＜正＞ Attachment of leukocytes to the vascular endothelium and the subsequent migration of cells into the vessel wall are early events in atherogenesis. Expression of endothelial adhesion molecules play an important role in this process. In the present study, we tested the effect of Epigllocatechin-3-gallate （ EGCG） on proatherogenic agent, angiotensin Ⅱ （Ang Ⅱ）-induced expression of adhesion molecules in vascular endothelial cells. We showed that EGCG inhibits Ang Li-stimulated VCAM-1 and ICAM-1 expression in HUVECs. Inhibition of Ang Ⅱ-in-duced adhesion molecules expression was manifested already on the transcriptional level. EGCG pretreatment inhibited Ang Ⅱ-stimulated activation of p38 MAPK and ERK 1/2. while EGCG did not exert any significant changes in activation of c-Jun N-terminal kinase （JNK）. In addition, a specific p38 MAPK inhibitor, SB202190 or ERK 1/2 inhibitor, PD98059, suppressed Ang Ⅱ-stimulated VCAM-1 and ICAM-1 expression. Conclusion: These results suggest that EGCG inhibits Ang Ⅱ-induced adhesion molecules expression, which is regulated by p38 MAPK and ERK 1/2 signaling pathways

背景强迫症是一种较为常见的慢性、致残的疾病，患者出现无法自控的强迫思维及强迫行为。10％以上的强迫症病人对药物和行为治疗无效。对一些常规治疗无效的难治性强迫症患者，神经外科已经开展一些手术治疗，例如：扣带前回毁损、脑白质切断术、尾状核下束切断、内囊前支毁损。扣带前回毁损相对来说比较安全同时没有明显的副作用，但是也有术后存在认知障碍的报道。为判断立体定向双侧扣带前回毁损是否是一种适合难治性强迫症的治疗，我们评价了立体定向双侧扣带前回毁损术的长期疗效及其副作用。方法患者筛选：在1999年3月至2003年4月间，17例难治性强迫症患者（男10例，女7例）接受了双侧扣带前回毁损术，并且接受了超过2年的随访。患者的选择标准如下：患者符合美国精神障碍诊断与统计手册第4版（DSM-Ⅳ）强迫症的诊断标准，病程超过3年，疾病导致社会心理功能降低。每位患者皆由两位精神科医师依照DSM-Ⅳ的AxisⅠ和AxisⅡ的疾病诊断标准行临床诊断。如伴有精神病性症状，就先用一线药物彻底治疗。手术方法：立体定向的靶点通过与MRI配套的Leksell适配器，在GESigma1．5 T的机器上得以确认。局部麻醉下，在两侧的冠状缝中线外20mm颅骨钻孔。将尖端直径．1．8mm，长度10mm的电极插至靶点，用85℃，90S进行射频毁损。在双侧扣带前回，每侧各4个射频毁损灶，2个针道。第一个射频毁损灶在两侧侧脑室前角后缘15mm，侧脑室顶部上2mm，旁开中线7mm处。射频电极接着回撤8mm，制作第二个射频毁损灶。第三个射频毁损处在两侧侧脑室前角后缘22mm，侧脑室顶部上2mm，旁开中线7mm处。再往上8mm，制作第四个射频毁损灶。结果是出现类似椭圆柱形的射频毁损灶，大约高18mm、前后径13mm，横径6mm。术前和术后随访评估：两位精神科医师用Yale-Brown强迫症量表（Y-BOCS）、临床总体印象量表（CGI）、汉密顿抑郁量表（HAMD）、汉密顿焦虑量表（HAMA），分别在手术前1个月、术后12个月、术后24个月对患者进行评估。评估的精神科医师接受了培训，Kappa值超过0．75，以确保评估的准确性。在我们研究中，患者的Y-BOCS得分改善35％或更高、CGI评分为1（极其显著改善）或者2（显著改善）；由专业的临床精神科医生对患者进行一套固定的神经心理测试，测试分别在术前1个月、术后12个月、24个月进行。这些测试包括：K—WAIS测验，视觉空间功能（RCFT），Hopkins语言学习测验（HVLT），威斯康星卡片分类测验（WCST），口语流畅测验（COWAT）。K-WAIS评分由韦氏成人智力量表（WAIS—R）改编而来，以产生总智商（IQ）、言语IQ和行为IQ来测量智力水平。RCFT评分用于测试空间构建能力。记忆力也包括回忆的精确性、瞬时回忆、延迟回忆和识别指数等的得分。HVLT用于测量口头记忆能力和学习能力。使用WCST和COWAT评分来检查认知的灵活性和执行功能。在WCST中，正确、错误反应的总数，完成的项目数，持续的反应，持续的错误数都用于评分。COWAT评分采用字母翻译和目录翻译的单词数目，字母翻译用来检查语音的流利，目录翻译用来评价语义流利性。使用Wileoxon符号秩检验对术前的Y—BOCS，HAMD，HAMA评分和术后6、12个月的评分进行比较，使用Wileoxon符号秩检验对手术前和手术后12个月的K—WAIS，RCFT，HVLT，WCST，COWAT评分进行比较。统计软件采用SPSS10。结果扣带回毁损术后强迫、抑郁、焦虑症状均改善：Y-BOCS，CGI、HAMD，HAMA评分显示了扣带回毁损术的有效性，在12个月的随访中，与术前Y-BOCS评分相比，改善率为36％，在24个月的随访中，改善率升至48％。术前、术后认知功能和记忆的改变：手术前和手术后24个月的K-WAIS，RCFT，HVLT，COWAT评分没有显著性差异。在WCST评分中，与术前基线相比，术后的错误总数、持续错误数和反应数明显减少。术后的错误总数、持续错误数和反应数的改变说明在扣带回毁损术后执行功能的提高。在术后2—3个月，3例患者有记忆障碍，但持续时间少于2个月。结论强迫症的扣带回毁损术术后24个月的随访结果显示症状明显改善，但没有认知功能的改变，包括智商、言语、视觉空间记忆和执行功能等均没有改变。

