Management of liver transplantation biliary stricture: Results from a tertiary hospital

AIM: To review results of endoscopic treatment for anastomotic biliary strictures after orthotopic liver transplantation(OLT) during an 8-year period. METHODS: This is a retrospective review of all endoscopic retrograde cholangiopancreatographys(ERCps) performed between May 2006 and June 2014 in deceased OLT recipients with anastomotic stricture at a tertiary care hospital. patients were divided into 2 groups, according to the type of stent used(multiple plastic or covered self-expandable metal stents), which was chose on a case-by-case basis and their characteristics. The primary outcome was anastomotic stricture resolution rate determined if there was no more than a minimum waist at cholangiography and a 10 mm balloon could easily pass through the anastomosis with no need for further intervention after final stent removal. Secondary outcomes were technical successrate, number or ERCps required per patient, number of stents placed, stent indwelling, stricture recurrence rate and therapy for recurrent anastomotic biliary stricture(AS). Stricture recurrence was defined as clinical laboratorial and/or imaging evidence of obstruction at the anastomosis level, after it was considered completely treated, requiring subsequent interventional ***: A total of 195 post-OLT patients were assessed for eligibility. One hundred and sixty-four(164) patients were diagnosed with anastomotic biliary stricture. ERCp was successfully performed in 157/164(95.7%) patients with AS, that were treated with either multiple plastic(n = 109) or metallic billiary stents(n = 48). Mean treatment duration, number of procedures and stents required were lower in the metal stent group. Acute pancreatitis was the most common procedure related complication, occurring in 17.1% in the covered self-expandable metal stents(c SEMS) and 4.1% in the multiple plastic stent(MpS) group. Migration was the most frequent stent related complication, observed in 4.3% and 5.5%(c SEMS and MpS respectively

Final results of a 2:1 control-case observational study using interferon beta and interleukin-2,in addition to first-line hormone therapy,in estrogen receptor-positive,endocrine-responsive metastatic breast cancer patients

Aim:We conducted a pilot study that combines immunotherapy(cyclic interleukin-2 interferon-beta sequence)and hormone therapy(HT)to overcome endocrine resistance in metastatic breast ***:The final results of a 2:1 control-case retrospective observational study are here shown following 22 additional months of postoperative follow-up and 6 further *** were 95 controls and 42 cases in *** 95 controls were ER+/HER2-metastatic breast cancer patients who underwent first-line HT with aromatase inhibitors(AIs)or ***-eight of them(28.9%)also received biological drugs including cyclin kinase inhibitors(CKIs).The 42 cases were ER+metastatic breast cancer patients who received interferon beta-interleukin-2 immunotherapy in addition to first-line *** estrogen receptor modulators/down-regulators(SERMs/SERDs)were used for HT in 39(92.9%)of them and AIs in the remaining ***:Median progression-free survival(pFS)and overall survival(OS)were significantly longer in the 42 studied patients who received hormone immunotherapy(HIT)than in the 95 controls(median time 33 vs.18 months,p=0.002,and 81 vs.62 months,p=0.019).In the analysis adjusted for disease-free interval(DFI),hormone receptor,HER2 status,visceral involvement,AIs,and biological therapy,the pFS and OS hazard ratio(HR)further increased in favor of the 42 cases(p=0.004 and p=0.044 respectively).In the same ER+/HER2-metastatic breast cancer patients treated with both AIs and CKIs,a median pFS ranging from 25.3 to 28.18 months and a median OS of 37.5 months were ***:This study strongly suggests multi-center randomized clinical trials should be performed to enter our proposed immunotherapy into clinical practice.

