An enhanced functional ability questionnaire(faVIQ) to measure the impact of rehabilitation services on the visually impaired

AIM: To develop a short,enhanced functional ability Quality of Vision(faVIQ) instrument based on previous questionnaires employing comprehensive modern statistical techniques to ensure the use of an appropriate response scale,items and scoring of the visual related difficulties experienced by patients with visual impairment. · METHODS: Items in current quality-of-life questionnaires for the visually impaired were refined by a multi-professional group and visually impaired focus groups. The resulting 76 items were completed by 293 visually impaired patients with stable vision on two occasions separated by a month. The faVIQ scores of 75 patients with no ocular pathology were compared to 75 age and gender matched patients with visual impairment. ·RESULTS: Rasch analysis reduced the faVIQ items to 27. Correlation to standard visual metrics was moderate(r =0.32-0.46) and to the NEI-VFQ was 0.48. The faVIQ was able to clearly discriminate between age and gender matched populations with no ocular pathology and visual impairment with an index of 0.983 and 95% sensitivity and 95% specificity using a cut off of 29.·CONCLUSION: The faVIQ allows sensitive assessment of quality-of-life in the visually impaired and should support studies which evaluate the effectiveness of low vision rehabilitation services.

AIM To investigate the functional effects of abnormal esophagogastric(EGJ) measurements in asymptomatic healthy volunteers over eighty years of age. METHODS Data from 30 young controls(11 M, mean age 37 ± 11 years) and 15 aged subjects(9 M, 85 ± 4 years) were compared for novel metrics of EGJ-function: EGJcontractile integral(EGJ-CI), "total" EGJ-CI and bolus flow time(BFT). Data were acquired using a 3.2 mm, 25 pressure(1 cm spacing) and 12 impedance segment(2 cm) solid-state catheter(Unisensor and MMS SolarGI system) across the EGJ. Five swallows each of 5 m L liquid(L) and viscous(V) bolus were analyzed. Mean values were compared using Student's t test for normally distributed data or Mann Whitney U-test when non-normally distributed. A P value < 0.05 was considered *** EGJ-CI at rest was similar for older subjects compared to controls. "Total" EGJ-CI, measured during liquid swallowing, was increased in older individuals when compared to young controls(O 39 ± 7 mm *** vs C 18 ± 3 mm ***; P = 0.006). For both liquid and viscous bolus consistencies, IRP4 was increased(L: 11.9 ± 2.3 mm Hg vs 5.9 ± 1.0 mm Hg, P = 0.019 and V: 14.3 ± 2.4 mm Hg vs 7.3 ± 0.8 mm Hg; P = 0.02) and BFT was reduced(L: 1.7 ± 0.3 s vs 3.8 ± 0.2 s and V: 1.9 ± 0.3 s vs 3.8 ± 0.2 s; P < 0.001 for both) in older subjects, when compared to young. A matrix of bolus flow and presence above the EGJ indicated reductions in bolus flow at the EGJ occurred due to both impaired bolus transport through the esophageal body(i.e., the bolus never reached the EGJ) and increased flow resistance at the EGJ(i.e., the bolus retained just above the EGJ).CONCLUSION Bolus flow through the EGJ is reduced in asymptomatic older individuals. Both ineffective esophageal bolus transport and increased EGJ resistance contribute to impaired bolus flow.

Diurnal variation in fecal concentrations of acid-detergent insoluble ash and alkaline-peroxide lignin from cattle fed bermudagrass hays of varying nutrient content

