Atypical presentation of a hepatic artery pseudoaneurysm:A case report and review of the literature

Classically, hepatic artery pseudoaneurysms(HAPs) arise secondary to trauma or iatrogenic causes. With an increasing prevalence of laparoscopic procedures of the hepatobiliary system the risk of inadvertent injury to arterial vessels is increased. Pseudoaneurysm formation post injury can lead to serious consequences of rupture and subsequent hemorrhage, therefore intervention in all identified visceral pseudoaneurysms has been advocated. A variety of interventional methods have been proposed, with surgical management becoming the last step intervention when minimally invasive therapies have failed. The authors present a case of a HAP in a 56-year-old female presenting with jaundice and pruritis suggestive of a Klatskin's tumor. This presentation of HAP in a patient without any significant past medical or surgical intervention is atypical when considering that the majority of HAP cases present secondary to iatrogenic causes or trauma. multiple minimally invasive approaches were employed in an attempt to alleviate the symptomology which included jaundice and associated inflammatory changes. Ultimately, a right hepatic trisegmentectomy was required to adequately relieve the mass effect on biliary outflow obstruction and definitively address the HAP. The presentation of a HAP masquerading as a malignancy with jaundice and pruritis, rather than the classic symptoms of abdominalpain, anemia, and melena, is unique. This presentation is only further complicated by the absent history of either trauma or instrumentation. It is important to be aware of HAPs as a potential cause of jaundice in addition to the more commonly thought of etiologies. Furthermore, given the morbidity and mortality associated with pseudoaneurysm rupture, intervention in identifiable cases, either by minimally invasive or surgical interventions, is recommended.

Quality of life issues in men undergoing androgen deprivation therapy： a review

Androgen deprivation therapy （ADT） has been an essential treatment option for treating prostate cancer （PCa）. The role for hormonal treatment initially was restricted to men with metastatic and inoperable, locally advanced disease. Now it has been extended to neoadjuvant or adjuvant therapy for surgery and radiotherapy, for biochemical relapse after surgery or radiation, and even as primary therapy for non-metastatic disease. Fifty percent of PCa patients treated will receive ADT at some point. There is growing concern about the adverse effects and costs associated with more widespread ADT use. The adverse effects on quality of life （QoL）, including physical, social and psychological well-being when men are androgen-deprived, may be considerable. This review examines the QoL issues in the following areas： body feminisation, sexual changes, relationship changes, cognitive and affective symptoms, fatigue, sleep disturbance, depression and physical effects. Further suggestions for therapeutic approaches to reduce these alterations are suFuzested.

Fate and transport of theβ-adrenergic agonist ractopamine hydrochloride in soil–water systems

The feed additive ractopamine hydrochloride was fortified at four concentrations into batch vials containing soils that differed in both biological activity and organic matter（Om）.Sampling of the liquid layer for 14 days demonstrated that ractopamine rapidly dissipated from the liquid layer. Less than 20% of the fortified dose remained in the liquid layer after 4 hr,and recoveries of dosed ractopamine ranged from 8 to 18% in the liquid layer at 336 hr. Sorption to soil was the major fate for ractopamine in soil：water systems, i.e., 42%–51% of the dose at14 days. The major portion of the sorbed fraction was comprised of non-extractables; a smaller fraction of the sorbed dose was extracted into water and acetone, portions which would be potentially mobile in the environment. Partitioning coefficients for all soils suggested strong sorption of ractopamine to soil which is governed by hydrophobic interactions and cation exchange complexes within the soil Om. Ractopamine degradation was observed, but to mostly non-polar compounds which had a higher potential than ractopamine to sorb to soil. The formation of volatiles was also suggested. Therefore, despite rapid and extensive soil sorption,these studies indicated a portion of ractopamine, present in manures used to fertilize soils,may be mobile in the environment via water-borne events.

Synchronous formation of the‘forearc’Bay of Islands ophiolite and its basal high-temperature metamorphic sole constrained by U-Pb zircon ages

The welded metamorphic sole at the base of the Bay of Islands Ophiolite Complex(BOIC)in the Northern Appalachians of Newfoundland shows a typical inverted pressure-temperature(P-T)metamorphic gradient from HT-mP granulite to LT-LP greenschist *** incorporates mafic volcanic/plutonic protoliths mixed with pelagic,hemi-pelagic and coarser epiclastic sedimentary *** LA-ICP-mS U–Pb concordia ages,trace elements,and Ti-in-zircon geothermometry for -250 zircon analyses from three metabasites of the upper HT sole amphibolites with N-mORB-like protoliths are *** samples collected within meters of the ophiolite peridotite-sole contact of the Blow me Down mountain and North Arm mountain massifs yielded the oldest comparable concordia ages of 487.7±2.6 ma and 489.1±3.1 ma,respectively,that are both within error of the igneous age of 488.3±1.5 ma of the directly overlying BOIC ophiolite,which formed at a supra-subduction zone(SSZ)forearc spreading center.A third slightly younger age of 484.2±2.4 ma was obtained for an upper HT amphibolite sample with similar phase assemblages but collected30 m below the peridotite contact of the Table mountain *** crystals analyzed have similar size and morphologies,subparallel rare earth element(REE)variation patterns,and steep heavy REE-enrichments((Lu/Gd)_(cn)>20),significant positive Ce anomalies(dominantly>5)and slight positive to dominantly negative Eu anomalies(1.2–0.4).Zircon shows Th/U mean values of 0.37–0.48 with little to no rim to core *** Ti-in-zircon mean crystallization temperatures range from764–787℃.These neocrystallized zircon crystals appear to be derived from thin leucosomes within the three *** other samples also from the upper HT sole show evidence of inherited detrital zircon with core dates spanning the Cambrian Notre Dame Arc through older Laurentian-like basement and rift age *** of the sole took place below a hot forearc asthenospheric wed