MINI-EX:Integrative inference of single-cell gene regulatory networks in plants

House geckos in the genus Hemidactylus are highly successful colonizers of regions beyond their native range,with colonization often resulting in displacement of native gecko species through competitive interactions for daytime refuge(crevices)and prey *** report on data collected from nighttime surveys undertaken in April-May 2014 on Barbados,West Indies,that focused on the distribution and abundance of the endemic Barbados leaf-toed gecko(phyllodactylus pulcher)and the introduced tropical house gecko(Hemidactylus mabouia)along unlit coastal walls and among boulders in the grounds of a hotel *** contrast to patterns of displacement of native species by *** seen elsewhere,*** was more abundant than *** on coastal walls,whereas the latter was found in greater numbers using boulders at this *** and boulders differed with regard to availability of diurnal refugia suitable for geckos,with the walls having high frequency of small crevices with openingsparation was a result of differences in diurnal refuge use between the species,we conducted experimental trials in which geckos were allowed to select between refugia with different *** species selected for narrower and warmer refugia,and refugia that had been previously occupied by the other *** shared preferences for refugia type suggest that other factors underlie the niche separation observed in the *** supporting high densities of ***,coastal walls could offer important secondary habitat by augmenting the natural cliff side habitat of this endemic gecko,a finding that could be exploited for the conservation of this candidate species for Critically Endangered classification.

Influence of Thermal Radiation on Hot Cluster Decay Rates and Abundances

The influence of radiative cooling on the unimolecular decay rates of free, hot clusters and molecules with unspecified excitation energies is quantified. Two different regimes, dedined by the magnitude of the energy of the photons emitted, are identified and the boundary between them is given. The boundary is determined in terms of the photon emission rate constants and thermal properties of the particles. Also the abundance spectra are calculated for the continuous cooling case, corresponding to small photon energies. The two regimes correspond to continuous cooling and single photon quenching of the unimolecular decay. The radiative effect can be parametrized by a redefinition of the time each individual cluster has available to undergo evaporation, expressed by an effective radiative time constant.

The global tectonics of Mercury is dominated by contractional features mainly represented by lobate scarps,high relief ridges,and wrinkle *** structures are the expression of thrust faults and are linear or arcuate features widely distributed on ***,these structures are arranged in long systems characterized by a preferential orientation and non-random spatial *** this work we identified five thrust systems,generally longer than 1000 *** were named after the main structure or crater encompassed by the system as:Thakur,Victoria,Villa Lobos,Al-Hamadhani,and *** order to gain clues about their formation,we dated them using the buffered crater counting technique,which can be applied to derive the ages of linear landforms such as faults,ridges and *** estimate the absolute age for the end of the thrust system's activity,we applied both Le Feuvre and Wieczorek production Function and Neukum production ***,to further confirm the results obtained with the buffered crater counting method,the classic stratigraphic approach has been adopted,in which a faulted and an unfaulted craters were dated for each *** results gave consistent ages and suggested that the most movements along major structures all over Mercury most likely ended at about 3.6-3.8 *** gives new clues to better understand the tectonics of the planet and,therefore,its thermal ***,the early occurrence of tectonic activity in the planet's history,well before than predicted by the thermophysical models,coupled with the orientation and spatial distribution of the thrust systems,suggests that other processes beside global contraction,like mantle downwelling or tidal despinning,could have contributed to the first stage of the planet's history.

Down-regulation of DpH2L gene during cellular differentiation/apoptosis:Use of mRNA differential display

To characterize the genes associated with differentiation/apoptosis induced by all-trans retinoic acid (ATRA) in human lung cancer cells, mRNA differential display was employed. Six cDNA fragments have been isolated, and one of them corresponds to a sequence-known gene DpH2L with unknown function, which was first isolated from the critical region of deletion on chromosome 17p13.3 in human ovarian carcinoma, and regarded as a candidate tumor suppressor gene. Results show that DpH2L is a wide expressed gene, and has another major transcript besides the previously reported 2.3 kb transcript. It is proved that the DpH2L gene is dawn-regulated during differentiation or apoptosis in several kinds of cancer cells induced by all-trans retinoic acid and N-(4-hydroxyphenyl) retinamide (4HpR). This may suggest that DpH2L does not play a role as a tumor suppressor gene, on the contrary, its down-regulation may be functionally involved in the transition from cell growth to differentiation or apoptosis.
