Implication of neurohormonal-coupled mechanisms of gastric emptying and pancreatic secretory function in diabetic gastroparesis

Recently, diabetic gastroparesis(DGP) has received much attention as its prevalence is increasing in a dramatic fashion and management of patients with DGP represents a challenge in the clinical practice due to the limited therapeutic options. DGP highlights an interrelationship between the gastric emptying and pancreatic secretory function that regulate a wide range of digestive and metabolic functions, respectively. It well documented that both gastric emptying and pancreatic secretion are under delicate control by multiple neurohormonal mechanisms including extrinsic parasympathetic pathways and gastrointestinal(GI) hormones. Interestingly, the latter released in response to various determinants that related to the rate and quality of gastric emptying. Others and we have provided strong evidence that the central autonomic nuclei send a dual output(excitatory and inhibitory) to the stomach and the pancreas in response to a variety of hormonal signals from the abdominal viscera. most of these hormones released upon gastric emptying to provide feedback, and control this process and simultaneously regulate pancreatic secretion and postprandial glycemia. These findings emphasize an important link between gastric emptying and pancreatic secretion and its role in maintaining homeostatic processes within the GI tract. The present review deals with the neurohormonal-coupled mechanisms of gastric emptying and pancreatic secretory function that implicated in DGP and this provides new insights in our understanding of the pathophysiology of DGP. This also enhances the process of identifying potential therapeutic targets to treat DGP and limit the complications of current management practices.

胃癌（Gastric cancer,GC）是目前世界上最常见的恶性肿瘤之一,其发病率和死亡率均呈上升趋势。随着手术、化疗、放疗和生物治疗技术的不断发展,胃癌的治疗取得了长足的进展。但是,由于胃癌浸润性高,易转移,导致早期不易发现,治疗效果较低,死亡率较高。因此,开发能够有效抑制胃癌细胞侵袭转移的候选新药对于胃癌的治疗具有重要意义。芦竹碱是一种具有多种生物活性的吲哚类生物碱,但其抗胃癌活性较低。已有研究表明,吲哚类化合物具有潜在的抗肿瘤活性,开发基于吲哚骨架的抗肿瘤药物具有可行性。受此启发,本课题基于药效团融合策略,在芦竹碱的结构基础上,通过引入不同的含氮五元杂环和末端炔基,该论文合成了 29个全新的芦竹碱衍生物,并对其抗胃癌活性进行了深入研究。构效关系（SAR）表明末端炔基有助于增殖抑制活性,其中,化合物16h对胃癌mGC803细胞的抗增殖活性最好,IC50值为3.74μm。进一步研究证明16h能够诱导线粒体介导的细胞凋亡,导致细胞周期在G2/m期阻滞。此外,16h还能成功抑制mGC803细胞的转移能力。综上所述,这些研究为优化芦竹碱衍生物的抗胃癌活性提供了新的思路,也为胃癌的治疗提供了潜在的候选药物。

The fate of prazosin and levonorgestrel after electrochemical degradation process: monitoring by-products using LC-TOF/mS

Prazosin（PRZ） and levonorgestrel（LNG） are widely used as an anti-disease drugs due to their biological activity in the human body. The frequent detection of these compounds in water samples requires alternative technologies for the removal of both compounds. After electrochemical degradation of PRZ and LNG, the parent compounds could be completely removed after treatment, but the identification and characterization of by-products are necessary as well. In this study, the effects of NaCl concentration and applied voltage were investigated during the electrochemical degradation process. The results revealed that the increase of NaCl concentration and applied voltage could promote the generation of hypochlorite OCl-and then enhance the degradation of PRZ and LNG. After initial study, 6 V and 0.2 g NaCl were selected for further experiments（96% and 99% removal of PRZ and LNG after 40 min, respectively）. Energy consumption was also evaluated and calculated for PRZ and LNG at 3, 6 and 8 V. Solid phase extraction（SPE） method plays an important role in enhancing the detection limit of by-products. Furthermore, characterization and identification of chlorinated and non-chlorinated by-products were conducted using an accurate liquid chromatography-time of flight/mass spectrometry LC-TOF/mS instrument. The monitoring of products during the electrochemical degradation process was performed at6 V and 0.2 g NaCl in a 50 m L solution. The results indicated that two chlorinated products were formed during the electrochemical process. The toxicity of by-products toward E. coli bacteria was investigated at 37°C and 20 hr incubation time.

