BACKGROUND Acute fibroid complications are ***,failure to recognize and treat acute complications expeditiously when they occur can lead to catastrophic,even deadly,*** is a rare but potentially fatal condition resulting from infarction and infection of a fibroid through bacterial seeding and direct,hematogenous,or lymphatic *** though the diagnosis is established through clinical and laboratory findings,imaging is an important complementary method to support the suspected *** SUMMARY Herein,we report a case of a pyomyoma in a nulliparous woman previously diagnosed with uterine leiomyomatosis according to ultrasound *** patient had previously attended the emergency room due to hypogastric pain unresponsive to *** a week of persistent pain,she developed sepsis without any identifiable *** resonance imaging revealed findings compatible with uterine myomatosis with red degeneration,and a possible diagnosis of a pyomyoma was made according to the imaging findings along with the patient’s clinical *** decided to perform myomectomy(which is an infrequently performed surgical treatment due to the procedure’s intrinsic implic-ations)due to the patient’s desire to preserve *** results revealed a uterine leiomyoma with coagulative and liquefactive necrosis,while the tissue culture showed gram-negative cocci bacteria,which were successfully treated using antibiotic *** patient’s health status improved after several *** The main diagnostic tools to evaluate pyomyomas are the clinical and laboratory findings as well as tissue ***,magnetic resonance imaging can help to corroborate these findings as well as to better characterize myomas with its different complications.

Hypopigmented Mycosis Fungoides in a 7-Year-Old Boy

Hypopigmented mycosis fungoides (HMF) is an uncommon variant of cutaneous T-cell lymphoma. It is more frequent in dark-skinned people, particularly children. The HMF diagnose is difficult, especially in early stages because this condition resembles benign skin diseases. Thus is histopathological analysis very important for the diagnosis. We report a case of a 7-year-old child with widespread HMF confirmed by histopathology that showed cells tagging along the dermal/epidermal junction and extending into the epidermis in a pattern of epidermotropism and focal cell aggregates in the epidermis (Pautrier’s microabscess). We demonstrate the importance of clinical suspicion for this cutaneous neoplasia in patients with hypopigmentated lesions.

Adenosquamous carcinoma of the head and neck (ASC) is a rare and aggressive variant of squamous cell carcinoma, a locally aggressive malignancy characterized by the presence of two distinct components: a squamous cell carcinoma and an adenocarcinoma. The purpose of this study was to report an additional rare case of adenosquamous carcinoma affecting the maxilla, with clinical microscopic features and a complex mid-facial rehabilitation.

Corrosion in Control Systems Decrease the Lifetime of the Electronic Devices of the Industrial Plants of Mexicali,BC,Mexico

