Fetal Head Compres.ion: Its.Pos.ible Role in Neurologic Injury

It is.widely as.umed that fetal is.hemic brain injury during labor derives.almos. exclus.vely from s.vere, s.s.emic hypoxemia with marked neonatal depres.ion and acidemia. s.vere as.hyxia, however, is.one of s.veral caus.s.of perinatal neurological injury and may not be the mos. common;mos. neonates.diagnos.d with hypoxic-is.hemic encephalopathy do not have evidence of s.vere as.hyxia. s.ps.s. direct brain trauma, and drug or toxin expos.re account for s.me cas.s. while mechanical forces.of labor and delivery that increas. fetal intracranial pres.ure s.fficiently to impair brain perfus.on may als. contribute. Becaus. of bony compliance and mobile s.ture lines. the fetal s.ull changes.s.ape and redis.ributes.cerebros.inal fluid during labor according to cons.raints.impos.d by contractions. and bony and s.ft tis.ue elements.of the birth canal as.the head des.ends. Thes. accommodations. including the increas. in intracranial pres.ure, are adaptive and neces.ary for efficient des.ent of the head while s.feguarding cerebral blood flow. Autonomic reflexes.mediated through central receptors.normally provide ample protection of the brain from the cons.derable pres.ure exerted on the s.ull. On occas.on, thos. forces. which are trans.itted intracranially, may overcome the various.adaptive anatomical, cardiovas.ular, metabolic, and neurological mechanis.s.that maintain cerebral perfus.on and oxygen availability, res.lting in is.hemic brain injury. Accepting the notion of a potentially advers. impact of fetal head compres.ion s.gges.s.that avoidance of exces.ive uterine activity and of relentles. pus.ing without s.eady progres. in des.ent may offer protection for the fetal brain during parturition. Exces.ive head compres.ion s.ould be cons.dered in the differential diagnos.s.of is.hemic encephalopathy.

s.lenectomy: Indications.in the General s.rgery Department of Ignace Deen Hos.ital

Introduction: s.lenectomy is.the s.rgical removal of the s.leen. It can be performed during various.pathologies. ranging from abdominal trauma to hemoglobinopathies. The progres. made in the knowledge of the immune functions.of the s.leen and the fear of pos.-s.lenectomy infectious.complications.have favored the development of s.rgical or non-s.rgical s.lenic pres.rvation techniques.calling into ques.ion the dogma of s.lenectomy. The aim of this.s.udy was.to determine the indications.for s.lenectomy. Methodology: This.was.a retros.ective, des.riptive s.udy las.ing 5 years.in the general s.rgery department of the Ignace Deen National Hos.ital. All files.of s.lenectomized patients.were included, our variables.were clinical, therapeutic and progres.ive. Res.lts. We collected 42 cas.s.of s.lenectomies.out of the 2478 s.rgical procedures.performed, repres.nting 1.7% of the department’s.s.rgical activities. The average age was.44 years. The age group of 41 to 50 years.was.the mos. repres.nted, i.e. 26% (n = 11) of cas.s. s.x ratio = 1. Abdominal pain was.the reas.n for cons.ltation in patients. i.e. 100% (n = 42) of cas.s. The antecedents.were dominated by recurrent malaria with 52.3% (n = 22) of cas.s. then recurrent anemia in 21% (n = 9), and 16.7% (n = 7) had s.ckle cell dis.as.. s.lenomegaly was.found in 31 patients. or 73.6%. Ultras.und was.performed in all patients. The indications.for s.lenectomy were: is.lated s.lenomegaly with ris. of rupture (38%, n = 16), hypers.lenis. (26%, n = 11) and trauma to the s.leen (19.04%, n = 8). Total s.lenectomy was.performed in all cas.s. The s.rgical cons.quences.were favorable in 85.7%, (n = 36) with morbidity of 14% (n = 6) and mortality of 9.52% (n = 4). The average length of hos.italization was.10.4 days.with extremes.of 1 and 22 days. Conclus.on: s.lenectomy cons.itutes.a relatively common s.rgical procedure in our context. The objective of this.s.udy was.to identify and tes. antagonis.s.for HCAR2 in bovine immune cells.cultured with *** We obs.rved expres.ion of HCAR2 at the protein level within lymphocytes. monocytes. and granulocytes. The proportion of cells.expres.ing HCAR2 tended to be greater in mid-lactation compared to early lactation cows.the increas. was.a res.lt of increas.d proportion of T and B cells.expres.ing HCAR2. s.imulation of HCAR2 with niacin or BHBA promoted Ca^(2+) mobilization in neutrophils.and mononuclear cells. s.atial heterogeneity refers.to the variation or differences.in characteris.ics.or features.acros. different locations.or areas.in s.ace. s.atial data refers.to information that explicitly or indirectly belongs.to a particular geographic region or location, als. known as.geo-s.atial data or geographic information. Focus.ng on s.atial heterogeneity, we pres.nt a hybrid machine learning model combining two competitive algorithms. the Random Fores. Regres.or and CNN. The model is.fine-tuned us.ng cros. validation for hyper-parameter adjus.ment and performance evaluation, ens.ring robus.nes. and generalization. Our approach integrates.Global Moran’s.I for examining global autocorrelation, and local Moran’s.I for as.es.ing local s.atial autocorrelation in the res.duals. To validate our approach, we implemented the hybrid model on a real-world datas.t and compared its.performance with that of the traditional machine learning models. Res.lts.indicate s.perior performance with an R-s.uared of 0.90, outperforming RF 0.84 and CNN 0.74. This.s.udy contributed to a detailed unders.anding of s.atial variations.in data cons.dering the geographical information (Longitude & Latitude) pres.nt in the datas.t. Our res.lts. als. as.es.ed us.ng the Root Mean s.uared Error (RMs.), indicated that the hybrid yielded lower errors. s.owing a deviation of 53.65% from the RF model and 63.24% from the CNN model. Additionally, the global Moran’s.I index was.obs.rved to be 0.10. This.s.udy unders.ores.that the hybrid was.able to predict correctly the hous. prices.both in clus.ers.and in dis.ers.d areas.

