Prognostic value of claudin 18.2 expression in gastric adenocarcinoma

Effect of interferon and ribavirin combined with amantadine in interferon and ribavirin non-responder patients with chronic hepatitis C (genotype 1)

AIM: To evaluate the efficacy of amantadine plus interferonalpha and ribavirin in non-responder patients with chronic hepatitis ***: Twenty-six non-responder patients received the regimen of IFN-α-2a at a dose of 6 million units three times a week, 1 000-1 200 mg of ribavirin daily, and 200 mg of amantadine daily in divided doses over 48 wk. After the end of treatment, at the 72nd wk, a sustained viral response rate was ***: An early (after 12 wk of therapy) response was seen in 34.6% (9/26) of patients. Response rate at the 24th wk was 42.3% (11/26). End of treatment response (ETR) was 53.8% (14/26). Sustained viral response (SVR) was 42.3% (11/26). There was a statistically significant difference between 0 and 12 wk (P = 0.04), 0 and 24 wk (P = 0.01), 0 and 48 wk (P = 0.00), and 0 and 72 wk (P = 0.001). No patient had severe adverse effects during the ***: Combination regimen of interferon-α,ribavirin and amantadine can enhance sustained viral response on IFN-α and ribavirin non-responder patients with HCV. Triple therapy with amantadine should be evaluated in further studies.

Honey prevents hepatic damage induced by obstruction of the common bile duct

AIM:To examine the possible effects of honey supplementation on hepatic damage due to obstruction of the common bile duct in an experimental rat model. METHODS:The study was performed with 30 male rats divided into three groups:a sham group,an obstructive jaundice group,and an obstructive jaundice plus honey group. At the end of the study period,the animals were sacrificed,and levels of nitric oxide(NO) ,and NO synthase(NOS) activities were measured in liver tissues,and levels of adenosine deaminase(ADA) and alanine transaminase(ALT) activities were measured in serum. RESULTS:Blood ALT and ADA activities were significantly elevated in the jaundice group as compared to those of the sham group. In the obstructive jaundice plus honey group,blood ALT and ADA activities were significantly decreased as compared to those of the jaundice group. In erythrocytes and liver tissues,NO levels were found to be significantly higher in the obstructive jaundice plus honey group compared to those of the sham group. Additionally,NO levels were found to be significantly higher in liver tissues from the animals in the obstructive jaundice plus honey group than those of the jaundice ***:Honey was found to be beneficial in the prevention of hepatic damage due to obstruction of the common bile duct.

A rare case: Spontaneous cutaneous fistula of infected splenic hydatid cyst

Hydatid disease is caused by the larval stage of the genus Echinococcus. Live hydatid cysts can rupture into physiologic channels, free body cavities or adjacent organs. Although hydatid disease can develop anywhere in the human body, the liver is the most frequently involved organ, followed by the lungs. Cysts of the spleen are unusual. There are only five case reports of spontaneous cutaneous fistulization of liver hydatid cysts in the literature. But there isn＇t any report about cutaneous fistula caused by splenic hydatid cyst. We report a first case of spontaneous cutaneous fistula of infected splenic hydatid cyst. A 43-year-old man was admitted to our Emergency Service with abdominal pain and fluid drainage from the abdominal wall. He has been suffering from a reddish swelling on the abdominal wall skin for four months. After a white membrane had been protruded out from his abdominal wall, he was admitted to our Emergency Service. On physical examination, a white membrane was seen to protrude out from the 2 crux I cm skin defect on the left superolateral site of the umblicus. Large, complex, cystic and solid mass of 9.5 cm-diameter was located in the spleen on ultrasonographic examination. At operation, partial cystectomy and drainage was performed. After the operation, he was given a dosage of 10 mg/kg per day of albendazole, divided into three doses. He was discharged on the postoperative 10^th d. It should be kept in mind that splenic hydatid cysts can cause such a rare complication.

Effects of microalgae chlorella species crude extracts on intestinal adaptation in experimental short bowel syndrome

AIM： To evaluate the effects of chlorella crude extract （CCE） on intestinal adaptation in rats subjected to short bowel syndrome （SBS）. METHODS： Wistar rats weighing 230-260 g were used in the study. After anesthesia a 75% small bowel resection was performed. Rats were randomized and divided into groups. Control group （n = 10）： where 5% dextrose was given through a gastrostomy tube, Enteral nutrition （EN） group （n = 10）： Isocaloric and isonitrogen EN （Alitraq, Abbott, USA）, study group （n = 10）： CCE was administrated through a gastrostomy tube. Rats were sacrificed on the fifteenth pcstoperative day and blood and tissue samples were taken. Histopathologic evaluation, intestinal mucosal protein and DNA levels, intestinal proliferation and apoptosis were determined in intestinal tissues, and total protein, albumin and citrulline levels in blood were studied. RESULTS： In rats receiving CCE, villus lengthening, crypt depth, mucosal DNA and protein levels, intestinal proliferation, and serum citrulline, protein and albumin levels were found to be significantly higher than those in control group. Apoptosis in CCE treated rats was significantly reduced when compared to EN group rats. CONCLUSION： CCE has beneficial effects on intestinal adaptation in experimental SBS.