A graphical index is a numeric value corresponding to a graph which is structurally invariant and in molecular graph theory these invariants are known as topological indices. In the field of Chemical and medical Sciences, the topological indices are used to study the chemical, biological, medical and pharmaceutical features of drugs. With reference to the previous deadly diseases, the COVID-19 pandemic has considered to be the biggest life threatening issue that modern medicines have ever tackled. COVID-19 is immedicable and even the existing treatments are only helping the certain group of sufferers. Scientists have tested available antiviral agents and got a favorable impact on recovering from pandemic. Some of these antiviral agents are remdesivir, chloroquine, hydroxychloroquine, theaflavin and dexamethasone. Keeping in view of the importance of topological indices in the study of pharmaceutical and chemical drugs, in this paper, we calculate the m>mdnm>-Polynomial, some downhill Zagreb topological indices and some downhill Zagreb polynomials of some of the anti viral agents remdesivir, chloroquine, hydroxychloroquine, theaflavin and dexamethasone. The results thus obtained may be useful for the finding new medicine and vaccine for the treatment of COVID-19.

How Long to Treat with Bisphosphonates?

Bisphosphonate class of drugs, the most commonly prescribed for the treatment of osteoporosis, is effective in preventing and treating bone loss and fractures. However, the treatment duration and the applicability of “drug holidays” for bisphosphonates need optimization in order to minimize long-term exposure. Drug holidays may prevent potential adverse events while still maintaining some degree of antifracture efficacy via residual antiresorptive activity by retained bisphosphonates. Patients receiving bisphosphonates, who are at low-moderate risk of fracture, are potential candidates for a drug holiday. However, for high-risk patients or patients with previous history of fragility fractures, the benefits of continuing bisphosphonate therapy considerably out-weigh their potential harm. Evidence-based guidelines regarding starting and stopping a drug holiday are not available;therefore, it is appropriate to monitor patients on a drug holiday to assess a declining antiresorptive effect. In case of a significant rise in bone turnover markers or significant decrease in bone mineral density, it may be time to restart therapy.

Square Patch Antenna for Breast Cancer Diagnosis at 2.45 GHz

This paper presents a model of cancer diagnosis using principal electrical parameters of tumor cells such as the relative permittivity and the conductivity. The proposed model involves a square microstrip antenna and breast phantom comprising a tumor cell. The radiation properties of the designed antenna at the ISm bands, such as the Return Loss (RL), the current density, the electrical field and the Specific Absorption Rate (SAR) are exploited for diagnosing purposes. The Ansoft HFSS (13.0) simulated results show that the difference in terms of the RL, the current density, the electrical field and the SAR is higher than 2 dB, 40 A/m2, 100 V/m and 20 W/kg respectively, once the tumor exists inside the breast model. This proposed technique in turn can be exploited to distinguish malignant cells inside the women breast in earlier stages as compared to other traditional techniques such as mammography, X-ray, ultra-sound, tomography and mRI.

SADI-S with Extended Duodeno-Bulb Preservation: Case Report

Obesity has been growing in Brazil and in the world. It is reaching epidemic proportions, and bariatric surgery is the most effective treatment for patients with this disease. Among the procedures described in the literature, ileal surgeries such as biliopancreatic diversion with duodenal switch (BPD-DS) and single-anastomosis duodeno-ileal bypass with sleeve gastrectomy (SADI-S) present better long-term results in terms of weight loss and comorbidities control. However, there are concerns regarding long term nutritional problems with these procedures. In this case report the aim is to demonstrate the technical feasibility of preserving an extended duodenal bulb segment, in the SADIS-S procedure, when there are difficulties in dissecting the retrobulbar region, as occurred here, due to fibrosis in this area. This assures the maintenance of the proposed surgical technique, in such a situation. The dissection and transection of the duodenum was done 7 cm distally to the pylorus, under endoscopic view, proximally to the papillae, where the tissue was normal. Additionally, due the importance of the duodenal mucosa on minerals and trace elements absorption and the release of important hormones in this region, this case report elicits the evaluation of the impact of this technical modification, which occurred casually, in the nutritional, hormonal and metabolic results, long term. In this case report, the extended duodenal length has demonstrated reasonable weight loss, adequate comorbidities control and good nutritional status, so far. These aspects must be evaluated in the long term, by clinical trials.

