Objective:To assess the role of polymerase chain reaction(PCR)in serum sauples,in the diagnosis of osteoarticular tuberculosis(OTB)in a setting where only clinical and imaging diagnoses determine the ***:A total of 44 consecutive serum specimens were collected from clinically suspected OTB patients,based on clinical and radiological[X-ray or magnetic resonance imagng/computecl tomography]*** were scrcened by in-house nested *** addition,a few specimens were examined by Gram stain,acid-fast bacilli stain,histand routine bacterial culture.A total of 39 specimens were collected from patients suffering from other bone diseases of nontuberculous origin and included as negative ***:of the 44 clinically suspected OTB patients,in-house nested PCR was positive in 40(91%)cases;PCR was negative in 38(97%)negative *** and specificity of our in—house nested PCR was 90.3%and 97.4%,*** PCR report was available within 48 *** was possible to standardize serum PCR technique and in positive cases,a good n was observed in terms of an adequate treatment ***:Nested PCR in serum samples is a rapid,highly sensitive and specific modality for OTB detection,PCR should be performed in addition to clinical evaluation,imaging studies,acidfast bacilli staining,culture and histopathology diagnosis,if possible.

Arbovirus infections have increased worldwide in the last years and specific differential diagnosis may be difficult owing to their similar clinical presentations. Here we describe and compare the clinical manifestations of dengue and chikungunya fever. Material and Methods: In order to portray the clinical behavior of both diseases, a cross-sectional study was performed including patients insured by the Mexican Institute for Social Security (IMSS), their demographic data, and clinical signs and symptoms. Laboratory parameters were analyzed. Results: One thousand four hundred and ninety-five dengue and 1810 chikungunya cases were studied. All patients presented fever, whereas headache, myalgias, mild arthralgias, and ocular pain were more frequent in dengue cases. Severe incapacitating polyarthralgias, arthritis, and conjunctivitis were present only in chikungunya cases. Also, edema and exanthema were more frequent in patients with chikungunya. Liquid leak to tissues and hemorrhage were more frequent among dengue cases. Platelet counts were lower in hemorrhagic dengue (P < 0.001). Laboratory parameters were altered between the second and third day of evolution and tended to return to normal from the fifth day. Discussion: The clinical features observed in this group of patients may contribute to the differential diagnosis of both diseases. The cardinal signs of severity were not present in all cases of any of the two diseases. It is important to consider these issues in order to establish an early diagnosis and improve the prognosis of patients.

Near-surface wind speed exerts profound impacts on many environmental issues,while the long-term(≥60 years)trend and multidecadal variability in the wind speed and its underlying causes in global high-elevation and mountainous areas(e.g.,Tibetan Plateau)remain largely ***,by examining homogenized wind speed data from 104 meteorological stations over the Tibetan Plateau for 1961-2020 and ERA5 reanalysis datasets,we investigated the variability and long-term trend in the near-surface wind speed and revealed the role played by the westerly and Asian *** results show that the homogenized annual wind speed displays a decreasing trend(-0.091 m s^(−1)per decade,p<0.05),with the strongest in spring(-0.131 m s^(−1)per decade,p<0.05),and the weakest in autumn(-0.071 m s^(−1)per decade,p<0.05).There is a distinct multidecadal variability of wind speed,which manifested in an prominent increase in 1961-1970,a sustained decrease in 1970-2002,and a consistent increase in *** observed decadal variations are likely linked to large-scale atmospheric circulation,and the correlation analysis unveiled a more important role of westerly and East Asian winter monsoon in modulating near-surface wind changes over the Tibetan *** potential physical processes associated with westerly and Asian monsoon changes are in concordance with wind speed change,in terms of overall weakened horizontal air flow(i.e.,geostrophic wind speed),declined vertical thermal and dynamic momentum transfer(i.e.,atmospheric stratification thermal instability and vertical wind shear),and varied Tibetan Plateau *** indicates that to varying degrees these processes may have contributed to the changes in near-surface wind speed over the Tibetan *** study has implications for wind power production and soil wind erosion prevention in the Tibetan Plateau.

Although it is recognized that the post-harvest system is most responsible for the loss of soybean quality,the real impact of this loss is still *** regulation allows 15%and 30%of broken soybean for group I and group II(quality groups),***,the industry is not informed about the loss in the quality parameters of soybeans and its impacts during long-term ***,the objective was to evaluate the effect of the breakage kernel percentage of soybean stored for 12 *** of 15% of breakage kernels did not affect soybean ***,content of 30% of breakage kernels affected significantly soybean quality,which was evidenced by the increase of up to 75% in moldy soybeans,72% in acidity,50% in leached solids,27% in electrical conductivity,and the decrease of up to 12% in soluble protein,6.4% in germination and 3.5% in thousand kernel weight after 8 months of *** the legislation establishes a percentage limit,it is recommended to store soybeans with up to 15% breakage *** the contrary,values higher than that can cause a significant reduction in soybean quality,resulting in commercial losses.

目的在重症监护病房(IntensiveCareUnit,ICU),护土难免会遇到需要限制治疗努力(limiting therapeutie efort,LTE)的患者,尽管多数护士还缺乏为这些临终患者及其家庭提供支持的准备。本综述旨在描述ICU护士在LTE中需要具备的能力。方法对2010年--2023年发表的文献进行综述。系统检索了5个数据库:Medline、Wiley Online Library、SciELO、ScienceDirect和Web of Science。使用关键评估项目工具西班牙语版(Critical Appraisal Skills Programme in Spanish,CASPe)作为评判性阅读工具。使用牛津循证医学中心(Center for Evidence-Based Medicine.,CEBM)评估工具确定证据级别和推荐强度。结果共纳入25篇文献。ICU护士LTE护理的能力包括对患者的直接护理、以家庭为中心的护理以及护士在团队中的作用。该综述确定了3个类别的能力:作为患者、家庭与跨学科团队之间维护者的能力,在LTE方面的决策能力,在患者生命末期治疗和护理干预的能力。结论ICU护士在LTE方面需要具备相应的能力,这对患者的生活质量死亡尊严以及患者家庭应对悲伤的全面支持至关重要。
