Peripheral nerve injuries induce a severe motor and sensory deficit. Since the availability of autologous nerve transplants for nerve repair is very limited, alternative treatment strategies are sought, including the use of tubular nerve guidance conduits(tnGcs). However, the use of tnGcs results in poor functional recovery and central necrosis of the regenerating tissue, which limits their application to short nerve lesion defects(typically shorter than 3 cm). Given the importance of vascularization in nerve regeneration, we hypothesized that enabling the growth of blood vessels from the surrounding tissue into the regenerating nerve within the tnGc would help eliminate necrotic processes and lead to improved regeneration. In this study, we reported the application of macroscopic holes into the tubular walls of silk-based tnGcs and compared the various features of these improved silk^(+) tnGcs with the tubes without holes(silk^(–) tnGcs) and autologous nerve transplants in an 8-mm sciatic nerve defect in rats. Using a combination of micro-computed tomography and histological analyses, we were able to prove that the use of silk^(+) tnGcs induced the growth of blood vessels from the adjacent tissue to the intraluminal neovascular formation. A significantly higher number of blood vessels in the silk^(+) group was found compared with autologous nerve transplants and silk^(–), accompanied by improved axon regeneration at the distal coaptation point compared with the silk^(–) tnGcs at 7 weeks postoperatively. In the 15-mm(critical size) sciatic nerve defect model, we again observed a distinct ingrowth of blood vessels through the tubular walls of silk^(+) tnGcs, but without improved functional recovery at 12 weeks postoperatively. Our data proves that macroporous tnGcs increase the vascular supply of regenerating nerves and facilitate improved axonal regeneration in a short-defect model but not in a critical-size defect model. BAcKGROUnD Video-capsule endoscopy(VcE)is an efficient tool that has proven to be highly useful in approaching several gastrointestinal *** was implemented in colombia in 2003,however current characterization of patients undergoing VcE in colombia is limited,and mainly comes from two investigations conducted before the SARS-coV-2 pandemic *** To describe the characteristics of patients undergoing VcEs and establish the main indications,findings,technical limitations,and other outstanding *** A descriptive study was carried out using data from reports of VcE(Pillcam SB3 system)use in a Gastroenterology Unit in Bogotá,colombia between September 2019 and January *** and clinical variables such as indication for the VcE,gastric and small bowel transit times(GTT,SBTT),endoscopic preparation quality,and limitations were described[n(%),median(IQR)].RESULTS A total of 133 VcE reports were *** were in men with a median age of 70 *** majority had good preparation(96.2%),and there were technical limitations in 15.8%of *** main indications were unexplained anemia(91%)or occult bleeding(23.3%).The median GTT and SBTT were 14 and 30 minutes,*** frequencies of bleeding stigma(3.79%)and active bleeding(9.09%)were low,and the most frequent abnormal findings were red spots(28.3%),erosions(17.6%),and vascular ectasias(12.5%).cOncLUSIOn VcE showed high-level *** main indication was unexplained *** bleeding was the most frequent *** with artificial intelligence,VcE can improve diagnostic precision and targeted therapeutic interventions.

Development of Extraction and Detection Method for a chemotherapeutic Drug with Phenytoin in Biological Samples

Dexamethasone is classified as a corticosteroid and is commonly used among cancer patients to decrease the amount of swelling around the tumor. Among patients with cancer, in particular brain tumors, seizures can become a daily routine in their everyday lives. To counteract the seizures, an antiepileptic drug such as phenytoin is administered to act as an anticonvulsant. Phenytoin and dexamethasone are frequently administrated concurrently to brain cancer patients. A previous study has shown that phenytoin serum concentration decreases when administrated concurrently with dexamethasone. Thus, it is important to monitor the concentration of these two drugs in biological samples to ensure that the proper dosages are administrated to the patients. This study aims to develop an effective extraction and detection method for dexamethasone and phenytoin. A reverse-phase high-performance liquid chromatography (HPLc) method with UV/Vis detection has been developed to separate phenytoin and dexamethasone at 219 nm and 241 nm respectively from urine samples. The mobile phase consists of a mixture of 0.01 M KH2PO4, acetonitrile, and methanol adjusted to pH 5.6 (48:32:20) and is pumped at a flow rate of 1.0 mL/min. calibration curves were prepared for phenytoin and dexamethasone (r2 > 0.99). An efficient solid-phase extraction (SPE) method for the extraction of dexamethasone and phenytoin from urine samples was developed with the use of c-18 cartridges. The percent recovery for phenytoin and dexamethasone is 95.4% (RSD = 1.15%) and 81.1% (RSD = 3.56%) respectively.

