Efficacy of cap-assisted colonoscopy according to lesion location and endoscopist training level

AIM: To evaluate the efficacy of cap-assisted colonoscopy(CAC) for detection of colorectal polyps and adenomas according to the lesion location and endoscopist training ***: Patients 20 years or older, who underwent their first screening colonoscopy in a single tertiary center from May 2011 to December 2012 were enrolled in this study. All patients underwent either CAC or standard colonoscopy(SC), and all of the procedures were performed by 11 endoscopists(8 trainees and 3 experts). All procedures were performed with highdefinition colonoscopes and narrow band imaging. The eight trainees had experiences of performing 150 to 500 colonoscopies, and the three experts had experiences of performing more than 3000 colonoscopies. A 4-mmlong transparent cap was attached to the end of a colonoscope in the CAC group. We retrospectively evaluated the number of polyps and adenomas, polyp detection rate(PDR), and the number of adenomas and adenoma detection rate(ADR) according to the lesion location and endoscopist training level between CAC and SC. We also evaluated the number of polyps and adenomas according to their size between CAC and ***: Overall, PDR and ADR using CAC were significantly higher than those using SC for both whole colon(48.5% vs 40.7%, P = 0.012; 35.7% vs 28.3%, P = 0.012) and right-side colon(35.3% vs 26.6%, P = 0.002; 27.0% vs 16.9%, P < 0.001). The number of polyps and adenomas per patient using CAC was significantly higher than that using SC for both the whole colon(1.07 ± 1.59 vs 0.82 ± 1.31, P = 0.008; 0.72 ± 1.32 vs 0.50 ± 1.01, P = 0.003) and right-side colon(0.66 ± 1.18 vs 0.41 ± 0.83, P < 0.001; 0.46 ± 0.97 vs 0.25 ± 0.67, P < 0.001). In the trainee group, the PDR and ADR using CAC were significantly higher than those using SC for both the whole colon(46.7% vs 39.7%, P = 0.040; 33.9% vs 26.0%, P =0.012) and right-side colon(34.2% vs 26.5%, P = 0.015; 25.3% vs 15.9%, P = 0.001). CYP3A5 unexpectedly regulates glucose metabolism through the AKT-TXNIP-GLUT1 axis in pancreatic cancer

Mixed-dimensional composite structures using zero-dimensional(0D)quantum dots(QDs)and two-dimensional(2D)transition metal dichalcogenides(TMDs)materials are expected to attract great interest in optoelectronics due to the potential to generate new optical ***,we report on the unique optical characteristics of a devices with mixed dimensional vertically stacked structures based on tungsten diselenide(WSe_(2))/CdSeS QDs monolayer/molybdenum disulfide(MoS_(2))(2D/0D/2D).Specifically,it exhibits an ambipolar photoresponse characteristic,with a negative photoresponse observed in the 400-600 nm wavelength range and a positive photoresponse appeared at 700 nm *** resulted in the high negative responsivity of up to 52.22 mA·W^(−1)under 400 nm,which is 163 times higher than that of the photodetector without CdSeS *** also demonstrated the negative photoresponse,which could be due to increased carrier collision probability and non-radiative *** modeling and simulation reveal that Auger recombination among the types of non-radiative recombination is the main cause of negative photocurrent ***,we discovered ambipolar photoresponse near a specific wavelength corresponding to the energy of quantum *** study revealed interesting phenomenon in the mixed low-dimensional stacked structure and paved the way to exploit it for the development of innovative photodetection materials as well as for optoelectronic applications.

Reliable beacon transmission based MAC protocol for LR-WPANs over WLAN interferences

The use of IEEE 802.15.4 standard based application systems has been rapidly increasing, for example, in medical services, sensor networks, public safety systems, and home automation systems. However, issues arise from the fact that IEEE 802.15.4 standard based low rate wireless personal area networks(LR-WPANs) use the same frequency bands as wireless local area networks(WLANs), and they interfere with each other. Based on past research on this issue, the interference has a more serious impact on LR-WPANs' performance than on WLANs' performance. In this paper we propose a method to improve LR-WPANs' performance while coexisting with WLANs, which is called the reliable beacon transmission based medium access control(MAC) protocol. Since the reliability of a beacon frame is important, in this method, only the beacon frame is transmitted in interference-free channels, and the data packets are transmitted in interfered channels instead of abandoning the channels altogether. This method increases the reliability of beacon frames as well as overall channel utilizations. The effectiveness of the proposed method was evaluated through extensive simulations, and this paper proves that this method improves the performance of IEEE 802.15.4 based wireless sensor networks(WSNs) over WLANs' interferences.