Background： The effect of time of fecal sampling on the accuracy of acid-detergent insoluble ash （ADIA） and alkaline-peroxide lignin （APL） for the prediction of fecal output （FO） in cattle was evaluated. Eight ruminally cannulated cows （594 _＋ 35.5 kg） were allocated randomly to 4 bermudagrass [Cynodon dactylon （L.） Pers.] hay diets markedly different in crude protein concentration （79-164 g/kg） with 2 replicates per diet for 3 periods. Cows were offered hay individually at 20 g DM/kg of body weight daily in equal feedings at 08：00 and 16：00 h for a 10-d adaptation period followed by 5-d of total fecal collection. Fecal grab samples also were taken each day during the fecal collection period at 06：00, 12：00, 18：00, and 24：00 h either directly from the rectum or from freshly voided feces. Samples were composited within cow and time across the 5 d total fecal collection period. Additionally, forage, ort, and fecal samples were analyzed for concentrations of APL and ADIA. Results： Fecal concentrations of ADIA and APL were not affected by sampling time （P 〉 0.22）, even though diet affected （P 〈 0.01） fecal ADIA and APL concentrations. There were no diet x sampling time interactions （P ≥ 0.60）. Estimates of FO and dry matter digestibility （DMD） from ADIA and APL were not affected （P 〉 0.16） by sampling time or the diet x sampling time interaction （P 〉 0.74）. Estimates of FO and DMD from markers from different sampling times or all different combinations of sampling time were not different （P 〉 0.72） from those of total collection among internal markers. Conclusion： Little variation in concentrations of ADIA and APL in daily fecal excretion across time increases flexibility in fecal grab sampling schedules for predicting FO and DMD.

The aim of the present study was the in vitro and in vivo analysis of a bi-layered 3D-printed scaffold combining a PLA layer and a biphasic PLA/bioglass G5 layer for regeneration of osteochondral defects in vivo Focus of the in vitro analysis was on the(molecular)weight loss and the morphological and mechanical variations after immersion in *** in vivo study focused on analysis of the tissue reactions and differences in the implant bed vascularization using an established subcutaneous implantation model in CD-1 mice and established histological and histomorphometrical *** scaffold parts kept their structural integrity,while changes in morphology were observed,especially for the PLA/G5 *** properties decreased with progressive degradation,while the PLA/G5 scaffolds presented higher compressive modulus than PLA *** tissue reaction to PLA included low numbers of BMGCs and minimal vascularization of its implant beds,while the addition of G5 lead to higher numbers of BMGCs and a higher implant bed *** revealed that the use of a bi-layered scaffold shows the ability to observe distinct in vivo response despite the physical proximity of PLA and PLA/G5 ***,the results showed that the addition of G5 enables to reduce scaffold weight loss and to increase mechanical ***,the addition of G5 lead to a higher vascularization of the implant bed required as basis for bone tissue regeneration mediated by higher numbers of BMGCs,while within the PLA parts a significantly lower vascularization was found optimally for chondral ***,this data show that the analyzed bi-layered scaffold may serve as an ideal basis for the regeneration of osteochondral tissue ***,the results show that it might be able to reduce the number of experimental animals required as it may be possible to analyze the tissue response to more than one implant in one experimental animal.

BACKGROUND:Ectopic pancreas is defined as pancreatic tissue found outside its usual anatomical position,with no ductal or vascular communication with the native pancreas. We describe a case of ectopic pancreas of the small bowel and mesentery causing recurrent episodes of pancreatitis, initially suspected on computed tomography(CT)and magnetic resonance cholangiopancreatography(MRCP),and confirmed on histological review of the resection. METHODS:A 67-year-old woman presented with clinical symptoms and biochemical evidence of *** had similar episodes over the past 30 years with unrevealing investigations,and was concluded to have idiopathic pancreatitis. She underwent CT and MRCP,with findings suggestive of ectopic pancreas,a diagnosis confirmed on histology of the resection. RESULTS:MRCP identified a mass in the proximal small bowel mesentery isointense to the native pancreas,with a small duct draining into a proximal jejunal *** resected specimen consisted of normal parenchyma with lobulated acinar tissue with scattered islets of Langerhans,an occasional ductular structure,and admixed areas of adipose *** patient remained asymptomatic with normal biochemistry six months post-operatively. CONCLUSION:In an individual with abdominal pain,elevated serum amylase/lipase,but imaging findings of a normal native pancreas,ectopic pancreatitis should be considered,and can be evaluated by CT and MRCP.

Is analysis of lower esophageal sphincter vector volumes of value in diagnosing gastroesophageal reflux disease?