Introduction: Cesarean scar pregnancy (CSP) is defined as implantation of gestational sac at the site of cesarean scar. It’s a serious diagnosis that has become more prevalent in recent years and related to the increasing rate of cesarean sections reported worldwide. Identifying these cases and treating them is challenging, with no agreed upon universal protocol for successful treatment. We aim to evaluate the success rate and outcome of medical management for Cesarean scar pregnancy. methods: It was a retrospective descriptive study of all cesarean scar pregnancies managed at fetal medicine unit at Latifa Hospital in Dubai, UAE the main obstetrics & Gynecology tertiary hospital in Dubai Emirate, UAE from 2015 to 2017. Certainly, a set of diagnostic criteria were implemented to confirm the diagnosis of CSP. The cases were then offered our proposed management which is systemic methotrexate injections ± KCL. Follow up made by serial βhcg and ultrasound scan. The data were collected used specified data collection sheet for this purpose then analyzed and presented using statistical package for social sciences (SPSS) version 26. Results: 33 cases of cesarean scar pregnancies are confirmed and medically managed during the study period. The study subjects composed of a group of patients who had a viable CSP and received local KCL injection + systemic methotrexate, a second group who had non-viable CSP and received systemic methotrexate 20 cases were viable pregnancies who received combined local potassium chloride plus systemic methotrexate, and 13 were non-viable received systemic methotrexate only. The mean gestational age at diagnosis was 8 weeks (SD ± 1.8). On average our cases had a history of 3 previous cesarean sections (range 1 - 6). Overall, the success rate calculated from our study population for medical management of CSP was 77.8%;this varied between viable CSP which had a success rate of 66.7% and non-viable CSP which had a success rate of 100%. Obesity is a significant and escalating health issue both in Brazil and globally, with over 650 million overweight adults worldwide. The treatment of obesity can be performed clinically, endoscopically or surgically;surgical treatment proves to be safe and more effective in terms of weight loss and long-term maintenance. Objective: This study aimed to monitor the progress of weight loss and comorbidity control in patients undergoing sleeve gastrectomy with duodenal bipartition. methods: This pilot project involved 8 patients divided into 2 arms. In the first arm, patients underwent sleeve gastrectomy with Roux-en-Y duodenoileal transit bipartition (S-RYDITB), while in the second arm, patients underwent sleeve gastrectomy with Roux-en-Y duodenojejunal transit bipartition (S-RYDJTB). Both procedures involved Roux-en-Y reconstruction without duodenal exclusion. In S-RYDITB, the duodenal-ileal anastomosis was performed 300 cm from the ileocecal valve (ICV), creating a 250 cm common channel and a 50 cm alimentary channel. In S-RYDJTB, a biliopancreatic loop was created 200 cm from the angle of Treitz, with a 1 m alimentary channel. Results: Five patients underwent the procedures, with one undergoing S-RYDITB and four undergoing S-RYDJTB. No adverse events such as hospitalizations, readmissions, reoperations, fistulas, bleeding, pulmonary embolism, diarrhea, dumping syndrome, or hypoglycemia occurred during the study period. The mean length of hospital stay was 2 days. The average BmI decreased from 37.27 kg/m2 preoperatively to 29.48 kg/m2 after 6 months. The significant percentage of weight loss was 21.22%, with excess weight loss of 63.6%. Ninety-five percent remission of comorbidities, including hypercholesterolemia, hypertriglyceridemia, diabetes, hypertension, steatosis, and pre-diabetes. Two patients underwent sleeve gastrectomy with duodenal bipartition using a single anastomosis. This study included isolation and identification of E. sakazakii from 51 different samples of powdered infant formula milk involved (Dialak 1 & 2, Celia 1 & 2, Primalac, Biomil 1 & 2, Similac, Nictalia 1 & 2, mAmi, Novolac (AD), Novolac (AR), Novolac (Allernova), S-26 AR, Nursoy, S-26 PDF gold. The results showed that one batch from three of the batches identified of Novolac and Dialak were contaminated and all the types of other infants were non-contaminated. The strains code given as (E1, E2, E3, E4);the bacteria showed resistance to antibiotics used was cephalosporin batch;the third generation showed sensitive to antibiotics life results through inhibition processes such as (Cefotaxime, Sifutetan and Siftadizim), where the diameters of inhibition zone for Siftadizim (18 mm), Sifutetan (22 mm) & Cefotaxime (25 mm) confirmed the bacteria by API and Vitek Compact- 2 (biomero).
