Disturbances in bone mineralization are frequent in cystic fibrosis but few st udies have focused on the use of biphosphonates in this indication, and none on the use of oral etidronate. We report our experience using this latter treatment . Methods. -The study was retrospective and included five children and three ad ults with cystic fibrosis (six males and two females) aged seven to 30:years wit h Z-scores for lumbar bone density lower than-2:SD after one year of calcium ( 1:g/day) and vitamin D (900:UI/day and 300:000:UI/6:months) supplementation. All were treated during one year with etidronate: four courses of 15:days (one cour se per trimester) with doses ranging from 4 to 8:mg/kg per day. Calcium and vita min D supplementation was continued between the etidronate treatment course. Tot al body and lumbar bone mineral density (BMD) were measured three times: at the beginning and the end of the year of calcium and vitamin D supplementation and at the end of the year of supplementation plus the four courses of etidronate treatment. Resu lts. -The increase in BMD in absolute value (g/cm2) and in Z-score was signifi cantly higher (P < 0.05) after the year of combined supplementation and etidron ate treatment (total body g/cm2: 3 ±1%, Z-score: 2 ±1%and lumbar spine g/cm 2: 6 ±5%, Z-score: 3 ±4%) than after supplementation alone (total body g/cm 2:-1 ±3%, Z-score:-4 ±3%and lumbar spine g/cm2:-1 ±3%, Z-score:-4 ± 4%). Supplementation alone improved the total BMD in only one patient and the l umbar BMD in three, whereas after etidronate treatment the total and lumbar BMD were improved in the eight patients. None of the patients presented with side ef fects that could be attributed to the treatment. Conclusion. -Oral etidronate t reatment is well-tolerated and capable of improving bone mineralization in pati ents with cystic fibrosis. Further work will be necessary to determine the optim al dosage and the optimal frequency for the treatment series.

Gastric emptying evaluation by ultrasound prior colonoscopy:An easy tool following bowel preparation

AIM: To investigate the gastric emptying after bowel preparation to allow general anaesthesia. METHODS: A prospective, non-comparative, and nonrandomized trial was performed and registered on Eudra CT database(2011-002953-80) and on ***(NCT01398098). All patients had a validated indication for colonoscopy and a preparation using sodium phosphate(NaP) tablets. The day of the procedure, patients took 4 tablets with 250 mL of water every 15 min, three times. The gastric volume wasestimated every 15 min from computed antral surfaces and weight according to the formula of Perlas et al(Anesthesiology, 2009). Colonoscopy was performed within the 6 h following the last ***: Thirty patients were prospectively included in the study from November 2011 to May 2012. The maximum volume of the antrum was 212 mL, achieved 15 min after the last intake. 24%, 67% and 92% of subjects had an antral volume below 20 mL at 60, 120 and 150 min, respectively. 81% of patients had a Boston score equal to 2 or 3 in each colonic segment. No adverse events leading to treatment discontinuation were ***: Gastric volume evaluation appeared to be a simple and reliable method for the assessment of gastric emptying. Data allow considering the NaP tablets bowel preparation in the morning of the procedure and confirming that gastric emptying is achieved after two hours, allowing general anaesthesia.

Aims: To determine whether central corneal thickness (CCT) is a significant pr edictor of visual field and optic disc progression in open angle glaucoma. Metho ds: Data were obtained from a prospective study of glaucoma patients tested with static automated perimetry and confocal scanning laser tomography every 6 month s. Progression was determined using a trend based approach called evidence of ch ange (EOC) analysis in which sectoral ordinal scores based on the significance o f regression coefficients of visual field pattern deviation and neuroretinal rim area over time are summed. Visual field progression was also determined using t he event based glaucoma change probability (GCP) analysis using both total and p attern deviation. Results: The sample contained 101 eyes of 54 patients (mean (S D) age 56.5 (9.8) years) with a mean follow up of 9.2 (0.7) years and 20.7 (2.3) sets of examinations every 6 months. Lower CCT was associated with worse baseli ne visual fields and lower mean IOP in the follow up. In the longitudinal analys is CCT was not correlated with the EOC scores for visual field or optic disc cha nge. In the GCP analyses, there was a tendency for groups classified as progress ing to have lower CCT compared to nonprogressing groups. In a multivariate analy ses accounting for IOP, the opposite was found, whereby higher CCT was associate d with visual field progression. None of the independent factors were predictive of optic disc progression. Conclusions: In this cohort of patients with establi shed glaucoma, CCT was not a useful index in the risk assessment of visual field and optic disc progression.