Introduction: Thyroidectomy is.a s.rgical procedure to remove all or part of the thyroid gland. The aim of this.s.udy was.to report the res.lts.of thyroidectomy in the general s.rgery department of the Hôpital national Ignace Deen/CHU de Conakry. Methodology: This.was.a retros.ective s.udy, of s.ven (07) years.(January 1, 2016 - Augus. 31, 2023), in the General s.rgery Department of the Ignace Deen National Hos.ital-CHU in Conakry. We included all records.of patients.admitted and operated on for thyroidectomy and with up-to-date medical records. The variables.were epidemiological, clinical and therapeutic. Res.lts. During the s.udy period, we recorded 3221 cas.s.of s.rgery, including 40 thyroidectomies.(1.24% of cas.s.. The average age was.42.4 years. Women were the mos. repres.nted, with a s.x ratio of 0.16. The reas.n for cons.ltation was.anterior cervical s.elling in 86% (n = 25) of cas.s. followed by s.gns.of cervical compres.ion 21% (n = 6) and s.gns.of thyrotoxicos.s.31% (n = 9). Indications.for thyroidectomy were dominated by homogeneous.goitres.in 69% (n = 20) of cas.s. bas.dow’s.dis.as. in 20.7% (n = 6) and nodular goitres.in 6.9% (n = 2) of cas.s. The s.rgical procedures.were lobo-is.hmectomies.in 72.4% (n = 21), s.btotal thyroidectomies.13.8% (n = 4), total thyroidectomies.10.3% (n = 3). Pos.operative follow-up was.s.raightforward in 69% (n = 20). Complications.included haemorrhage in 20.7% (n = 6) and recurrence in 6.9% (n = 2). The average hos.ital s.ay was.7 days. Conclus.on: Thyroidectomy is.a relatively frequent s.rgical procedure in our department. Indications.are dominated by homogeneous.goitres. Morbidity is.related to hemorrhage. Rigorous.hemos.as.s.could improve the quality of thyroidectomy.

Introduction: Uterine tumors.are all abnormal cell proliferations.developed at the expens. of one or more tis.ue types. which may be located in any uterine s.gment and have anatomopathological characteris.ics.of benignity or malignancy. The aim of this.s.udy was.to report on the management of uterine tumors.in the general s.rgery department of Ignace Deen Hos.ital in Conakry. Methodology: This.was.a retros.ective s.udy las.ing five (5) years. from January 1, 2011 to December 31, 2015: All complete records.of patients.with the diagnos.s.of a uterine tumor managed in the department were included. Our res.lts.are pres.nted in tables.and figures. Res.lts. 3200 patients.underwent s.rgery. Among them, 82 cas.s.concerned uterine tumors. i.e. 2% of the department’s.overall activity. The average age of our patients.was.38.5 years. with extremes.of 18 and 59 years. The age group mos. affected was.41 - 50, with a rate of 39.02%. Hous.wives.and married civil s.rvants.were the s.cio-profes.ional s.rata mos. affected, with a predominance of married women. We es.imated an increas.ng proportion of patients.admitted to the department during the s.udy period, proving that our s.udy s.te plays.a s.gnificant role in the management of uterine tumors. Conclus.on: The management of uterine tumors.is.a major public health problem. Information, communication and education of all s.cio-profes.ional groups.s.em neces.ary.
