Prescription practice of antihistamines for acute upper respiratory tract infections in pediatric patients in a local emergency department in Hong Kong

BACKGROUND: Currently there is very limited data in the literature assessing the prevalence of antihistamine prescription, and there is no local prevalence data about the prescription of antihistamine agents among primary practitioner and emergency physicians. The objectives are 1) to report the prevalence of antihistamine prescription for children less than 6 years old with acute upper respiratory infection and 2) to explore the associated factors for the prescription ***: This is a cross-sectional study. All consecutive cases of paediatric patients aged 6 or below who presented to the emergency department during a study period of one week from April 1 to July 4, 2009 with diagnosis of acute upper respiratory infection were included. Totally 162 patients were ***: Among the 162 cases, 141(87%) patients were prescribed one antihistamine of any group. Sixty(37%) patients were prescribed two or more antihistamines. In multivariate logistic regression model, age was found to be signi? cantly(P<0.001) associated with multiple antihistamine prescription(OR=1.042, 95%CI=1.02 to 1.06). Years of graduation of attending physician for more than 5 years was also a strong predictor of multiple antihistamine prescription(OR=4.654, 95%CI=2.20 to 9.84, P<0.001).CONCLUSION: In the local emergency department, patients' age and the years of graduation from medical school of the attending physician were predictors of multiple antihistamine prescription for acute upper respiratory infections for children aged less than 6.

A Stable Open-Shell Conjugated Diradical Polymer with Ultra-High Photothermal Conversion Efficiency for NIR-Ⅱ Photo-Immunotherapy of Metastatic Tumor

Massive efforts have been concentrated on the advance of eminent near-infrared(NIR) photothermal materials(PTMs) in the NIR-Ⅱ window(1000–1700 nm), especially organic PTMs because of their intrinsic biological safety compared with inorganic PTMs. However, so far, only a few NIR-Ⅱresponsive organic PTMs was explored, and their photothermal conversion efficiencies(PCEs) still remain relatively low. Herein, donor–acceptor conjugated diradical polymers with open-shell characteristics are explored for synergistically photothermal immunotherapy of metastatic tumors in the NIR-Ⅱ window. By employing side-chain regulation, the conjugated diradical polymer TTB-2 with obvious NIR-Ⅱ absorption was developed, and its nanoparticles realize a record-breaking PCE of 87.7% upon NIR-Ⅱ light illustration. In vitro and in vivo experiments demonstrate that TTB-2 nanoparticles show good tumor photoablation with navigation of photoacoustic imaging in the NIR-Ⅱ window, without any side-effect. Moreover, by combining with PD-1 antibody,the pulmonary metastasis of breast cancer is high-effectively prevented by the efficient photo-immunity effect. Thus, this study explores superior PTMs for cancer metastasis theranostics in the NIR-Ⅱ window, offering a new horizon in developing radical-characteristic NIR-Ⅱ photothermal materials.

Utilization of the Accident & Emergency Departments by Chinese elderly in Hong Kong

BACKGROUND: The epidemiological data on elderly patients attending Accident and Emergency Departments(AEDs) in Hong Kong is lacking. The study aimed to examine the epidemiology of geriatric patient visits to AEDs in Hong Kong, including demographic data and predictors of life-saving interventions(LSI) and ***: A retrospective cross-sectional study of geriatric patients older than 64 years old attending three AEDs during the year 2012, with a sample of 1 200 patient visits recruited. The data were retrieved from the medical records of the respective hospitals. Descriptive characteristics of the visits were provided. Multivariate logistic regression was performed to evaluate the predictors of LSI and hospital ***: The mean age of the patients was 79.1 years. Totally 49.7% of the patients were male. "Diseases of the respiratory system" was the commonest diagnosis in AEDs as well as that required admission. The admission rate was 56.8%. Logistic regression demonstrated that dependent activity of daily living(ADL), arrival by ambulance, and the higher number of co-morbidities were predictors of LSI, while advanced age, dependent ADL, institutionalized patients, arrival by ambulance, and higher number of co-morbidities were predictors of hospital ***: Ageing population is creating an imminent burden on the emergency service in Hong Kong. Previously unavailable epidemiological information about geriatric attendance to AEDs was described. This forms the basis for development of future studies concerning the medical services on this specific group of patients.

