An evaluation of the role played by remote sensing technology following the World Trade Center attack

Remote sensing technology has been widely recognized for contributing to emergency response efforts after the World Trade Center attack on September 11th, 2001. The need to coordinate activities in the midst of a dense, yet relatively small area, made the combination of imagery and mapped data strategically useful. This paper reviews the role played by aerial photography, satellite imagery, and LIDAR data at Ground Zero. It examines how emergency managers utilized these datasets, and identifies significant problems that were encountered. It goes on to explore additional ways in which imagery could have been used, while presenting recommendations for more effective use in future disasters and Homeland Security applications. To plan adequately for future events, it was important to capture knowledge from individuals who responded to the World Trade Center attack. In recognition, interviews with key emergency management and geographic information system (GIS) personnel provide the basis of this paper. Successful techniques should not be forgotten, or serious problems dismissed. Although widely used after September 11th, it is important to recognize that with better planning, remote sensing and GIS could have played an even greater role. Bank erosion in an Andean páramo river system: Implications for hydro-development and carbon dynamics in the neotropical Andes

The páramo of the Northern Andes provide critically important ecosystem services to the Northern Andean region in the form of water provisioning and carbon sequestration, both of which are a result of the páramo?s organic-rich soils. Little is known, however, about the hydro-geomorphic characteristics of the rivers that drain these ecosystems. With impending plans for widespread hydro-development and increasing implementation of carbon-sequestering compensation for ecosystem services programs in the region it is imperative that we develop a thorough understanding of the hydrogeomorphic role that rivers play in this unique ecosystem. The objective of this study was to quantify bank erosion along an Amazonian headwater stream draining a small, relatively undisturbed páramo catchment to gain a better understanding of the natural erosion regime and the resulting sediment contributions from this unique ecosystem. This study implemented a combination of field, laboratory, and Geographic Information Systems techniques to quantify bank erosion rates and determine a bank erosion sediment yield from the Ningar River, a small páramo catchment(22.7 km^2) located in the eastern Andean cordillera of Ecuador. Results show that bank erosion rates range from 3.0 to ≥ 390.0 mm/yr, are highly episodic, and yield at least 487 tons of sediment annually to the Ningar River. These results imply that 1) páramo ecosystems substantially contribute to the sediment load of the Amazon River basin; 2) bank erosion is a potentially significant flux component of basin-scale carbon cycles in páramo ecosystems; and 3) hydrologic alteration campaigns(dam building) will likely critically alter these contributions and concomitantly disconnect a critical source of sediment and nutrients to downstream ecosystems.

Is there an acute exercise-induced physiological/biochemical threshold which triggers increased speed of cognitive functioning? A meta-analytic investigation

Purpose： The purpose of this study was to examine, using meta-analytic measures, the evidence regarding the optimal exercise intensity at which improvements in speed of cognitive function are triggered. Specifically, it was hypothesized that the catecholamine, lactate, and ventilatory thresholds is the point at which significant improvements in speed of cognitive function are observed. Methods： We compared mean effect sizes for threshold studies and for those studies where exercise intensity was classed as moderate （40%-- 79% VO2 or equivalent） but in which the thresholds were not measured. Results： Random effects meta-analysis showed significant, moderate, mean effect sizes for studies at the threshold （g = 0.58, Z = 2.98, p 〈 0.003） and for those during moderate intensity exercise but in which the threshold was not measured （ g = 0.54, Z = 5.0l, p 〈 0.001）. There was no significant difference between mean effect sizes, which suggests that the thresholds are unlikely to represent a trigger point. Conclusion： Moderate intensity exercise, even below the thresholds, can induce improved speed of cognition, possibly due to a combination of increased peripheral catecholamine concentrations inducing vagal/nucleus tractus solitarii pathway activation and central increases due to perceptions of stress.

