Laparoscopy Assisted Distal Gastrectomy for Cancer at a Tertiary Center in Egypt. Is It Safe and Feasible?

Background: Laparoscopic radical gastrectomy for cancer has significant short- and long-term advantages. The feasibility and safety of laparoscopicdistal gastrectomy for cancer (LADG) is unclear in low to middle income countries as resources are limited. Therefore, the aim of this study was to evaluate the safety and feasibility of (LADG) in low to middle income countries;Egypt as an example. Methods: Thirty four Patients with stage I-II cancer at the pylorus and antrum have been enrolled for LADG between 2012 and 2015 with the reuse of single use vascular sealing device has been evaluated. Results: Finally 27 patients had been included in the study and successful LADG has been done for all selected cases. The average operative time was 151 ± 10 minutes. The average estimated blood loss was 73.3 ± 13 ml. No intra-operative complications have been recorded. The average time for post-operative patient ambulation was 9 hours (SD ± 1.8) and for oral fluid intake was 3.5 SD ± 1 days. The average duration of the hospital stay was 9.3 ± 1.2 days. The average number of retrieved lymph nodes was 21.7 ± 3.8 days. All the cases had free surgical margin. The median number of reuse of the vascular sealing device was 3.8 times (3 - 5 times). Conclusion: Laparoscopic distal gastrectomy for cancer could be safe and feasible in developing countries and give similar results for that of developed countries. Safe reuse of single use expensive parts of some instruments for laparoscopy could help in utilization of these advanced surgeries in low to middle income countries. Long term follow up as well as comparative studies with open surgery are required.

Evaluation of antibiotic resistance patterns in clinical isolates of Klebsiella pneumoniae in Bangladesh

The treatment of *** infections has become more difficult due to the emergence of multidrugresistant *** study aimed to evaluate antimicrobial resistance patterns and detect different types of beta-lactamase genes among MDR and non-MDR Klebsiella pneumoniae in various clinical samples.A total of 150 Klebsiella pneumoniae were identified from different clinical samples by conventional microbiological *** sensitivity was detected by the Kirby-Bauer disc diffusion ***-spectrum betalactamase,Metallo-β-lactamase,and carbapenemase were phenotypically detected by double-disk synergy test,combined disk assay,and modified Hodge *** also detected differentβlactamase genes and virulence *** majority of Klebsiella pneumoniae were multi-drug resistant(82%).The bacteria were found resistant to mostβ-lactam antibiotics,aminoglycosides,ciprofloxacin,cotrimoxazole,carbapenem,piperacillin,and ***,only about 0.7%colistin resistance was observed.A double-disk synergy test was performed among the isolates for phenotypic detection of ESBL producing Klebsiella *** 16.6%of isolates were found to be ESBL ***β-lactamase genes,NDM(23.34%),OXA-48(8%),and KPC(7.3%)were *** prevalentβlactamase genes VIM and IMP were not *** summary,the prevalence of MDR Klebsiella pneumoniae is high in Bangladesh,which may complicate the treatment of hospitalized patients.

Penetrating injuries to anterior chest may result in life-threatening complications such as massive haemothorax,?as a result of injury to the internal mammary artery.?Isolated internal mammary injury is a very rare cause of massive haemothorax and associated with high mortality.?We are presenting this?32-year-old gentleman who sustained a thoracic stab wound and had an emergency right anterior mini-thoracotomy?by extending the stab wound rather than standard thoracotomy or sternotomy.?This case of isolated penetrating IMA injury managed with mini-thoracotomy is the only documented case so far. We are publishing this case report with patient’s both written and informed consent and institutional approval.?This potentially life-threating injury can be managed by mini-thoracotomy with enhanced recovery;however, it is case specific and needs proper judgement.

