Impact of conditioning regimen on peripheral blood hematopoietic cell transplant

AIM To investigate infused hematopoietic cell doses and their interaction with conditioning regimen intensity +/-total body irradiation(TBI) on outcomes after peripheral blood hematopoietic cell transplant(PBHcT).METHODS Our retrospective cohort included 247 patients receiving a first, T-replete, human leukocyte antigen-matched allogeneic PBHcT and treated between 2001 and2012. correlations were calculated using the Pearson product-moment correlation coefficient. Overall survival and progression free survival curves were generated using the Kaplan-Meier method and compared using the log-rank *** Neutrophil engraftment was significantly faster after reduced intensity TBI based conditioning [reduced intensity conditioning(RIc) + TBI] and > 4 × 10~6 cD34+cells/kg infused. A higher total nucleated cell dose led to a higher incidence of grade II-IV acute graft-versus-host disease in the myeloablative + TBI regimen group(P = 0.03), but no significant difference in grade III-IV graft-versus-host disease. A higher total nucleated cell dose was also associated with increased incidence of moderate/severe chronic graft-versus-host disease, regardless ofconditioning regimen. Overall and progression-free survival were significantly better in patients with a RIc + TBI regimen and total nucleated cell dose > 8 ×10~8/kg(3 years, overall survival: 70% vs 38%, P = 0.02, 3 years, progression free survival: 64% vs 38%, P = 0.02).cONcLUSION TBI and conditioning intensity may alter the relationship between infused cell doses and outcomes after PBHcT. Immune cell subsets may predict improved survival after unmanipulated PBHcT.

BAcKGROUND:This study aimed to evaluate the discriminatory performance of 11 vital sign-based early warning scores(EWSs)and three shock indices in early sepsis prediction in the emergency department(ED).METHODS:We performed a retrospective study on consecutive adult patients with an infection over 3 months in a public ED in Hong *** primary outcome was sepsis(Sepsis-3 definition)within 48 h of ED *** c-statistics and the DeLong test,we compared 11 EWSs,including the National Early Warning Score 2(NEWS2),Modified Early Warning Score,and Worthing Physiological Scoring System(WPS),etc.,and three shock indices(the shock index[SI],modified shock index[MSI],and diastolic shock index[DSI]),with Systemic Inflammatory Response Syndrome(SIRS)and quick Sequential Organ Failure Assessment(qSOFA)in predicting the primary outcome,intensive care unit admission,and mortality at different time ***:We analyzed 601 patients,of whom 166(27.6%)developed ***2 had the highest point estimate(area under the receiver operating characteristic curve[AUROc]0.75,95%cI 0.70-0.79)and was significantly better than SIRS,qSOFA,other EWSs and shock indices,except WPS,at predicting the primary ***,the pooled sensitivity and specificity of NEWS2≥5 for the prediction of sepsis were 0.45(95%cI 0.37-0.52)and 0.88(95%cI 0.85-0.91),*** discriminatory performance of all EWSs and shock indices declined when used to predict mortality at a more remote time ***:NEWS2 compared favorably with other EWSs and shock indices in early sepsis prediction but its low sensitivity at the usual cut-off point requires further modification for sepsis screening.

Understanding the forces acting in self-assembly and the implications for constructing three-dimensional （3D） supercrystals

The assembly of nanocrystals into ordered structures called supercrystals or superstructures has become a pivotal frontier owing to numerous useful applications such as correlating the arrangements of atoms in macroscopic crystals and tuning the collective properties to meet the demands of various applications. In this article, recent progress in the preparation of three-dimensional superlattices of nanocrystals is outlined, with a particular emphasis on the driving forces and evolutionary routes beyond orderly assembly. First, the leading or repulsive forces that internally and externally govern the formation of three-dimensional supercrystals are systematically identified and discussed with respect to their origins and functions in three-dimensional self-organization. Then a synoptic introduction of commonly applied means of nanocrystal self-assembly based on growth scenarios such as droplet evaporation and a liquid/liquid interface is presented with specific cases and detailed analyses. Finally, the existing challenges and prospects for this field are briefly highlighted.