Intrahepatic vascular changes in non-alcoholic fatty liver disease: potential role of insulin-resistance and endothelial dysfunction

Metabolic syndrome is a cluster of several clinical conditions characterized by insulin-resistance and high cardiovascular risk. Non-alcoholic fatty liver disease is the liver expression of the metabolic syndrome, and insulin resistance can be a frequent comorbidity in several chronic liver diseases, in particular hepatitis C virus infection and/or cirrhosis. Several studies have demonstrated that insulin action is not only relevant for glucose control, but also for vascular homeostasis. Insulin regulates nitric oxide production, which mediates to a large degree the vasodilating, antiinflammatory and antithrombotic properties of a healthy endothelium, guaranteeing organ perfusion. The effects of insulin on the liver microvasculature and the effects of IR on sinusoidal endothelial cells have been studied in animal models of non-alcoholic fatty liver disease. The hypotheses derived from these studies and the potential translation of these results into humans are critically discussed in this review.

Disclosure, Multiple Sex Partners, and Consistent Condom Use among HIV Positive Adults on Antiretroviral Therapy in johannesburg, South Africa

Inconsistent condom use among persons on antiretroviral treatment (ART) is a major public health concern because of the risk of HIV transmission. This study examined the association between socio-demographic variables and knowing partners’ HIV status, multiple sex partners, and consistent condom use among 400 HIV-infected adults who had received ART for at least six months in johannesburg, South Africa. The study used a cross-sectional survey and a structured interviewer administered questionnaire. Over half (n = 225, 56.3%) of participants were on ART for more than two years. Two thirds (n = 234, 63.2%) were aware of partner’s HIV status. Over a third (n = 136, 34.0%) reported having more than one sex partners. Three quarters (n = 279, 75.8%) reported consistent condom use with regular partner. Discussing HIV testing (aOR = 2.28, CI: 1.31 -3.95), awareness of partner’s HIV status (aOR = 2.59, CI: 1.50 -4.46), level of education (aOR = 0.64, CI: 0.42 -0.98), and duration on ART (aOR = 0.71, CI: 1.31 -3.95) were predictors for consistent condom use. Awareness of partner’s HIV status was associated with multiple partnership (aOR = 0.38, CI: 0.21 -0.66), living with partner (aOR = 4.75, CI: 2.86 -7.91), discussing HIV testing (aOR = 2.43, CI: 1.48 -3.99), and duration on ART (aOR = 2.04, CI: 1.43 -2.92). While gender (aOR = 5.68, CI: 3.46 -9.34), marital status (aOR = 0.44, CI: 0.25 -0.77), and awareness of partner’s HIV status (aOR = 0.52, CI: 0.30 -0.89) were associated with multiple partnerships. Risky sexual behaviours occurred in all types of partners and knowing partner’s HIV status was a predictor for consistent condom use with all types of partners. It is essential that HIV prevention strategies create an enabling environment for disclosure and reductions of risky sexual behaviours by HIV-infected persons on ART.

Antabuse for Lyme Disease: The Way Forward

Despite leadership by dedicated lay organization and patient-based advocacy groups, the way forward for prospective candidates for disulfiram treatment has been difficult. This article provides the background, overview of disulfiram, in particular its intended use in post-treatment Lyme disease syndrome after a standard of care course of antibiotics fails to alleviate symptoms of tick borne disease. A series of recommendations are offered to guide patients and clinicians.

This editorial is an open invitation to trauma doctors to participate in the international CRASH-2 trial the rationale for which is outlined below. Those interested in this trial can register via the trial website www. crash2. lshtm, ac. uk. For people at ages 5 to 45 years, trauma is second only to HIV/AIDS as a cause of death. Each year, worldwide, over three million people die as a result of trauma, many after reac-hing hospital . Among trauma patients who do survive to reach hospital, exsanguination is a common cause of death, accounting for nearly half of in-hospital trauma deaths . Central nervous system injury and multi-organ failure account for most of the remainder, both of which can be exacerbated by severe bleeding.