The levered-dual response(LEDAR)coulomb-damped system attains near resonant vibration isolation by differential preloads/offsets in linear *** takes the advantages of both the preloads/offsets in linear springs and the guiderail friction for realizing different levels of vibration *** isolation capacities are investigated on the strategies with both the horizontal and vertical guiderails,with the horizontal rail only,and without *** compressive preloads generally result in the consumption of most of the initial excitation energy so as to overcome the potential *** isolation onsets at the frequency ratio of 1∓0.095 on the left-hand side(LHS)and the right-hand side(RHS)of the lever are relative to the load plate *** observed near resonant isolation thus makes the LEDAR system a candidate for the isolation of the mechanical systems about resonance while opening a path for simultaneous harvesterisolation functions and passive functions at extreme frequencies.

BAcKGROUnD The use of uncemented cups during total hip arthroplasty(THA)has gained popularity in recent *** Robert Mathys(RM)pressfit cup,an uncemented monoblock implant is expected to preserve bone density due to its composition and external surface,while reducing backside wear with its monoblock *** factors should lead to a high survival rate of the *** To evaluate the mid-term survival and functional outcome of the RM Pressfit cup in a large study *** Between 2011 and 2020,we included 1324 patients receiving a primary THA using the RM pressfit *** clinical follow-up was performed at 2 years postoperatively with the Dutch arthroplasty register used to assess implant status *** for acetabular failure and reason for revision were reported to evaluate implant survival,while the hip disability and osteoarthritis outcome score(HOOS)scores were used to assess functional *** The mean age at surgery was 64.9 *** mean follow-up was 4.6 *** the 1324 THAs performed,13 needed cup revisions within 5 years after index THA:5 due to aseptic loosening,6 due to infection,2 due to dislocation and 2 due to other *** resulted in a 5-year cup survival of 98.8%(95%cI:98.1-99.5).nine of the cup revisions occurred within the first year after index *** scores increased significantly in all domains during the first year and levelled out during the second *** In the present study,the RM pressfit cup demonstrated excellent clinical outcomes at mid-term follow-up;however,future studies are needed to assess the long-term outcomes of this acetabular implant.

The effectiveness of different wear indicators in quantifying wear on railway wheels of freight wagons

Railway infrastructure relies on the dynamic interaction between wheels and rails;thus,assessing wheel wear is a critical aspect of maintenance and *** paper focuses on the wheel-rail wear indicator T-gamma(Tγ).Amidst its use,it becomes apparent that Tγ,while valuable,fails to provide a comprehensive reflection of the actual material removal and actual contact format,which means that using only Tγas a target for optimization of profiles is not *** this work,three different freight wagons are evaluated:a meter-gauge and a broad-gauge heavy haul vehicles from South American railways,and a standard-gauge freight vehicle operated in Europe,with different axle loads and dissimilar new wheel/rail *** vehicles are subjected to comprehensive multibody simulations on various *** simulations aimed to elucidate the intricate relationship between different wear indicators:Tγ,wear index,material removal,and maximum wear depth,under diverse curves,non-compensated lateral accelerations(A_(nc)),and *** findings showed a correlation of 0.96 between Tγand wear depth and 0.82 between wear index and material removed for the outer *** the results,the Tγis better than the wear index to be used when analyzing wear depth while the wear index is more suited to foresee the material *** results also show the low influence of A_(nc)on wear index and Tγ.By considering these factors together,the study aims to improve the understanding of wheel-rail wear by selecting the best wear analysis approaches based on the effectiveness of each parameter.