The translocon on the outer membrane of mitochondria （TOM） facilitates the import of nuclear-encoded proteins. The principal machinery of mitochondrial protein transport seems conserved in eukaryotes; however, divergence in the composition and structure of TOM components has been observed between mammals, yeast, and plants. TOM9, the plant homolog of yeast Tom22, is significantly smaller due to a truncation in the cytosolic receptor domain, and its precise function is not understood. Here we provide evidence showing that TOM9.2 from Arabidopsis thaliana is involved in the formation of mature TOM com- plex, most likely by influencing the assembly of the pore-forming subunit TOM40. Dexamethasone- induced RNAi gene silencing of TOM9.2 results in a severe reduction in the mature TOM complex, and the assembly of newly imported TOM40 into the complex is impaired. Nevertheless, mutant plants are fully viable and no obvious downstream effects of the loss of TOM complex, i.e., on mitochondrial import ca- pacity, were observed. Furthermore, we found that TOM9.2 can bind calmodulin （cAM） in vitro and that caM impairs the assembly of TOM complex in the isolated wild-type mitochondria, suggesting a regula- tory role of TOM9.2 and a possible integration of TOM assembly into the cellular calcium signaling network.

The Younger the Patients the Less Adverse Effect of Diminished Oocyte Reserve on Outcome Following in vitro Fertilization-embryo Transfer as Long as the Proper Ovarian Stimulation Protocol is Used

Objective To determine the confounding effect of oocyte reserve and age on pregnancy rates per oocyte retrieval and a comparison of the average number of babies born from a given oocyte harvest. Methods In vitro fertilization-embryo transfer （IVF-ET） cycles were retrospectively reviewed over a l O-year period They were stratified according to whether they had normal oocyte reserve （day 3 serum FSH 〈12 mlU/ml and serum estradiol 〈50 pg/ml） or diminished oocyte reserve. Mild stimulation was used if low oocyte reserve. The pregnancy rate per retrieval included all pregnancies （fresh or frozen embryo transfer） resulting from one retrieval. The average number of babies born was also compared according to reserve. Results For the diminished oocyte reserve group, the live delivered pregnancy rates per transfer were 80% as good in women aged 〈35 years, 70% as good in women 3639 years, but only 43% as good as women aged 40-42 years. However, because of more total embryos the pregnancy rate per given oocyte retrieval was only 55% as good for women with diminished oocyte reserve aged 〈39 years. The average number of babies born per retrieval was twice as high as those with normal reserve for this age group. conclusion The use of mild follicle stimulating hormone （FSH） stimulation allows a live delivered pregnancy rate per transfer that is 75% as good for women aged 〈39 years but because of less embryos a likelihood of pregnancy rate per retrieval only half as good as women with normal reserve.

Endoscopic ultrasound staging for early esophageal cancer: Are we denying patients neoadjuvant chemo-radiation?

AIM To evaluate the accuracy of endoscopic ultrasound(EUS) in early esophageal cancer(Ec) performed in a highvolume tertiary cancer center. METHODS A retrospective review of patients undergoing esophagectomy was performed and patients with c T1 N0 and c T2 N0 esophageal cancer by EUS were evaluated. Patient demographics, tumor characteristics, and treatment were reviewed. EUS staging was compared to surgical pathology to determine accuracy of EUS. Descriptive statistics was used to describe the cohort. Student's t test and Fisher's exact test or χ~2 test was used to compare variables. Logistic regression analysis was used to determine if clinical variables such as tumor location and tumor histology were associated with EUS *** Between 2000 and 2015, 139 patients with clinical stage Ⅰ or Ⅱ?A esophageal cancer undergoing esophagectomy were identified. There were 25(18%) female and 114(82%) male patients. The tumor location included the middle third of the esophagus in 11(8%) and lower third and gastroesophageal junction in 128(92%) patients. Ninety-three percent of patients had adenocarcinoma. Preoperative EUS matched the final surgical pathology in 73/139 patients for a concordance rate of 53%. Twenty-nine patients(21%) were under-staged by EUS; of those, 19(14%) had unrecognized nodal disease. Positron emission tomography(PET) was used in addition to EUS for clinical staging in 62/139 patients. Occult nodal disease was only found in 4 of 62 patients(6%) in whom both EUS and PET were negative for nodal involvement. cONcLUSION EUS is less accurate in early Ec and endoscopic mucosal resection might be useful in certain settings. The addition of PET to EUS improves staging accuracy.

