AiM To study factors associated with esophageal and nonesophageal cancer morbidity among barrett's esophagus(BE) patients. METHODs A cohort study within a single tertiary center included 386 consecutive patients with biopsy proven BE, who were recruited between 2004-2014. Endoscopic and histologic data were prospectively recorded. Cancer morbidity was obtained from the national cancer registry. Main outcomes were BE related(defined as esophagus and cardia) and non-BE related cancers(all other cancers). Cancer incidence and all-causemortality were compared between patients with highgrade dysplasia(HGD) and with low-grade or no dysplasia(non-HGD) using Kaplan-Meier curves and cox regression *** Of the 386 patients, 12 had HGD, 7 had a BE related cancer. There were 75(19.4%) patients with 86 cases of lifetime cancers, 76 of these cases were non-BE cancers. seven(1.8%) and 18(4.7%) patients had BE and non-BE incident cancers, respectively. Twelve(3.1%) patients had HGD as worst histologic result. Two(16.7%) and 16(4.4%) incident non-BE cancers occurred in the HGD and non-HGD group, respectively. Ten-year any cancer and non-BE cancer free survival was 63% and 82% in the HGD group compared to 93% and 95% at the non-HGD group, respectively. Log-rank test for patients with more than one endoscopy, assuring longer follow up, showed a significant difference(P spectively). All-cause mortality was not significantly associated with BE *** Patients with BE and HGD, may have a higher risk for all-cause cancer morbidity. The implications on cancer prevention recommendations should be further studied.

AiM:The pathogenesis of hypogonadism in liver cirrhosis is not well *** results from our laboratory showed that iGF-1 deficiency might play a pathogenetic role in hypogonadism of *** administration of iGF-1 for a short period of time reverted the testicular atrophy associated with advanced experimental cirrhosis. The aim of this study was to establish the historical progression of the described alterations in the testes, explore testicular morphology,histopathology,cellular proliferation,integrity of testicular barrier and hypophyso- gonadal axis in rats with no ascitic cirrhosis. METHODs:Male Wistar rats with histologically-proven cirrhosis induced with carbon tetrachloride(CCi_4)for 11 wk, were allocated into two groups(n=12,each)to receive recombinant iGF-1(2 μg/100 gd,sc)for two weeks or *** rats receiving vehicle were used as control group(n=12). REsULTs:Compared to controls,rats with compensated cirrhosis showed a normal testicular size and weight and very few histopathological testicular abnormaliti.s. However,these animals showed a significant diminution of cellular proliferation and a reduction of testicular transferrin *** addition,pituitary-gonadal axis was altered,with significant higher levels of FsH(Pscontrols)and increased levels of LH in untreated cirrhotic ***,iGF-1 treatment normalized testicular transferrin expression and cellular proliferation and reduced serum levels of LH(P=ns vs controls,and Ps untreated cirrhotic group). CONCLUsiON:The testicular barrier is altered from an early stage of cirrhosis,shown by a reduction of transferrin expression in sertoli cells,a diminished cellular proliferation and an altered gonadal *** treatment with iGF-1 could be also useful in this initial stage of testicular disorder associated with compensated cirrhosis.

Background:After promising initial results from the administration of a third(booster)dose of the BNT162b2 messenger RNA vaccine(Pfizer-BioNTech)to persons 60 years of age or older,the booster campaign in israel was gradually expanded to persons in younger age groups who had received a second dose at least 5 months ***:We extracted data for the period from July 30 to October 10,2021,from the israel Ministry of Health database regarding 4,696,865 persons 16 years of age or older who had received two doses of BNT162b2 at least 5 months *** the primary analysis,we compared the rates of confirmed coronavirus disease 2019(Covid-19),severe illness,and death among those who had received a booster dose at least 12 days earlier(booster group)with the rates among those who had not received a booster(nonbooster group).in a secondary analysis,we compared the rates in the booster group with the rates among those who had received a booster 3 to 7 days earlier(early postbooster group).We used Poi.s.n regression models to estimate rate rati.s.after adjusting for possible confounding factors.

Molybdenum is nitrided by a 100-Hz pulsed DC glow discharge technique for various time durations and fill gas pressures to study the effects on the surface properti.s.of molybdenum. X-ray diffractometry （XRD）, scanning electron microscopy （sEM）, and atomic force microscopy （AFM） are used for the structural and morphological analysis of the nitrided layers. Vickers＇ microhardness tester is utilized to investigate surface microhardness. Phase analysis shows the formation of more molybdenum nitride molecules for longer nitriding durations at fill gas pressures of 2 mbar and 3 mbar （1 bar = 105 Pa）. A considerable increase in surface microhardness （approximately by a factor of 2） is observed for longer duration （10 h） and 2-mbar pressure. Longer duration （10 h） and 2-mbar fill gas pressure favors the formation of homogeneous, smooth, hard layers by the incorporation of more nitrogen.

The study aimed at examining by quantitative research design to what extent management style affects organizational climate of schools A, B, and C of Arab sector in israel, while comparing a test group of school A to a control group of schools B＋C. The research question was： What is the impact of a principal＇s management style on creation of a positive school organizational climate？ Quantitative data collection was conducted by means of validated questionnaires from relevant scientific literature. The findings indicate that a principal has a central role in a teacher＇s perception of professional autonomy. The sense of professional autonomy of a teacher is reinforced the more a teacher senses that the principal is an educational figure or a leader whom he can consult with regarding personal and professional problems. Differences were found among teachers in perception of school characteristi.s.as providing various levels of autonomy.