Objective: To determine the effects of 8 weeks of therapy with amlodipine, ramipril or telmisartan on the autonomic system over 24 h in hypertensives. Methods: After a placebo run-in, 57 patients were included in a prospective randomized open-label design protocol for therapy with amlodipine(5 mg for 4 weeks followed by 10 mg for 4 weeks, n=22), or ramipril(2.5 mg for 1 week, 5.0 mg for 3 weeks and 10 mg for 4 weeks, n=17) or telmisartan(80 mg for 8 weeks, n=18). Autonomic functions were assessed by norepinephrine(NE) and epinephrine(E), as well as by the spectral analysis of heart rate variability(HRV). Results: The 24-h ambulatory blood pressure, plasma NE and HRV demonstrated the characteristic day-night circadian rhythm in hypertensives. Higher values for SBP and DBP and for NE levels, as well as for spectral analysis components-low frequency band(LF) and low frequency/high frequency(LF/HF) ratio-were found during the day, whereas the HF was higher during the night. In patients treated with amlodipine, the HF decreased significantly during the night, while the LF and the LF/HF ratio increased during the day in association with the rise in NE. The therapy with telmisartan did increase the HF during the night and the day, while ramipril did not influence all HRV components during the night but significantly increased the HF, and decreased the LF/HF ratio during the day. No changes were observed in plasma NE with telmisartan or ramipril, but a 50%increase in NE levels throughout the 24-h period was found in amlodipine-treated patients. Conclusion: These data suggest a sympathetic activation during the day and a decrease in parasympathetic activity during the night after therapy with amlodipine, correlated with increases in plasma NE. In contrast, the therapy with telmisartan significantly increased parasympathetic activity without changes in NE during the night and day. The therapy with ramipril increased the parasympathetic activity only during the day.

Does Dust from Arctic Mines Affect Caribou Forage?

This study explores how dust from the Ekati Diamond Mine potentially affects the availability and quality of forage on the seasonal range of the Bathurst caribou herd. Understanding the effects of dust as a source of disturbance is important because the Bathurst caribou population has declined by 93% since the middle 1980s and there are reports that caribou in general may avoid mining projects. There are several challenges for quantifying dust impacts: 1) Natural variations (e.g., topography, natural disturbance, and soil pH) may also impact forage availability and quality for caribou. To minimize their masking effect, we stratified survey sites into seven land cover classes and selected the most populous class (i.e., the dwarf shrub) for assessing the impact. 2) Within class variation (e.g., the proportion of area covered by rocks where vascular plants and lichen do not grow) can further skew the analysis. We eliminated this problem by examining only the area not covered by rocks. 3) Coarse and fine suspended particulates have different spatial coverages, chemical compositions, and pH values. Consequently, their impacts on caribou forage can be different. To distinguish their impacts, we sampled two areas: transects from the Misery Haul Road that has been in active use vs. those from a rarely used spur road outside the Misery Camp. We sampled percent vegetation cover, soil pH, and dust on leaves along these transects during the summers of 2015 and 2016. Our results indicated that the amount of dust on leaves in a zone of ~1000 m from the Misery Haul Road was 3 - 9 times than that of background sites. The zone of reduced lichen percent cover was also about 1000 m. In contrast, these road dust-induced changes in caribou forage were not observed for the dust-free transect from the spur road.

Optimization of the generator settings for endobiliary radiofrequency ablation

AIM:To determine the optimal generator settings for endobiliary radiofrequency ablation. METHODS:Endobiliary radiofrequency ablation was performed in live swine on the ampulla of Vater,the common bile duct and in the hepatic parenchyma. Radiofrequency ablation time,"effect",and power were allowed to vary. The animals were sacrificed two hours after the procedure. Histopathological assessment of the depth of the thermal lesions was performed. RESULTS:Twenty-five radiofrequency bursts were applied in three swine. In the ampulla of Vater(n = 3),necrosis of the duodenal wall was observed starting with an effect set at 8,power output set at 10 W,and a 30 s shot duration,whereas superficial mucosal damage of up to 350 μm in depth was recorded for an effect set at 8,power output set at 6 W and a 30 s shot duration. In the common bile duct(n = 4),a 1070 μm,safe and efficient ablation was obtained for an effect set at 8,a power output of 8 W,and an ablation time of 30 s. Within the hepatic parenchyma(n = 18),the depth of tissue damage varied from 1620 μm(effect = 8,power = 10 W,ablation time = 15 s) to 4480 μm(effect = 8,power = 8 W,ablation time = 90 s). CONCLUSION:The duration of the catheter application appeared to be the most important parameter influencing the depth of the thermal injury during endobiliary radiofrequency ablation. In healthy swine,the currently recommended settings of the generator may induce severe,supratherapeutic tissue damage in the biliary tree,especially in the high-risk area of the ampulla of Vater.
