Conceptual Model of Aquifers in the Bedrock Zone of the MarahouéWatershed (Centre-West of Ivory Coast)

Marahoué watershed, located in the Center West of Côte d’Ivoire, has experienced significant population growth in recent decades. And a major economic boom linked to intense agricultural activity and the presence of certain industries. This population growth is also accompanied by seasonal water shortages. Hence it needs to better manage the basin’s groundwater, which is a permanent resource and more resistant than surface water to climatic hazards. The objective of this study is therefore to propose a conceptual model of hydrogeological flow for the sustainable exploitation of groundwater resources in the Marahoué watershed. The establishment of the conceptual model was carried out in two stages. The first step consisted in defining the stratigraphic units. For this purpose, three units have been defined. These are the layer of alterite, the useful fissured horizon and the sound basement. The thickness of the layer of alterite varies from 0 to 80 m with an average of 26 m. As for the useful fissured horizon, its thickness is between 43 and 46.5 with an average of 45 m. In addition, the roof of the basement presents a slightly uneven morphology with a North-West, South-East dip and the altitudes are between 150 and 390 m. The second step corresponds to the phase of determining the hydrodynamic parameters. During this phase, the crack porosity, the transmissivity, the conductivity, the storage coefficient, the hydrological balance and the piezometric map were determined. Indeed, these parameters (the crack porosity, the transmissivity, the conductivity and the storage coefficient) confirm not only the heterogeneity of the medium but that the cracked horizon is sufficiently porous to be assimilated to an equivalent continuous medium during the simulation.

Using Water Quality Index for Assessing of Physicochemical Quality of Quaternary Groundwater in the Southern Part of Abidjan District (Côte d’Ivoire)

In this study, fourteen physicochemical parameters were monitored at 61 groundwater sampling points located in the southern part of Abidjan District. Each water sampling location was sampled in March and July 2007, representing respectively the long dry season and the long wet season. Groundwater evaluation index and statistical methods were used to assess Quaternary groundwater physicochemical quality in term of drinking and irrigation purposes. The groundwater samples irrespective of seasons fell in mixed K-Na-HCO3 and K-Na-Cl types. Groundwater quality index values varied from 14.01 to 593.44 and from 20.88 to 567.44, in dry and rainy seasons, respectively. In both seasons, 9.83% exhibited poor water quality and 3.27% indicated unsuitable water for drinking purposes. The studied water wells were heavily contaminated with nitrate, nitrite and ammonium in the strongly urbanized areas at Treichville, Koumassi, Marcory, Port-Bou?t and Grand-Bassam. The degree of correlation between physicochemical parameters suggested that groundwater quality was mainly related to geogenic (rock–water interaction) and anthropogenic sources (domestic sewage), and intrusion of marine and lagoon waters in the study area. Sodium adsorption ratio was generally low during study period, indicating most groundwater sampled was suitable for irrigation purposes.

Multi-Model Approach for Assessing the Influence of Calibration Criteria on the Water Balance in Ouémé Basin

Hydrological models are very useful tools for evaluating water resources, and the hydroclimatic hazards associated with the water cycle. However, their calibration and validation require the use of performance criteria which choice is not straightforward. This paper aims to evaluate the influence of the performance criteria on water balance components and water extremes using two global rainfall-runoff models (HBV and GR4J) over the Ouémé watershed at the Bonou and Savè outlets. Three (3) Efficacy criteria (Nash, coefficient of determination, and KGE) were considered for calibration and validation. The results show that the Nash criterion provides a good assessment of the simulation of the different parts of the hydrograph. KGE is better for simulating peak flows and water balance elements than other efficiency criteria. This study could serve as a basis for the choice of performance criteria in hydrological modelling.