A historical re-analysis of the calamitous midget typhoon passing through Hong Kong region on 18 September 1906 and its storm surge impact to Hong Kong region

A typhoon passed through Hong Kong region suddenly and unexpectedly on 18 September 1906(the“Typhoon 1906”)and had a disastrous impact on Victoria Harbour and its surroundings in just a couple of *** the year 1906 was the“Bingwu”year in the Chinese calendar,the typhoon is also known historically as the“Bingwu Typhoon”.Tremendous loss of lives and property resulted,and the shipping and fishing communities were *** mysteries arising from this calamitous typhoon have existed to date:1)Why the Hong Kong region Observatory was not able to provide any forewarning?2)whether the storm surge reported in some contemporary records is entirely credible?This paper will focus on both of *** this paper,we re-analyse historical weather observations recorded in various historical documents and estimate the possible storm size,intensity and track of Typhoon 1906 using tropical cyclone *** on the re-analyses,the storm surges,storm tides and wave heights in Hong Kong region are also estimated using storm surge and wave *** results reveal that Typhoon 1906 was a midget typhoon,with a radius of maximum winds of 11 km or smaller,during its passage through Hong Kong *** explains why it was technically impossible for a forewarning to be given at that time when real-time weather observations from ships,meteorological satellites and radars were *** also estimate that the maximum storm surges(storm tides)in Hong Kong region were not higher than 0.82 m(2.43 mCD)and 1.98 m(4.15 mCD)in Victoria Harbour and Tolo Harbour *** figures are found to be limited by the intensity and the storm size of the ***,we conclude that the previously documented storm surge figures are not supported by the present study.

Bevacizumab for neovascular age-related macular degeneration in Chinese patients in a clinical setting

AIM：To determine the outcome of non-investigational treatment with intravitreal bevacizumab（IVB） in neovascular age-related macular degeneration（AMD）patients.· METHODS：Retrospective chart review of 81 eyes with neovascular AMD followed-up for at least 12 mo and received 3-monthly loading IVB ***-treat was based upon the individual clinician＇s *** visual acuity（BCVA） and optical coherence tomography measurements of central foveal thickness outcomes were evaluated at 12,24 mo.· RESULTS：Eighty-one eyes（of 75 patients） completed12 mo of follow-up and 44 eyes（of 41 patients） completed24 mo of *** mean baseline logMAR BCVA significantly improved from 0.94±0.69 to 0.85±0.68 at12mo（P〈0.001） and from 0.91±0.65 to 0.85±0.60（P=0.004）at 24 *** proportion of eyes that lost 〈15 logMAR letters at 12 mo was 90.1%and at 24 mo was 81.8%.IVB was effective in improving visual acuity in both treatment naive and previous photodynamic therapy（PDT）-treated *** naive patients required significantly fewer injections than patients with prior *** regression analysis identified that poorer baseline visual acuity was associated with greater improvement in visual acuity（P=0.015）.· CONCLUSION：Fewer injections in clinical practice may result in suboptimal visual outcomes compared with clinical trials of IVB in neovascular AMD *** baseline visual acuity and prior PDT treatment may also improve vision after *** safety and durability of effect was maintained at 24 mo.

