Infants at risk of significant hyperbilirubinemia in poorly-resourced countries:evidence from a scoping review

Background:Neonatal hyperbilirubinemia is frequently associated with disproportionately high rates of bilirubin-induced mortality and long-term morbidities in low-and middle-income countries(LMICs).This scoping review aimed to identify possible et iological/risk factors for clinically significant hyperbilirubinemia in LMICs so as to guide intervention and future research *** sources:We systematically searched PubMed,Scopus,Excerpt Medica Database,Cumulative Index to Nursing and Allied Health Literature,WHo Library Database,African Index Medicus,African Journals online,Latin American and Caribbean Health Sciences Literature,and Indian Medical Journals for reports published between January 1990 and August 2014 in LMICs with per capita income of≤US$*** included studies on the etiology of neonatal hyperbilirubinemia or hyperbilirubinemia as signifi cant morbidity for relevant maternal,perinatal and neonatal disorders without restriction on study ***:A total of 131 studies were identified in 23 LMICs from different regions of the *** factors most frequently associated with neonatal hyperbilirubinemia(in approximately 10%of all studies)were ABo and Rhesus incompatibilities,diabetes mellitus,glucose-6-phosphate dehydrogenase defi ciency,prematurity/low birth weight,infection,birth trauma,and drug-induced *** role of exclusive breast-feeding and genetic factors was sparsely ***:Several maternal,perinatal and neonatal factors are associated with neonatal hyperbilirubinemia in *** research efforts and strategies to address these factors are warranted to curtail the disease burden in these countries.

Facility-based constraints to exchange transfusions for neonatal hyperbilirubinemia in resource-limited settings

Several clinical guidelines for the management of infants with severe neonatal hyperbilirubinemia recommend immediate exchange transfusion(ET) when the risk or presence of acute bilirubin encephalopathy is established in order to prevent chronic bilirubin encephalopathy or kernicterus. However, the literature is sparse concerning the interval between the time the decision for ET is made and the actual initiation of ET, especially in low- and middle-income countries(LMICs) with significant resource constraints but high rates of ET. This paper explores the various stages and potential delays during this interval in complying with the requirement for immediate ET for the affected infants, based on the available evidence from LMICs. The vital role of intensive phototherapy, efficient laboratory and logistical support, and clinical expertise for ET are highlighted. The challenges in securing informed parental consent, especially on religious grounds, and meeting the financial burden of this emergency procedure to facilitate timely ET are examined. Secondary delays arising from posttreatment bilirubin rebound with intensive phototherapy or ET are also discussed. These potential delays can compromise the effectiveness of ET and should provide additional impetus to curtail avoidable ET in LMICs.

Qualitative and quantitative phytochemical analysis of selected mosses with different alcoholic solvents

This study was conducted to investigate the qualitative and quantitative phytochemical content of the crude extracts of Archidium ohioense,Pelekium gratum,and Hyophila involuta with different alcoholic solvents(ethanol,methanol,Seaman's Schnapps,fresh oil-palm wine,and fresh Raffia-palm wine).The mosses were collected from their natural populations on the central campus of the obafemi Awolowo University,Ile-Ife,*** yield of the extracts was weighed for all the solvents,and the qualitative and quantitative evaluations of the extracts were carried out using standard *** results of phytochemical analysis of the crude extracts from the mosses showed the presence of saponins,cardiac glycosides,triterpenes,alkaloids,flavonoids,and *** quantitative phytochemical analysis of the crude extracts showed that ethanolic extracts of Hyophila involuta had the highest flavonoid content(288.37±0.10 mg RE/g),and Raffiapalm-wine extracts of Hyophila involuta had the highest saponin content(224.70±0.02 mg/g),while the methanolic extract of Archidium ohioense had the highest cardiac glycosides content(63.71±0.14 mg/g),and the Raffia-palm wine extract of Hyophila involuta had the highest alkaloids content(102.50±0.12 mg/g).Raffia-and oil-palm wines were observed to be the most effective solvents for all the mosses studied,followed by Seaman's Schnapp,while methanol and ethanol were less *** study concluded that the extracts of the mosses studied contain pharmacologically active constituents that can be used for therapeutic purposes.

Effects of nitrogen nutritional stress on the morphological and yield parameters of tomato(Solanum lycopersicum L.)

This investigation was carried out to better understand the effects of nitrogen stress on the growth and yield of tomato （Solanum lycopersicon L.）. Seeds of S. lycopersicon （Ife No. 1 variety） were collected from the osun-State Ministry of Agriculture, oshogbo, Nigeria and planted in analyzed top soil. The plants were grown for a period of five weeks within which they were supplied with water and kept under optimum environmental conditions that enhanced normal growth. After this period, the plants were subjected to different levels of nitrogen stress which include： plants supplied with dis- tilled water only （n）, plants supplied with complete nutrient solution （N）, plants supplied with nutrient solution in which nitrogen concentration sources was increased by a factor of 5 （N5）, and plants supplied with nutrient solution in which nitrogen concentration sources was increased by a factor of 10 （N10）. Analysis of Variance （ANoVA） results shows that there is no significant effect of stress on the growth and morphological parameters of tomato plants. However, there was a significant effect of nitrogen stress on the yield parameters. Nitrogen stress also caused an increase in the number and size of fruits produced in plants subjected with high nitrogen concentration.

Isolation, characterization and modulatory potentials of β-stigmasterol, ergosterol and xylopic acid from Anchomanes difformis on mitochondrial permeability transition pore in vitro

objective: Secondary metabolites and polyphenolic compounds from medicinal plants have been demonstrated to have multiple biological functions with promising research and development prospects. This study examined the effect of β-stigmasterol(with ergosterol) and xylopic acid isolated from Anchomanes difformis on liver mitochondrial permeability transition pore(mPTP).Methods: The compounds were isolated by vacuum liquid chromatography. Mitochondrial swelling was assessed as changes in absorbance under succinate-energized ***:1H and13C NMR spectroscopic elucidation of the isolates affirmed the presence of β-stigmasterol with ergosterol(1:0.3) and xylopic acid. The isolates reversed the increase in lipid peroxidation and inhibited the opening of mitochondrial permeability transition pores caused by calcium and *** inhibition of m PTP offers a promising therapeutic target for the treatment of mitochondrial-associated ***: Reduction in the activity of calcium ATPase and the expression of Caspase-3 and-9 were observed, suggesting that they could play a role in protecting physicochemical properties of membrane bilayers from free radical-induced severe cellular damage and be useful in the management of diseases where much apoptosis occurs.

Development of a Model for International Traveler’s Check-In Process Using Arena Software Tool

The increasing demand for airline services amidst limited resources results in passenger dissatisfaction and dwindling revenue for airports and airlines. The dynamics of service demand and resource supply results in check-in issues for the stakeholders in the commercial aviation industry. This has the effect of impacting negatively on service performance, cost of operations, customer satisfaction, and overall throughput. Hence, this paper modelled the travelers’ check-in process at the “D Wing” of the Departure Section of Murtala Muhammed International Airport (MMIA), Ikeja, using Arena Software Tool. The work was carried out by determining the parameters of the queues at designated service points in the check-in process. The primary data required to develop the model were acquired by direct observation of passenger flow and oral interview. Thus, the average check-in time was determined. Thereafter, a model of the international check-in system of the MMIA was developed using Arena software in combination with Microsoft office tools. The data collected were therefore inputted into the model and simulated;the real result was compared with the simulation result of 133 completions and there was no significant difference. The result showed that the model is a representation of the real system under study. Further work will be tailored towards simulation (i.e. the model will be subjected to experimentation in order to have different scenario).