AIM: With successful surgical treatment of gastroesophagealreflux disease (GERD), there is interest in understandingthe anti-reflux barrier and its mechanisms of failure. To date,the potential use of vector volumes to predict the DeMeesterscore has not been adequately ***: 627 patients in the referral database receivedesophageal manometry and ambulatory 24-hour pHmonitoring. Study data included LES resting pressure (LESP),overall LES length (OL) and abdominal length (AL), totalvector volume (TW) and intrabdominal vector volume (IVV).RESULTS: In cases where LESP, TVV or IVV were all belownormal, there was an 81.4 % probability of a positiveDeMeester score. In cases where all three were normal,there was an 86.9 % probability that the DeMeester scorewould be negative. Receiver-operating characteristics (ROC)for LESP, TW and IW were nearly identical and indicatedno useful cut-off values. Logistic regression demonstratedthat LESP and IVV had the strongest association with apositive DeMeester score; however, the regression formulawas only 76.L % ***: While the indices based on TVV, IW andLESP are more sensitive and specific, respectively, than anysingle measurement, the measurement of vector volumesdoes not add significantly to the diagnosis of GERD.

Comparison of acid-detergent lignin,alkaline-peroxide lignin,and acid-detergent insoluble ash as internal markers for predicting fecal output and digestibility by cattle offered bermudagrass hays of varying nutrient composition

Background： The potential for acid-detergent insoluble ash （ADIA）, alkaline-peroxide lignin （APL）, and acid-detergent lignin （ADL） to predict fecal output （FO） and dry matter digestibility （DMD） by cattle offered bermudagrass [Cynodon doctylon （L） Pers.] hays of different qualities was evaluated. Eight ruminally cannulated cows （594 ± 35.5 kg） were allocated randomly to 4 hay diets： low （L）, medium low （ML）, medium high （MH）, and high （H） crude protein （CP） concentration （79, 111,131, and 164 g CP/kg on a DM basis, respectively）. Diets were offered in 3 periods with 2 diet replicates per period and were rotated across cows between periods. Cows were individually fed 20 g DM/kg of body weight in equal feedings at 08：00 and 16：00 h for a 10-d adaptation followed by a 5-d total fecal collection. Actual DM intake （DMI）, DMD, and FO were determined based on hay offered, ort, and feces excreted. These components were then analyzed for ADL, APL, and ADIA concentration to determine marker recovery and marker-based estimates of FO and DMD. Results： Forage DMI was affected by diet （P = 0.02）, and DMI from MH and H was greater （P 〈 0.05） than from L. Apparent DMD tended （P = 0.08） to differ among diets while FO （P = 0.20） was not affected by diet treatments. Average ADL recovery （1.16） was greater （P 〈 0.05） than that of ADIA （1.03） and APL （1.06）, but ADIA and APL did not differ （P = 0.42）. Estimates of FO and DMD derived using APL and ADIA were not different （P≥0.05） from total fecal collection while those using ADL differed （P 〈 0.05）. There was no diet by marker interaction （P≥ 0.22） for either FO or DMD. Conclusion： Acid-detergent insoluble ash and APL accurately predicted FO and DMD of cattle fed bermudagrass hay of varying nutrient composition. These internal markers may facilitate studies involving large numbers of animals and forages. Previously we established Zygote Electroporation of Nucleases（ZEN） technology as an efficient and high-throughput way to generate genetically modified mouse ***,there were significant variations of the targeting efficiency among different genomic loci using our previously published *** this study,we improved the ZEN technology by delivering Cas9 protein into mouse zygotes through a series of *** this approach,we were able to introduce precise nucleotide substitutions,large segment deletion and short segment insertion into targeted loci with high efficiency.

Carbon star CGCS 673 identified as a semi-regular variable star

This study reports that the carbon star CGCS 673 is a semi-regular(SR)variable star with a period of 135 d and an amplitude of 0.18mag in the *** light curve obtained by this study correlates well with the SR classification as the photometric data obtained show noticeable periodicity in the light changes of CGCS 673 that is occasionally interrupted by a period of irregular *** derived period and colour index obtained from our data and those from professional databases indicate that the attributes of this star fall within the parameters of the SR class of variable *** our notification of the discovery that this star is a variable source,CGCS 673 has received the AAVSO Unique Identifier of(AAVSO UID)000-BMZ-492.
