Effectiveness of dry needling on reducing pain intensity in patients with myofascial pain syndrome： a Meta-analysis

OBJECTIVE: To summarize the literature about the effectiveness of dry needling(DN) on relieving pain and increasing range of motion(ROM) in individuals with myofascial pain syndrome(MPS).METHODS: Papers published from January 2000 to January 2013 were identified through an electronic search in the databases MEDLINE, Dialnet, Cochrane Library Plus, Physiotherapy Evidence Database(PEDro) and Spanish Superior Council of Scientific research(CSIC). The studies included were randomized controlled trials written in English and/or Spanish about the effectiveness of DN on pain and ROM in individuals with ***: Out of 19 clinical trials that were potentially relevant, a total of 10 were included in the Meta-analysis. regarding pain intensity reduction when measured before and immediately after the intervention, DN achieved improvement compared with the placebo treatment [d =-0.49; 95% CI(-3.21, 0.42)] and with the control group [d =-9.13;95% CI(- 14.70,- 3.56)]. However, other treatments achieved better results on the same variable compared with DN, considering the measurements for pre-treatment and immediately after [d = 2.54;95% CI(-0.40, 5.48)], as well as the pre-treatment and after 3-4 weeks [d = 4.23; 95% CI(0.78, 7.68)].DN showed a significantly increased ROM when measured before the intervention and immediately after, in comparison with the placebo [d = 2.00;95% CI(1.60, 2.41)]. However, other treatments achieved a significant better result regarding ROM when it was measured before the intervention and immediately after, as compared with DN [d =-1.42;95% CI(-1.84,-0.99)].CONCLUSION: DN was less effective on decreasing pain comparing to the placebo group. Other treatments were more effective than DN on reducing pain after 3-4 weeks. However, on increasing ROM,DN was more effective comparing to that of placebo group, but less than other treatments.

Duodenal biopsy may be avoided when high transglutaminase antibody titers are present

AIM:To evaluate the predictive value of tissue transglutaminase (tTG) antibodies for villous atrophy in adult and pediatric populations to determine if duodenal biopsy can be ***: A total of 324 patients with celiac disease (CD; 97 children and 227 adults) were recruited prospectively at two tertiary centers. Human IgA class anti-tTG antibody measurement and upper gastrointestinal endoscopy were performed at diagnosis. A second biopsy was performed in 40 asymptomatic adults on a gluten-free diet (GFD) and with normal tTG ***: Adults showed less severe histopathology (26% vs 63%, Pren. Levels of tTG antibody correlated with Marsh type in both populations (r=0.661, Pregression revealed that only tTG antibody was an independent predictor for Marsh type 3 lesions, but clinical presentation type and age were not. A cut-off point of 30 U tTG antibody yielded the highest area under the receiver operating characteristic curve (0.854). Based on the predictive value of this cut-off point, up to 95% of children and 53% of adults would be correctly diagnosed without biopsy. Despite GFDs and decreased tTG antibody levels, 25% of the adults did not recover from villous atrophy during the second year after ***: Strongly positive tTG antibody titers might be sufficient for CD diagnosis in children. However, duodenal biopsy cannot be avoided in adults because disease presentation and monitoring are different.

新冠肺炎给世界各地的教育系统带来了危机,威胁着人们的受教育权和学习过程的连续性。因此,各国出台了基于网络技术的政策来保证教育。但是,许多国家的技术基础设施并不能够胜任疫情期间在线教育的要求。委内瑞拉制定了“居家上大学（University at Home,Universidad en Casa）政策,以保证大学部门在新冠大流行期间的运行。本研究以委内瑞拉中央大学为例,探究疫情期间委内瑞拉为保证大学的正常教学活动而实施的“居家上大学”政策,研究目的在于明确委内瑞拉于疫情下的教育政策对保障大学正常教学的作用,借助混沌和复杂理论解释战略性政策帮助优化远程教学的实践效果,提出保证教育权的教育政策建议。研究对委内瑞拉中央大学的管理层（包括3名院长,1名学校主任,1名教职学术协调员,1名学校学术协调员和1名平台专家）进行了半结构性访谈,并采用内容分析方法对这些质性资料进行了归纳分析。研究结果表明,由于线上教育的条件,国家宪法要求的受教育权难以保障,而且政策的制定未征询大学管理者意见。混沌和复杂性理论原则也没有得到合理应用,因为该理论要求适应性、弹性和计划性,但疫情下委内瑞拉的事实并非如此。大学依靠自己的资源来完成教学任务,不乏有一些好的教学实践,诸如学习、信息和协作等。源于线上教学的特性,教学活动也需要更多弹性和分析。总结而言,此项研究考察了疫情期间委内瑞拉如何采取教育保障政策,以保证大学的正常教学,并提供了相关建议。委内瑞拉在疫情期间制定了“居家上大学”的教育保障政策,但政策的实施未取得理想效果。

