BACKGROUND The management of idiopathic scoliosis(IS)in skeletally immature patients should aim at three-dimensional deformity correction,without compromising spinal and chest *** 2019,the US Food and Drug Administration approved the first instrumentation system for anterior vertebral body tethering(AVBT),under a Humanitarian Device Exception,for skeletally immature patients with curves having a Cobb angle between 35°and 65°.AIM To summarize current evidence about the efficacy and safety of AVBT in the management of IS in skeletally immature *** From January 2014 to January 2021,Ovid Medline,Embase,Cochrane Library,Scopus,Web of Science,Google Scholar and PubMed were searched to identify relevant *** methodological quality of the studies was evaluated and relevant data were *** Seven clinical trials recruiting 163 patients were included in the present *** studies out of seven were classified as high quality,whereas the remaining two studies were classified as moderate quality.A total of 151 of 163 AVBT procedures were performed in the thoracic spine,and the remaining 12 tethering in the lumbar *** 117 of 163(71.8%)patients had a nonprogressive curve at skeletal ***-three of 163(14.11%)patients required unplanned revision surgery within the follow-up *** to posterior spinal fusion(PSF)was performed in 18 of 163(11%)*** AVBT is a promising growth-friendly technique for treatment of IS in growing ***,it has moderate success and perioperative complications,revision and conversion to PSF.

Climate change and rapid urbanization are pressing environmental and social concerns, with approximately 56% of the global population living in urban areas. This number is expected to rise to 68% by 2050, leading to the expansion of cities and encroachment upon natural areas, including wetlands, causing their degradation and fragmentation. To mitigate these challenges, green and blue infrastructures (GBIs), such as constructed wetlands, have been proposed to emulate and replace the functions of natural wetlands. This study evaluates the potential of eight constructed wetlands near Beijing, China, focusing on their ecosystem services (ESs), cost savings related to human health, growing/maintenance expenses, and disservices using an emergy-based assessment procedure. The results indicate that all constructed wetlands effectively purify wastewater, reducing nutrient concentrations (e.g., total nitrogen, total phosphorus, and total suspended solids). Among the studied wetlands, the integrated vertical subsurface flow constructed wetland (CW-4) demonstrates the highest wastewater purification capability (1.63E+14 sej/m^(2)/yr) compared to other types (6.78E+13 and 2.08E+13 sej/m^(2)/yr). Additionally, constructed wetlands contribute to flood mitigation, groundwater recharge, wildlife habitat protection, and carbon sequestration, resembling the functions of natural wetlands. However, the implementation of constructed wetlands in cities is not without challenges, including greenhouse gas emissions, green waste management, mosquito issues, and disturbances in the surrounding urban areas, negatively impacting residents. The ternary phase diagram reveals that all constructed wetlands provide more benefits than costs and impacts. After the first description of fatty pancreas in 1933,the effects of pancreatic steatosis have been poorly investigated,compared with that of the liver. However,the interest of research is increasing. Fat accumulation,associated with obesity and the metabolic syndrome(Met S),has been defined as "fatty infiltration" or "nonalcoholic fatty pancreas disease"(NAFPD). The term "fatty replacement" describes a distinct phenomenon characterized by death of acinar cells and replacement by adipose tissue. Risk factors for developing NAFPD include obesity,increasing age,male sex,hypertension,dyslipidemia,alcohol and hyperferritinemia. Increasing evidence support the role of pancreatic fat in the development of type 2 diabetes mellitus,Met S,atherosclerosis,severe acute pancreatitis and even pancreatic cancer. Evidence exists that fatty pancreas could be used as the initial indicator of "ectopic fat deposition",which is a key element of nonalcoholic fatty liver disease and/or Met S. Moreover,in patients with fatty pancreas,pancreaticoduodenectomy is associated with an increased risk of intraoperative blood loss and post-operative pancreatic fistula.

Expression of matrix metalloproteinase-11 is increased under conditions of insulin resistance

AIM To investigate matrix metalloproteinase-11(MMP-11) expression in adipose tissue dysfunction, using in vitro and in vivo models of insulin *** Culture of mouse 3T3-L1 preadipocytes were induced to differentiation into mature 3T3-L1 adipocytes. Cellular insulin resistance was induced by treating differentiated cultured adipocytes with hypoxia and/or tumor necrosis factor(TNF)-α, and transcriptional changes were analyzed in each condition thereafter. For the in vivo studies, MMP-11 expression levels were measured in white adipose tissue(WAT) from C57BL/6J mice that underwent low fat diet or high-fat feeding in order to induce obesity and obesity-related insulin resistance. Statistical analysis was carried out with GraphP ad Prism *** MMP-11 m RNA expression levels were significantly higher in insulin resistant 3T3-L1 adipocytes compared to control cells(1.46±0.49vs0.83±0.21, respectively;P<0.00036). The increase in MMP-11 expression was observed even in the presence of TNF-α alone(3.79±1.11vs1±0.17, P<0.01) or hypoxia alone(1.79±0.7vs0.88±0.1, P<0.00023). The results obtained in in vitro experiments were confirmed in the in vivo model of insulin resistance. In particular, MMP-11 m RNA was upregulated in WAT from obese mice compared to lean mice(5.5±2.8vs1.1±0.7, respectively; P<3.72E-08). The increase in MMP-11 levels in obese mice was accompanied by the increase in typical markers of fibrosis, such as collagen type Ⅵ alpha 3(Col6_α3), and fibroblast-specific protein *** Our results indicate that dysregulation of MMP-11 expression is an early process in the adipose tissue dysfunction, which leads to obesity and obesity-related insulin resistance.
