Effects of Health Record Computerization in the Context of the COVID-19 Pandemic in Burundi’ Hospitals: Propensity Score Matching

Introduction: A great number of software are currently used to digitalize the patient records in order to optimize the quality of services offered to patients. The obj.ctive of this study was to evaluate the effects of Electronic Health Records use in Burundi’s hospitals, taking into account the COVID-19 pandemic context. Methods: This was a quasi-experimental study based on difference in difference method. Ten district hospitals were included in the study, five of them had the Electronic Health Records and five of which did not yet have the Electronic Health Records. The hospital’s control group were chosen using the propensity score matching method. The period before the proj.ct’s implementation was 2014 and the period after were 2019 and 2020. Results: After 5 years of the Electronic Health Record’s implementation, the results showed an increase in outpatient consultation (70%), deliveries (more than 100%), caesarean sections (56%) and maj.r surgeries (43%) indicators. The overall quality score of hospitals’ care had a regressive effect of 37% and the income from performance-based funding had an increase by 31%. The indicators which were affected by the context of the COVID-19 pandemic were especially outpatient consultation, caesarian section, income from performance-based funding decreased by 3%, 5% and 20% respectively. Conclusion: The effects of Electronic Health Records use are effective. As the COVID-19 pandemic impacted the hospital’s indicators negatively, the resilient strategies alongside the potential shocks are recommended.

Assessing Community Health Interventions in Reducing Infant and Child Mortality in the Southeastern Benin

Background: To reduce infant and child mortality in Benin, a package of high-impact interventions per healthcare level was implemented in 2009. This study aimed to assess the quality of community-based health interventions in reducing infant and child mortality within the municipality of Pobè in southeastern Benin. Methods: This was a cross-sectional evaluative study carried out in November 2021 focused on children aged 0 - 59 months, their mothers, health workers, community facilitators, community health workers and the Town Hall health focal point. Mothers and their children were targeted by cluster sampling, and exhaustive selection was used to recruit all other participants. Predetermined scores based on rating criteria were used to assess the quality of community health interventions using the “input, process and outcome” of Donabedian approach. Results: Over 300 mother-child couples, 46 community health workers, 7 health agents, 1 community facilitator and 1 health focal point from Pobè town hall were surveyed. Intervention quality was j.dged as “average”, with a score of 73.80%. The “inputs” and “outcomes” components were the weakest links. Conclusion: Improving access to the inputs needed by community health workers can enhance the quality of PIHI interventions.

Background:The mobilization and redistribution of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)specific T-cells and neutralizing antibodies(nAbs)during exercise is purported to increase immune surveillance and protect against severe coronavirus disease 2019(COVID-19).We sought to determine if COVID-19 vaccination would elicit exercise-responsive SARS-CoV-2 T-cells and transiently alter nAb ***:Eighteen healthy participants completed a 20-min bout of graded cycling exercise before and/or after receiving a COVID-19 *** maj.r leukocyte subtypes were enumerated before,during,and after exercise by flow cytometry,and immune responses to SARS-CoV-2 were determined using whole blood peptide stimulation assays,T-cell receptor(TCR)-βsequencing,and SARS-CoV-2 nAb ***:COVID-19 vaccination had no effect on the mobilization or egress of maj.r leukocyte subsets in response to intensity-controlled graded ***,non-infected participants had a significantly reduced mobilization of CD4+and CD8+naive T-cells,as well as CD4+central memory T-cells,after vaccination(synthetic immunity group);this was not seen after vaccination in those with prior SARS-CoV-2 infection(hybrid immunity group).Acute exercise after vaccination robustly mobilized SARS-CoV-2 specific T-cells to blood in an intensity-dependent *** groups mobilized T-cells that reacted to spike protein;however,only the hybrid immunity group mobilized T-cells that reacted to membrane and nucleocapsid *** increased significantly during exercise only in the hybrid immunity ***:These data indicate that acute exercise mobilizes SARS-CoV-2 specific T-cells that recognize spike protein and increases the redistribution of nAbs in individuals with hybrid immunity.

The secondary inj.ry cascade after spinal cord inj.ry: an analysis of local cytokine/chemokine regulation

After spinal cord inj.ry, there is an extensive infiltration of immune cells, which exacerbates the inj.ry and leads to further neural degeneration. Therefore, a maj.r aim of current research involves targeting the immune response as a treatment for spinal cord inj.ry. Although much research has been performed analyzing the complex inflammatory process following spinal cord inj.ry, there remain maj.r discrepancies within previous literature regarding the timeline of local cytokine regulation. The obj.ctives of this study were to establish an overview of the timeline of cytokine regulation for 2 weeks after spinal cord inj.ry, identify sexual dimorphisms in terms of cytokine levels, and determine local cytokines that significantly change based on the severity of spinal cord inj.ry. Rats were inflicted with either a mild contusion, moderate contusion, severe contusion, or complete transection, 7 mm of spinal cord centered on the inj.ry was harvested at varying times post-inj.ry, and tissue homogenates were analyzed with a Cytokine/Chemokine 27-Plex assay. Results demonstrated pro-inflammatory cytokines including tumor necrosis factor α, interleukin-1β, and interleukin-6 were all upregulated after spinal cord inj.ry, but returned to uninj.red levels within approximately 24 hours post-inj.ry, while chemokines including monocyte chemoattractant protein-1 remained upregulated for days post-inj.ry. In contrast, several anti-inflammatory cytokines and growth factors including interleukin-10 and vascular endothelial growth factor were downregulated by 7 days post-inj.ry. After spinal cord inj.ry, tissue inhibitor of metalloproteinase-1, which specifically affects astrocytes involved in glial scar development, increased more than all other cytokines tested, reaching 26.9-fold higher than uninj.red rats. Further Studies of the Cap Pushing Response in Honey Bees (Apis mellifera)

