Laparoscopic natural orifice specimen extraction-colectomy: A systematic review

Over the last 20 years, laparoscopic colorectal surgery has shown equal efficacy for benign and malignant colorectal diseases when compared to open surgery. However, a laparoscopic approach reduces postoperative morbidity and shortens hospital stay. In the quest to optimize outcomes after laparoscopic colorectal surgery, reduction of access trauma could be a way to improve recovery. To date, one method to reduce access trauma is natural orifice specimen extraction(NOSE). NOSE aims to reduce access trauma in laparoscopic colorectal surgery. The specimen is delivered via a natural orifice and the anastomosis is created intracorporeally. different methods are used to extract the specimen and to create a bowel anastomosis. Currently, specimens are delivered transcolonically, transrectally, transanally, or transvaginally. Each of these NOSEprocedures raises specific issues with regard to operative technique and application. The presumed benefits of NOSE-procedures are less pain, lower analgesia requirements, faster recovery, shorter hospital stay, better cosmetic results, and lower incisional hernia rates. Avoidance of extraction site laparotomy is the most important characteristic of NOSE. Concerns associated with the NOSE-technique include bacterial contamination of the peritoneal cavity, inflammatory response, and postoperative outcomes, including postoperative pain and the functional and oncologic outcomes. These issues need to be studied in prospective randomized controlled trials. The aim of this systematic review is to describe the role of NOSE in minimally invasive colorectal surgery.

social media have dramatically changed the mode of information *** models and algorithms have been developed to model information diffusion and address the influence maximization problem in complex social ***,it appears difficult for state-of-the-art models to interpret complex and reversible real interactive *** this paper,we propose a novel influence diffusion model,i.e.,the Operator-Based Model(OBM),by leveraging the advantages offered from the heat diffusion based model and the agent-based *** OBM improves the performance of simulated dissemination by considering the complex user context in the operator of the heat diffusion based *** experiment obtains a high similarity of the OBM simulated trend to the real-world diffusion process by use of the dynamic time warping ***,a novel influence maximization algorithm,i.e.,the Global Topical Support Greedy algorithm(GTS-Greedy algorithm),is proposed corresponding to the *** experimental results demonstrate its promising performance by comparing it against other classic algorithms.

In the last 10 years the mortality rate of colorectal cancer(CRC)has decreased by more than 20%due to the rising developments in diagnostic techniques and optimization of surgical,neoadjuvant and palliative *** methods currently used in the evaluation of CRC are heterogeneous and can vary within the countries and the *** article aims to discuss in depth currently applied imaging modalities such as virtual computed tomography colonoscopy,endorectal ultrasound,computed tomography(CT)and magnetic resonance imaging(MRI)in the diagnosis of *** focus is put on the potential of recent diagnostic developments as diffusion weighted imaging MRI,MRI biomarkers(dynamic enhanced MRI),positron emission tomography with 2-(fluorine-18)-fluoro-2-deoxy-d-glucose(FdG-PET)combined with computed tomography(PET/CT)and new hepatobiliary MRI contrast *** precise role,advantage and disadvantages of these modalities are evaluated controversially in local staging,metastatic spread and treatment monitoring of ***,the authors will touch upon the future perspectives in functional imaging evaluating the role of integrated FdG-PET/CT with perfusion CT,MRI spectroscopy of primary CRC and hepatic transit time analysis using contrast enhanced ultrasound and MRI in the detection of liver *** of these newer imaging techniques may lead to significant improvements in the management of patients with colorectal cancer.

There is an unprecedented need for new treatments for renal failure,as the incidence of this disease is increasing disproportionately to advancements in *** treatments are limited by the availability of viable organs,for which there is a worldwide *** treatment modalities also require a substantial amount of infrastructure,significantly limiting the access to care in most *** tissue engineering approaches promise to develop alternative solutions that address many of the inadequacies in current *** many advancements have been made—primarily in the past decade—in biofabrication and whole-organ tissue engineering,many challenges *** major hindrance to the progress of current tissue engineering approaches is establishing successful vascularization of developed engineered tissue *** review focuses on the recent advancements that address the vascular challenge,including the biofabrication of vasculature,whole-organ engineering through decellularization and recellularization approaches,microscale organogenesis,and vascularization using organoids in the context of kidney tissue *** also highlight the specific challenges that remain in developing successful strategies capable of clinical translation.

The presence ofα/αon priorβ/βgrain boundaries directly impacts the final mechanical properties of the titanium ***β/βgrain boundary variant selection of titanium alloys has been assumed to be unlikely owing to the high cooling rates in laser powder bed fusion(L-PBF).However,we hypothesize that powder characteristics such as morphology(non-spherical)and particle size(50–120μm)could affect the initial variant selection in L-PBF processed Ti-6Al-4V alloy by locally altering the cooling *** the high cooling rate found in L-PBF,results showed the presence ofβ/βgrain boundaryαlath growth inside two adjacent priorβ*** backscatter diffraction micrographs confirmed the presence ofβ/βgrain boundary variant selection,and synchrotron X-ray high-speed imaging observation revealed the role of the“shadowing effect”on the locally decreased cooling rate because of keyhole depth reduction and the consequentβ/βgrain boundaryαlath *** self-accommodation mechanism was the main variant selection driving force,and the most abundantα/αboundary variant was type 4(63.26°//[10553¯]).The dominance of Category IIαlath clusters associated with the type 4α/αboundary variant was validated using the phenomenological theory of martensite transformations and analytical calculations,from which the stress needed for theβ→αtransformation was calculated.

despite a high prevalence of hepatitis C virus(HCV)infection,the vast majority of persons who inject drugs(PWId)have not engaged in HCV care due to a large number of *** about the infection among both PWId and providers remains an important challenge as does discrimination faced by PWId in conventional health care *** providers also remain hesitant to prescribe antiviral therapy due to concerns about adherence and relapse to drug use resulting in ***,however,as a result of improvements in treatment efficacy combined with professional society and government endorsement of HCV treatment for PWId,a pressing need exists to develop strategies to engage these individuals into HCV *** this article,we propose several strategies that can be pursued in an attempt to engage PWId into HCV *** advocate that multidisciplinary approaches that utilize health care practitioners from a wide range of specialties,as well as co-localization of medical services,are strategies likely to result in increased numbers of PWId entering into HCV *** of HCV therapy after stabilization through drug treatment is an additional strategy likely to increase PWId engagement into HCV *** full impact of direct acting antivirals for HCV will only be realized if innovative approaches are pursued to engage all HCV infected individuals into treatment.
