(50%). current clinical, biological and imaging techniques still lack sufficient sensitivity to diagnose Pc in its initial stages, when cRS plus HIPEc has a greater impact and a higher cure rate. Second-look surgery with HIPEc or prophylactic HIPEc at the time of the first intervention have been proposed as means of preventing and/or anticipating clinical or radiological relapse in at-risk patients. Both techniques have shown a significant decrease in peritoneal relapses and should be considered essential weapons in the management of colorectal cancer.

WORLD HERITAGE AND SPAcE TEcHNOLOGY The convention concerning the Protection of the World cultural and Natural Heritage(WHc),adopted by United Nations Educational,Scientific and cultural Organization(UNEScO)on November 16,1972,aims to ensure the identification,protection,conservation,presentation,and transmission to future generations of the world’s cultural and natural *** WHc works toward these goals by emphasizing the Outstanding Universal Value(OUV)of heritage sites and the unique contribution such places can make to conservation and human development agendas.1 As of the end of January 2023,theWHc has been signed by 194 state parties,covering 1,157 sites(including 900 cultural,218 natural,and 39 mixed properties),55 of which are considered to be in *** sites,totaling an area of more than 370 million hectares are designated as World Heritage(WH)sites(https://***/en/list/).WH sites have played a significant role in the sustainable development of society globally and helped effectively maintain and preserve the cultural diversity and global biodiversity of the Earth.

构建与增强型绿色荧光蛋白基因相连的糖基化磷脂酰肌醇(glycosyl phosphatidylinositol,GPI)序列的真核表达质粒,并检测其在A549细胞中的表达。分离人外周血淋巴细胞,提取总RNA,以RT-PcR法扩增cD24基因的243bpGPI锚定序列,双酶切后定向克隆入pEGFP-c1质粒中,构建并鉴定pEGFP-c1-GPI质粒。经脂质体介导转染A549细胞后,在荧光显微镜下观察目的蛋白在真核细胞内的表达情况。经酶切和测序鉴定证实,所克隆的cD24GPI序列正确,荧光显微镜观察pEGFP-c1-GPI质粒转染A549细胞可见围绕细胞膜的强绿色荧光,而对照pEGFP-c1质粒转染A549细胞仅见胞内均匀荧光。成功构建与EGFP相连的GPI真核表达质粒,且能在A549细胞膜上锚定表达EGFP-GPI融合蛋白,为构建锚定表达型肿瘤疫苗奠定基础。

Background: Previous surveys of children’ s diabetes service provision in the UK have shown gradual improvements but continuing deficiencies. Aim: To determine whether further improvements in services have occurred. Methods: A questionnaire was mailed to all paediatricians in the UK identified as providing care for children and adolescents with diabetes. Responses were compared with results of three previous surveys, and with recommendations in the Diabetes NSF and the NIcE type 1 diabetes guidelines. Results: Replies were received from 187 consultant paediatricians in 169 centres looking after children; 89% expressed a special interest in diabetes, 98% saw children in a designated diabetic clinic, and 95% clinics now have more than 40 patients. In 98% of the clinics there was a specialist nurse (82% now children’ s trained), but 61% clinics had a nurse:patient ratio clinics did not have a paediatric dietician and in 78% there was no access to psychology/ psychiatry services in clinics. Glycated haemoglobin was measured routinely at clinics in 86% , annual screening for retinopathy performed in 80% , and microalbuminuria in 83% . All centres now have local protocols for ketoacidosis, but not for children undergoing surgery (90% ) or severe hypoglycaemia (74% ). Mean clinic HbA1c levels were significantly lower in the clinics run by specialists (8.9% ) than generalists (9.4% ). There have been incremental improvements over the last 14 years since the surveys began, but only two clinics met all the 10 previously published recommendations on standards of care. conclusions: The survey shows continuing improvements in organisational structure of services for children with diabetes but serious deficiencies remain. Publication and dissemination of the results of the previous surveys may have been associated with these improvements and similar recurrent service review may be applicable to services for other chronic childhood conditions.