The Cap Pushing Response (CPR) is a free-flying technique used to study learning and memory in honey bees (Apis mellifera). The series of experiments outlined in this paper aimed to test whether honey bees exhibit the cognitive concept of “expectancy” utilizing the CPR in a weight differentiation paradigm. Five previous experiments in our laboratory have explored whether the concept of expectancy can account for honey bee performance and have all failed to support the cognitive interpretation. The first experiment examined if bees could differentiate between the two caps in the amount of force they used to push the cap and the distance the cap was pushed when the caps were presented one at a time. The second experiment explored cap weight preference by presenting bees with a choice between the two caps. The third and fourth experiments tested the bee’s ability to expect reward or punishment based on cap weight. Results revealed that bees were found to have a strong preference for the light cap and therefore were not able to expect reward or punishment based on cap weight. These experiments contribute to the debate on whether bees have “cognitive” representations and continue to support the behaviorist interpretation.

Comparative analysis of microstructure,mechanical,and corrosion properties of biodegradable Mg-3Y alloy prepared by selective laser melting and spark plasma sintering

This work explored possibilities of biodegradable magnesium alloy Mg-3Y preparation by two modern powder metallurgy techniques–spark plasma sintering(SPS)and selective laser melting(SLM).The powder material was consolidated by both methods utilising optimised parameters,which led to very low porosity(∼0.3%)in the SLM material and unmeasurably low porosity in the SPS *** main aim of the study was the thorough microstructure characterisation and interrelation between the microstructure and the functional properties,such as mechanical strength,deformability,and corrosion *** materials showed comparable strength of∼110 MPa in tension and compression and relatively good deformability of∼9%and∼21%for the SLM and SPS materials,*** corrosion resistance of the SPS material in 0.1 M NaCl solution was superior to the SLM one and comparable to the conventional extruded *** digital image correlation during loading and the cross-section analysis of the corrosion layers revealed that the residual porosity and large strained grains have the dominant negative effect on the functional properties of the SLM *** the other hand,one of the primary outcomes of this study is that the SPS consolidation method is very effective in the preparation of the W3 biodegradable alloy,resulting in material with convenient mechanical and degradation properties that might find practical applications.

Floristic Diversity, Stand Structure and Plant Life Traits in the Forest-Savanna Mosaic at Ndj.le (Centre Cameroon)

With the aim of assessing floristic diversity, stand structure and vegetation ecology, the present work was carried out in the savanna mosaic forest of the locality of Ndj.le (Centre Cameroon). A total of 30 floristic plots of 40 m × 40 m (1600 m2) were delineated in the different ecosystems (forest, forest-savanna contact zone and the savanna). Within each plot, all individuals with a diameter at breast height ≥ 10 cm at 1.30 m were taken into account. ANOVA was used to compare the means of the diversity indices of the different ecosystems, and the DUNCAN test at 5% significance level (XLSTAT 2016) to separate these means. A total of 85 species belonging to 67 genera and 33 families were recorded. That is, 68 species in semi-caducifolia forest, 45 in forest-savanna contact zone and 18 in savanna. Urticaceae, Combretaceae, Apocynaceae, Myristicaceae and Euphorbiaceae are the most diverse families in the forest. On the other hand, in contact zones, the Euphorbiaceae, Fabaceae, Annonaceae and Moraceae families are the most diversified, while in the savannas, the Combretaceae, Fabaceae and Annonaceae families are the most diversified. Shannon’s diversity index values (1.7 to 2.37 bits) reflect low specific diversity;Piélou’s equitability (0.86 to 0.9) shows good distribution of individuals within species. The diametric structure of woody species shows an inverted “j. shape in all plant communities. This indicates a high regeneration potential characterized by a greater number of small trees than large trees. Mesophanerophytes and megaphanerophytes are dominant in forests and forest-savanna contact zones, and microphanerophytes in the savannas. Endozoochory is the main mode of diaspore dissemination. As regards the distribution of phytogeographical types, it shows the overall dominance of Guineo-Congolese species in all plant communities. Prevalence and Predictors of Double Burden of Malnutrition within Households in Africa: A Systematic Review

In recent years, there has been growing interest in the emergence of double burden of malnutrition (DBM) in Africa. In this study, we reviewed the literature on double burden of malnutrition in households, reviewing previous studies investigating the prevalence of DBM in Africa and the factors associated with it. To identify relevant studies, we consulted the PubMed and Cochrane electronic databases, using specific search terms. A total of seventeen articles met the eligibility criteria. These articles were published between 2012 and 2022, and their data were collected between 2000 and 2019. Twelve of these studies used secondary data, including demographic and health surveys. The age of children and adults varied from study to study. All studies used Body Mass Index as a nutritional indicator for adults. For children, the height-for-age Z-score was most commonly used, while weight-for-age, weight-for-height and Body Mass Index-for-age were less commonly used. The national prevalence of double nutritional burden in households ranged from 1.71% to 38.7%, depending on the country and the year. However, direct comparisons between studies were limited due to differences in combinations of undernutrition, overweight or obesity. Among the factors associated with double nutritional burden within households, the most frequently cited in the selected articles were urban/rural residence, income or socioeconomic status, age of child and mother, household size and mother’s level of education. However, no study assessed physical activity, and very few examined the diet of household members. It is essential to take these different parameters into account when designing and implementing interventions to prevent the DBM in Africa. Community and societal factors will also need to be studied and taken into account in these interventions.