Acupuncture ameliorates atopic dermatitis by modulating gut barrier function in a gut microbiota-dependent manner in mice

Objective: Atopic dermatitis(AD) is a chronic inflammatory skin disease that may be linked to changes in the gut microbiome. Acupuncture has been proven to be effective in reducing AD symptoms without serious adverse events, but its underlying mechanism is not completely understood. The purpose of this study was to investigate whether the potential effect of acupuncture on AD is gut ***: AD-like skin lesions were induced by applying MC903 topically to the cheek of the *** was done at the Gok-ji(LI11) acupoints. AD-like symptoms were assessed by lesion scores,scratching behavior, and histopathological changes; intestinal barrier function was measured by fecal output, serum lipopolysaccharide levels, histopathological changes, and mRNA expression of markers involved in intestinal permeability and inflammation. Gut microbiota was profiled using 16S rRNA gene sequencing from fecal ***: Acupuncture effectively improved chronic itch as well as the AD-like skin lesions with epidermal thickening, and also significantly altered gut microbiota structure as revealed by β-diversity indices and analysis of similarities. These beneficial effects were eliminated by antibiotic depletion of gut microbiota,but were reproduced in gut microbiota-depleted mice that received a fecal microbiota transplant from acupuncture-treated mice. Interestingly, AD mice had intestinal barrier dysfunction as indicated by increased intestinal permeability, atrophy of the mucosal structure(reduced villus height and crypt depth), decreased expression of tight junctions and mucus synthesis genes, and increased expression of inflammatory mediators in the ileum. Oxidative stress and inflammatory signaling in cerulein pancreatitis

Oxidative stress is considered to be an important regulator of the pathogenesis of acute pancreatitis. Reactive oxygen species (ROS) regulate the activation of inflammatory cascades, the recruitment of inflammatory cells and tissue damage in acute pancreatitis. A hallmark of the inflammatory response in pancreatitis is the induction of cytokine expression, which is regulated by a number of signaling molecules including oxidant-sensitive transcription factors such as nuclear factor-κB (NF-κB) and activator protein-1 (AP-1), signal transducer and activator of transcription 3 (STAT3), and mitogen-activated protein kinases (MAPKs). Cross-talk between ROS and pro-inflammatory cytokines is mediated by NF-κB, AP-1, STAT3, and MAPKs; this crosstalk amplifies the inflammatory cascade in acute pancreatitis. Therapeutic studies have shown that antioxidants and natural compounds can have beneficial effects for patients with pancreatitis and can also influence the expression of proinflammatory cytokines in cerulein-induced pancreatitis. Since oxidative stress may activate inflammatory signaling pathways and contribute to the development of pancreatitis, antioxidant therapy may alleviate the symptoms or prevent the development of pancreatitis. Since chronic administration of high doses of antioxidants may have deleterious effects, dosage levels and duration of antioxidant treatment should be carefully determined.

Immunotherapy for pancreatic cancer

Pancreatic cancer, a highly lethal cancer, has the lowest 5-year survival rate forseveral reasons, including its tendency for the late diagnosis, a lack of serologicmarkers for screening, aggressive local invasion, its early metastaticdissemination, and its resistance to chemotherapy/radiotherapy. Pancreaticcancer evades immunologic elimination by a variety of mechanisms, includinginduction of an immunosuppressive microenvironment. Cancer-associatedfibroblasts interact with inhibitory immune cells, such as tumor-associatedmacrophages and regulatory T cells, to form an inflammatory shell-like desmoplasticstroma around tumor cells. Immunotherapy has the potential to mobilizethe immune system to eliminate cancer cells. Nevertheless, althoughimmunotherapy has shown brilliant results across a wide range of malignancies,only anti-programmed cell death 1 antibodies have been approved for use inpatients with pancreatic cancer who test positive for microsatellite instability ormismatch repair deficiency. Some patients treated with immunotherapy whoshow progression based on conventional response criteria may prove to have adurable response later. Continuation of immune-based treatment beyond diseaseprogression can be chosen if the patient is clinically stable. Immunotherapeuticapproaches for pancreatic cancer treatment deserve further exploration, given theplethora of combination trials with other immunotherapeutic agents, targetedtherapy, stroma-modulating agents, chemotherapy, and multi-way combinationtherapies.
