Background:Ocular surface disease in glaucoma patients is a significant ocular co-morbidity that can affect 40%to 59%of these patients *** current study was aimed at evaluating the potential clinical benefit of an intense pulsed light(IPL)-based treatment in glaucomatous patients with ocular surface disease due to prolonged hypotensive eyedrop *** our knowledge,this is the first series analyzing the therapeutic effect of this treatment option in this type of ***:This non-comparative prospective case series study enrolled a total of 30 glaucoma patients ranging in age from 57 to 94 years old and treated with hypotensive eyedrops for years with dry eye *** patients received four sessions of IPL treatment using the Optima IPL system(Lumenis,Yokneam,Israel)adjusted to the official optimized Lumenis *** in symptomatology,corneal staining,conjunctival hyperemia,non-invasive break-up time(NIBUT),tear osmolarity,tear meniscus height(TMH),meiboscore and meibomian gland expressibility was analyzed after ***:Statistically significant reductions were observed after IPL treatment in the symptomatology scores measured with different questionnaires[ocular surface disease index(OSDI),standard patient evaluation of eye dryness(SPEED)and symptom assessment questionnaire in dry eye(sanDE)]as well as with the visual analogue scale(P<0.001).Mean change in OSDI was−15.0±11.3.A significant reduction was found after treatment in the corneal staining score(P<0.001).A significant reduction was found in tear film meniscus height(P=0.012),as well as in tear film osmolarity(P=0.001).A significant reduction was also found in meibomian gland expressibility(P=0.003),changing the percentage of grade 3 eyes from 44.4%before IPL to 17.2%after ***:IPL therapy combined with meibomian gland expression(MGX)seems to be an effective option to improve symptomatology in glaucomatous patients with ocular surface diseas

Suppression of hepatic steatosis in non-alcoholic steatohepatitis model by modified Xiaoyao san formula:Evidence,mechanisms and perspective

In this letter,we comment on a recent publication by Mei et al,in the World Journal of Hepatology,investigating the hepatoprotective effects of the modified Xiaoyao san(MXS)formula in a male rat model of non-alcoholic steatohepatitis(NASH).The authors found that MXS treatment mitigated hepatic steatosis and inflam-mation in the NASH model,as evidenced by the reduction in lipid droplets(LDs),fibrosis markers and lipogenic ***,these hepatoprotective effects were associated with androgen upregulation(based on metabolomics analysis of male steroid hormone metabolites),adenosine 5’-monophosphate-activated protein kinase(AMPK)activation,and restoration of phosphatase and tensin homolog(PTEN)***,the authors did not clearly discuss the relationships between MXS-induced hepatic steatosis reduction in the NASH model,and androgen upregulation,AMPK activation,and restoration of PTEN *** editorial emphasizes the reported mechanisms and explains how they act or interact with each other to reduce hepatic steatosis and inflammation in the NASH *** a perspective,we propose additional mechanisms(such as autophagy/lipophagy activation in hepatocytes)for the clearance of LDs and suppression of hepatic steatosis by MXS in the NASH model.A proper understanding of the mechanisms of MXS-induced reduction of hepatic steatosis might help in the treatment of NASH and related diseases.

Experience with the surgical treatment of hepatic hydatidosis: Case series with follow-up

AIM: To describe patient characteristics and the results of surgical treatment for hepatic hydatidosis (HH) in the Hospital Hernán Henríquez Aravena from December 2001 to March 2005. METHODS: Subjects older than 16 years with suspected HH were included based on the clinical picture and radiological studies. Variables examined included clinical and laboratory aspects, outcome, features of the parasite and surgical aspects. Descriptive statistics were used, calculating central tendency, dispersion and extreme tendency. RESULTS: The series was comprised of 122 patients, with an average age of 44 ± 16.9 years. The most frequently used surgical technique was subtotal cys-tectomy in 90% of the patients, followed by hepatic resection (hepatectomy, segmentectomy and subsegmentectomy) in 5%. In 2%, a combination of subtotal cystectomy and segmentectomy was performed. In addition, 28% of the series presented complications in the postoperative period and mortality was 2%. CONCLUSION: The most frequently used surgical technique for HH was subtotal cystectomy and the morbidity and mortality rates in this Chilean series are comparable to other national and international series.

BACKGROUND Endoscopic resection of non-invasive lesions is now the standard of care for lesions in the GI ***,resection techniques require extensive training,are not available in all endoscopy centers and are prone to *** mucosal ablation(EMA)is a combination of resection and ablation techniques and it may offer an alternative in the management of such *** SUMMARY In this case series we report the successful treatment of three flat colonic polyps using the EMA *** lesions were treatment naïve and 1 was a recurrence after an endoscopic mucosal *** sizes ranged from 2 to 4 *** three polyps were ablated successfully with no immediate or delayed *** recurrence rate at 1 year of follow up was 0%.CONCLUSION Based on this initial experience,we conclude that EMA is a safe and effective technique for the treatment of non-invasive colonic polyps when endoscopic resection techniques are not available.

