对链球菌性咽炎的WHO临床诊断规则的评价

Aims: To prospectively assess the WHO clinical decision rule (CDR) for group A beta haemolytic streptococcal (GABHs)-pharyngitis in three countries. Methods: A prospective, observational cohort study in urban outpatient clinics in Rio de Janeiro, Cairo, and Zagreb. There were 2225 children aged 2-12 years with cough, rhinorrhoea, red or sore throat; 1810 of these with sore throat were included in the analysis. Results: The proportion of children presenting with sore throat and found to have GABHs pharyngitis ranged from 24.6% (Brazil) to 42.0% (Croatia). WHO CDR sensitivity was low for all sites in both age groups. In children age 5 or older, sensitivity ranged from 3.8% in Egypt to 10.8% in Brazil. In children under 5, sensitivity was low(0.0-4.6% ) specificity was high in both age groups in all countries (93.8-97.4% ). Conclusions: In these populations, the current WHO CDR has high specificity, but low sensitivity; it did not detect up to 96.0% of children who have laboratory confirmed GABHs pharyngitis. A CDR with higher sensitivity should be developed for use in regions where rheumatic fever and rheumatic heart disease are still major health problems.

In this White Paper we present the potential of the enhanced X-ray Timing and Polarimetry(eXTP) mission for studies related to Observatory science targets. These include flaring stars, supernova remnants, accreting white dwarfs, low and high mass X-ray binaries, radio quiet and radio loud active galactic nuclei, tidal disruption events, and gamma-ray bursts. eXTP will be excellently suited to study one common aspect of these objects: their often transient nature. Developed by an international Consortium led by the Institute of High Energy Physics of the Chinese Academy of science, the eXTP mission is expected to be launched in the mid 2020s.

Background: Histological studies on the human optic nerve have documented decreasing axonal nerve fiber counts with age. In patients with optic atrophy, a nonpathological dropout of ganglion cell axons as part of the normal aging process may become clinically significant. Objective: To describe the occurrence of delayed visual loss in patients with presumably “ stable” optic neuropathy. Methods: We reviewed the medical records of 3 patients who experienced slowly progressive visual loss in adulthood after suffering childhood optic nerve injury. Results: All 3 patients had a monophasic illness in childhood that caused bilateral optic atrophy and visual impairment. Following decades of stability, each suffered a gradual, symptomatic visual decline that extended over years. No new ophthalmologic, systemic, or neurologic disorder was found that explained the visual decline in any of these patients. Conclusion: We hypothesize that the late visual decline in these 3 patients resulted from deleterious effects of age-related axonal loss on an already depleted population of neurons.

Objective: To evaluate a clinical classification system,the Clinical Age-Related Maculopathy staging (CARMs) system,for age-related maculopathy (ARM) using a simple grading scale designed for clinical practice and clinical research protocols. Participants: Two hundred forty-six male and female participants with various stages of ARM who were enrolled during the first 4 years of a longitudinal study were selected for the evaluation of the CARMs system. Design: Cross-sectional comparison study. Methods: The CARMs system divides patients into 5 mutually exclusive categories based on slit-lamp assessment of drusen,retinal pigment epithelial irregularities,geographic atrophy,retinal pigment epithelial detachment,and choroidal neovascularization. Fundus photographs and clinical data of the subjects were used to evaluate this scale. Clinical grades assigned for 492 eyes of 246 patients with varying stages of ARMwere compared with grades obtained from photographs evaluated by a reading center. To compare grades obtained from an inexperienced grader with those of an experienced grader,observations based on photographs from 50 randomly selected patients were reviewed. To quantify intraobserver agreement for photographic grades,observations from one observer were compared with those made at a later date by the same observer. Main Outcome Measures: Reliability and validity of the CARMs system in grading various stages of ARM based on clinical examination and photography. Results: The degree of overall agreement between the clinically assigned grade and photographic assessment for 492 eyes was substantial (exact overall agreement,75% ; unweighted κ ,0.63; weighted κ ,0.78). For advanced age-related macular degeneration,the sensitivity was 0.83,and specificity was 0.97. Transgrediens et progrediens palmoplantar keratoderma, known as Greither’ s syndrome,was originally described in 1952 and is characterized by diffuse keratoderma of the palms and soles,extending to the back aspects(transgrediens)and involving the skin over the Achilles’ tendon. Patchy hyperkeratosis also develops on the shins,knees,elbows,and sometimes on the skin flexures. We describe two unrelated families affected with Greither’ s syndrome, in which the same dominant missense mutation gave rise to the amino acid change N188s in K1. The previously reported cases of Greither’ s syndrome showed phenotypic variability suggestive of different underlying gene defects. Our findings suggest that at least some cases of Grei- ther’ s syndrome are caused by keratin mutations.

Objective: To evaluate the effect of treatment by lamina puncture, a novel procedure to create a perivascular opening within the optic nerve head by a transvitreal approach, on visual acuity after central retinal vein occlusion (CRVO) in older patients. Methods: The patients comprised a nonrandomized, consecutive, interventional case series of older patients being seen with CRVO. Patients 65 years or older with CRVO and a visual acuity of 20/200 or worse were treated with vitrectomy and lamina puncture of the optic disc. Preoperative visual acuity, clinical examination results, and fluorescein angiography results were compared with postoperative results. Results: Twenty patients (12 men and 8 women), an average age of 72 years, were enrolled. The mean duration of CRVO was 5.4 months; 14 eyes had nonischemic CRVO when first seen, while 6 had substantial ischemia. The mean preoperative visual acuity was in the counting fingers range,and the mean postoperative visual acuity was also in the counting fingers range. Complications included 5 eyes with iris neovascularization, of which 4 progressed to neovascular glaucoma; also, preoperative ischemia seemed to predispose to neovascular complications. Conclusion: Lamina puncture does not restore visual acuity in older patients with CRVO.