The aim of this study was to identify the most frequent yeasts in the oral cavity of adult individuals without immune disorders and to associate the presence of these oral yeasts with different characteristics of each individual. Oral rinse samples were obtained from 96 healthy adults and cultured in Sabouraud dextrose agar media and CHROMagar. Yeasts were identified by sequencing the D1/D2 region of the 28 S rRNA gene. Probable association among the socio-demographic characteristics, body mass index, family and personal medical history,oral hygiene, tobacco and/or alcohol consumption habits and presence of oral fungi was analyzed. Contingency tables and logistic regression were employed to evaluate possible relationships between the presence of oral fungi and mixed colonization with these variables. 57.3% of the healthy individuals had oral yeasts and 21.8% had mixed colonization. The most prevalent yeasts were Candida albicans(52%), C. parapsilosis(17.9%), and ***(7.57%). Yeasts with most frequently mixed colonization were C. albicans and C. parapsilosis. No relationships were found among the variables analyzed. However, the presence of mixed colonization was related to the presence of dental prostheses(Prevalent oral yeasts C. albicans, C. parapsilosis and C. dublinensis and an association of mixed colonization with the use of dental prostheses and aparatology and poor hygiene.

Simultaneous Determination of Four Estrogens in Compost Based on Ultrasonic Solvent Extraction, Solid-Phase Extraction Clean-Up and Analysis by UHPLC-MS/MS

A rapid, cost effective and reliable analytical method was developed and validated for the simultaneous determination of four estrogens (17 β-estradiol, 17 α-ethinylestradiol, estrone, and estriol) in compost samples from the biodegradation of biological infectious hazardous wastes. Ultrasonic solvent extraction, using methanol as extraction solvent, coupled with SPE clean-up, using cartridges HLB 60 mg - 6 ml Supelco® and acetonitrile for reconstitution of eluents, was used for the simultaneous extraction of the four estrogens. Mean recoveries in the range of 98% - 107% were obtained. All compounds were separated in a single gradient run by UHPLC KinetexTM 2.6 μm XB-C18 100 ÅLC (50 × 4.6 mm) column. Analytes were detected via multiple reaction monitoring (MRM) using an AB SCIEX API-5000TM triple quadrupole (Applied Biosystems/MDS SCIEX) with electrospray ionization in negative mode. Isocratic mobile phase of Water:ACN (50:50) resulted to be the optimum. Limits of detection and quantification were on the order of 0.66 ng·g-1 and 2 ng·g-1 for all the estrogens. These limits were lower than most of the values reported in the literature for similar matrices. Suitable level of linearity, good repeatability and reproducibility with coefficients of variation is lower than 11.7%, 6.8% and 8.3%, respectively.

Clinical Outcomes with a New Model of Extended Depth of Focus Intraocular Lens

Purpose: To evaluate the early clinical outcomes of a new diffractive extended depth-of-focus (EDOF) intraocular lens (IOL). Methods: Prospective case series enrolling patients undergoing cataract surgery with implantation of the EDOF IOL AT LARA (Carl Zeiss Meditec, Jena, Germany). Distance, intermediate and near visual acuity were evaluated during a follow-up of 1 month. Likewise, defocus curve, binocular mesopic contrast sensitivity, perception of photic phenomena, patient satisfaction and spectacle independence were assessed. results: The study enrolled 38 eyes of 19 patients with a mean age of 69.5 years. A significant reduction of manifest refraction and improvement in corrected distance visual acuity (CDVA) was observed with surgery (p refractive predictability, 97.4% of eyes had a 1-month postoperative spherical equivalent within ±0.50 D. The level of patient satisfaction with distance, intermediate and near vision was high, with 95% of patients reporting that they would have the same surgical procedure with the same IOL again. Difficulties in night driving or associated to halos were low. A total of 89% of patients reported to be spectacle independent postoperatively. Conclusions: The EDOF IOL AT LARA is able to provide a functional restoration of the visual function across distances after cataract surgery, with very high levels of patient satisfaction and minimal incidence of photic phenomena.
