Liver transplantation in patients with incidental hepatocellular carcinoma/cholangiocarcinoma and intrahepatic cholangiocarcinoma： a single-center experience

BACKGROUND： Reports of fiver transplantation （LT） in patients with mixed hepatocellular carcinoma/cholangiocarcinoma （HCC/CC） and intrahepatic cholangiocarcinoma （ICC） are modest and have been mostly retrospective after pathological categorization in the setting of presumed HCC. Some studies suggest that patients undergoing LT with small and unifocal ICC or mixed HCC/CC can achieve about 40%-60% 5-year post-transplant survival. The study aimed to report our experience in patients undergoing LT with explant pathology revealing HCC/CC and ICC. METHODS： From a prospectively maintained database, we performed cohort analysis. We identified 13 patients who underwent LT with explant pathology revealing HCC/CC or ICC. RESULTS： The observed recurrence rate post-LT was 31% （4/13） and overall survival was 85%, 51%, and 51% at 1, 3 and 5 years, respectively. Disease-free survival was 68%, 51%, and 41% at 1, 3 and 5 years, respectively. In our cohort, four patients would have qualified for exception points based on updated HCC Organ Procurement and Transplantation Network imaging guidelines. CONCLUSIONS： Lesions which lack complete imaging characteristics of HCC may warrant pre-LT biopsy to fully elucidate their pathology. Identified patients with early HCC/CC or ICC may benefit from LT if unresectable. Additionally, incorporating adjunctive perioperative therapies such as in the case of patients undergoing LT with hilar cholangiocarcinoma may improve outcomes but this warrants further investigation.

Solution for fractional potential KdV and Benjamin equations using the novel technique

In this paper,we find the solutions for fractional potential Korteweg-de Vries(p-KdV)and Benjamin equations using q-homotopy analysis transform method(q-HATM).The considered method is the mixture of q-homotopy analysis method and Laplace transform,and the Caputo fractional operator is considered in the present *** projected solution procedure manipulates and controls the obtained results in a large admissible ***,it offers a simple algorithm to adjust the convergence province of the obtained *** validate the q-HATM is accurate and reliable,the numerical simulations have been conducted for both equations and the outcomes are revealed through the plots and *** between the obtained solutions with the exact solutions exhibits that,the considered method is efficient and effective in solving nonlinear problems associated with science and technology.

Attitudes and diagnostic practice in low back pain:A qualitative study amongst Greek and British physiotherapists

AIM To explore current diagnostic practice and attitudes of Greek and United Kingdom physiotherapists(PTs)on assessing low back pain(LBP)*** Three focus groups were undertaken,followed by a structured questionnaire-type survey comprising 23 health professionals and a random stratified sample of 150 PTs,***-nine themes relating to LBP diagnostic practice *** were then given to 30 British PTs assessing their level of agreement with their Greek *** was performed by percentage agreements andχ2 *** The survey was divided into three subsections;PTs’attitudes on LBP assessment,patients’attitudes and diagnostic/healthcare issues,each constituting 14,7 and 8 statements,*** half of the statements fell within the 30%-80%agreement between Greece and United Kingdom whereas,5 statements reported low(90%)PT percentage *** across British and Greek PTs were detected in history taking methods and in the way PTs feel patients perceive physiotherapy practice whereas,re-assessment was undertaken less frequently in *** according to 91%of the Greek PTs is considered a“privilege”which is exclusive for doctors in Greece(only 17%British PTs agreed)and is accompanied with a great overuse of medical *** percent of Greek PTs(compared to 0%of British)consider themselves as“executers”,being unable to interfere with treatment plan,possibly implying lack of *** Although similarities on history taking methods and on patients’attitudes were detected across both groups,gross differences were found in re-assessment procedures and diagnostic issues between Greek and British physiotherapists,highlighting differences in service delivery and professional autonomy.

