Spectrum of Primary Glomerular Diseases in Patients Presenting with Urinary Abnormalities

Introduction: Primary Glomerular Diseases are a spectrum of renal disorders of unknown aetiology with distinct characteristics, specific natural history and prognosis. A thorough evaluation is prerequisite to establish the diagnosis since many systemic diseases and secondary aetiology masquerade as primary diseases. Methods: This prospective observational study was conducted at a tertiary care centre and included 30 patients, with clinical features suggestive of primary glomerular diseases, of which 23 patients (76.6%) were males and 07 patients (23.33%) were females. The mean age at presentation was 37.23 ± 12.89 years. Among the observed spectrum of Primary Glomerular Diseases, IgA Nephropathy (IgA N) was seen in 26.67% patients, Focal Segmental Glomerulosclerosis (FSGS) in 20% patients and Membranous Glomerulopathy (MGN) in 13.33% patients, whereas the incidence of other abnormalities had less percentage contribution. Proteinuria was the commonest presentation seen in 60% patients, followed by Microscopic Haematuria in 20%. Mean Serum Creatinine was 0.99 ± 0.16 mg/dl. Mean Serum Albumin was 2.51 ± 0.76 gm/dl. Overall Nephrotic range proteinuria was observed in 15 (50%) patients. Results: IgA N, FSGS & MGN were the commonest observed Primary Glomerular Diseases. Proteinuria, Haematuria, Anasarca and Pedal Oedema were the commonest observed clinical presentations. Conclusions: In this studied series IgA Nephropathy, FSGS and MGN were the most prevelant diagnoses in the patients presenting with Urinary Abnormality. Nephrotic range Protenuria was the major indication for biopsy, there is a temporal variation in glomerulopathies wherein there is increase in incidence of IgA Nephropathy and decrease in incidence of FSGS.

The Combination of Moberg Flap with V-Y Advancement and Reverse Adipofascial Cross Finger Flap for Coverage of Degloving Injury of the Thumb-Case Report

We report a case of 22 years old male patient who is a worker in a factory and sustained degloving injury of his left thumb in a machine while working. There was loss of the pulp of the thumb extending circumferentially to the dorsal aspect with loss of the skin of the terminal phalanx and part of the proximal phalanx. The nail and germinal matrix were lost with exposure of the bone and extensor pollicis longus tendon insertion. The thumb was totally covered with a combination of two flaps: Moberg flap with V-Y advancement was used to cover most of the volar surface of the thumb and reverse adipofascial cross finger flap from the adjacent index finger was used to cover the dorsal surface and the tip of the thumb. The reverse adipofascial cross finger flap was covered with split thickness skin graft. Three weeks later this flap was divided and the thumb was mobilized freely. The patient had a full range of movement of the thumb and index finger with few settings of physiotherapy postoperatively. We recommend combining both of these flaps to reconstruct degloving injury of the thumb as they provide near adjacent tissue of similar texture, preserve sensation at the volar aspect of the thumb and also avoid the complications of the distant flaps.

Location Based Performance of WiMAX Network for QoS with Optimal Base Stations (BS)

In this paper we are given concept of our WiMAX (Worldwide Interoperability for Microwave Access) network perfor-mance for QoS monitoring and optimization solution for BS (Base Station) with multimedia application. In the communication sector, the optimal objective is to equate quality and cost. Due to its large coverage area, low cost of deployment and high speed data rates. WiMAX is a promising technology for providing wireless last-mile connectivity. Physical and MAC layer of this technology refer to the IEEE 802.16e standard, which defines 5 different data delivery service classes that can be used in order to satisfy Quality of Service (QoS) requirements of different applications, such as VoIP, videoconference, FTP, Web, etc. In this paper we have made six scenarios. Here two types of MAC layer QoS are used and they are UGS and rtPS having application of Voice over IP (VoIP) and MPEG respectively. Also the traffic priority for UGS is high as compared to rtPS. In each scenario the number of fixed nodes (Fixed Subscriber Stations) and Mobile nodes (Mobile Subscriber Stations) are different. To cover more nodes or if nodes are outside the coverage area more than one BS are required. All the simulation results based on optimized networks and area we have considered between Bhusaval and Jalgam.