Cholinesterase Activity as an Indicator of Health Risks among Kou Valley Farmers

Introduction: Pesticides are currently an essential component of agricultural production techniques for controlling pests and weeds. In Burkina Faso, non-compliance with good practice in the use of pesticides poses a real health problem for the population. This study examines the health risks associated with pesticide management in rice-growing areas. Material and Methods: A field survey was conducted in Bama, involving farmers, focusing on their socio-demographic characteristics, pesticide usage, and health effects. Cholinesterase levels were measured in subsample of farmers using a portable device. Data were analysed using Microsoft Excel, calculating means and percentages for various practices. Health consultations, protection methods, and pesticide management were studied. Erythrocyte acetylcholinesterase activity was compared before and after treatment. Data were categorised into classes based on inhibition levels, and correlation analyses determined relationships between variables such as age, years of experience, and cholinesterase activity. Results: The results indicate that rice cultivation is mainly carried out by a fairly young population, with nearly 63% being under the age of 50. Common poor practices in pesticide use include improper storage and reuse of leftover pesticides. Seven types of pesticides were identified, including organophosphates such as glyphosate, which was used in 26.7% of cases. This organophosphate has resulted in class B poisoning, causing a 30% - 50% reduction in erythrocyte cholinesterase activity. The health effects of pesticide use are felt by agricultural farmers through various symptoms of poisoning. Conclusion: To reduce the occurrence of pesticide poisoning, it is essential to launch information and awareness campaigns among the population and farmers to promote safe practices in pesticide use in Bama, Burkina Faso.

Predictors of Fatal Outcome in Hospitalised Adult Patients with Acute Kidney Injury at Two Tertiary Hospitals in Sub-Saharan Africa

Introduction: Data on mortality in acute kidney injury (AKI) derives from high-income countries where AKI is hospital-acquired and occurs in elderly patients with a high burden of cardiovascular disease. In sub-Saharan Africa (SSA), AKI is community-acquired occurring in healthy young adults. We aimed to identify predictors of fatal outcomes in patients with AKI in two tertiary hospitals in Cameroon. Methods: Medical records of adults with confirmed AKI, from January 2018 to March 2020 were retrieved. The outcomes of interest were in-hospital deaths and presumed causes of death. We used multiple logistic regressions modeling to identify predictors of death. The study was approved by the ethics boards of both hospitals. Values were considered significant for a p-value of 0.05. Results: We included 285 patient records (37.2% females). The mean (SD) age was 50.1 (19.0) years. Hypertension (n = 97, 34.0%), organ failure (n = 88, 30.9%), and diabetes (n = 60, 21.1%) were the main comorbidities. The majority of patients had community-acquired AKI (78.6%, n = 224), were KDIGO stage 3 (88.8%, n = 253), and needed dialysis (52.6%, n = 150). Up to 16.7% (n = 25) did not receive what was needed. The in-hospital mortality rate was 29.1% (n = 83). Lack of access to dialysis (OR = 27.8;CI: 5.2 - 149.3, p = 0.001), hypotension (OR = 11.8;CI: 1.3 - 24.8;p = 0.001) and ICU admission (OR = 5.7;CI: 1.3 - 24.8, p = 0.001) were predictors of mortality. The presence of co-morbidities or underlying diseases (n = 46, 55%) were the main causes of death. Conclusions: In-hospital AKI mortality is high, as in other low- and middle-income economies. Lack of access to dialysis and the severity of the underlying illness are major predictors of death.

Thoracoscopy in Cameroon: Indications, Technics and Short Term Results

Introduction: Thoracoscopy is a video-assisted surgical approach that mirrors the techniques used in thoracotomy. Our aim is to map the current state of thoracoscopy practice in Cameroon’s hospitals. Methods: This was a descriptive study that collected both retrospective and prospective data over 57 months across four hospitals in Cameroon. It included 13 patients and focused on variables such as socio-demographic factors, clinical profiles, surgical procedures, and postoperative follow-up. Results: Thirteen patients, predominantly male (84.6%, n = 11), with a mean age of 37.5 ± 16 years, were enrolled. Alcohol use (61.5%) and smoking (38.5%) were the most common past histories. The major complaints were dyspnea (84.6%) and chest pain (58.3%). The primary surgical procedure was clot-free thoracoscopy in 30.8% of cases, mainly for persistent hemothorax (41.8%). Most surgeries were elective (76.9%) and performed under general anesthesia with selective intubation (61.5%). The most common approach was single-port thoracoscopy (U-VATS) (76.9%), with no reported difficulties;however, one intraoperative incident occurred and was successfully treated. Drainage was performed systematically in all patients, and one case required conversion to open surgery. Postoperative complications were minor (Clavien-Dindo grade I) and mainly consisted of pain, with a mortality rate of 15.4%. No significant association was found between risk factors and the occurrence of complications or postoperative mortality. Conclusion: Thoracoscopy, a novel approach in our context, primarily focuses on minor thoracic surgeries.