AIM： To evaluate the predictive value of hepatocyte proliferation and hepatic angiogenesis for the occurrence of Hepatocellular carcinoma （Hcc） in hepatitis c virus （HcV） cirrhotic patients.METHODS： One hundred-five patients （69 males, 36 females; age range, 51-90 year; median 66 year） with biopsy proven HcV cirrhosis were prospectively monitored for Hcc occurrence for a median time of 64 too. Angiogenesis was assessed by using microvessel density （MVD）, hepatocyte turnover by MIB1 and PcNA indexes at inclusion in liver biopsies.RESULTS： Forty six patients （43.8%） developed Hcc after a median time of 55 （6-120） mo while 59 （56.2%） did not. Patients were divided into two groups according to the median value of each index. The difference between patients with low （median MVD = 3; range 0-20） and high （median MVD = 7; range 1-24） MVD was statist（caUy s（gn（ficant （χ^2= 22.06; P 〈 0.0001） wh（ch was not the case for MIB1 or PcNA （MIB-I： χ^2 = 1.41; P = 0.2351; PcNA： χ^2 = 1.27; P = 0.2589）. The median MVD was higher in patients who developed Hcc than in those who did not. HOe-free interval was significantly longer in patients with the MVD ≤ 4 （P = 0.0006）. No relationship was found between MIB1 or PcNA and MVD （MIB-1 r^2 = 0.00007116, P = 0.9281; PcNA： P =0.001950; P = 0.6692）. MVD only was able to predict the occurrence of Hcc in these patients. Among other known risk factors for Hcc, only male sex was statistically associated with an increased risk. cONcLUSION： Liver angiogenesis has a role for in HcV- related liver carcinogenesis and for defining patients at higher risk.

Synchronous colorectal liver metastases: a national survey of surgeon opinions on simultaneous resection and multidisciplinary cooperation

Background: The management of patients with synchronous colorectal liver metastases (scRLM) has evolved significantly (improved chemotherapy, hepatic surgery advancements, colonic stenting, consultation synergies). We sought to better understand surgeon viewpoints on optimal referral patterns and the delivery of simultaneous resections. Methods: A 40 question on-line survey was offered to members of the canadian surgical community. Statistical analysis was descriptive. Results: A total of 52 surgeons responded. Most colorectal surgeons (cRS) had access to and a good working relationship with regional hepatobiliary (HPB) surgeons (86%) and medical oncologists (100%). The majority (92%) believed there was a role for simultaneous resection of scRLM, with 69% having first hand experience. Many cRS (62%) discussed all cases of known hepatic metastases with HPB prior to any resection. When a lesion was asymptomatic/minimally symptomatic, most cRS (92%) discussed them with medical oncology/HPB prior to resection (8%). Bilobar metastases (58%), patient comorbidities (35%), portal lymphadenopathy (35%), and patient age (15%) restricted cRS from obtaining HPB consultations. Many cRS (46%) did not believe that resecting hepatic metastases prior to the primary lesion might be beneficial. Most cRS (60%) reported they could not accurately predict hepatic resectability, with only 27%familiarity with evidence-based guidelines. Despite working in smaller hospitals with less access to HPB and less experience with simultaneous resections, non-cR general surgeons more commonly supported a 'liver-first' approach. conclusions: There was general agreement between cRS and general surgeons on numerous topics, but additional education is required with regard to HPB surgical capabilities and to provide truly individualized patient-centered care.

Pseudomonas aeruginosa remains an important pathogen. Our purpose was to determine the minimum inhibitory con-centration (MIc) and pharmacodynamic (PD) parameters predicting a positive response to therapy with piperacil-lin-tazobactam. Medical records were retrospectively reviewed at 3 centers. Data were recorded to assess age, type of disease, renal function, weight (body mass), MIc, antimicrobial treatment, and clinical outcome. Success was response to piperacillin-tazobactam alone, or in combination with another active agent;failure was lack of response. Of 78 eva-luable patients, 63 responded (7 UTI;56 non-UTI) and 15 did not;26 responding received combination therapy and 37 monotherapy. Piperacillin-tazobactam treatment was successful in 53 of 63 of non-UTI disease with a MIc of ≤64/4 μg/mL, but in only 3 of 7 with a MIc of >64/4 μg/mL (P = 0.023);overall 9 of 10 infections by strains with MIcs = 32 - 64 μg/mL had a successful outcome. Piperacillin estimated time above MIc at 20% separated those responding from those that did not (P = 0.019).

通过对下载的16株小反刍兽疫病毒(PPRV)N基因片段进行序列分析,合成了PPRV N基因。用NP1和NP2引物扩增。纯化的PPRV N基因在T4DNA连接酶的作用下,和PET32-a载体进行连接反应构建重组质粒pET-a-PPRV-N。经诱导表达和纯化后,得到了大量的PPRV N蛋白。用PPRV N蛋白制备免疫血清,经国际标准血清标化后作为参考阳性血清,同时以PPRV N蛋白为抗原建立间接ELISA(I-ELISA)方法。对山东省的467份羊血清检测,使用S/P(样品值/阳性值)平均值加3倍标准差的方法,计算出其临界点为0.28。应用间接ELISA和针对H蛋白的单抗为基础的c-ELISA检测来自于疫区的69份血清,结果两者的符合率为79.7%。