Finite Difference Approximation for Solving Transient Heat Conduction Equation of Copper

In this paper, a numerical technique is proposed to obtain the solution for transient heat conduction equation of Copper. The copper element is characterized by many characteristics;the most important of which is its high ability to conduct heat and electrical conductivity, in addition to being a flexible and malleable metal that is easy to form without being broken, making it one of the basic minerals that humans have benefited from for thousands of years, it is one of the first minerals. That has been discovered and extracted, and still plays a major role in the development of societies. The obtained solutions are compared with the available exact solutions and the obtained solutions using the finite difference method. The results indicate that the finite difference method is a highly effective method for obtaining approximate solutions for the thermal conductivity equation for copper. It is also clear from the numerical results from copper in the high conductivity of heat and electricity.

Etching and Micro-Topographical Studies of Barium Oxalate Crystals Grown in Agar Gel

In the present study, the selection of an etchant for a particular crystal was purely made on empirical basis. The gel grown barium oxalate crystals were etched by HCl, HNO3, BaCl2, NH4Cl, and NH4Cl-HCl solutions. Micro-topographical studies have been made and it was found that elongated triangular etch pits and pits within pits were formed. Rectangular growth layers in the form of staircase and leaf like dendrite pattern were seen. Kinetics of etching was studied. Quantitative estimation of dissolved crystals in etchants was used for the determination of activation energy of reaction and pre-exponential factor with the help of Arrhenius equation.

Maternal and Neonatal Death Review (MNDR): A Useful Approach to Identifying Appropriate and Effective Maternal and Neonatal Health Initiatives in Bangladesh

Objectives: To identify the effects of Maternal and Neonatal Death Review (MNDR) in terms of improving maternal and neonatal health at the community level in Bangladesh. Methods: Both quantitative and qualitative methods were undertaken for collecting data in Kashipur Union, Bangladesh. Death notifications from households, subsequent data collections from a focus-group discussion (FGD), a group discussion (GD) and in-depth interviews (IDIs) were obtained using structured tools and guidelines. Results: A total of four maternal deaths, 21 neonatal deaths and 15 still births were reported in the MNDR death notification system at Kashipur Union in 2010. Data were presented to local programme managers, who took various initiatives including awareness programmes, pregnancy registration, antenatal care, birth planning, and also the revitalization of a community clinic. The coverage of antenatal care, delivery in clinics, postnatal care and referral of complications increased through the active participation of the community. Community healthcare providers, care recipients and members of the community expressed satisfaction with the quality of maternal and neonatal services. In the preceding two years, maternal and neonatal deaths substantially reduced in Kashipur (in 2011 maternal death = 1, neonatal death = 20, still birth = 8;in 2012 maternal death = 1, neonatal death = 8, still birth = 13). Conclusions: The MNDR system successfully delivered notification of all maternal and neonatal deaths in the defined area and collected information for the formulation and implementation of specific interventions, which resulted in visible and tangible changes in care-seeking and client satisfaction.

Prioritization of Promising Wadi Wala Watershed (Southern Jordan) Using GIS-Based Morphometric Analysis and Multivariate Statistics

GIS-based morphometric analysis was employed for prioritization of the Wadi Wala catchment, southern Jordan. Twenty three fourth-order sub-basins were prioritized based on morphometric analysis, then appropriate soil and water conservation measures were proposed. Digital Elevation Model (DEM) and Arc GIS were used to delineate watersheds and to extract the drainage networks, and other required thematic maps (elevation and slope). LANDSAT data was used to prepare land use/land cover map, and a soil map was digitized using Arc GIS software. Linear and shape parameters were computed to prioritize 23 sub-watersheds, and ranks were designated based on the calculated compound parameter (Cp). Sub-basins grouped under a high priority class are exposed to high erosion risk;thus, they are of high potential for applying soil and water conservation measures. The current study substantiates the capability of morphometric analysis method, and geospatial technology in watershed prioritization. The Discriminant Analysis (DA) employed validates the priority classes (high, moderate, and low priority) achieved based on morphometric analysis, where they found statistically distinct from each other. Thus, it can be concluded that prioritization based only on morphometric analysis method is consistent, reliable, and of high capacity using GIS platform. Priority map along with soil, land user/cover, and slope information will help decision makers to execute proper soil and water conservation programs in the rainfed highlands of Jordan.