Background: Coupling biomass models with nutrient concentrations can provide sound estimations of carbon and nutrient contents, enabling the improvement of carbon and nutrient balance in forest ecosystems. Although nutrient concentrations are often assumed to be constant for some species and specific tree components, at least in mature stands,the concentrations usually vary with age, site index and even with tree density. The main objective of this study was to evaluate the sources of variation in nutrient concentrations in biomass compartments usually removed during harvesting operations, covering a range of species and management conditions: semi-natural forest, conventional forest plantations and short rotation forestry(SRF). Five species(Betula pubescens, Quercus robur, Eucalyptus globulus, Eucalyptus nitens and Populus spp.) and 14 genotypes were considered. A total of 430 trees were sampled in 61 plots to obtain 6 biomass components:leaves, twigs, thin branches, thick branches, bark and wood. Aboveground leafless biomass was pooled together *** concentrations of C, N, K, P, Ca, Mg, S, Fe, Mn, Cu, Zn and B were measured and the total biomass of each sampled tree and plot were determined. The data were analysed using boosted regression trees and conventional ***: The main sources of variation in nutrient concentrations were biomass component > > genotype(species) ≈ age >tree diameter. The concentrations of Ca, Mg and K were most strongly affected by genotype and age. The concentrations of P, K, Ca, Mg, S and Cu in the wood component decreased with age, whereas C concentrations increased, with a trend to reach 50% in the older trees. In the SRF, interamerican poplar and P. trichocarpa genotypes were comparatively more efficient in terms of Ca and K nutrient assimilation index(NAI)(+65-85%) than eucalypts, mainly because leafless biomass can be removed. Development of Extraction and Detection Method for a Chemotherapeutic Drug with Phenytoin in Biological Samples

Dexamethasone is classified as a corticosteroid and is commonly used among cancer patients to decrease the amount of swelling around the tumor. Among patients with cancer, in particular brain tumors, seizures can become a daily routine in their everyday lives. To counteract the seizures, an antiepileptic drug such as phenytoin is administered to act as an anticonvulsant. Phenytoin and dexamethasone are frequently administrated concurrently to brain cancer patients. A previous study has shown that phenytoin serum concentration decreases when administrated concurrently with dexamethasone. Thus, it is important to monitor the concentration of these two drugs in biological samples to ensure that the proper dosages are administrated to the patients. This study aims to develop an effective extraction and detection method for dexamethasone and phenytoin. A reverse-phase high-performance liquid chromatography (HPLC) method with UV/Vis detection has been developed to separate phenytoin and dexamethasone at 219 nm and 241 nm respectively from urine samples. The mobile phase consists of a mixture of 0.01 M KH2PO4, acetonitrile, and methanol adjusted to pH 5.6 (48:32:20) and is pumped at a flow rate of 1.0 mL/min. Calibration curves were prepared for phenytoin and dexamethasone (r2 > 0.99). An efficient solid-phase extraction (SPE) method for the extraction of dexamethasone and phenytoin from urine samples was developed with the use of C-18 cartridges. The percent recovery for phenytoin and dexamethasone is 95.4% (RSD = 1.15%) and 81.1% (RSD = 3.56%) respectively.

Validity of automated refraction after segmented refractive multifocal intraocular lens implantation

AIM： To evaluate the clinical utility of automated refraction（AR） and keratometry（KR） compared with subjective or manifest refraction（MR） after cataract or refractive lens exchange surgery with implantation of Lentis Mplus X（Oculentis GmbH） refractive multifocal intraocular lens（IOL）.METHODS： Eighty-six eyes implanted with the Lentis Mplus X multifocal IOL were included. MR was performed in all patients followed by three consecutive AR measurements using the Topcon KR-8000 autorefractor. Assessment of repeatability of consecutive AR before and after dilation with phenylephrine 10%, and comparison of the AR and KR with MR using vector analysis were performed at 3 mo ***： Analysis showed excellent repeatability of the AR measurements. Linear regression of AR versus MR showed good correlation for sphere and spherical equivalent, whereas the correlation for astigmatism was low. The mean difference AR-MR was-1.28±0.29 diopters（D） for sphere. Astigmatism showed better correlation between KR and MR. CONCLUSION： We suggest AR sphere plus 1.25 D and the KR cylinder as the starting point for MR in eyes with a Lentis Mplus X multifocal IOL. If AR measurements are equal to MR, decentration of the IOL should be suspected.
