The study investigated the improvement of slowly digestible starch fraction of cardaba banana via octenyl succinic anhydride(OSA)modification process.A nonlinear(Response surface methodology[RSM]and artificial neural network[ANN])and linear(partial least square[PLS])modelswere employed and their predictabilitywas *** result revealed that all the modelling techniques were accurate in predicting the experimental *** optimized RSM values for the production of slowly digestible starch(SDS)fraction were OSA concentration of 4%,reaction time of 47.49 min,and pH of 10 with a predicted SDS value of 44.64%.Among the modelling techniques,ANNwas adjudged as the predictivemodel for improving the SDS *** regression coefficient coupled with the variable important in the projection(VIP)values of the PLS model indicated that the OSA concentrationwas themost important factors responsible for high SDS ***,a structural comparison of the optimized starch against native starch revealed the formation of high ordered crystalline structure of the starch due to the impregnation of the modifying agent to the hydroxyl group of the cardaba banana starch.

AIM:To present statistical tools to model and optimize the cost of a randomized clinical trial as a function of the stringency of patient inclusion ***: We consider a two treatment, dichotomous outcome trial that includes a proportion of patients who are strong responders to the tested intervention. Patients are screened for inclusion using an arbitrary number of test results that are combined into an aggregate suitability score. The screening score is regarded as a diagnostic test for the responsive phenotype, having a specific cutoff value for inclusion and a particular sensitivity and specificity. The cutoff is a measure of stringency of inclusion criteria. Total cost is modeled as a function of the cutoff value, number of patients screened, the number of patients included, the case occurrence rate, response probabilities for control and experimental treatments, and the trial duration required to produce a statistically significant result with a specified power. Regression methods are developed to estimate relevant model parameters from pilot data in an adaptive trial design. RESULTS: The patient numbers and total cost are strongly related to the choice of the cutoff for inclusion. Clear cost minimums exist between 5.6 and 6.1 on arepresentative 10-point scale of exclusiveness. Potential cost savings for typical trial scenarios range in millions of dollars. As the response rate for controls approaches 50%, the proper choice of inclusion criteria can mean the difference between a successful trial and a failed trial. CONCLUSION: Early formal estimation of optimal inclusion criteria allows planning of clinical trials to avoid high costs, excessive delays, and moral hazards of Type II errors.

BACKGROUND: Hepatic abscess secondary to Salmonella is extremely rare in the United States and other Western countries. METHODS: A 43-year-old Caucasian man, with a history of chronic alcohol abuse, was admitted to the hospital for intermittent abdominal pain, fever and diarrhea. Clinical radiological and laboratory results were analyzed. Medical literature in PubMed pertaining to similar cases was reviewed. RESULTS: Stool culture was positive for Salmonella serotype B and a CT scan of the abdomen with contrast was consistent with a solitary hepatic abscess. Appropriate intravenous antibiotics followed by oral maintenance therapy for six weeks resulted in a complete clinical recovery and radiographic resolution. CONCLUSIONS: The cause of Salmonella hepatic abscess in our patient was most likely associated with decreased mucosal resistance to the bacteria, seeding of infection via transient portal bacteremia and loss of host immunity. Our case highlights the fact that appropriate antibiotic alone is sufficient in the management of a solitary hepatic abscess less than 3-5 cm.

Background:The transition metal copper enhances amyloidβaggregation and neurotoxicity,and in models of concomitant amyloid and tau pathology,copper also promotes tau *** it is not clear if the effects of environmental copper upon tau pathology are dependent on the presence of pathological amyloidβ,we tested the effects of copper overload and complexing in disease models which lack pathological amyloidβ.Methods:We used cell culture and transgenic murine models to test the effects of environmental copper on tau *** used oral zinc acetate as a copper lowering agent in mice and examined changes in blood and brain metals through inductively coupled plasma mass *** effects of copper lowering were assessed with Morris water maze and novel object recognition *** in tau phosphorylation were examined by phosphorylation specific antibodies on Western ***:In human neuroblastoma cells,excess copper promoted tau phosphorylation and a copper complexing agent,tetrathiomolybdate,attenuated tau *** a transgenic mouse model expressing wild type human tau,copper-lowering by oral zinc suppressed plasma and brain levels of copper,and resulted in a marked attenuation of tau *** significant changes in behavior were observed with copper lowering,but a trend to improved recognition of the novel object was observed in zinc acetate treated ***:We propose that reduction of brain copper by blocking uptake of copper from the diet may be a viable strategy for modulating tau pathology in Alzheimer’s *** potential benefits of this approach are tempered by the absence of a behavioral benefit and by the health risks of excessive lowering of copper.

BACKGROUND: Chronic pancreatitis following acute fatty liver of pregnancy is rarely ***: We treated a 34-year-old woman who developed acute fatty liver of pregnancy (AfLP) after delivery by caesarean section at 32 weeks of gestation. AfLP was complicated by acute pancreatitis and multiple organ failure. The management of the disease was primarily supportive. She recovered from acute fulminant liver failure and multi-organ failure, apart from the development of symptomatic chronic pancreatitis thereafter. RESULTS: Investigations failed to identify any other causes of chronic pancreatitis. The patient responded very well to pancreatic enzyme supplement for the treatment of ***: To our knowledge, this is the first report of chronic pancreatitis as a consequence of multi-organ dysfunction caused by AfLP.