MicroRNA-142-3p and microRNA-142-5p are downregulated in hepatocellular carcinoma and exhibit synergistic effects on cell motility

MicroRNAs （miRNAs）, an important class of small non-coding RNAs, regulate gene expression at the post-transcriptional level, miRNAs are involved in a wide range of biological processes and implicated in different diseases, including cancers. In this study, miRNA profiling and qRT-PCR validation revealed that miR-142-3p and miR-142-5p were significantly downregulated in hepatocellular carcinoma （HCC） and their expression levels decreased as the disease progressed. The ectopic expression of miR-142 significantly reduced HCC cell migration and invasion. Overexpression of either miR-142-3p or miR-142-5p suppressed HCC cell migration, and overexpression of both synergistically inhibited cell migration, which indicated that miR-142-3p and miR-142-5p may cooperatively regulate cell movement, miR-142-3p and miR-142-5p, which are mature miRNAs derived from the 3＇- and 5＇-strands of the precursor miR-142, target distinct pools of genes because of their different seed sequences. Pathway enrichment analysis showed a strong association of the putative gene targets of miR-142-3p and miR-142-5p with several cell motility-associated pathways, including those regulating actin cytoskeleton, adherens junctions, and focal adhesion. Importantly, a number of the putative gene targets were also significantly upregulated in human HCC cells. Moreover, overexpression of miR-142 significantly abrogated stress fiber formation in HCC cells and led to cell shrinkage. This study shows that mature miR-142 pairs collaboratively regulate different components of distinct signaling cascades and therefore affects the motility of HCC cells.

Quality indicators in pediatric colonoscopy in a low-volume center:Implications for training

AIM To study implications of measuring quality indicators on training and trainees' performance in pediatric colonoscopy in a low-volume training *** We reviewed retrospectively the performance of pediatric colonoscopies in a training center in Malaysia over 5 years(January 2010-December 2015), benchmarked against five quality indicators: appropriateness of indications, bowel preparations, cecum and ileal examination rates, and complications. The European Society of Gastrointestinal Endoscopy guideline for pediatric endoscopy and North American Society for Pediatric Gastroenterology, Hepatology and Nutrition training guidelines were used as *** Median(± SD) age of 121 children [males = 74(61.2%)] who had 177 colonoscopies was 7.0(± 4.6) years. On average, 30 colonoscopies were performed each year(range: 19-58). Except for investigations of abdominal pain(21/177, 17%), indications for colonoscopies were appropriate in the remaining 83%. Bowel preparation was good in 87%. One patient(0.6%) with severe Crohn's disease had bowel perforation. Cecum examination and ileal intubation rate was 95% and 68.1%. Ileal intubation rate was significantly higher in diagnosing or assessing inflammatory bowel disease(IBD) than non-IBD(72.9% vs 50.0%; P = 0.016). Performance of four trainees was consistent throughout the study period. Average cecum and ileal examination rate among trainees were 97% and 77%.CONCLUSION Benchmarking against established guidelines helps units with a low-volume of colonoscopies to identify area for further improvement.

A Survey of Mental Health of New Patients at Traditional Chinese Medical Clinic

In Chinese societies, traditional Chinese medical clinics are popular and play a significant role in primary healthcare. The current study aimed to examine the mental health of new patients at Chinese medical clinic. A cross-sectional questionnaire survey was conducted. Subjects were new patients of a Chinese medical clinic recruited through 100% consecutive sampling during a 5-week period. A response rate of 84.2% and a sample size of 367 were achieved. Sample included 111 (30.2%) men and 256 (69.8%) women. Mean age was 44.0 (SD = 17.1). Presenting problems were all somatic, with 68.3% lasted over 1 month and 37.1% lasted over 1 year. In the month prior to first consultation, 26.4%, 33.2% and 19.3% patients consulted other public, private and Chinese medical clinic on average 1.5, 1.9 and 3.0 times respectively. Besides, 7.4% and 6.5% patients self-administered Western and Chinese drugs on average 1.3 and 2.4 times respectively. Using a cut-off of ? 5 on GHQ-12, prevalence of common mental disorders was estimated at 21.0%. In conclusion, findings revealed that common mental disorders were prevalent among new Chinese medicine patients al-though presenting problems of subjects were all somatic. Many of these presenting medical conditions were chronic, with 68.3% lasting over 1 month and 37.1% lasting over 1 year. Over 70% patients had been seek-ing various forms of treatment in the month prior to first consultation. Mental health and illness behavior of Chinese medicine patients warrants further research and attention.