Pretreatment and enzymatic hydrolysis of sunflower hulls for fermentable sugar production

Sunflower is a widely adapted crop and can be grown in every temperature *** the U.S.,two million acres were cultivated with sunflowers in *** industrial processing,large quantities of hulls are obtained as a waste product from the dehulling *** study focused on converting the sunflower hulls into fermentable sugars by dilute acid pretreatment and enzymatic *** sunflower hulls are composed ofβ-glucan(34%±1.1%),lignin(25%±0.95%),xylan and arabinan(27%±1.56%),extractives(13%±2.5%)and traces of *** hulls were first subjected to pretreatment by varying three independent factors:1)acid concentration(0.5%-2%);2)reaction temperatures(140-160℃);3)reaction time(10-30 min).Slurry samples obtained after pretreatment were separated into liquid and solid *** fractions were analyzed for monomeric and oligomeric sugars and inhibitor products by High Pressure Liquid Chromatography(HPLC).Enzymatic saccharification was then performed on pretreated solid fractions to convert remaining cellulose(β-glucan)into fermentable *** results showed an increase in acid concentration and reaction temperature gave high xylose yield in the liquid ***,an increase in reaction time resulted in degradation of xylose into furfural.A quadratic model for xylose yield was formulated based on the experimental *** maximum xylose yield predicted by the model was 62%at 158℃for 20 min at 1.75%acid *** maximumβ-glucan digestibility of the enzymatic saccharification was 53.5%at 160℃for 30 min at 2%acid concentration.

Verification and Dosimetric Impact of Acuros XB Algorithm for Stereotactic Body Radiation Therapy (SBRT) and RapidArc Planning for Non-Small-Cell Lung Cancer (NSCLC) Patients

Purpose: The experimental verification of the Acuros XB (AXB) algorithm was conducted in a heterogeneous rectangular slab phantom, and compared to the Anisotropic Analytical Algorithm (AAA). The dosimetric impact of the AXB for stereotactic body radiation therapy (SBRT) and RapidArc planning for 16 non-small-cell lung cancer (NSCLC) patients was assessed due to the dose recalculation from the AAA to the AXB. Methods: The calculated central axis percentage depth doses (PDD) in a heterogeneous slab phantom for an open field size of 3 ×3 cm2 were compared against the PDD measured by an ionization chamber. For 16 NSCLC patients, the dose-volume parameters from the treatment plans calculated by the AXB and the AAA were compared using identical jaw settings, leaf positions, and monitor units (MUs). Results: The results from the heterogeneous slab phantom study showed that the AXB was more accurate than the AAA;however, the dose underestimation by the AXB (up to ?3.9%) and AAA (up to ?13.5%) was observed. For a planning target volume (PTV) in the NSCLC patients, in comparison to the AAA, the AXB predicted lower mean and minimum doses by average 0.3% and 4.3% respectively, but a higher maximum dose by average 2.3%. The averaged maximum doses to the heart and spinal cord predicted by the AXB were lower by 1.3% and 2.6% respectively;whereas the doses to the lungs predicted by the AXB were higher by up to 0.5% compared to the AAA. The percentage of ipsilateral lung volume receiving at least 20 and 5 Gy (V20 and V5 respectively) were higher in the AXB plans than in the AAA plans by average 1.1% and 2.8% respectively. The AXB plans produced higher target heterogeneity by average 4.5% and lower plan conformity by average 5.8% compared to the AAA plans. Using the AXB, the PTV coverage (95% of the PTV covered by the 100% of the prescribed dose) was reduced by average 8.2% than using the AAA. Implantable deep brain stimulation(DBS)systems are utilized for clinical treatment of diseases such as Parkinson's disease and chronic ***,long-term efficacy of DBS is limited,and chronic neuroplastic changes and associated therapeutic mechanisms are not well *** and mechanistic investigation,typically accomplished in small animal models,is difficult because of the need for chronic stimulators that currently require either frequent handling of test subjects to charge battery-powered systems or specialized setups to manage tethers that restrict experimental paradigms and compromise *** overcome these challenges,we demonstrate a fully implantable,wireless,battery-free platform that allows for chronic DBS in rodents with the capability to control stimulation parameters digitally in real *** devices are able to provide stimulation over a wide range of frequencies with biphasic pulses and constant voltage control via low-impedance,surface-engineered platinum *** devices utilize off-the-shelf components and feature the ability to customize electrodes to enable broad utility and rapid *** of the system is demonstrated with a readout of stimulation-evoked neural activity in vivo and chronic stimulation of the medial forebrain bundle in freely moving rats to evoke characteristic head motion for over 36 days.