The principal factor to determine the economical value of the products manufactured in the electronics industry is due to the productive yielding. This is important for the cost of the articles fabricated in this type of industrial plants installed in Mexicali city, where around 80% of companies are, and which fabricate electronic devices and systems, or have industrial electronic systems and machines to their manufacturing process. Mexicalicity is located in theBaja CaliforniaStateof the northwest ofMexico, which is a border city with Calexico in theCaliforniaStateof the United States of America (USA). The region located in Mexicali, is a desert area. Geothermal plant is located in this area, which is an important industry and supplies electricity to this city and its valleys and some cities on southwest of United States for daily activities. This company emits hydrogen sulfide (H2S) as a main air pollutant that reacts with oxygen in the atmosphere, generating sulfur oxides (SOX). This chemical is dispersed to the city of Mexicali in which industrial plants are located with electronic control systems, and penetrates to indoor rooms. Those cause the corrosion process. The presence of corrosion leads to the deterioration of electrical connectors, the connections of electronic systems and the decreasing of the lifetime of these control systems. Other air pollutants that are considered as chemical agents which cause damage to materials used in the electronics industry, are the sulfurs and nitrogen oxides (NOX), emitted from the traffic vehicle and some industries. This causes the low productive yielding of electrical and electronic devices and systems used in the companies of this city, and is a major concern to specialized people, managers and owners. AIM To investigate any changing trends in the etiologies of hepatocellular carcinoma(HCC) in Argentina during the last years. METHODS A longitudinal cohort study was conducted by 14 regional hospitals starting in 2009 through 2016. All adult patients with newly diagnosed HCC either with pathology or imaging criteria were included. Patients were classified as presenting non-alcoholic fatty liver disease(NAFLD) either by histology or clinically, provided that all other etiologies of liver disease were ruled out, fatty liver was present on abdominal ultrasound and alcohol consumption was excluded. Complete follow-up was assessed in all included subjects since the date of HCC diagnosis until death or last medical *** A total of 708 consecutive adults with HCC were included. Six out of 14 hospitals were liver transplant centers(n = 484). The prevalence of diabetes mellitus was 27.7%. Overall, HCV was the main cause of liver disease related with HCC(37%) including cirrhotic and non-cirrhotic patients, followed by alcoholic liver disease 20.8%, NAFLD 11.4%, cryptogenic 9.6%, HBV 5.4% infection, cholestatic disease and autoimmune hepatitis 2.2%, and other causes 9.9%. A 6-fold increase in the percentage corresponding to NAFLDHCC was detected when the starting year, i.e., 2009 was compared to the last one, i.e., 2015(4.3% vs 25.6%; P < 0.0001). Accordingly, a higher prevalence of diabetes mellitus was present in NAFLD-HCC group 61.7% when compared to other than NAFLD-HCC 23.3%(P < 0.0001). Lower median AFP values at HCC diagnosis were observed between NAFLD-HCC and non-NAFLD groups(6.6 ng/m L vs 26 ng/m L; P = 0.02). In Silico Screening of Potential Drug with Antileishmanial Activty and Validation of their Activity by in Vitro and in Vivo Studies

The research on discovery and development of new treatments for cutaneous leishmaniasis has been declared as priority. Using bioinformatics approaches, this study aimed to identify antileishmanial activity in drugs that are currently used as anti-inflammatory and wound healing by such anti-Leishmania activity was validated by in vitro and in vivo assays. In silico analysis identified 153 compounds from which 87 were selected by data mining of DrugBank database, 22 and 44 were detected by PASS （http：//***） and BLAST （http：//***. gov/） alignment, respectively. The majority of identified drugs are used as skin protector, anti-acne, anti-ulcerative （wound healer） or anti-inflammatory and few of them had specific antileishmanial activity. The efficacy as antileishmanial was validated in vitro in 12/23 tested compounds and in all seven compounds that were evaluated in in vivo assays. Notably, this is the first report of antileishmanial activity for adapalene. In conclusion, bioinformatics tools not only can help to reduce time and cost of the drug discovery process but also may increase the chance that candidates identified in silico which have a validated antileishmanial activity by combining different biological properties.

Determinants of percutaneous coronary intervention success in repeat chronic total occlusion procedures following an initial failed attempt

AIM To investigate the rates and determinants of success of repeat percutaneous coronary intervention(PCI) following an initial failed attempt at recanalising the chronic total occlusions(CTO) *** In 445 consecutive first attempt CTO-PCI procedures in our institution,procedural failure occurred in 149(33.5%). Sixty-four re-PCI procedures were performed in 58 patients(39%) all had a single CTO. Procedural and outcome data in the re-PCI population was entered into the institutional database. A retrospective analysis of clinical,angiographic and procedural data was performed. RESULTS Procedural success was achieved in 41(64%) procedures. Univariate analysis of clinical and angiographic characteristics showed that re-PCI success was associated with intravascular ultrasound(IVUS) guidance(19.5% vs 0%,P = 0.042),while failure was associated with severecalcification(30.4% vs 9.7%,P = 0.047) and a JCTO score > 3(56.5% vs 17.1% P = 0.003). Following multiple regression analysis the degree of lesion complexity(J-CTO score > 3),IVUS use,involvement of an experienced CTO operator and LAD CTO location were significant predictors of successful re-PCI. Overall the complication rate was low,with the only MACCE two periprocedural MI's neither of which required intervention. CONCLUSION Re-PCI substantially increases the overall success rate of CTO revascularization. Predictors of re-PCI success included the use of IVUS,the involvement of an experienced CTO operator in the repeat attempt and the location of the CTO.