Prioritization of W. Mujib Catchment (South Jordan) through Morphometric and Discriminant Analysis, GIS, and RS Techniques

GIS and remote sensing were utilized for prioritizing the W. Mujib catchment. Fifty three fourth-order sub-watersheds were prioritized based on morphometric analysis of linear and shape parameters. ASTER DEM (v.2), topographical maps, and Arc GIS (10.1) software, have been employed to delineate the 53 sub-basins, to extract the drainage networks, and to compute the required basic, linear, and shape parameters, and to compile the necessary thematic maps such as elevation and slope categories. The land use/land cover map was generated using ERDAS Imagine (2015), LANDSAT 8 image, and supervised classification (Maximum Likelihood Method). Soil map was digitized using the Arc GIS tool. Each sub-basin is prioritized by assigning ranks based on the calculated compound parameter (Cp). The final score for each sub-basin is ascribed as per erosion threat. The 53 sub-watersheds were grouped into four categories of priority: very high (15 sub-basins, 28.3% of the total), high (17 sub-basins, 32% of the total), moderate (16 sub-basins, 30.2% of the total), and low (5 sub-basins, 9.5% of the total). Sub-basins categorized as very high and high priority (60.3% of the total) are subjected to high erosion risk, thus, creating an urgent need for applying soil and water conservation measures. The validity of the prioritized four groups was tested statistically by means of Discriminant Analysis (DA), and a significant difference was found between the four priority classes. A relatively complete separation exists between the recognized priority classes;thus, they are statistically valid, distinct, and different from each other. The present results intend to help decision makers pay sufficient attention to soil and water conservation programs, and to encourage tree plantation over the government-owned sloping land. Such procedures are essential in order to minimize soil erosion loss, and to increase soil moisture on farms, thus, reducing the impact of recurrent droughts and the possibili

AIMTo analyze serum levels of nitric oxide （NO）, an indicator of cardiovascular health, in post-menopausal females with and without hypothyroidism. METHODSNO was analyzed colorimetrically in 30 newly diag-nosed hypothyroid postmenopausal females and 30 postmenopausal females with normal thyroid profle. Results were compared and subjected to appropriate statistical analysis. RESULTSThe levels of serum NO were found to be significantlydecreased in postmenopausal females with hypothyroidismas compared to the levels in those with normal thyroidprofile （P value 〈 0.05）. A negative correlation of NOwas observed with thyroid stimulating hormone whereasa positive correlation of NO was observed with free T3 （FT3）, free T4 （FT4）, T3 and T4 though it was statistically signifcant only for FT4 among postmenopausal females with *** hypothyroid females may be at a risk of compromised cardiovascular health as indicated by low NO levels. Regular monitoring and risk assessment is essential for timely intervention.

WiMAX System Simulation and Performance Analysis under the Influence of Jamming

This paper presents simulation of WiMAX based system under jamming. The performance of the system was found out to greatly differ with the use of different jamming signals, allowing central areas to be identified, where system devel-opment should be focused on. In addition, from the basic theory point of view, rather surprising results were also found. This work should give a clear picture of how the studied WiMAX system performs under jamming as well as without jamming. The results show that some forms of interference degrade the performance of the system rapidly, thus the form of incoming jamming should be known and considered before deploying the system. Single carrier jamming and multi–carrier jamming are discussed here. The issues related to jamming and jamming reduction techniques are also covered. Jamming can destroy communication in the targeted area. Multi–carrier jamming is challenge in WiMAX because WiMAX is having OFDM based physical layer. Simulation is the main approach in this paper. OPNET MODELER 14.5 is the software used for the simulation purpose.