Relationship between retinopathy and cirrhosis

AIM: To evaluate ophthalmic disorders with special attention to retinopathy in cirrhotic patients. Vitamin A deficiency-related ophthalmopathy, xerophthalmia, and color blindness may be documented in cirrhosis due to various etiologies. Retinopathy is an obscure feature of cirrhosis. METHODS: Thirty-two cirrhotic patients, who were followed up by Clinics of Gastroenterology, Izmir Ataturk Teaching and Research Hospital, were enrolled to the study. Associated systemic diseases such as diabetes mellitus and hypertension were excluded. Thirty-two healthy volunteers took part as the control subjects. All participants had ophthalmologic examination in the same hospital. RESULTS: Five (15.6%) of the cirrhotic subjects had soft exudate in the retina. None of the control subjects had retinopathy (P＜0.05). Intraocular pressure (IOP) measured for both eyes were also significantly lower in the cirrhotics (P＜0.05 vs P = 0.01). There were no statistically significant differences between the two groups in terms of other ophthalmic pathologies. The ophthalmic findings did not show up any differences according to the etiology of cirrhosis. CONCLUSION: Soft exudates may develop in cirrhotic patients probably due to loss of synthetic function of liver and hemodynamic effects of portal hypertension. Retinopathy must be sought in cirrhosis because of its severe morbidity.

Geographically Weighted Regression and Secondary Variables for Mapping of Meteorological Data

GA （geostatistical analyst） is an indispensable tool to analyze various and plenty of data in GIS （geographic information system）. Spatial distribution is the most effective factor for predicting of meteorological maps at the point of performance or reliability of the model. Generally, classical interpolation methods may not be sufficient to produce accurate maps. GA is more considerable in this state. Secondary variables affect the precious of prediction models especially meteorological data mapping. In this study 245 meteorological data stations have been evaluated to produce precipitation model maps in Turkey. Long term （25 years） mean annual and monthly precipitation data from Turkish State Meteorological Service and elevation, slope and aspect values from DEM （Digital Elevation Model） were registered. OK （Ordinary Kriging）, OCK （Ordinary Co-Kriging） and GWR （Geographically Weighted Regression） have been used as a method to compare the models. With the study if there are effects of secondary variables to precipitation models have been illustrated on the prediction maps. Besides comparing statistical values, regional effects of secondary variables have been determined and illustrated on the maps numerically. As a result to define precipitation distribution spatially R2 values between measured and predicted values have been calculated 0.55 for Kriging, 0.67 for OCK and 0.86 for GWR. Cross validation indicated that GWR interpolation yields the smallest prediction error with elevation, slope and aspect. Spatial distribution of meteorological stations is also other important factor for similar studies.

Oxidant/antioxidant balance in the aqueous humor of patients with glaucoma

AIM： To evaluate total antioxidant status （TAS）, total oxidant status （TOS）, and the oxidative stress index （OSI） of the aqueous humor （AH） in patients with glaucoma. METHODS： The prospective study was composed of a study group （n-31） and a control group （n=31）. Fifteen patients in the study group were diagnosed with primary open angle glaucoma （POAG）, and 16 patients were diagnosed with pseudoexfoliation glaucoma （PEG）. The control group was composed of non-glaucomatous patients with cataracts. AH samples were collected and analyzed for TAS, TOS, and OSI levels. RESULTS： Mean AH TAS level was significantly higher in patients with glaucoma than that in the control group （P〈0.01）. Mean TOS and OSI levels tended to increase in patients with glaucoma. No significant differences in TAS, TOS, or OSI levels were observed between patients with POAG and PEG. CONCLUSION： High levels of TAS were observed in patients with glaucoma, which was likely a response to the increased oxidative stress observed in these patients.

Effects of extra-corporeal shock waves on penile hemodynamics and histopathology in rats

Aim: To study the effect of extra-corporeal shock wave (ESW) on the penile hemodynamics and histopathology in rats. Methods: Adult male Sprague-Dawley rats were divided at random into 3 groups. ESW application was performed with a Siemens Lithostar with the rats under anesthesia lying prone on the balloon probe. Rats in Group Ⅰ received a total of 1000 shocks at 18 kV and immediately underwent hemodynamic evaluation performed by direct electrostimulation of the cavernous nerve and measurement of intracavernous pressure (ICP). Rats in Group Ⅱ received 3 times 1000 shocks at 18 kV at weekly intervals and hemodynamic evaluation was performed 1 month after the last ESW application. Group Ⅲ served as the control. Histopathological examinations of penile tissues were done on Masson's trichrome and hematoxylin and eosin stained sections. Results: Penile hemodynamic evaluation showed a trend toward a diminished mean maximal ICP, duration of erection, ICP during the plateau phase and maximal ICP/blood pressure ratio in Group Ⅰ, although there was no significant significance. The mean latency period in Groups Ⅰ and Ⅱ was prolonged. Petechial bleeding within tunical layers and small loci of hemorrhage within the corpora cavernosa were observed in Group Ⅰ. However, histopathological examination failed to reveal any significant differences between the groups in terms of smooth muscle content, tunical thickness, organization of collagen bundles and elastic fiber-lattice framework.