BACKGROUND Intramuscular corticosteroid injection may cause adverse effects such as dermal and/or subcutaneous atrophy,alopecia,hypopigmentation,and *** cutaneous atrophy can spontaneously resolve,several treatment options have been suggested for this *** SUMMARY In this paper,we report a case of corticosteroid injection induced lipoatrophy treated with autologous whole blood(AWB)injection,as the condition had been unresponsive to fractional laser therapy.A 29-year-old female patient visited the dermatology clinic complaining of skin depression on her right buttock area,which had appeared six months *** had been only subtle improvement at the margins after fractional CO_(2) laser treatment;therefore,after obtaining informed consent from the patient,AWB treatment was *** month after the first AWB injection,the size and depth of the lesion had noticeably improved,and a slight improvement was also observed in *** Close observation is the initial treatment of choice for steroid induced skin atrophy;however,for patients in need of immediate cosmetic improvement,AWB injection may be a safe and cost-effective alternative.

Background:Endoscopic transpapillary gallbladder stenting(ETGBS)has been used as an alternative to percutaneous cholecystostomy in patients with acute cholecystitis who are considered unfit for ***,there are few data on the efficacy and safety of ETGBS replacement of percutaneous cholecystostomy in high-risk surgical *** study aimed to evaluate the feasibility,efficacy,and safety of ETGBS to replace percutaneous cholecystostomy in high-risk surgical ***:This single center retrospective study reviewed the data of patients who attempted ETGBS to replace percutaneous cholecystostomy between January 2017 and September *** technical success,clinical success,adverse events,and stent patency were ***:ETGBS was performed in 43 patients(24 male,mean age 80.7±7.4 years)to replace percutaneous cholecystostomy due to high surgical *** technical success rate and clinical success rate were 97.7%(42/43)and 90.5%(38/42),***-related adverse events and stent-related late adverse events occurred in 7.0%(3/43)and 11.6%(5/43),*** the patients who successfully underwent ETGBS(n=42),only one had recurrent acute cholecystitis during *** median stent patency was 415 days(interquartile range 240–528 days).Conclusions:ETGBS,as a secondary intervention for the purpose of internalizing gallbladder drainage in patients following placement of a percutaneous cholecystostomy,is safe,effective,and technically ***,conversion of percutaneous cholecystostomy to ETGBS may be considered as a viable option in high-risk surgical patients.

