Background:The development and spread of artemisinin-resistant Plasmodium falciparum malaria in Greater Mekong Subregion has created impetus for continuing global monitoring of efficacy of artemisinin-based combination therapies(ACTs). This post analyses is aimed to evaluate changes in early treatment response markers 10 years after the adoption of ACTs as first-line treatments of uncomplicated falciparum malaria in ***: At 14 sentinel sites in six geographical areas of Nigeria, we evaluated treatment responses in 1341 children under 5 years and in additional 360 children under 16 years with uncomplicated malaria enrolled in randomized trials of artemether-lumefantrine versus artesunate-amodiaquine at 5-year interval in 2009-2010 and 2014-2015 and at 2-year interval in 2009-2010 and 2012-2015, respectively after deployment in ***: Asexual parasite positivity 1 day after treatment initiation (APPD1) rose from 54 to 62% and 2 days after treatment initiation from 5 to 26% in 2009-2010 to 2014-2015(P=0.002 and Phin the same time period (Ph). Enrolment parasitaemia > 75 000 μl^-1, haematocrit > 27% 1 day post-treatment initiatiortreatment with artemether-lumefantrine and enrolment in 2014-2015 independently predicted APPD1. In paralle , Kaplan-Meier estimated risk of recurrent infections by day 28 rose from 8 to 14% (P=0005) and from 9 to 15%(P=0.02) with artemether-lumefantrine and artesunate-amodiaquine, respectively. 叶芝《第二次降临》的主题框架是(世界)历史的进程。诗歌将两个不同层次的叙事序列结合在一起。一方面,在其持续尝试叙述和理解历史的发展中,抒情人的话语代表了历史进程的不同版本。他以描述秩序和正义普遍下降的剧烈变化作为开端,继而根据基督教的末世论阐释了无序状况的泛滥,以此作为世界末日和基督重临的先声,并作了最后的展望。另一方面,这些意图的延续,本身就构成了一个叙事,一个抒情人理解和叙述历史过程中认知发展的心理故事,并直接在他言说的展开中进行,参照各种脚本构架出来:首先,是历史退化的一般模式,接着,是基督教的历史观,最后,是集体无意识。在持续试图把握历史的进程中,抒情人声称至少已经对即将发生的事情有了部分了解。但这一心理过程的前景也引出了抒情人可靠性的问题。各种事件和决定性的转折,发生在两个叙事序列中,分别是对基督"第二次降临"的传统预期的骤然失望,以及对抒情人一方预言的幡然顿悟。

背景局部创伤性脑损伤大鼠过度通气4小时对其神经认知行为、运动功能、协调能力和病损大小的远期影响。方法对大鼠进行为期14天的神经认知行为学训练，包括孔板实验、木梁行走实验、木梁平衡实验。21只SD大鼠（369±15g），氟烷麻醉后气管插管，机械通气，行控制性皮质撞击损伤（深1．75mm，直径5mm，速度4m／s）。将大鼠随机分入正常通气组（n=10；Paco2=38-42mmhg）和过度通气组（n=11；Paco2=28-32mmhg），机械通气4小时。创伤后持续检测大鼠的行为学和运动能h20天。随后，将大鼠在深麻醉下断头取脑，冰冻脑组织，切片，评估病损大小。结果过度通气组与正常通气组相比外显记忆受损严重。两组大鼠在损伤初期的运动功能和协调能力的测试（木梁行走和木梁平衡实验）中表现均有欠缺，过度通气组更为严重。但是，在实验结束时，两组并无明显差异。创伤后第20天，过度通气组大鼠测得的病损大小（69．7±13．0mm^3）明显大于正常通气组（48．3±15．6mm^3）。结论创伤性脑损伤后过度通气4／1,时（Paco2=28-32mmhg）虽然增加了组织学损伤，但并没有对大鼠远期的神经认知功能造成不良影响。

本文试图从沃尔夫副校长卸任演讲的哲学背景出发,去理解沃尔夫关于依赖和独立的设想所具有的特点。本文从笛卡尔开始,首先从“依赖”概念出发指出笛卡尔和沃尔夫之间的区别。就像沃尔夫在其以数学方法的精神搭建的实践哲学框架中所做的那样,笛卡尔本来也完全可以在自己的(在1637年《谈谈方法》中发展出来的)“一套临时的道德”的框架下诉诸孔子。本文还简短讨论了康德对启示信仰所做批判的一个特殊面向,并回顾20世纪马堡神学家鲁道夫·布尔特曼的去神秘计划。与沃尔夫截然不同,布尔特曼将我们在信仰中经验到的依赖解释为克服我们的畏惧的那把钥匙。

Brain antigens in functionally distinct antigen-presenting cell populations in cervical lymph nodes in MS and EAE

Drainage of central nervous system(CNS) antigens to the brain-draining cervical lymph nodes(CLN) is likely crucial in the initiation and control of autoimmune responses during multiple sclerosis(MS). We demonstrate neuronal antigens within CLN of MS patients. In monkeys and mice with experimental autoimmune encephalomyelitis(EAE) and in mouse models with non-inflammatory CNS damage, the type and extent of CNS damage was associated with the frequencies of CNS antigens within the cervical lymph nodes. In addition, CNS antigens drained to the spinal-cord-draining lumbar lymph nodes. In human MS CLN, neuronal antigens were present in pro-inflammatory antigen-presenting cells(APC), whereas the majority of myelincontaining cells were anti-inflammatory. This may reflect a different origin of the cells or different drainage mechanisms. Indeed, neuronal antigen-containing cells in human CLN did not express the lymph node homing receptor CCR7, whereas myelin antigen-containing cells in situ and in vitro did. Nevertheless, CLN from EAE-affected CCR7-deficient mice contained equal amounts of myelin and neuronal antigens as wild-type mice. We conclude that the type and frequencies of CNS antigens within the CLN are determined by the type and extent of CNS damage. Furthermore, the presence of myelin and neuronal antigens in functionally distinct APC populations within MS CLN suggests that differential immune responses can be evoked.

Context and objective: The COVID-19 pandemic has become a major public health problem and has mobilized many innovative means of diagnosis. The Central African Republic is not spared. The emergence of variants and their impact require health monitoring despite the obligation of vaccination. The purpose of this campaign was to determine the circulation of pending second-wave variants. Patients and Methods: A second mass screening campaign took place from 02 to 22 July 2021 in the main land and river entry points of Bangui (Exit North-PK12, Exit South-PK9, Port Beach) and at the LNBCSP. Antigenic and RT-PCR tests carried out on nasopharyngeal samples made it possible to select strains which were finally sequenced. Results: Of 2687 participants included in the study, 53 (1.97%) were positive for SARS-CoV-2. Thirteen (1.53%) were male and 40 (2.18%) female. The analyses carried out on the LumiraDx analyzer were positive for 109 samples against 53 on the RT-PCR. The prevalence was higher in the most tested age groups (30 to 50 years) with two clusters identified. B.1.617.2 (Delta) variants were predominant (57%). Conclusion: SARS-CoV-2 continues to circulate. The acquisition of automated antigenic tests (LumiraDx®) with sensitivity and specificity close to those of the reference test (RT-PCR) will allow better mass diagnosis for an optimization of the surveillance of COVID-19 in our countries with limited resources. The predominance of the B.1.617.2 (Delta) variant would suggest a third wave in the Central African Republic.
