BACKGROUND Osteochondromas are the most common benign bone tumor,accounting for 36%of benign bone *** found within the appendicular skeleton,osteochondromas of the spine are rare,comprising 4%to 7%of primary benign spinal *** SUmmARY We report a case of a solitary lumbar osteochondroma in an 18-year-old male with a history of a suprasellar pineal germinoma treated with combined chemotherapy and *** underwent mass excision and partial laminectomy with the ultrasonic bone scalpel(misonix,Farmingdale,NY,United States)at the L5 Level without the use of *** patient returned to work and full activities without back pain at 3 mo *** Osteochondromas are common tumors of the appendicular skeleton but rarely occur within the *** case discussion supplements current osteochondroma literature by describing an unusual presentation of this tumor.

BACKGROUND: The current study evaluated factors influencing survival in patients diagnosed with human immunodeficiency virus (HIV)-related primary central nervous system lymphoma (PCNSL), with a focus on the effects of therapeutic radiotherapy (RT) and highly active antiretroviral therapy (HAART). mETHODS: A retrospective chart review of patients with a diagnosis of HIV-related PCNSL at one of five university hospitals between 1987 and 1998 was performed. Clinical details including antiretroviral agent use, brain imaging scan results, RT use, and survival outcomes were recorded. RESULTS: One hundred eleven patients with HIV-related PCNSL were identified. The annual incidence decreased significantly between 1992 and 1995 and between 1996 and 1998 (P=0.04).

AIm: To describe the disease and psychosocial outcomes of an inflammatory bowel disease (IBD) transition cohort and their ***: Patients with IBD, aged > 18 years, who had moved from paediatric to adult care within 10 years were identified through IBD databases at three tertiary hospitals. Participants were surveyed regarding demographic and disease specific data and their perspectives on the transition process. Survey response data were compared to contemporaneously recorded information in paediatric service case notes. Data were compared to a similar age cohort who had never received paediatric IBD care and therefore who had not undergone a transition ***: There were 81 returned surveys from 46 transition and 35 non-transition patients. No statistically significant differences were found in disease burden, disease outcomes or adult roles and responsibilities between cohorts. Despite a high prevalence of mood disturbance (35%), there was a very low usage (5%) of psychological services in both cohorts. In the transition cohort, knowledge of their transition plan was reported by only 25/46 patients and the majority (54%) felt they were not strongly prepared. A high rate (78%) of discussion about work/study plans was recorded prior to transition, but a near complete absence of discussion regarding sex (8%), and other adult issues was recorded. Both cohorts agreed that their preferred method of future transition practices (of the options offered) was a shared clinic appointment with all key ***: Transition did not appear to adversely affect disease or psychosocial outcomes. Current transition care processes could be optimised, with better psychosocial preparation and agreed transition plans.

Irritable bowel syndrome (IBS) is one of the most common gastrointestinal disorders causing patients to seek medical treatment. It is relatively resource intensive and the source of significant morbidity. Recent insights into the pathophysiology and treatment of IBS has given clinicians more options than ever to contend with this disorder. The purpose of our paper is to review older, “classic” treatments for IBS as well as newer agents and “alternative” therapies. We discuss the evidence base of these drugs and provide context to help develop appropriate treatment plans for IBS patients.

Arsenic variability and groundwater age in three water supply wells in southeast New Hampshire

Three wells in New Hampshire were sampled bimonthly over three years to evaluate the temporal variability of arsenic concentrations and groundwater *** samples had measurable concentrations of arsenic throughout the entire sampling period and concentrations in individual wells had a mean variation of more than 7 μg/*** time series data from this sampling effort showed that arsenic concentrations ranged from a median of 4 mg/L in a glacial aquifer well (SGW-65) to medians of 19 μg/L and 37 μg/L in wells (SGW-93 and KFW-87) screened in the bedrock aquifer,*** high arsenic concentrations were associated with the consistently high pH (median ≥- 8) and low dissolved oxygen (median mg/L) in the bedrock aquifer wells,which is typical of fractured crystalline bedrock aquifers in New *** from the glacial aquifer often has high dissolved oxygen,but in this case was consistently *** pH also is generally acidic in the glacial aquifer but in this case was slightly alkaline (median =7.5).Also,sorption sites may be more abundant in glacial aquifer deposits than in fractured bedrock which may contribute to lower arsenic *** groundwater ages were less than 50 years old in all three wells and correlated with conservative tracer concentrations,such as chloride;however,mean age was not directly correlated with arsenic *** concentrations at KFW-87 did correlate with water levels,in addition,there was a seasonal pattern,which suggests that either the timing of or multiple sampling efforts may be important to define the full range of arsenic concentrations in domestic bedrock *** geochemically reduced conditions and alkaline pHs are common to both bedrock and glacial aquifer wells in this study,groundwater age correlates less strongly with arsenic concentrations than geochemical *** also is evidence of direct hydraulic connection between the glacial and bedrock aquifers,which can influe

AIm: To assess the impact of bolus volume on the characteristics of small intestinal (SI) impedance ***: Concurrent SI manometry-impedance measurements were performed on 12 healthy volunteers to assess the pattern of proximal jejunal fluid bolus movement over a 14 *** subject was given 34 boluses of normal saline (volume from 1 to 30 mL) via the feeding tube placed immediately above the proximal margin of the studied segment.A bolus-induced impedance event occurred if there was > 12% impedance drop from baseline,over ≥ 3 consecutive segments within 10 s of bolus injection.A minor or major imped-ance event was defined as a duration of impedance drop minimum volume required for a detectable SI impedance event was 2 mL.A direct linear relationship between the SI bolus volume and the occurrence of impedance events was noted until SI bolus volume reached 10 mL,a volume which always produced an impedance flow *** was a moderate correlation between the bolus volume and the duration of impedance drop (r = 0.63,P mber of propagated channels (r = 0.50,P me boluses were associated with more major impedance events (≥ 10 mL boluses = 63%,3 mL boluses = 17%,and mL boluses = 0%,P = 0.02).CONCLUSION: Bolus volume had an impact on the type and length of propagation of SI impedance events and a threshold of 2 mL is required to produce an event.

The ability to determine the locations of phosphorous dopants in silicon is crucial for the design,modelling,and analysis of atom-based nanoscale devices for future quantum computing ***,several papers showed that a metrology of scanning tunnelling microscopy(STm)imaging combined with atomistic tight-binding simulations could be used to determine coordinates of a dopant buried close to a Si *** identify effects which play a crucial role in the simulation of STm images and have to be precisely modelled for STm imaging of buried dopants and multi-dopant clusters to provide reliable position *** contrast to previous work,we demonstrate that a metrology combining STm imaging with tight-binding simulations may lead to pronounced uncertainty due to tip orbital model,effects of dangling bonds and choice of local atomic basis for the tight-binding *** work is still needed to obtain a reliable STm metrology of buried dopant position.