Superior and Inferior Gastric Pouch Dilatation Post-Sleeve Surgery: Does Revision Work? A Systematic Review and Case Reports

Longitudinal sleeve gastrectomy (LSG) has become an acceptable option in the management of morbidly obese patients. Complications in the form of pouch dilatation may occur in post-sleeve surgery. However, revision sleeve gastrectomy procedures are effective in correcting such complications. The aim of this study was to systematically review all published cases that reported revision surgeries following the initial sleeve gastrectomy complicated by pouch dilatation. In addition, we are presenting two cases reported from our military/teaching hospital. A systematic literature search was conducted from English-language studies published from 2000 to 2014 from the following databases: PubMed, CINAHL, MEDLINE, EMBASE, Clinical evidence, Dara, Scopus, Web of Sciences, TRIP, Health Technology Database, Cochrane library, and Psych INFO. A total of 5340 articles were identified in the initial search and 5339 articles were excluded based on the exclusion criteria. Only one case study met the inclusion criteria for this systematic review, involving two patients. Also, we are reporting two cases with revision surgery performed for pouch dilatation post-sleeve surgery from our teaching hospital.?There was only one study that reported revision surgery as a management of gastric pouch dilatation post-sleeve surgery. Authors believe that the incidence of revision surgery for the management of pouch dilatation post-sleeve surgery is higher but underreported. This assumption may be valuable, especially because some surgeons consider pouch dilatation post-sleeve surgery a complication that arises due to surgical technique rather than the procedure itself;therefore, it is unreported.

All patients who underwent allogeneic haemopoietic stem cell transplant （SCT） during study period were included in the study. Data for transfusion of red cell concentrates （RCC）, random donor platelets （RDP）, single donor platelets （SDP）, fresh frozen plasma （FFP） and cryoprecipitate was collected by reviewing the clinical charts and record of transfusion.

Prevalence of Antibiotic Resistant Bacteria on Tomato Surfaces and Effectiveness of Disinfectants in Reducing the Microbial Load

This study was conducted to isolate and identify antibiotic resistant bacteria in fresh tomato sold in three different local markets of Dhaka city. On an average 〉 5.0 log CFU/g of natural microbial populations and coliform bacteria was present in the samples. Prevalence of Salmonella spp., E. coli, Listeria spp., and Yersinia spp., were recorded through cultivation dependent assay of tomato samples of different markets. Almost all of the isolates were uniformly resistant to rifampicin, erythromycin, clindamycin and oxacillin （100%）, vancomycin （93%）, amoxicillin （87%）, whereas 60% of the isolates were found to be resistant to ampicillin and cephalexin. Antibiotic sensitivity test of the presumptive E. coli and Salmonella spp. and corresponding plasmid profiling indicated a correlation of plasmid mediated multidrug resistance （MDR） of these environmental bacteria. Washing with sanitizing agents such as acidified sodium chlorite （ASC） and 200 ppm chlorinated water was able to reduce _〈 2.0 log CFU/g of natural microflora and coliform bacteria. In contrast, ASC was able to reduce 〈 2.0 log CFU/g of other food borne pathogens and, chlorinated water was able to reduce up to undetectable level. Therefore, washing of tomato with 200 ppm chlorinated water could be useful in reducing the pathogen populations on tomatoes.

One hundred twenty-five endophytic microorganisms were isolated from the roots,stems,and leaves of four prominent rice cultivars growing in temperate *** potential to combat rice blast disease and promote plant growth was *** dual culture tests highlighted the strong antagonistic activity of five fungal(ranging from 89%–70%)and five bacterial(72%–61%)*** examination focused on volatile compounds produced by selected isolates to counter the blast *** these,the highest chitinase(13.76μg mL−1)and siderophore(56.64%),was exhibited by Aspergillus flavus,and the highest HCN production was shown by Stenotrophomonas maltophilia(36.15μM mL−1).In terms of growth promotion traits,Aspergillus flavus and Enterobacter cloacae excelled in activities viz,phosphorous solubilization,ammonia production,auxin and gibberellic acid production,and nitrogen *** Identity of these endophytes was confirmed through molecular analysis as Trichoderma afroharzianum,Trichoderma harzianum,Penicillium rubens,Aspergillus flavus,Stenotrophomonas rhizophila,Stenotrophomonas maltophilia,Bacillus cereus,Enterobacter cloacae,and Bacillus *** greenhouse conditions,the highest disease control was shown by isolate Bacillus licheniformis and *** with an inhibition of 79%,followed by ***(77%)and ***(73%).The overall results of this study showed that Bacillus licheniformis and Stenotrophomonas rhizophila have great potential to be used as bio-stimulant and biocontrol agents to manage rice blast disease.