Evidence of proteins, chromosomes and chemical markers of DNA in exceptionally preserved dinosaur cartilage

A histological ground-section from a duck-billed dinosaur nestling(Hypacrosaurus stebingeri) revealed microstructures morphologically consistent with nuclei and chromosomes in cells within calcified *** hypothesized that this exceptional cellular preservation extended to the molecular level and had molecular features in common with extant avian cartilage. Histochemical and immunological evidence supports in situ preservation of extracellular matrix components found in extant cartilage, including glycosaminoglycans and collagen type II. Furthermore, isolated Hypacrosaurus chondrocytes react positively with two DNA intercalating stains. Specific DNA staining is only observed inside the isolated cells, suggesting endogenous nuclear material survived fossilization. Our data support the hypothesis that calcified cartilage is preserved at the molecular level in this mesozoic material, and suggest that remnants of once-living chondrocytes, including their DNA, may preserve for millions of years.

The role of emergency incident type in identifying first responders' health exposure risks

Variation in resource competition traits among microcystis strains is affected by their microbiomes

Freshwater harmful algal blooms are often dominated by microcystis,a phylogenetically cohesive group of cyanobacteria marked by extensive genetic and physiological *** have previously shown that this genetic diversity and the presence of a microbiome of heterotrophic bacteria influences competitive interactions with eukaryotic *** this study,we sought to explain these observations by characterizing monod equation parameters for resource usage(maximum growth rateμmax,half-saturation value for growth Ks,and quota)as a function of N and P levels for four strains(NIES-843,PCC 9701,PCC 7806[WT],and PCC 7806ΔmcyB)in presence and absence of a microbiome derived from microcystis isolated from Lake *** indicated limited differences in maximum growth rates but more pronounced differences in half-saturation values among microcystis *** largest impact of the microbiome was reducing the minimal nitrogen concentration sustaining growth and reducing half saturation values,with variable results depending on the microcystis *** strains also differed from each other in their N and P quotas and the extent to which microbiome presence affected *** data highlight the importance of the microbiome in altering microcystis-intrinsic traits,strain competitive hierarchies,and thus bloom *** quota,μmax,and Ks are commonly used in models for harmful algal blooms,our data suggest that model improvement may be possible by incorporating genotype dependencies of resource-use parameters.

Cost-Efficient medium Frequency Propagation Research with Software Defined Radio

medium Frequency radio holds significance in modern society as it supports broadcasting and individual communications in the public, government, and military sectors. Enhancing the availability and quality of these communications is only possible by enhancing the understanding of medium frequency propagation. While traditional methods of radio wave propagation research can have a high material demand and cost, software defined radio presents itself as a versatile and low-cost platform for medium frequency signal reception and data acquisition. This paper details a research effort that utilizes software defined radio to help characterize medium frequency signal strength in relation to ionospheric and solar weather propagation determinants. Signal strength data from seven medium frequency stations of unique transmission locations and varying transmission powers were retrieved in 24-hour segments via a receiving loop antenna, Airspy HF+ Discovery software defined radio, and SDR Sharp software interface network. Retrieved data sets were visualized and analyzed in mATLAB for the identification of signal strength trends, which were subsequently compared to historical ionospheric and space weather indices in pursuit of a quantifiable correlation between such indices and medium frequency signal strengths. The results of the investigation prove that software defined radio, when used in conjunction with a receiving antenna and data analysis program, provides a versatile mechanism for cost-efficient propagation research.

Caring for the breast cancer survivor's health and wellbeing

The breast cancer care continuum entails detection, diagnosis, treatment, and survivorship. During this time, focus on the whole woman and medical concerns beyond the breast cancer diagnosis itself is essential. In this comprehensive review, we critically review and evaluate recent evidence regarding several topics pertinent to and specific for the woman living with a prior history of breast cancer. more specifically, we discuss the most recent recommendations for contraceptive options including long-acting reversible contraception and emergency contraception, fertility and pregnancy considerations during and after breast cancer treatment, management of menopausal vasomotors symptomsand vulvovaginal atrophy which often occurs even in young women during treatment for breast cancer. The need to directly query the patient about these concerns is emphasized. Our focus is on non-systemic hormones and non-hormonal options. Our holistic approach to the care of the breast cancer survivor includes such preventive health issues as sexual and bone health,which are important in optimizing quality of life. We also discuss strategies for breast cancer recurrence surveillance in the setting of a prior breast cancer diagnosis. This review is intended for primary care practitioners as well as specialists caring for female breast cancer survivors and includes key points for evidence-based best practice recommendations.
