AIM： To assess the efficacy of premedicaton with pronase or N-acetylcysteine （NAC） at 20 min before upper gastrointestinal （UGI） endoscopy and to determine whether pronase or NAC pretreatment influences the reliability of the rapid urease test. METHODS： A total of 146 patients were prospectively and randomly assigned into the study groups according to different premedications before endoscopy. One endoscopist assessed mucosal visibility （MV） with scores ranged from 1 to 4 at four sites in the stomach. The sum of the MV scores from these four locations was defined as the total mucosal visibility （TMV） score. Identification of Hpylori was performed using CLO test, histology, and serology. RESULTS： The Group with pronase premedication had a significantly lower TMV score than did the groups with gascon and gascon water （P 〈 0.001 and P 〈 0.01, respectively）. The group with NAC had a significantly lower TMV score than the group with gascon （P 〈 0.01） and a trend of a lower MV score than the group with gascon water （P = 0.06）. The THV score did not significantly differ between the group with pronase and the group with NAC （P = 0.39 and P = 0.14, respectively）. The sensitivity and specifidty of the CLO test were 92.5% and 93.9%, respectively, in groups premedicated with pronase and NAC together.CONCLUSION： Premedication with pronase or NAC at 20 min before UGI endoscopy improves the mucosal visibility of the stomach. Neither pronase nor NAC produces any obvious interference with the CLO test for the identification of H pylori infection.

Objective:To investigate whether major dengue outbreaks in the last two decades in Kaohsiung follow a precise temporal ***:Government daily lab-confirmed dengue case data from three major dengue outbreaks occurring during the last two decades in Kaohsiung in2002,2014 and 2015,is utilized to compute the corresponding weekly cumulative percentage of total case *** divide each of the three time series data into two periods to examine the corresponding weekly cumulative percentages of case numbers for each ***’s correlation coefficient was calculated to compare quantitatively the similarity between the temporal patterns of these three ***:Three cutoff points produce the most interesting comparisons and the most different ***’s correlation coefficient indicates quantitative discrepancies in the similarity between temporal patterns of the three years when using different cutoff ***:Temporal patterns in 2002 and 2014 are comparatively more similar in early *** 2015 outbreak started late in the year,but ended more like the outbreak in 2014,both with record-breaking number of *** retrospective analysis shows that the temporal dynamics of dengue outbreaks in Kaohsiung can strongly vary from one year to another,making it difficult to identify any common predictor.

Background and Aims:Early detection of hepatocellular carcinoma(HCC)is crucial for improving survival in patients with chronic *** GALAD algorithm combines gen-der(biological sex),age,α-fetoprotein(AFP),Lens culinaris agglutinin-reactive fraction of AFP(AFP-L3),and protein in-duced by vitamin K absence or antagonist-II(PIVKA-II)for HCC ***,the GAAD algorithm incorporates gender(biological sex),age,AFP,and *** study aimed to assess the clinical utility of AFP-L3 in the GALAD algorithm and its potential synergies with *** compared the clinical performance of GALAD with GAAD;AFP;AFP-L3;and PIVKA-II,with or without ultrasound,in Taiwan residents ***:A total of 439 serum samples were analyzed using a Cobas®e 601 analyzer(healthy con-trols,n=200;chronic liver disease controls,n=177;HCC cases,n=62).Performance was assessed through receiver operating characteristic curve analyses to calculate the area under the ***:The area under the curve for dif-ferentiating early-stage HCC from patients with chronic liver disease was optimal for PIVKA-II(84.9%),GAAD(79.8%),and GALAD(79.4%),with slightly improved performance for detecting all-stage *** performance was unaffected by disease stage or *** for early-stage HCC was highest for GAAD(57.6%)and GALAD(57.6%).Sen-sitivity for each strategy was further enhanced when com-bined with ultrasound,regardless of disease stage or etiology(P<0.01).Conclusions:These findings indicate that the role of AFP-L3 in the GALAD algorithm is minimal,supporting the use of GAAD for HCC detection.A combination of GAAD,GALAD,or PIVKA-II with ultrasound may improve diagnostic efficiency compared with recommended strategies.

The experimental study described in this paper is to investigate the control of thermal nitrogen oxides emissions from a 2.28 MW gas-fired test furnace. Tests, including changing axial or radial air flow rate, adding cooling water, and adding staged air, were performed to characterize and opimize the fuel-rich burning zone and the fuel-lean burnout zone independently. Detailed measurements of O_2, CO_2, CO, NO and NOx were made at the fuel-rich burning zone and furnace exit. The influence of forming CO, NO and NOx was examined. Results indicated that adding staged air in the fuel-rich burning zone (75 cm from burner) will reduce the mawximum NO and NO_x emissions. Adding cooling water in a right position may further lower the NO and NO_x emissions. In addition, the least formation of thermal nitrogen oxides in the first stooge fuel-rich bunting zone will occur at the stoichiometric ratio's inverse value, (φ_1)^(-1), 0.65 to 0.7.

Aim:Despite the high cure rate of interferon-free directly acting antivirals(DAAs)for chronic hepatitis C(CHC)patients,the treatment efficacy for patients with preexisting hepatocellular carcinoma(HCC)remains *** aimed in the present study to address the issue by using novel DAAs in treating CHC patients who were adherent to treatment in ***:CHC patients with or without HCC were consecutively *** primary objective was sustained virological response(SVR)defined as undetectable HCV RNA throughout 12 weeks of a post-treatment follow-up period(SVR12).Only patients with available SVR12 were enrolled for final ***:A total of 1237 patients(1113 non-HCC,101 inactive HCC and 23 active HCC)were *** overall SVR12 rate was 98.9%,and was similar between HCV patients with and without pre-existing HCC(98.4%vs.98.9%,P=0.64).While HCC patients were classified as those who had active or inactive HCC,the SVR12 was also similar between patients with and without active HCC(95.7%vs.99.0%,P=0.34).Among the 101 patients without viable HCC at the time of DAA initiation,eighty-four patients exhibited curative therapy and the other 17 ;patients experienced HCC recurrence before *** the 23 patients with viable HCC at the time of DAA treatment,10 patients had received curative therapy for HCC whereas the remaining 13 patients had HCC that was never *** SVR12 rates were also similar among the four subpopulations,being 98.8%(83/84),100%(17/17),90%(9/10)and 100%(13/13)***:CHC patients with HCC who were adherent to potent DAAs achieved similar SVR12 rate compared to those without HCC and could be effectively treated.