Gomphrena serrata L. (Amaranthaceae) is a medicinal plant widely used traditionally in rural areas to treat many diseases, including gastrointestinal helminthiasis of small ruminants. This work aims to estimate the plant extracts phytochemical potential and study the activities of the aqueous decoction (GS_AD), aqueous macerate (GS_AM), and hydroalcoholic macerate (GS_HEM) of Gomphrena serrata extracts on two developmental stages (eggs and adult worms) of Haemonchus contortus. Phytochemical screening was carried out. The acute toxicity test was performed according to the Organisation for Economic Co-operation and Development (OECD) test guideline 423. The Haemonchus contortus eggs hatching inhibition test was performed left to incubate for 48 hours in the presence of the different plant extracts at increasing concentrations of 0.125;0.25;0.5;0.625;1;2;3 mg/mL. For the motility inhibition test on adult worms, the worms were contacted with the extracts at concentrations of 0.75;1;1.25;1.5;2;3;4 mg/mL and left to incubate for 24 hours. Albendazole and levamisole were used as standard. Phytochemical screening revealed the presence of tannins, saponins, reducing compounds, coumarins and derivatives, anthocyanins, steroids, triterpenes, and flavonoids. An acute toxicity study showed that the oral LD50 value of G. serrata was estimated to be 5000 mg/kg. All extracts showed an effect on both stages of parasite development. The hatching inhibition rate was 100% from 3 mg/mL for all three extracts. The rate of inhibition of motility was 100% from 4 mg/mL for each extract. HEM is more efficient on eggs and Haemonchus contortus adult worms than other extracts with IC50 of 0.457 mg/mL and LC50 of 1.329 mg/mL, respectively. These results suggest that Gomphrena serrata, a plant used by farmers, has anthelmintic properties.

Clinical Epidemiology and Surgical Treatment of Spontaneous Perforations of the Terminal Ileum: A Multicentre Study in Cameroon

Background: Perforations of the terminal ileum are a frequent clinical situation and a therapeutic challenge. Surgical treatment is controversial. Several surgical modalities have been proposed and postoperative morbidity and mortality remain high. The aim of this study was to review our experience in the surgical management of perforations of the terminal ileum. Patients and methods: This is a descriptive cross-sectional study with retrospective data collection from January 1, 2017 to December 31, 2021, in five hospitals in Cameroon. Patients’ demographic, clinical presentation, surgical findings and 30-days postoperative outcomes data were collected. Results: We collected 34 files. The sex ratio was 1.4 and the average age was 20.28 years. The average consultation time was 9.1 days. Abdominal pain was present in all our patients. Peritoneal irritation was present in 46 cases (88.5%). The mean time to treatment after admission was 17.5 hours. The perforation was unique in 40 cases (76.9%) and located between 6 and 10 cm from the ileocecal valve in 29 cases (55.8%). Typhoid was the most suspected etiology intraoperatively in 38 cases (73.1%). A simple suture was performed in 18 patients (34.6%), resection with anastomosis in 11 patients (21.2%) and an ileostomy in 5 patients (9.6%). The average length of hospitalization was 21.74 days. Postoperative morbidity and mortality were 32.7% and 17.3% respectively. Conclusion: Perforations of the terminal ileum are common and affect young people. Surgical procedures are varied. Reducing consultation and care times and respecting treatment principles could improve postoperative morbidity and mortality, which remain high.

Assessment of the Drafting Quality of Request Forms Submitted to the Malaria and Parasitology Units at the Institut Pasteur de Cote d’Ivoire

Biological tests provide information on the medical analysis requested by both the patient and the prescriber. It is a communication link between the prescriber and the laboratory staff. The lack of some information on request forms not only affects the drafting quality of the test and patient care, but could also make thousands of data produced by healthcare centers unusable. The aim of this study was to assess the drafting quality of request forms submitted to the Malaria and Parasitology Units at the Institut Pasteur de Côte d’Ivoire. Methods: It was a descriptive cross-sectional study to assess the drafting quality of request forms of various prescribers received at the Institut Pasteur de Côte d’Ivoire. This study was conducted at the Malaria and Parasitology Units, department of Parasitology and Mycology (Institut Pasteur de Côte d’Ivoire), from 6th December 2020 to 6th December 2021. The information on each request forms was recorded on a data collection form designed for this purpose. Each data collection form corresponds to a request forms and each test to a patient. Results: Out of a total of 1990 request forms received, the patient’s age and sex were missing on 18% and 26.8% of the tests respectively. More than half (51.80%) of request forms did not indicate the patient’s place of residence. Clinical information was not provided on 45.90% of the tests. Prescribers omitting their signatures were 51%, stamps were 50.3% and contacts were 71.2%. Only 5.4% of request forms were of good drafting quality. Providing all the required information on the forms could facilitate the use and analysis of data and samples.