This article describes the integrated modeling approach for planning the size and the operation of constructed wetlands for maximizing retention of nonpoint source pollutant loads and reservoir water-quality improvement at a catchment scale. The experimental field-scale wetland systems （four sets, 0.88 ha each） have been in operation since 2002, where water depth was maintained at 30-50 cm and hydraulic loading rate was at 6.3-18.8 cm/day. The wetland system was found to be adequate for treating polluted stream water with stable removal efficiency even during the winter. The integrated modeling system （modified-BASINS） was applied to the Seokmoon estuarine reservoir watershed and calibrated with monitoring data from constructed wetland, stream, and reservoir. The calibrated integrated modeling system estimated that constructing wetlands on 0.5% （about 114 ha） of the watershed area at the mouth of reservoir could reduce 11.61% and 13.49% of total external nitrogen and phosphorus loads, respectively. It also might improve the nitrogen and phosphorus concentration of the reservoir by 9.69% and 16.48%, respectively. The study suggested that about 0.1%-1.0% of the watershed area should be allocated for constructed wetland to meet specified water-quality standards for the estuarine reservoir at the polder area where land use planning is relatively less complicated.

Many researches on critical nozzles have been performed to accurately measure the mass flow rate of gas flow,and to standardize the performance as a flow ***,much interest is being paid on the measurement of very small mass flow rate in industry fields such as MEMS ***,the design and performance data of the critical nozzles obtained so far have been applied mainly to the critical nozzles with comparatively large diameters,and the works available on miniature critical nozzles are *** the present study,a computational fluid dynamics method has been applied to investigate the influence of the diffuser angle on discharge coefficient of the miniature critical *** computations,the throat diameter of critical nozzle is varied from 0.2 mm to 5.0 mm and the diffuser angle is changed from 2 deg to 8 *** computational results are validated with some experimental data *** results show that the present computational results predict appropriately the discharge coefficient of the gas flows through miniature critical *** is known that the discharge coefficient is considerably influenced by the diffuser angle,as the throat diameter of nozzle becomes small below a certain *** implies that the miniature critical nozzles should be carefully designed.

Fractionation of heavy metals in runoff and discharge of a stormwater management system and its implications for treatment

A stormwater management system utilizing the mechanisms of sedimentation and filtration/infiltration was developed and constructed for the immobilization of suspended solids and heavy metal constituents （Fe, Zn, Pb, Cr, Cu, Cd, Ni） in runoff. Monitoring took place between May 2010 and November 2012 on a total of 24 storm events. This research on the fractionation of heavy metals in runoff and discharge of a stormwater system provided insight on the actual efficiency of the system and determined the implications for treatment of the particulate-bound and dissolved heavy metals in runoff. Results revealed that the partitioning of heavy metal load in runoff in either dissolved or particulate-bound were influenced by flow rate and total suspended solids load, and evident in high-intensity storm （rainfall-runoff event）. On the other hand, the partitioning of heavy metal load discharge from the stormwater system was more apparent during the early period of discharge having higher variability in dissolved than particulate-bound heavy metal. Findings revealed that fractionation of heavy metals played an important role in the performance of the stormwater system; thus, must be considered in designing stormwater systems. For the stormwater system to be effective, it is recommended to design the system treating not only the early period of a storm （first flush criteria） rather until the peak part of the hydrograph （high flow rate where partitioning was greatest） from a load basis.