A Case of Thomsen Disease in Pregnancy: Case Report

Thomsen disease is a rare genetic disorder which affects the cell membrane of skeletal muscles causing hyper excitability and periods of prolonged muscle contraction. This prolonged muscle contractions can be aggravated during pregnancy and can interfere with normal labour and delivery. In this case report, we describe the case of a gravid patient in her first pregnancy with Thomsen disease and how to minimise the risk. Also, we illustrate the importance of involving multidisciplinary team in the management of such case to achieve the best fetomaternal outcome.

Traditional medicinal plants are one of the potential sources of antimicrobial drugs and there is a great concern in the use and development of herbal medicine for the treatment of various infections. This study aimed to evaluate the antibacterial, and antioxidant activities of Dichrostachys cinera ethanolic leaves extract and to determine the components of the crude extract. D. cinera extract was evaluated against Staphylococcus aureus ATCC 25923, Escherichia coli ATCC 25922, and Pseudomonas aeruginosa ATCC 27853. The antibacterial, antioxidant activities and active constituents were determined using standard methods. Antibacterial activity of the crude extract findings showed that all bacterial candidates were susceptible where S. aureus represent MIC at 12.5 mg/ml and MBC at 25 mg/ml, E. coli and P. aeruginosa both showed MIC 25 mg/ml and MBC 50 mg/ml. In the free radical scavenging assay of the extract and the standard quercetin at concentrations of 250 μg/ml, 125 μg/ml, 50 μg/ml, 10 μg/ml, and 5 μg/ml. The radical scavenging activity for the extract was about 92%, 89.6%, 86.8%, 82.8% and 37.8% respectively, compared to quercetin which gave 89.7%, 85.8%, 62.1%, 55.5%, and 45% radical scavenging activity. The GC-Ms analysis of the total constituents demonstrated that 1,6-Anhydro-2,4-dideoxy-.beta.-D-ribo-hexo (21.26%) with different peaks, followed by Glycerin (11.56%), 1,2,3-Cyclopentanetriol (10.18%), 8,11,14-Eicosatrienoic acid, (Z,Z,Z)-(6.18%), 1H-Pyrrole, 1-methyl-(6.08%), Phytol (5.91%) and 7-Bromo-6-(2-diethylaminoethoxy)-2,3-dihyd (5.44%) as major components in the extract. Finally, this study provided useful information on the therapeutic potential of D. cinera as an antibacterial agent and recommended to be evaluated against a wide range of Bacterial and fungal strains using different solvents and different parts from the plant.

Comparison of Shisha Smoking Behavior among Medical and Pharmacy Students

Background: Health care workers are respected in every society and have a strong influence on the behaviour of the community. In order to reduce the shisha dependence in the Pakistani population, it is necessary to evaluate the perspective of future doctors and pharmacists regarding this addiction as they become role models for the future generations. Objective: The aim is to compare the knowledge, attitudes, and practices of shisha use among medical students and pharmacy students in order to assess their support in the anti-shisha campaigns of Pakistan. Methodology: Cross sectional study was performed in four major medical universities of Karachi, Pakistan through a modification of the WHO global health survey questionnaire. Result: 398 medical students and 470 pharmacy students were included in the study which showed that currently 9.6% pharmacy students and 4.8% medical students used shisha daily (p = 0.000). Pharmacy students started shisha use in less than 15 years of age while this number was 29.5% among medical students (P = 0.000). Moreover, 44.6% pharmacy students and 50% medical students were not planning to quit this addiction within the next six months. 14% of pharmacy students and 20.4% in medical students (P = 0.002) were unaware that passive smoking caused lung cancer, while 23.4% pharmacy students and 33.5% medical students rejected it as cause of heart diseases (p = 0.006). Conclusion: A significant number of medical and pharmacy students were unaware about some of the serious consequences of shisha smoking. Therefore, both groups of students need to be educated about the health hazards of shisha use, in order to control this growing menace.
